Objective: To introduce the laparoscopic type C1 hysterectomy based on the anatomic landmark of the uterus deep vein and its branched and to evaluate its feasibility and safety for cervical cancer and its effect to bladder function and to provide some reference to simplify the surgical procedures of laparoscopic type C1 hysterectomy. Methods: The clinicopathologic data of the patients with stage ⅠA2~ⅡB cervical cancer and who underwent the laparoscopic C1 hysterectomy based on anatomic landmark of the uterus deep vein and its branches between March 2010 and December 2015 was retrospectively analysed. Results: A total of 99 patients received laparoscopic type C1 hysterectomy based on the anatomic landmark of the uterus deep vein and its branches, in which 93 patients reserved unilateral or bilateral pelvic autonomic nerve successfully, the other 6 patients were transfered to receive type C2 hysterectomy due to adhesions, bleeding or the low possibility of curative resection. The failure rate of the surgery was 6.1% (6/99). The average age of these 93 patients was 44.4±8.2 years (range 25~61 years) and there was one case of stage ⅠA2, 84 stage ⅠB1, 2 stage ⅠB2, 5 stage ⅡA1 and 1 stage ⅡB. The number of patients with squamous cell carcinoma was 67, adenocarcinoma was 19, adenosquamous carcinoma was 3, small cell neuroendocrine carcinoma was 3 and mixed type was 1. The average operation time was 4.1±0.5 h, the average amount of intraoperative blood loss was 103.8±84.0 ml and the mean number of excisional pelvic lymph nodes was 29.7±8.9. There was no patient with positive parametrial margin, positive vaginal margin or intraoperative ureteral injury. The postoperative catheter extraction time was 20.3±8.4 d. The median follow-up time was 20 months (rang 5~44 months), the long-term bladder dysfunction rate was 8.6% (8/93). The numbers of locally uncontrolled and distantly metastasis case were both one and both patients died. The fatality rate were 2.2% (2/93). The two-year disease-free survival and overall survival rate were 97.6% and 96.2%, respectively. Conclusion: Laparoscopic type C1 hysterectomy based on the anatomic landmark of the uterus deep vein and its branches is a safe and feasible treatment method for cervical cancer and it provides a new approach for simplifying the surgical procedures of laparoscopic type C1 hysterectomy.

CONTEXT: Stroke is a leading cause of death and disability worldwide, being the third leading cause of death in the United States and the second and third most common causes of death in Chinese cities and rural areas, respectively. Evaluation of different rehabilitative modalities appears necessary to optimize treatment. OBJECTIVES: To compare acupuncture and physiotherapy for effectiveness and reliability in treating hemiplegic patients after stroke. DESIGN: The research team designed a multicentered, three-arm, randomized controlled trial. Power calculations revealed a targeted sample size of 310 participants. SETTING: The study took place at seven in-patient hospitals in China. PARTICIPANTS: The research team screened a total of 310 patients. Of that number, 274 completed the study, 15 did not meet the inclusion/exclusion criteria, and 21 dropped out. Adverse events were rare (less than 1%), mild, and temporary. INTERVENTION: The research team randomly divided participants into three groups that all received conventional care as needed-including psychological counseling, standard nursing care, and daily medical evaluation plus (1) acupuncture, (2) physiotherapy, or (3) acupuncture plus physiotherapy. The participants received treatments once a day, 6 days a week for 4 weeks. OUTCOME MEASURES: The research team evaluated all patients at baseline, after 2 weeks, and after 4 weeks using the Fugl-Meyer Assessment of Physical Performance (FMA), a modified Barthel Index (BI), and the Neurologic Defect Scale (NDS). RESULTS: No significant differences existed between the three groups at baseline. Compared to baseline, participants in all groups improved their FMA, BI, and NDS scores by the end of week 2 (P≤.05) and further improved by the end of week 4 (P≤.05) The study found no statistically significant differences in outcomes between the three groups after treatment (P>.05). CONCLUSION: Acupuncture plus conventional care was similar in effectiveness to physiotherapy treatment plus conventional care for poststroke rehabilitation. The study found no synergistic effects for the combination of acupuncture and physiotherapy in addition to conventional care; that combination of treatments was no more effective than either treatment by itself. The effectiveness and lack of adverse events associated with acupuncture in this study suggest that it may represent an additional treatment option for stroke patients.

