RESUMO

The state of immunity was studied in 52 patients with the post-thrombophlebitic syndrome complicated by drug allergy. Levels of immunoglobulins G, A, M were found to be increased considerably in serum protein fractions. Series of medicamentous immunocorrective measures including removal of autoimmunoaggressive focus from altered area and surgical correction of venous blood circulation in damaged extremity was suggested.

Myelography of spongiform substance of the injured os tibia was conducted in 134 patients with chronic arterial insufficiency in III-IV stages. Absence of immature forms of erythrocytes, thrombocytes, increase by 35.2% of quantity of immature forms of granulocytic row, and of lymphocytes by 25.5% was noted. Revision of the main vessels of femoro-popliteo--crural region was done, tibial osteotrepanation and autotransplantation of 7-8 ml of the bone marrow cells suspension into the medial malleolus was performed in the case of impossible reconstructive operation performance. Positive changes of clinical indexes and metabolic activity were observed after the bone marrow autotransplantation operation in soft tissues--by 50% and in the costal one--by 100%. Positive effect of the bone marrow autotransplantation operation was caused by prolonged life capacity of the cells transplanted, which enhances peripheral hemopoiesis and stimulates the bone marrow blood flow. In the III-IIIA stages of ischemia and the perfusion deficiency less than 40% such an intervention proved to be effective in 64% of the patients.

The content of soluble molecules of intercellular adhesion-1 (sICAM-1) in 60 patients with an acute thrombophlebitis (TP) of the lower extremities subcutaneous veins was studied preoperatively and 7 days after the operation using method of a solid phase immunoenzymatic analysis (IEA). The sICAM-1 level before treatment in men was increased at average by 75.8% (P < 0.001), in women--by 81.5% (P < 0.001) in comparison with control group, after treatment it was lowered somewhat. The gender differences of patients for the average sICAM-1 concentration was not noted. In ascending TP sICAM-1 higher level preoperatively and its normalization on 7th day postoperatively were observed in patients, admitted to in surgical unit and operated 3-5 days after the beginning of disease. In patients, to whom conservative treatment was performed initially (anti-inflammatory and thrombolytic therapy), but its efficacy was minimal, the content of cell adhesion molecules preoperatively and before the treatment did not differ. Positive dynamic of index was revealed after the intervention. The results of investigation trust the important role of cell adhesion in pathogenesis of an acute TP of the lower extremities subcutaneous veins and perspective of application of the sICAM-1 method determination in dynamic of the patients treatment to estimate the surgical efficacy intervention.

RESUMO

The results of treatment in 1080 patients with the lower extremities varicose disease (VD) were analyzed. For the subcutaneous veins recurrent varicose (SVV) there were operated 11.2% patients, including, once--76.1%, twice--17.4%, three times--6.5%. In all 146 the operations were performed. Sclerosing therapy of SVV was applied in 21 patients. Basic mistake during performance of the first operative intervention for the VD was insufficient evaluation of character of the blood outflow disorder.

Clinical estimation was performed of the application efficacy of Unguentum cum Olei Hippopheae preparation in treatment of 30 patients with the burn wounds, 7--with the lower extremities ulcer due to postthrombophlebitis syndrome existence, 8--on the diabetic angiopathy background. The epithelisation acceleration by (4.6 +/- 1.4) days, an ulcer square reduction by (2.3 +/- 0.25)% in comparison with a control group were established.
