RESUMO

A cumulative psychiatric case register was utilized to examine patterns of psychopathology in 1,334 adolescent patients. Consistent with studies of adult patients, seriousness of disorder was weighted toward being seen in an inpatient rather than outpatient settling; schizophrenia and personality disorder were relatively more common amoung lower-class patients and neurosis and situational disorder among middle- and upper-class patients; neurosis was more commonly diagnosed among female patients, and personality disorder more commonly diagnosed among males. A ten-year follow-up revealed 54.1% reasonable diagnostic stability on subsequent contacts and 62.2% complete subsequent agreement among patients originally diagnosed as schizophrenic. Aspects of the data demonstrate continuity in adolescent and adult psychopathology, the mythical nature of "normative adolescent turmoil," and what appears to be excesive use of situational disorder in diagnosing adolescent patients.

The authors studied 1,106 consecutive patients who called for an initial outpatient appointment at a medical center department of psychiatry during a one-year period. The 759 (68.6%) patients who kept their initial scheduled appointment differed from the 347 (31.4%) who did not in age, previous psychiatric treatment, source of referral, chief complaint, and number of days they waited for an appointment. A telephone follow-up conducted with almost 30% of those who did not keep their initial appointment revealed that of those contacted, 26 (25%) sought other routes for psychiatric help and 23 (22%) made a subsequent contact at the facility studied.

RESUMO

A brief report of the attitudes of therapists towards psychiatric outpatients, varying in socioeconomic background, insight levels and degrees of disturbances, is presented. Results suggest that the professional affiliation and sex of therapists influenced their attitudes towards the patients.

RESUMO

A questionnaire on teaching of the Rorschach technique was sent to all APA-approved graduate clinical psychology programs in the United States and Canada. The questionnaire asked for the degree of emphasis placed on the Rorschach in the primary assessment course, the Rorschach teaching experience of the respondent and the respondent's evaluation of the technique as a clinical tool, a teaching aid and a research instrument. The respondent in each case was the faculty member teaching the major diagnostic assessment course. There was a 100% return of the questionnaires. The major results show that (1) 81% of the programs place major emphasis on the Rorschach Technique in the assessment course; (2) only 24% of the programs offer the course for a full year; (3) respondents with more than 10 years teaching experience rated the Rorschach higher than respondents with less experience, and (4) respondents as a group rated the Rorschach highly as a clinical tool and teaching aid, but gave it generally low ratings as a research instrument.

RESUMO

Female and male neurotic depressives in an outpatient psychiatric clinic were compared on measures of subjective distress, interpersonal concerns, and types of treatment given. There were essentially no differences between the two groups on the patient self-report measures. However, female patients were found to have significantly more therapy sessions that the male neurotic depressives. Also, the female patients were more likely than the male patients to receive psychotropic medication, especially the more potent antidepressant variety.

Differences in patterns of utilization of psychiatric services were studied in 178 adult outpatients who either had dropped out of, or had terminated from treatment with the consent of their therapists. The Monroe County Psychiatric Case Register was employed to determine the use of psychiatric services by these patients within a period of two years before and two years after their admission to a community mental health center (CMHC). Dropping out of treatment was associated with a pattern of utilization of multiple psychiatric facilities. Of those individuals who made contact with a psychiatric facility both prior to and subsequent to their admission to the CMHC, the dropout group made, on the average, approximately twice as many contacts per person as the terminator group.

The numerical criteria of either mean or median number of sessions as a nethodological procedure for categorizing patients as dropouts were compared with the clinically based definition in a sample of 221 outpatients. Lack of agreement was noted in two primary areas. A large number of clinically defined terminators were classified incorrectly as dropouts, and a large number of clinically defined dropouts were not classified when the mean or median number of sessions was used as a criterion to classify the dropout. The results of the study supported the contention that definitions of dropping out based on number of sessions lead to categorizations that are not comparable to those based on knowledge of the clinical data.

RESUMO

Therapists from four professional groups--nursing, psychiatry, psychology and social work--were compared on Lerner's Democratic Values Scale. Significant professional differences were found on this scale. The findings raise questions about the predictive power of this scale when a variety of mental health professionals are studied.
