OBJECTIVE: The aim of this study was to investigate clinical aspects and predisposing factors for alveolar bone graft complications in persons born with oral clefts. DESIGN: A total of 105 patients, aged 7 to 57 years old, who received alveolar bone graft at the Cranio-maxillofacial Surgery Center in the National Institute of Traumatology and Orthopedics (INTO), Rio de Janeiro (RJ) from 2009 to 2014 were selected. Data were collected concerning the type of oral cleft, family history of cleft, medical and dental exam, donor area, type of graft material, repaired surgical treatment done, and postoperative follow-up examinations. RESULTS: Postoperative complications developed in 31 patients (32.9%). The mean age at grafting was 16.79 years for the group without complications (n = 63) and 20.13 years for the group with postoperative complications (n = 31). There was a positive association between age and type of graft and cases with alveolar bone graft complications. Patients aged 12 years or more had a four times more chance of developing alveolar bone graft complications. Particulate bone graft from iliac crest demonstrated better results compared with block graft or mixed graft. CONCLUSION: Patients with cleft lip and palate who were 12 years or older had a greater chance of developing complications after grafting the alveolar bone. Furthermore, particulate alveolar graft from iliac crest had significantly better outcomes.

Bone morphogenetic proteins (BMPs) play an important role during the initial process of enamel development and therefore may play a role in caries susceptibility. The purpose of this study was to evaluate the association between the polymorphisms in the BMP2, BMP4 and BMP7 genes and their association with caries experience and primary enamel microhardness characteristics. DNA from buccal cells as well as clinical and demographic information from 1,731 subjects from three different data sets from Brazil were included. Polymorphisms in BMP2, BMP4 and BMP7 were analyzed by real-time polymerase chain reaction from genomic DNA. Association between caries experience, genotype, and allele distribution in both cohorts was evaluated using χ(2) and logistic regression analyses. In the family-based set, the association between caries experience and alleles was tested using the transmission disequilibrium test. In the Rio de Janeiro cohort, microhardness data on 108 exfoliated primary teeth before and after demineralization and remineralization challenges was included. Associations between microhardness values and genotype and allele distribution were evaluated using χ(2) and logistic regression analyses. Differences between caries experience and some risk factors were statistically significant. In the cohort from Nova Friburgo, BMP2 was associated with caries experience in primary dentition during logistic regression analysis (p = 0.023; OR = 2.58; 95% CI 1.13-5.86). There was no association between genotype and allele distribution for BMP polymorphisms and primary enamel microhardness alterations. Our result suggests that BMP2 may be involved in caries experience in primary dentition from a Nova Friburgo cohort.

OBJECTIVE: To undertake a systematic review to assess if HIV-infected children and adolescents have an increased dental caries experience. METHODS: A search of MEDLINE, BIREME, EMBASE, GOOGLE SCHOLAR, SIGLE (Grey Literature) and reference lists of included studies was carried out. To be eligible the studies had to present HIV-infected and non-infected children/adolescents between 0-18 years old. To assess the methodological quality, the studies were categorized in scores from 'A' to 'C'. To perform a meta-analysis a random effect model was used with 95% confidence intervals and two distinct sub-group analyses were carried out in terms of caries progression: data for cavitated and non-cavitated lesions (sub-group 1) and data only for cavitated lesions (sub-group 2). RESULTS: Five studies fulfilled the selection criteria. Four studies (two ranked A and two B in the quality assessment) revealed higher caries scores in primary teeth in the HIV-infected patients with mean dmft/dmfs scores of 3.8-4.1/7.8-11.0 compared to the control group 1.5-2.4/3.4-5.1. No differences in caries index were found for permanent dentition. The meta-analysis excluded caries data of permanent teeth and showed a significant association between caries experience in primary dentition and HIV infection considering cavitated and non-cavitated lesions (OR = 2.33, 95% CI = 1.48-3.68) or only cavitated lesions (OR = 2.98, 95% CI = 1.59-5.59). CONCLUSION: Evidence exists that suggests HIV-infected children/adolescents have an increased caries experience in primary dentition.

