RESUMO

We evaluated the effect of oral molecular iodine supplementation and shock wave application under three different conditions on human MDA-MB231 cancer cell xenografts. After tumor volume reached 1 cm3, mice were randomly assigned to groups and treated for 3 weeks. The results revealed that high-dose shock wave treatment (150 shock waves at a pressure of 21.7 MPa, SW150/21.7) generated tissue lesions without decreasing tumor growth, canceled the antineoplastic action of iodine and promoted pro-tumor conditions (increased hypoxia-induced factor [HIF] and vascular endothelial growth factor [VEGF]). In contrast, moderate (SW35/21.7) and low (SW35/9.9) doses of shock waves had significant antineoplastic effects and, in combination with iodine supplement, attenuated the aggressiveness of these cells by decreasing expression of the markers of stem cells (CD44 and Sox2) and invasion (HIF and VEGF). These results allow us to propose the combination of shock waves and iodine as a possible adjuvant in breast cancer therapy.

BACKGROUND: Iodine is a crucial component of thyroid hormones, and several reports have shown that iodine per se is implicated in the physiopathology of other organs. METHODS: Innovative ion chromatography detection following a four-step temperature ramp microwave digestion in 25-50 mM nitric acid was developed to measure total iodine in biological fluids and tissue samples from female Sprague-Dawley rats supplemented with 0.05% molecular iodine (I2) or 0.05% potassium iodide (I(-)) in drinking water. RESULTS: The reported method allows the measurement of total iodine with a limit of quantification of 13.7 µg L(-1), recoveries of 96.3-100.3%, and intra- and inter-assay variations, of 3.5% and 7.4% respectively. Analysis of biological fluids showed that after 48 hours, iodine-supplemented animals exhibited significantly higher levels of total iodine in both serum and urine compared with those supplemented with iodide. The half-life of iodine in serum and urine measured over the first 48 h showed similar patterns for both the I2 (7.89 and 7.76 hours) and I(-) (8.27 and 8.90 hours) supplements. Differential uptake patterns were observed in tissues after 6 days of supplements, with I(-) preferentially retained by thyroid, lactating mammary gland, and milk, and a slightly but significantly higher capture of I2 in pituitary, ovary, and virgin mammary gland. CONCLUSIONS: We developed a rapid, selective, and accurate digestion method to process fluid and tissue samples that permits reproducible measurements of total iodine by ion chromatography; iodine or iodide supplement show a similar serum and urine half-life, but organ-specific uptake depends on the chemical form of the iodine supplement.

Objetivo: percibir la experiencia vivida de pacientes hospitalizados con falla cardiaca para elaborar una propuesta educativa que promueva estilos de vida saludable y autocuidado en esta población. Metodología: investigación cualitativa de tipo fenomenológico en el que se abordaron 19 pacientes a través de una entrevista en profundidad, que incluye pacientes hospitalizados en salas generales de una institución de salud de tercer nivel, con diagnóstico de Insuficiencia Cardiaca Congestiva.Resultados: de los 19 pacientes entrevistados, 68% (13) estaba conformado por mujeres, la edad promedio fue de 67.8 años. El factor de riesgo más representativo entre la población fue la Hipertensión Arterial.Se observó que los pacientes referían claramente estar enfermos del corazón y, en gran parte, atribuían su origen a causas sociales y, algunos, a la herencia. Reconocían estilos de vida saludable; sin embargo, no todos lo implementaban. Conclusiones: la falla cardiaca cambia la dinámica de la vida de las personas que la padecen, en tanto que limita las actividades diarias y repercute en su estado emocional y vida familiar.


Objective: collect the experience of patients hospitalized for heart failure in order to develop an educational proposal to promote healthy lifestyles and self-care styles in this population.Methods: phenomenological type, qualitative study addressing 19 patients through an in depth interview, which included patients hospitalized in general wards at a third complexity level, with the diagnosis of congestive heart failure.Results: 19 patients interviewed, 68% (13) were women, average age was of 67.8 years. The most representative risk factor in the population was arterial hypertension. It was noted that patients clearly stated to be heart-diseased and largely attributed its origin, to social causes, and some, to inheritance. They also recognized healthy lifestyles; however healthy lifestyles werenot practiced. Conclusions: Heart failure changes the dynamics of life in people suffering from it, while limiting daily activities and has repercussions on the emotional state and family life.

Este estudio se inicio en Febrero del 2003 y se terminó en Febrero del 2004 con 15 familiares allegados a pacientes críticos cardiovasculares de la Unidad de Terapia Intensiva (UTI), Clínica Bolivariana (CUB). Fue retrospectivo, interpretativo y fenomenológico. Se consideraron los sentimientos de los familiares. La mayoría eran mujeres, casadas y amas de casa, entre 31 y 50 años, de estrato 2 y católicas, con hijos preescolares y adolescentes; su rol: mantenimiento de la motivación, la moral, la conservación física y el orden. Permitió comprender el gran aprecio por el familiar enfermo, los pacientes manifestaron lo poco que fuerosn tenidos en cuenta y expresaban espontáneamente temor y tristeza. Los sentimientos se reviven a pesar del tiempo. Sus expectativas fueron de esperanza y percepción de muerte. Había aflicción, angustia, fe o esperanza en Dios, fortaleza, un gran desconocimiento de la enfermedad del familiar, algunos hablaban con propiedad, otros daban datos de lo que oían y muchos expresaban preocupación, ansiedad y, en especial, la preocupación por la gran responsabilidad de tomar decisiones. La mayoría estaban satisfechas por la forma como cuidaban a su familiar enfermo; expresaron lo bien que se sintieron por el espacio brindado para comentar sus experiencias. Se propone la "alfabetización emocional" para el personal que participa en el proceso y su intervención con los familiares.
