RESUMO

Analysis of the 5', upstream regions of the Norrie's disease gene (NDP) from 1380bp to +428 (relative to the putative transcription start site) has been undertaken by transfection analysis. Constructs in which a deletion series of sequences from the region were linked to a luciferase reporter gene have been introduced by electroporation into the retinoblastoma cell line, WeriB. Both positive and negative regulatory elements have been identified, and WeriB proved to be a useful cell-line in which to study some aspects of the regulation of this retinal expressed locus. The analysis, therefore, also provides an indication of additional sequences for mutation detection in those patients in which no alterations in the three exons of the gene have been detected.

An analysis of 130 consecutive cadaveric renal transplants has shown the effectiveness of simple surface cooling of organs before transplantation. The primary graft failure rate was 10%, and kidneys could be stored for periods of up to 12 hours including warm ischaemia times of 150 minutes. Forty-eight patients received haemodialysis before transplantation and 78 patients received peritoneal dialysis. There were significant differences in the incidence of tubular necrosis in the two groups: these may have been linked to the differences in fluid and osmotic load presented to the transplants.

A series of 245 patients was operated upon for abdominal aortic aneurysm during the years 1969-77, of whom 162 were elective cases while in 83 the aneurysm was leaking or had ruptured. The hospital mortality for the latter group fell from 77 per cent at the beginning of the study period to 30 per cent at the end. For elective operations, hospital mortality averaged 9.3 per cent, though during the last 2 years there were no deaths in the 51 consecutive cases. Operative methods and postoperative treatment changed during the years of the study, with few excision-replacement grafts and greater use of the inlay technique, most of the latter being simple unbranched Dacron tubes. Early graft infection, the most important and serious surgical complication, caused 9 deaths, an overall incidence of 3.7 per cent, of which 8 were in patients who received no perioperative antibiotics and only 1 among the patients who did receive them. All deaths were in the inlay group. The reasons for this are discussed. Since July 1976 perioperative antibiotic treatment has been routine for all arterial prosthetic graft patients in our hospital.

RESUMO

A review of 50 cases of acute dissection of the aorta managed over a period of almost 15 years showed that patients with either proximal or complex dissections had a better prognosis when managed surgically, whereas medical treatment offered a better change of survival in patients with distal dissection. Angiography was generally safe and reliable, most cases being correctly diagnosed by this means. The majority of patients can be managed conservatively in the initial stages.