AIM: To evaluate the association of the polymorphisms in the TGFB3 gene (rs2268626), and the BMP4 gene (rs17563) with dental caries in two different groups (noncleft group and oral cleft group) from a cleft center located at Rio de Janeiro, Brazil. MATERIALS AND METHODS: A total of 486 unrelated children and adolescents with or without caries were evaluated using a cohort design. Data on oral health habits was obtained through a questionnaire and caries data was collected by clinical examination. Genotyping of the selected polymorphisms for TGFB3 and BMP4 were carried out by real-time PCR using the TaqMan assay method from a genomic DNA isolated from buccal epithelial cells of all children and adolescents. RESULTS: No association was found between BMP4 polymorphism and caries among individuals from both groups. For TGFB3 polymorphism, significant differences were observed for allele and genotype frequencies between caries free and caries affected individuals in oral cleft group (p = 0.013 and 0.006 for allele and genotype frequencies respectively). CONCLUSION: Our findings provide evidence suggesting that TGFB3 may be involved in caries susceptibility in oral cleft group. CLINICAL SIGNIFICANCE: In the future, the possibility of identifying genes related to caries susceptibility can lead to counseling of the individual that carries gene alterations, with the aim of working on preventive measures for caries as well as bioengineering treatments.

Objective : Considering that oral clefts and tooth dimensions may be part of the same phenotypic spectrum, the aim of this study was to investigate tooth dimensions in permanent dentition and dental malformations, including tooth size discrepancies, of subjects born with clefts compared with individuals without clefts. Design : Cross-sectional study. Participants : The cleft group was composed of 66 subjects, and the noncleft group consisted of 66 healthy unrelated subjects. Main Outcome Measures : The mesio-distal and buccal-lingual crown diameter of fully erupted permanent teeth outside the cleft area was measured using a digital caliber. Clinical records and radiographs were used to evaluate the type of clefts and dental anomalies. Results : The lower second premolar was significantly reduced in the CLP and CP groups. The upper lateral incisor was found to be significantly smaller in the CP group, only for mesio-distal dimensions (P < .05). Dental agenesis was found in eight (12%) cleft subjects and supernumerary teeth in two (3%). Conclusions : Subjects born with oral clefts presented size reduction in specific dental groups.

Objective : To evaluate the association of the polymorphisms in the TGFB3 gene (rs2268626) and the BMP4 gene (rs17563) with nonsyndromic oral clefts. Design : A hospital-based case-control study was conducted with nonsyndromic oral clefts cases and unaffected controls. Cleft type and tooth agenesis data were collected by clinical examination and confirmed through medical records. Risk factors were obtained through a questionnaire. Genotyping of the selected polymorphisms for TGFB3 and BMP4 were carried out by real-time polymerase chain reaction using the Taqman assay method from a genomic DNA isolated from buccal epithelial cells of all individuals. Setting : The case group was ascertained through a public reference hospital of oral cleft rehabilitation in Rio de Janeiro, Brazil. The noncleft group consisted of unrelated subjects, with no history of oral cleft in the family, recruited at the Dental Clinic at the Federal University of Rio de Janeiro, Brazil. Participants : A total of 833 unrelated individuals (450 control individuals and 383 cases with nonsyndromic oral clefts). Results : No significant association in relation to genotype or allele distributions for TGFB3 polymorphism was found between oral cleft subgroups and the control group. For BMP4, there were significant differences in the genotype frequencies between the oral cleft and control groups (P = .0007). Regarding cleft type, there were statistically significant differences between the cleft lip and control groups (P = .00009). Conclusion : Our findings provide preliminary evidence suggesting that the risk of nonsyndromic oral clefts may be influenced by variation in the BMP4 gene.

OBJECTIVE: To evaluate the association of the polymorphisms in the TGFB3 gene (rs2268626) and the BMP4 gene (rs17563) with isolated human tooth agenesis. MATERIALS AND METHODS: One hundred and seventy-five unrelated individuals (125 control individuals without tooth agenesis and 50 cases with tooth agenesis) were evaluated using a case-control design. The participants of the study were recruited through the Dental School of the Federal University of Rio de Janeiro, Brazil. Genotyping of the selected polymorphisms for TGFB3 (48 individuals with tooth agenesis and 125 control cases) and BMP4 (46 individuals with tooth agenesis and 88 control cases) were carried out by real-time PCR using the Taqman assay method from a genomic DNA isolated from buccal epithelial cells of all individuals. RESULTS: Significant statistical differences were found for genotype frequencies between tooth agenesis and TGFB3 control samples (p = 0.026). In addition, significant differences were also observed for allele and genotype frequencies between unilateral tooth agenesis and TGFB3 control samples (p = 0.014 and 0.004 for allele and genotype frequencies, respectively). For BMP4, genotype distribution had a statistically significant difference between groups (p = 0.047). The GG genotype of BMP4 was more frequent in individuals with three or more missing teeth than in the control group (p < 0.0001). CONCLUSIONS: These results indicate that polymorphisms in the TGFB3 gene and in BMP4 genes contribute to tooth agenesis. Nonetheless, the extents to which this polymorphism may actually contribute to the tooth agenesis status should be clarified.

