RESUMO

A case of an eighteen month child with ocular melanocytosis is reported. The authors describe clinicopathological features of the affection. Ocular melanocytosis is a neural crest disorder more frequently found in pigmented populations. Differential diagnosis are naevus of Ota, blue sclera disease and scleritis. The risk of malignant degeneration especially in caucasian people explain that patient should be followed-up at regular intervals.

RESUMO

Four years after unexplained left eye visual loss and 15 days after surgical procedure on the other eye, a 93-year-old female developed a pigmented left conjunctival lesion. Ultrasound examination demonstrated this lesion to be in fact an extraocular extension of uveal melanoma. The patient was treated by external radiotherapy after expanded enucleation.

RESUMO

The orbital schwannoma or neurilemoma is a rare tumor. We report herein the single case of a 51-year-old woman presenting an inferior extraconal orbital schwannoma and multiple sclerosis. Surgical treatment with complete excision allowed healing.

RESUMO

We report the case of a 70-year-old patient who presented with Mooren's ulcer in her right eye following extracapsular cataract extraction combined with trabeculectomy. She was successfully treated by a sclerocorneal graft combined with a large conjunctival resection. Clinical and histological features, and therapeutic outcomes are discussed and compared to previously published data.

INTRODUCTION: The introduction of a porous, biocolonizable, orbital implant is difficult after a classic evisceration. We have developed a modification of the method that provides better results. MATERIAL AND METHODS: Four scleral squares pediculized on the right oculomotor muscles were cut. They were sutured two-by-two over the anterior part of the implant. RESULTS: Fifteen patients were operated. There were no complications during a mean follow-up of 12.4 months. DISCUSSION: The soft anterior tissues were protected by the two vascularized scleral layers. The diameter of the implant was 18 mm for three patients, 20 mm for eight patients, and 22 mm for four patients. Operating time was much shorter than with enucleation and covering of the implant with autologous sclera. CONCLUSION: Evisceration with the four-square technique is a safe and quick method, which can be used for most patients, except those with an intraocular neoplasia.

RESUMO

A clinico-pathological observation of one case of nonabsorbable sutures in the superior right eyelid was reported in a 79-year-old patient. The main clinical finding was a painful reddish eyelid. Surgical excision was performed. Histopathological analysis found a non-granulomatous chronic inflammatory reaction surrounding suture fragments. Uncommon in eyelids, such an inflammatory reaction against sutures attests the likely intolerance to the suture material.

RESUMO

A 76-year-old-female patient fell in her garden, inducing a right orbital wound in which no foreign body was primarily found. As pain and diplopia followed the orbital trauma, a CT-scan was performed, disclosing a linear external foreign body in the right orbit. An anterior orbitotomy was performed to remove an 18-mm-long wooden foreign body. Histopathology of the tissue surrounding the foreign body showed an inflammatory fibrous tissue with small granulomas around telluric particles. Improvement after the operation was dramatic with complete disappearance of diplopia the day following surgery. Antibiotics were prescribed and tetanus prophylaxis was performed.

RESUMO

The ICE syndrome includes 3 variants of the same disease, previously believed to be independent entities. All 3 share clinical and histopathological features such as endothelial abnormalities, iris damage, and progressive closure of the iridocorneal angle. Seven cases of ICE syndrome are reported, affecting 6 women and 1 man, with a mean age of 40 years. Corectopia was found in 5 eyes, a uveal ectropion in 2 cases, and other iris changes in 3 cases. The endothelial changes of the cornea were confirmed by a speculum microscopy in 5 cases. There were 5 cases of Chandler syndrome, 1 of Cogan-Reese syndrome, and 1 case of iris essential atrophy in our series. Ocular hypertony was observed in 3 cases, 2 of which needed to be operated. In one patient, a corneal edema was observed.

RESUMO

A 28-year-old male patient had a chalazion on his left eyelid. Increased discomfort led to the discovery of a conjunctival mass under the inner part of his upper eyelid. On clinical examination, a small reddish mass on the palpebral conjunctiva of the upper left eyelid was only visible on the underside of the lid; the mass was diffusely reddish. Additionally, a chalazion was present in the middle of the lower left eyelid. As this condition was a source of discomfort to the patient, the mass was surgically resected. The histological study found a typical pyogenic granuloma, a possible complication in the course of a chalazion.

RESUMO

Pseudo-tumoral presentation of a superficial conjunctival foreign body. A 4-year-old girl was seen in consultation for an apparent conjunctival tumor of the inner canthus of the right eye. With a soft traction, the tumor was detached of the conjunctiva, as it represented in fact a foreign body. Histopathology helped to diagnose that the foreign body was made of textile fiber. Superficial conjunctival foreign bodies may sometimes appear difficult to identify primarily especially in children.

