BACKGROUND: Reviewing the literature, no study on the rate of regrowth of tongue coatings after tongue cleaning was found. Therefore, the purpose of this study in young adults was to study the rate of reformation of tongue coatings after mechanical removal. MATERIAL AND METHODS: Thirty-five dental students participated in the present study. Following preparatory study instructions, baseline examinations were carried out followed by 3 days of observation. At baseline, tongue coating scores (prescraping) were obtained followed by tongue scrapings and determination of the wet weights of the coatings. A second tongue coating score was then obtained within 5 min of the first score (immediate post-scraping). The subjects returned for repeated tongue coating scores after 1 and 2 days and for final examination after 3 days, which included both tongue coating scores (prescraping and immediate post-scraping) and determination of the wet weights of the coatings. RESULTS: Prior to scraping the tongue at day 0 (baseline), mean tongue coating amounted to a surface extension of 33% of the entire dorsum of the tongue. Scraping the tongue reduced the score to 9%. On average, tongue coating scores had returned to baseline levels on day 2. The mean wet weights of tongue scrapings at days 0 and 3 were similar and amounted to 0.09 +/- 0.07 and 0.09 +/- 0.06 g, respectively. CONCLUSION: If tongue cleaning is to be recommended, the results of this study in dental students indicate that tongue cleaning should be performed on a daily basis.

Twenty-six mandibular class III furcation defects were treated in 26 subjects using a regenerative therapy that included citric acid root conditioning, placement of freeze-dried decalcified bone allograft and coronally positioned flaps secured by crown-attached sutures. In addition to this therapy, expanded polytetrafluoroethylene (ePTFE) membranes were placed in 14 of the 26 defects. The effect of these therapies was evaluated after 52 to 60 weeks through a series of soft and hard tissue probing measurements including residual furcation classification. For both treatments, mean improvements were observed for furcal probing attachment levels, furcal bone levels, and defect volumes. However, only a few of these through-and-through defects had become closed as judged by a panel of 3 independent examiners. Four defects in each of the 2 treatment groups had changed from Class III to Class I/II or Class II/II evaluated with the soft tissue in place. One defect treated without membrane and 3 defects treated with membranes had changed from Class III to Class II as recorded after reflection of the soft tissues at re-entry. Little difference was observed between defects treated with and without membranes. Further studies with larger sample sizes and longer observation times are needed to fully evaluate these regenerative procedures and their potential for healing Class III furcation defects.

RESUMO

The effect of tongue brushing on formation of dental plaque was studied in two groups of dental hygiene students. In one of the groups plaque formation during a 4-day period of no oral hygiene procedures was compared to a 4-day period of tongue brushing as the only hygiene procedure. In the other group tongue brushing as an adjunct to tooth brushing was evaluated on plaque formation during 7-day periods of partially ineffective tooth brushing. Both experiments failed to show any effect of the tongue brushing on plaque formation.

The thickness of the radiographic image of the maxillary sinus mucosa on intraoral radiographs was evaluated in 13 patients with advanced periodontal disease, prior to and 12 months following initial periodontal therapy. Before treatment, a relationship was observed between the thickness of the sinus mucosa and the mean probing depths of the teeth in the involved sextant. As many as 79% of the available sextants showed swelling of the mucosa prior to periodontal therapy, compared to only 17% after treatment. This report supports previous studies indicating that advanced periodontal disease may cause swelling of the maxillary sinus mucosa and that periodontal therapy will significantly reduce such swelling.

Twenty-seven mandibular class III furcation defects were treated in 27 subjects using a regenerative therapy that included citric acid root conditioning and coronally positioned flaps secured by crown-attached sutures. In addition to this therapy, 13 of the 27 defects received freeze-dried, decalcified allogenic bone grafts. The effect of these therapies was evaluated from soft tissue probing measurements, including furcation probings to determine soft tissue closure of the defects. At 6 months postsurgery, the mean vertical probing depth reduction and the mean probing attachment level gain in the furcation area were 2.6 mm and 2.2 mm for the non-grafted defects and 1.9 mm and 1.5 mm for the grafted defects. One of the 14 non-grafted defects and 3 of 13 grafted defects were judged to show soft tissue clinical closure by a panel of 3 independent examiners. No statistically significant differences were observed between defects treated with or without bone grafts.