OBJECTIVE: To assess whether individuals born with nonsyndromic oral clefts display a higher frequency of dental anomalies. DESIGN: A search of MEDLINE, BIREME, OVID ALL EMB Reviews, and The Cochrane Library was conducted. The methodologic quality of the papers selected was assessed and scored. Papers reporting observational controlled studies of nonsyndromic forms of oral cleft matched for dental anomalies in primary and/or permanent teeth were included without language restrictions. Eligible studies were scored as "A"-low risk of bias, "B"-moderate risk of bias, or "C"-high risk of bias and poor quality. Fixed and random effects models were used to aggregate individual odds ratios (OR) and to derive pooled estimates and 95% confidence intervals. RESULTS: Six studies fulfilled our selection criteria and were included in the meta-analysis. Three distinct subgroup analyses were carried out in terms of dental anomalies. In the tooth agenesis meta-analysis, a random effects model was used because of heterogeneity and showed a significant association between tooth agenesis and oral clefts (OR  =  12.31; 95% confidence interval [CI]  =  3.75 to 40.36). In the remaining analyses, the fixed effects model revealed a positive association between supernumerary (OR  =  4.99; 95% CI, 2.58 to 9.64) and crown morphologic abnormalities (OR  =  5.69; 95% CI, 3.96 to 8.19) with oral clefts. Most included studies were of low to moderate quality. CONCLUSION: Although general limitations in study design were observed, the evidence suggests that a higher number of dental anomalies in the permanent dentition are noted in individuals born with oral clefts.

BACKGROUND: The effect of smear layer (SL) removal on primary tooth pulpectomy outcome has not been well elucidated. AIM: To determine the effect of SL removal on primary tooth pulpectomy outcome. METHODS: This is a double-blind, randomized, and controlled clinical trial. Forty-eight patients were randomly divided into SL removal (G1 = 40 teeth) or smear layer nonremoval (G2 = 42 teeth) groups. Following the chemomechanical preparation with K-files and 2.5% sodium hypochlorite (NaOCl), teeth were irrigated with either 6% citric acid and 0.9% physiologic solution (G1) or only 0.9% physiologic solution (G2). Camphorated paramonochlorophenol was used as intracanal medication. At the second appointment, 1 week after, root canals were filled with zinc oxide-eugenol paste. Clinical and radiographical baseline criteria were stipulated equally for both groups. RESULTS: The success rate (G1 = 91.2%; G2 = 70.0%) was statistically different (P = 0.04) between the groups. In G2, the outcome was affected significantly by pulpal necrosis (P = 0.02), pre-operatory symptoms (P = 0.02), and periapical/inter-radicular radiolucency (P = 0.04). CONCLUSION: The pulpectomy outcome was improved by smear layer removal. The outcome for teeth with pulpal necrosis, pre-operatory symptoms, or periapical/inter-radicular radiolucency was significantly improved by removal of the smear layer.

PURPOSE: The aim of this study was to evaluate, via clinical and radiographic assessment, pulpectomy outcomes performed on primary anterior teeth both with and without a citric acid solution to enhance smear layer removal. METHODS: Patients with a matched pair of primary incisors (split-mouth design) with irreversible pulp changes were selected. A total of 36 teeth (18 children) received pulpectomies and were followed for 36 months. Pulpectomies were performed using sodium hypochlorite and saline solution as canal irrigants; during the last irrigation, the tooth was randomly selected to receive ( Group 1) or not receive (Group 2) the citric acid solution for smear layer removal. The roots were filled with ZOE paste. RESULTS: Overall pulpectomy success was 90.6%. Cases with smear layer removal were successful 82.3% of the time; those without smear layer removal, 88.2%, and there were no statistical differences (P=1.00). CONCLUSION: Pulpectomy with smear layer removal in primary incisors exhibited, after 36 months, a high success rate; however, comparable results were obtained when the smear layer was not removed.

Pulpectomy in primary teeth is a common technique that preserves teeth in the oral environment and maintains or recovers periapical tissues to a healthy condition. This article describes the ectopic eruption of permanent incisors whose primary predecessors underwent pulpectomy using ZOE filler paste. In a group of 135 teeth that received pulpectomy therapy due to caries, 10 primary maxillary incisors had overretention and were followed for at least 3.5 years (mean time of 4.2 years), both clinically and radiographically, until the permanent teeth erupted. The proposed treatment included extraction of the overretained primary incisors based on permanent successor eruption chronology and contralateral eruption. Seven permanent teeth erupted ectopically. Autocorrection of the permanent tooth positions was observed in five cases. It can be concluded that periodic clinical and radiographic assessments are essential to verify radicular and filling paste resorptions and to avoid overretention and any subsequent malocclusion.