RESUMO

Most conjunctival nevi are found during the two first decades of life. Beyond this period, the diagnosis must be assessed by histopathology. We report the case of a conjunctival nevus recently discovered in a 33-year-old patient who, additionally, was previously treated for herpetic disciform keratitis. The corneal scar decreased vision. A corneal graft and nevus excision were performed simultaneously. Histopathologic study confirmed the diagnosis of nevus and disclosed the corneal scar changes induced by herpes.

RESUMO

A clinico-pathologic case of a black hidrocystoma of the left eyelid margin is reported in a 45-year-old female patient. Clinically, a black nodule was disclosed after eversion of the superior left lid. A surgical full thickness eyelid focal resection was performed to remove the cyst. Histopathology found a cyst covered with an epithelium with a double layer of cells and filled with a brownish granular content, typical of a black hidrocystoma.

RESUMO

There are numerous variants of cutaneous tumors involving the eyelids. Tumors of a different nature may at times be observed simultaneously in the same area of the eyelid. A clinicopathologic case of a 36-year-old male patient with 2 different cutaneous tumors at the nasal part of the left eyelid is reported. One was a nodular tumor on the inner canthus with a pearly appearance; the other had a papillomatous pattern. After surgical removal, the histopathological study of the tumors disclosed a typical basal cell carcinoma and a squamous cell papilloma. Both tumors can be commonly observed on the eyelids and surgical excision cured the patient.

A case of choroidal melanoma associated to two other primitive malignancies is reported. The patient, a 65-year-old woman had an amelanotic choroidal tumor of her left eye. In her clinical history was found a previously treated kidney carcinoma eight years ago. A choroidal metastasis was therefore diagnosed. After radiation therapy, an initial regression was observed. Fifteen months later, the tumor grew again. Enucleation was performed, and histopathology concluded on choroidal malignant melanoma. Three years later, a mammal carcinoma was discovered and treated by mammectomy. Her ophthalmologic and general status remained normal until now. Amelanotic choroidal tumors are difficult to diagnose. Regular follow-up can lead to a change in the diagnosis. The occurrence of multiple cancers is still not fully understood.

RESUMO

The clinicopathologic case of a 69-year-old-female patient with a big caruncular melanoma is reported. Tumor was excised surgically and underwent external conventional radiation therapy with no recurrence two years later. Histopathology endorsed the clinical diagnosis and thickness of the tumor measured 5.5 mm. The bad prognosis of the caruncular location was reinforced by the tumor thickness. A better knowledge of conjunctival melanomas characteristics might allow an earlier diagnosis and a better prognosis as irradiation following surgical treatment appears efficient on unifocal limited melanomas.

RESUMO

A clinico-pathologic case of a 83-year-old female patient with a deeply pigmented inferior lid tumor is reported. The histopathological study of the tumor demonstrated a pigmented basal cell carcinoma. Numerous different tumors may affect the eyelids, melanocytic or not in nature. True melanomas are infrequent compared with the numerous other pigmented tumors, which always need a histological analysis.

RESUMO

A primary acquired melanosis occurred in the right bulbar conjunctiva of a 65 year-old caucasian woman. After four years of steadiness, this lesion gave raise to a malignant conjunctival melanoma. The evolution remained under control three years after the end of the therapeutic procedures. Clinical and histopathological data are presented.

RESUMO

A malignant ciliary body melanoma was treated by proton-beam irradiation. Ring-shaped recurrence with extraocular extension appeared thirty-six months later and required enucleation. Histologic study revealed a predominant epithelioid cell type tumor, flattened in previously treated area. Proton-beam irradiation efficiency depends on clinical evaluation, which remains questioned in ciliary body locations where circumferential extension could be misevaluated.

Chicken pox is a very common infectious disease in children. Its corneal involvement is less serious than with measles, which may lead to blindness in numerous developing countries. However, with occasional cases occur. A case of a 59-year-old male patient whose left cornea was involved during a chicken pox infection at the age of 7 is reported. More recently, the vision of the right eye was normal at 20/20 and reduced to visual perception in the affected left eye. Corneal sensitivity was maintained in the left eye, which, however exhibited a central epithelial defect. A central round opacity of the left corneal stroma was believed to be the scar resulting from a previous disciform keratitis. The left central cornea was thinned and there was neither an anterior chamber flare nor new corneal vessels. This corneal condition required a corneal allograft, performed quickly because of the potential risk of perforation. Histopathological study of the corneal button showed a central corneal thinning with an increase in epithelial thickness. The corneal stroma was disorganized, with irregular collagen bundles. No inflammatory cells could be observed, however. All the histopathological changes observed were those of a corneal scar.