This study evaluated healing, with emphasis on root resorption, following root surface treatment with 1% aqueous stannous fluoride (SnF2), saturated citric acid (CA), or saline control (C) in conjunction with periodontal flap surgery. Supraalveolar periodontal defects were surgically created and immediately treated in the mandibular premolars in 6 beagle dogs. The defect height approximated 5 to 6 mm from the reduced alveolar bone to the cemento-enamel junction. Root treatments were rotated between experimental teeth within jaw quadrants and duplicated in left and right quadrants in the dogs. Flaps were raised to cover most of the crowns of the teeth and sutured. The dogs were sacrificed 12 weeks after surgery and tissue blocks with teeth and adjacent structures were processed for histometric analysis. SnF2-treated teeth healed with significantly longer junctional epithelium, less connective tissue repair to the root surface, and less bone regeneration than CA and C-treated teeth. New cementum formation was limited in all treatment groups. Root resorption was observed in almost all teeth exhibiting connective tissue repair, however to a lesser amount and not as frequent in SnF2 treated teeth due to limited connective tissue repair. No differences were found in amount and frequency of root resorption in CA and C-treated teeth. An inhibitory effect on root resorption of SnF2 could not be disclosed in this experiment, however, it may be concluded that CA treatment of the root surface in conjunction with reconstructive periodontal flap surgery does not seem to enhance root resorption.

AIM: The purpose of this study was to determine inter-examiner reproducibility of two modified Winkel tongue coating indices and to study the relationship between each of these two indices and the amount of tongue coating as determined by wet weight of scrapings from the dorsum of the tongue. MATERIALS AND METHODS: Twenty-five participants were selected for the study from among dental students. The selection was made to assure that a wide range of degrees of tongue coating were represented. Two independent examiners scored the tongue coatings using the two modified Winkel tongue coating indices (mWTCI 1 + 2 and mWTCI 2). After completion of index recordings, tongue scrapings were performed by one of the examiners. RESULTS: Inter-examiner reproducibility of mWTCI 1 + 2 and mWTCI 2 using Pearson's coefficients of correlation amounted to r = 0.48 and 0.93 respectively. Overall, mWTCI 2 showed higher correlations to wet weight of tongue scrapings than mWTCI 1 + 2. CONCLUSION: The mWTCI 2 was found to be highly reproducible and also valid as related to wet weight of tongue scrapings. This index would seem suitable for further studies on tongue coatings; effect of tongue cleaning; and rate of reformation of coatings after cleaning.

The purpose of this report was to evaluate the magnitude of the regression towards the mean (RTM) relative to the observed changes in probing measurements following periodontal therapy in 2 groups of patients. Regression lines were calculated for observed changes in probing depths and probing attachment levels related to the initial probing depths of the individual sites. Regression lines, adjusted for the RTM effect, were also calculated. For the extreme subgroups of recorded shallow and deep sites, the RTM effect (mm) was found to amount to a maximum of 0.6 mm. It appears unlikely that erroneous conclusions have been made in previous clinical studies evaluating probing depth changes in deep sites, since the RTM accounts for a limited portion of the observed changes. For probing depth changes in shallow sites and for probing attachment level changes in both shallow and deep sites, the RTM accounts for a larger proportion of the observed mm changes. It therefore seems prudent to be careful about interpretation of these changes in clinical studies, where the RTM effect has not been adjusted for. Whenever possible, duplicate initial recordings should be taken to allow determination of and subsequent adjustment for the RTM effect.

Citric acid conditioning of the root surface as a supplement to replaced flap surgery of intraosseous periodontal defects was evaluated. Thirteen patients and a total of 45 proximal defects with residual probing pocket depth greater than or equal to 6 mm after initial preparation were used. The effect of treatment was studied in matched subsamples of the total material including one acid-treated and one nonacid-treated defect from each subject. The results demonstrated that an average gain of probing attachment level amounting to 2.l0 mm was obtained following acid treatment as compared to 1.1-1.2 mm for the nonacid-treated controls. Corresponding figures for gain or probing bone level were 1.2-1.3 mm for acid treatment and 0.8-0.9 mm for controls. The clinical significance of these findings was discussed.