OBJECTIVE: To explore the regulatory mechanism of lncRNA NORAD on proliferation and invasion of ovarian cancer cells through miR-199a-3p. PATIENTS AND METHODS: Eighty-six ovarian cancer tissues and 86 tissues adjacent to cancer, human ovarian cancer cell lines SKOV3, HO-8910, A2780, OVCAR-3, and human normal ovarian epithelial cell line IOSE80 were collected. MiR-199a-3p-mimics, miR-199a-3p-inhibitor, miR-NC, si-NORAD, Sh-NORAD, and NC were transfected into HO-8910 and A2780 cells, the expression levels of lncRNA NORAD and miR-199a-3p in ovarian cancer tissues and cells were detected by qRT-PCR, and the expression levels of N-cadherin, E-cadherin, and vimentin in cells were detected by WB. Cell Counting Kit-8 (CCK-8), transwell, and cell scratch tests were used to detect proliferation, invasion, and migration of cells, and the relationship between lncRNA NORAD and miR-199a-3p was confirmed by the Dual-Luciferase reporter assay. RESULTS: LncRNA NORAD was highly expressed and miR-199a-3p was lowly expressed in ovarian cancer, and the expression levels of LNCRNARAD and miR-199a-3p were negatively correlated. Cell experiments showed that inhibiting the expression of lncRNA NORAD or up-regulating the expression of miR-199a-3p could inhibit the proliferation, invasion, migration, and EMT of ovarian cancer cells, while up-regulating the expression of lncRNA NORAD or inhibiting the expression of miR-199a-3p could promote their proliferation, invasion, migration, and EMT. Dual-Luciferase reporter assay confirmed that there was a regulatory relationship between lncRNA NORAD and miR-199a-3p. CONCLUSIONS: LncRNA NORAD was highly expressed in ovarian cancer tissues, while silencing lncRNA NORAD expression could inhibit the proliferation, invasion, migration, and EMT of ovarian cancer cells by regulating miR-199a-3p, which might be a new target for the diagnosis and treatment of ovarian cancer.

Dental caries has been an intractable disease in spite of intense dental research. Glucosyltransferase (GTF) enzyme plays the most important role in the development of dental caries. In our previous studies, magnolol, a compound from Magnolia officinalis Rehder et Wilson (Magnoliaceae), was shown to possess a strong anti-GTF activity. The purpose of the present study was to examine the effect of magnolol on the functional domains of GTF for the purpose of defining its anti-GTF activity mechanism. GTF-I which was prepared from Streptococcus milleri transformant KSB8 cells expressing the gtfB gene was used. The results demonstrated magnolol reduced total glucan synthesis, depending on the magnolol concentration. There were no significant differences in Michaelis constant (K(m)) values between the presence and absence of magnolol as determined by Lineweaver-Burk plot, and maximum velocity (V(m)) in the presence of magnolol was lower than that in its absence. Magnolol significantly inhibited both sucrose hydrolysis and glucosyl transfer to glucan by GTF-I. Free glucose in the presence of magnolol was reduced by 33-48% as compared to in its absence, while the quantity of glucan was reduced by 75-82%. These findings suggested that magnolol inhibited both two sequential reaction phases of GTF non-competitively by operating on the glucan-binding domain, but not on the catalytic domain. Magnolol could be a valuable resource for the exploration of novel bioactive compounds in natural products.

Objective. To establish method in analysing EEG complexity as related to brain function state (Cs) and to examine Cs under conditions of taking rest and doing mental arithmetic, to explore their regular changes. Method. EEG of 10 male subjects aged 26-30 under the conditions of taking rest and doing mental arithmetic were recorded. Cs under the two conditions and their changes related to brain function states were analyzed with a special software for Cs analysis. Result. EEG Cs under mental arithmetic changed significantly. According to these changes, the subjects were divided into two groups: Group A, 8 subjects, their EEG Cs under mental arithmetic were significantly higher than Cs under rest at all electrode sites. The most outstanding areas were temporal region and parieto-occipital region. Group B, 2 subjects, their EEG Cs under mental arithmetic were significantly lower than Cs under rest. The most outstanding areas were prefrontal region and temporal region. Conclusion. The result showed that high correlation existed between mental arithmetic and Cs. This method of analysis was useful in studying brain cognitive activities and assessing mental workload.

Objective. The Application of Cs to mental workload evaluation was investigated. Method. EEG of 47 male (aged 23-33) were recorded during rest or doing mental arithmetic, and changes of Cs with relation to mental arithmetic load were analyzed and compared by means of Cs analysis method. Result. Cs distribution was laterally symmetry, higher in temporal and lower in occiput. The changes of Cs over some brain areas were outstanding and stable while doing mental arithmetic: type A, 38 subjects (81%), Cs under mental arithmetic were significantly higher than Cs under rest except over prefrontal, and the most outstanding areas were post-temporal and left parieto-occipital regions; type B, 9 subjects (19%), Cs over post-temporal and parieto-occipital under rest is higher than type A, and Cs under mental arithmetic descend significantly (the most outstanding areas were right central and mid-temporal, then left frontal-central and pre-temporal). Moreover there were some possible relationship among Cs, mental arithmetic score and subjective evaluation of mental workload. Conclusion. There exists high correlation between Cs and mental arithmetic activity, and Cs could well reflect brain function state. It provide a new method and a possible way for objective evaluation of mental workload and studies of brain cognitive activities.

