RESUMO

STATEMENT OF PROBLEM: Complete arch fixed implant-supported prostheses have gained substantial popularity as a treatment option for patients with edentulism or a failing dentition. While YouTube is a leading source of health-related information videos, the usefulness and quality of videos on YouTube about complete arch fixed implant-supported prostheses are unclear. PURPOSE: The purpose of this study was to evaluate the quality of videos on complete arch fixed implant-supported prostheses and assess their usefulness for the general population as an educational resource. MATERIAL AND METHODS: An electronic search for "complete arch fixed treatment using dental implants" was conducted on YouTube on October 10, 2021. The videos in English with a duration between 4 and 20 minutes were considered for the analysis. A total of 89 videos were subjected to quantitative analysis by using a usefulness score. Additionally, to evaluate video quality, 5-point global quality scale (GQS) criteria were applied. A 1-way analysis of variance (ANOVA) was used to compare mean scores among different subgroups. The Pearson correlation analysis investigated the relationship among the usefulness score, GQS score, and video demographics, which include video content, source, type (academic or nonacademic), total views, and upload days (the days since the video was uploaded to YouTube) (α=.05). RESULTS: The mean number of views for the 89 selected videos was 193 321 with a mean daily upload of 1241. The mean ±standard deviation usefulness score was 1.55 ±1.35, with 78.7% of the videos rated poor, 20.2% moderate, and 1.1% excellent. The majority of videos included procedures involved (44.9%) and advantages (41.9%), and the least discussed topics were contraindications (7.9%) and prognosis and survival (6.7%). The GQS scoring had a mean score of 1.98 ±0.78 out of a total score of 5. While statistically significant differences were not found among the video source, total views or upload days, and usefulness score (P>.05), significant differences were found between the type of videos and usefulness score (P<.001). CONCLUSIONS: The majority of videos on complete arch fixed implant-supported rehabilitation were rated as poor in usefulness for patient education. Quality videos on implant-supported fixed rehabilitation were lacking on YouTube (GQS score <1.50), with most videos consisting of marketing or promotional content rather than unbiased educational information.

INTRODUCTION: Denture disinfection is an indispensable procedure for preventing cross contamination and the maintenance of a healthy oral mucosa in patients rehabilitated with removable dental prosthesis. Nevertheless, they are known to cause changes in the physical and mechanical properties of denture base resins and acrylic resin denture teeth following immersion of a denture in a suitable chemical disinfectant solution or by undergoing microwave irradiation. One such mechanical property indicator for artificial tooth materials is hardness. AIM: To assess the surface hardness of acrylic resin teeth of three different commercial brands (Ivoclar, Newace, Acryrock) following chemical (2% glutaraldehyde, 1% sodium hypochlorite) and microwave disinfections. MATERIALS AND METHODS: Ten specimens of each of the three commercial brands were made for control and each simulated disinfection type and stored in distilled water at room temperature for 24 hours. After water storage, specimens were immersed in 2% glutaraldehyde and 1% sodium hypochlorite (one and three cycles) at room temperature for 10 minutes. Irradiation with microwave (one and three cycles) was done in domestic microwave for three minutes with the specimens immersed in 150 ml of distilled water. The specimens were stored in distilled water at room temperature for seven days after each disinfection cycle. Vickers hardness measurements were made using a hardness indenter under a load of 50 g force for 10 seconds. Data was subjected to repeated measure two-way ANOVA test and Tukey's test. RESULTS: There were statistically significant differences for the variables disinfection, tooth, and cycle (p<0.05 for teeth & disinfectant interaction, p<0.05 cycle and disinfectant interaction). The mean surface hardness following one microwave disinfection cycle was lower than control, glutaraldehyde and sodium hypochlorite. Comparison among cycles revealed that microhardness was significantly decreased for three cycles of microwave disinfection. CONCLUSION: It was concluded that there was no significant difference in microhardness when the teeth were subjected to chemical disinfection but three cycles of microwave disinfection produced decrease in the microhardness of different types of artificial teeth.

Maxillary sinus pneumatisation pose a grave clinical challenge for implant fixed rehabilitation in posterior maxilla owing to diminished bone volume. This necessitates sinus lift and grafting which increase the duration and cost and possible surgical complications. Pterygoid implant has a greater short term osseointegration and is a proven treatment method for rehabilitation of highly resorbed posterior maxilla. To overcome the limitations of sinus grafting techniques, the current case report describes the use of flapless, tilted and pterygoid implant for restoration of partially edentulous atrophic maxilla eliminating grafting (AU).


A pneumatização do seio maxilar representa um grande desafio clínico para a reabilitação fixa por implante na região posterior da maxila devido ao volume ósseo diminuído. Isso requer elevação do seio e enxerto, fatores que aumentam a duração, o custo e as possíveis complicações cirúrgicas. O implante pterigóide tem uma osseointegração maior em curto prazo e é um método de tratamento comprovado para reabilitação de maxila posterior altamente reabsorvida. Para superar as limitações das técnicas de enxerto de seio, o relato de caso atual descreve o uso de implante sem retalho, inclinado e pterigóide para restauração de maxila atrófica parcialmente edêntula eliminando o enxerto.(AU)