16 advanced periodontitis patients were subjected to initial periodontal treatment and monitored every 3rd month during 42 months. Clinical characteristics at baseline and during the 42-month maintenance period were investigated for their association with probing attachment loss over the 42-month period, both on a patient level and on a site level. On a patient level, averaged full-mouth plaque and bleeding on probing scores over the maintenance interval showed little association with probing attachment loss. Little association was also observed for % sites with depth > or = 6 mm at baseline. However, a notable relationship was seen for % sites > or = 6 mm at 3 months. This finding initiated a separation of the 16 subjects into 2 groups based upon % sites > or = 6 mm at 3 months (groups 'high' and 'low'). Site level analyses for these groups showed little association between frequent presence of plaque at the sites over the maintenance interval and probing attachment loss. Frequent bleeding on probing showed limited relationship with attachment loss for group 'low', but an appreciable association for group 'high'. The findings suggest that advanced periodontitis patients with multiple residual probing depths > or = 6 mm at re-evaluation run a greater risk of developing sites with additional attachment loss than patients with few such residual depths. For such higher risk patients, bleeding on probing at maintenance examinations may be a useful indicator of subsequent deterioration at a site level.

Periodontal sites which had undergone probing attachment loss were identified in 16 advanced periodontitis patients subjected to initial periodontal treatment and monitored every 3rd month for 42 months. Many sites with probing attachment loss showed no increase in probing depth and bled on probing at a few of the examinations during maintenance. Others showed increase in depth, frequent bleeding and suppuration. An arbitrary classification of 'questionable periodontitis' sites was used and included sites with the following characteristics: probing depth at 42 months < or = 3.5 mm + bleeding on probing at no more than 5 of the 14 examinations 3-42 months + no suppuration on probing 3-42 months. The % sites in this category ranged from 21-35%, depending upon method used to determine probing attachment loss. Widely varying characteristics of sites identified with probing attachment loss were evident for all surface locations and all subgroups of initial probing depth. From a traditional viewpoint, a significant proportion of the losing sites might not have been afflicted with periodonitis. On the other hand, deterioration does not necessarily have to be coupled with obvious inflammation, particularly following therapy.

Two plaque control programs were compared. One of the programs was given as a series of three consecutive 30-min instruction appointments during a 14-day period. The other program was condensed into one 15-min instruction appointment and two subsequent 15-min follow-up visits. Fifty-nine blue-collar workers served as subjects and were divided into three matched groups: two experimental groups (30-min and 15-min group) and one control group. The 30-min program as well as the 15-min program significantly reduced the plaque scores in the participants. No difference could be observed between the results of the two programs during the immediate 12-week observation interval or throughout the subsequent 24 months of maintenance care. The results of the present study together with results by Söderholm et al. (1980) and Woofter et al. (1980) indicate that comprehensive plaque control programs may not be necessary and may not increase the effect of basic plaque control instruction.

RESUMO

In recent years numerous reports have appeared in the dental literature comparing different methods of treating periodontitis. These researches have not indicated that surgical procedures have particular advantages over thorough hygiene therapy. This review considers several relevant hypotheses and the results of clinical research into this topic. Particular note has been taken of the research of Ramfjord et al. (1987) whose conclusions are typical of many others. This review discusses such conclusions from several standpoints and shows that there is a need for further research. It is derived from a review by the same authors in the journal of the Swedish Dental Association, Tandläkartidingen.

The purpose of the present study was to evaluate the effects of subgingival irrigations with tetracycline as a supplement to mechanical plaque control and root debridement on clinical conditions of periodontal furcation pockets. 20 subjects with molar teeth having furcation pockets of varying depths and different grades of furcation involvement served for the study. Following oral hygiene instruction and root debridement at baseline, test teeth were irrigated subgingivally by a professional with a solution of 50 mg/ml of tetracycline, and control teeth with saline every 2nd week for 3 months. Records of dental plaque, bleeding on probing, probing depth and probing attachment level were obtained at 0, 1, 2, 3, 6, 9 and 12 months. The results failed to demonstrate any significant differences between test and control teeth for any of the subgroups of furcation sites at any observation interval. It is suggested that future studies may need to be performed over longer periods of time, and that the antimicrobial agents may need to be administered in vehicles, which provide prolonged periods of active subgingival concentrations.