OBJECTIVE: To offer reference of the screening procedure for aircrew and astronaut selection. METHOD: The competitive frequency-fluctuation characteristics of the dominant frequency in the band of 7-12 Hz in EEG and power density spectra of alpha band power-fluctuation were analyzed with a newer system of electroencephalofluctuographic technology in 20 patients (male 13, female 7, aged 17-37 years) with paroxysmal headache. RESULT: During episodes of headache in the affected sites the cumulated dominant fluctuation probability of each frequency components was in the order of 7, 8, 9, 10, 11, 12 Hz from high to low; The relative entropy value was 92.25% +/- 3.41%; Two peaks having at least 25% higher value than those of its neighbors were observed at 1.86 mHz and 4.65 mHz in power density spectra of power-fluctuation. During the intervals of episodes the above-mentioned probability character disappeared; The relative entropy value was decreased to 69.08% +/- 4.16% ; A new peak was found at 0.93 mHz in spectra power-fluctuation, with a reduction in peak value power at 4.65 mHz. CONCLUSION: It not only offered a parameter for pathophysiological and functional evaluation of paroxysmal headache, but also provided reference of the screening procedure for aircrew and astronaut selection.

Objective. To study the characteristics of EEG after transient ischemic attack and to offer reference for screening procedure of aircrew and astronaut selection. Method. The dominant frequency uncertainty of alpha band EEG in 12 pilots (males; age 30 +/- 5) with transient ischemic episodes in middle cerebral artery (MCA) territories and in 20 normal healthy pilots was analyzed with frequency-fluctuation analysis. Result. The dominant probability of the main frequency coinciding with sites affected by transient ischemic attack (TIA) in patient pilots was higher than that in healthy pilots (P<0.01), and the dominant probability ratio logarithmic index I >or= 0 in all patient pilots with normal EEG, but I<0 in all healthy pilots. It was also found that not only I >or= 0, but the second component shifted to lower frequency (8 Hz) in patients with slight focal EEG alterations, i.e. slowing of frequency. The relative entropy values (percentage) were decreased significantly in pilots with TIA as compared with healthy pilots (P<0.05). Conclusion. The dominant frequency uncertainty analysis of alpha band showed clear superiority of computerized evaluation over routine visual assessment for the diagnosis of minor cerebral ischemia. It offers not only a possibility of studying pathophysiological functional parameter, but also the reference for screening procedure in aircrew and astronaut selection.

Objective. To study the influences of short-term simulated weightlessness on human EEG. Method. EEG of 12 male subjects (aged 18-22) was recorded, pre-HDBR (-6 degrees head down bed rest), mid-HDBR (3rd, 4th, 6th day) and post-HDBR (3rd day). Then these EEG recordings were analyzed by power spectral analysis. Result. During HDBR, peak frequency of EEG gradually slowed down (6th day, P<0.05). Alpha-band activities were enhanced (alpha1: P<0.001. Alpha2: 3rd and 6th day, P<0.05; 4th day, P<0.001) and reached maximum on the 4th day. Alpha1/alpha2 ascended (6th day, C3 and C4: p<0.02; F4, P4, T3, T4, T5: P<0.05). Theta-band activities enhanced (P<0.001), and reached maximum on the 3rd day at frontal regions, on the 4th day at parieto-occipital regions. Theta/alpha ascended obviously (6th day, P3, O1: P<0.05). Meanwhile, beta1, activities also enhanced (P<0.001), and reached maximum on the 3rd day at frontal regions. After HDBR, the index that recovered to the pre-HDBR level first was EEG peak frequency. Conclusion. HDBR caused obvious changes of EEG power spectrum characteristic. There exists a potential influence on brain function.

We report the atomic-scale imaging with concurrent transport measurements of the breakdown of individual multiwall carbon nanotubes inside a transmission electron microscope equipped with a piezomanipulator. We found unexpectedly three distinct breakdown sequences: namely, from the outermost wall inward, from the innermost wall outward, and alternatively between the innermost and the outmost walls. Remarkably, a significant amount of current drop was observed when an innermost wall is broken, proving unambiguously that every wall is conducting. Moreover, the breakdown of each wall in any sequence initiates in the middle of the nanotube, not at the contact, proving that the transport is not ballistic.

RESUMO

A system for the automation of radiotherapy treatment planning is described, implemented by a personal computer, which can be employed with both electromagnetic and corpuscular radiation beams, once the characteristics of the beams employed have been recorded. The use of both moving-field and fixed-field techniques is considered. Along the beams, wedge filters and limitation blocks can be interposed. When calculating the radiation dose, both beam obliquity and inhomogeneous tissue structure are to be considered. Dose distribution can be optimized and adapted to the patient's needs.