Healing events after nonsurgical periodontal therapy in patients with periodontal pockets 4--7 mm deep were investigated. Incisors, cuspids and premolars in 15 patients were treated by plaque control and supra- and subgingival debridement using hand or ultrasonic instruments in a split mouth approach. The results were evaluated by recordings of plaque scores, bleeding on probing, probing pocket depths and probing attachment levels. All these parameters were improved during the initial 4--5 months after start of therapy. Little change occurred during the rest of the 13-month observation period. No difference of results could be observed comparing hand and ultrasonic instrumentation or comparing the results of two different operators. Initially a total of 106 sites demonstrated probing pocket depths greater than or equal to 6 mm. At 13 months only 13 such sites were observed. The apparently successful results of conservative treatment of patients with 4--7 mm deep pockets in the present study raise the question to what extent nonsurgical therapy is feasible also in patients with severely advanced lesions.

Facial gingival surfaces over cuspids and bicuspids in 20 patients were monitored for 5 years to evaluate the importance of attached gingiva in the maintenance of a stable periodontium. 43 surfaces "at risk" were compared to 36 "non-risk" surfaces within these patients. Surfaces "at risk" had to meet the following 3 criteria as established by both of 2 independent examiners: width of keratinized gingiva less than or equal to 1.0 mm; absence of attached gingiva; mobility of the gingival margin. During the 5 years of observation, "non-risk" surfaces showed a trend toward decrease in mean width of keratinized gingiva, while this measurement remained unchanged for the surfaces "at risk" Probing depths remained unaltered for both groups. A trend toward gingival recession and loss of probing attachment was observed for both "risk" and "non-risk" surfaces. Thus, this study failed to demonstrate that "unattached" and mobile facial gingival surfaces are more susceptible to periodontal breakdown than "attached" surfaces.

The present study compared surgical therapy to root planing alone in the treatment of periodontal intraosseous defects. 25 defects in 14 patients were subjected to root planing only and another 25 defects in the same patients were surgically exposed and citric acid treated. The healing response was evaluated 6 months after treatment. The mean gain of probing attachment level was 0.8 mm in the root-planed defects as compared to 1.3 mm for the surgically exposed and acid-treated defects. The probing bone level improved an average of 0.2 mm for the root-planed areas as compared to 0.6 mm for the acid-treated defects. The mean preoperative probing pocket depths of 6.7 mm and 6.8 mm for the 2 groups were reduced to 5.2 mm and 4.1 mm, respectively. The differences in these parameters were statistically significant between the 2 groups. However, both groups demonstrated limited regeneration.

Incisors, cuspids and premolars in 49 patients with advanced chronic periodontitis were treated with initial, nonsurgical periodontal therapy. The results were monitored by probing attachment level measurements for 6 sites of each tooth every 3rd month during a period of 24 months. Amongst sites with initial probing depth greater than or equal to 4.0 mm, sites with probing attachment loss were identified using regression analysis. Scores for plaque, bleeding, suppuration on probing and probing depth, obtained for these sites during the 24-month study, were analyzed to determine whether any of the scores could be used diagnostically as an indicator of probing attachment loss. Diagnostic sensitivity and predictability were calculated for different levels of each of the scores. The results showed that sites with probing attachment loss were more frequent for sites with high scores for plaque, bleeding, residual probing depth and suppuration than in sites with low scores. However, the diagnostic sensitivity and predictability of these clinical indicators was generally low. Thus, records of plaque, bleeding, suppuration and probing depth do not obviate the need for probing attachment level measurements for identification of sites with probing attachment loss following initial, nonsurgical periodontal therapy.

The purpose of the present study was to determine the diagnostic value of clinical scores of supragingival plaque, bleeding, suppuration and probing depth to predict probing attachment loss in patients on maintenance following nonsurgical periodontal therapy. Non-molar teeth in 39 subjects were monitored and the above scores were repeatedly obtained throughout 5 years of observation following initial treatment. Probing attachment loss between 0-60 months was determined by a combination of linear regression analysis and end-point analysis. The results revealed that all the investigated scores were associated with probing attachment loss. This association was demonstrated by improved diagnostic predictability along with increased frequency or magnitude of the various scores. Also, the diagnostic predictability improved with increase in length of time for recording of the scores. The diagnostic predictability of either accumulated plaque scores and accumulated bleeding scores reached a maximum of about 30%. Residual probing depth greater than or equal to 7 mm reached a predictability of around 50% and increase in probing depth greater than or equal to 1.0 mm reached about 80% after 60 months. Thus, of the clinical scores investigated, increase in probing depth was found to be most valuable in predicting probing attachment loss.