Preschool children are frequently affected by traumatic dental injuries, most of them during the first three years of life. Due to the resiliency of the alveolar bone surrounding the primary teeth, most of these injuries fit into tooth luxations category. In this paper two different cases of early loss of primary incisors resulting from luxations are described. Removable esthetic space maintainers were placed using the natural crown of the traumatized teeth and the children were evaluated over a 12 month period. The treatment given could be considered an excellent rehabilitation option since it restores the esthetics and masticatory function, allows speech development, benefits oral hygiene and prevents the establishment of tongue habits and malocclusions.

Recent studies suggested that genetics contribute to differences in dental fluorosis (DF) susceptibility among individuals having the same environmental exposure. This study evaluated if MMP2, MMP9 and MMP20 are expressed during enamel development and assessed the association between polymorphisms in these genes with DF. Mice susceptible and resistant to DF were used to evaluate if MMPs were candidate genes for DF. The animals received fluoride and their enamels were used for immunohistochemistry. Additionally, 481 subjects from a city with fluoridation of public water supplies were recruited. Genotyping was performed using real time PCR. Allele/genotype frequencies were compared between groups. MMP2, MMP9 and MMP20 immunostaining was detected in both animal groups. DF was observed in 22.4% of the subjects. A borderline association was observed in MMP2 (rs243865), MMP9 (rs17576) and in MMP20 (rs1784418) (p = 0.06, p = 0.08 and p = 0.06 respectively). Briefly, MMPs were expressed during enamel maturation and genetic polymorphisms were not associated with DF.

An aesthetic smile is usually associated with healthy appearance and success in many areas of life. Currently, individuals access the internet and social media in search of health information. The aim of this study was to analyse the quality and reliability of information in videos available on the YouTubeTM platform about ultra-thin ceramic laminates. Methods: YouTubeTM was searched using the therms "dental ceramic laminates" and "dental contact lens" for videos uploaded on ultra-thin ceramic laminates. The first 100 videos in Portuguese were selected and rated for quality and reliability. The content and source of videos were also noted. The quality of the videos was determined by evaluating the presence or absence of reliable information and distributed into seven domains. To compare the distributions of quantitative variables between the classifications of source, content, and quality, the Kruskal-Wallis and Dunn's post-hoc tests were performed. Results: Seventy-two videos were included for qualitative analysis. Regarding quality rating, most videos were rated with poor quality. The reliability of the evaluated videos was low. Conclusion: YouTubeTM showed a large number of videos with many views on the topic, but it does not contribute with good quality and reliable information to the population

The aim of this study was to measure the satisfaction of patients treated in a dental hospital using a validated questionnaire on patient-dentist communication and investigate factors associated with satisfaction. Patients in the dental clinic answered 2 questionnaires: Dental Patient Feedback on Consultation (DPFC) and another regarding their oral health perceptions using a visual analog scale (VAS) and sociodemographic backgrounds. The correlation between patient's satisfaction and other numerical variables was determined using Spearman's test. The KruskalWallis and Mann-Whitney tests were used to compare the patient's satisfaction scores and the categories of the independent variables (p≤0.05). The median DPFC score was 2.9. All participants were very satisfied considering the active dentist listening (Q3) and the dental treatment advices (Q16). The lowest score was observed about family habits and medical history in oral health (Q9:2.0). With respect to their oral health perceptions, the majority of the participants demonstrated satisfaction with their chewing, smile and dental aesthetics. Being younger than 40 years influenced the satisfaction with oral health satisfaction when the VAS score was analyzed. The skin colour (p=0.001 and p=0.005) and the marital status (p=0.001 and p=0.006) of the participants also demonstrated a statistically positive signiticant correlation between DPFC and VAS scores, respectively. There was a high level of patient's satisfaction with the ability of the dentist to communicate in the dental hospital analyzed. Age, color skin, and the marital status of patients interfered in the satisfaction about patient-dentist communication and their oral health perception.(AU)


O objetivo deste estudo foi mensurar a safisfação de pacientes tratados em um hospital utilizando um questionário validado baseado na comunicação dentista-paciente e investigar os fatores associados a esta satisfação. Os pacientes da clínica responderam 2 questionários: Dental Patient Feedback on Consultation (DPFC) e outro a respeito das percepções relacionadas a saúde utilizando uma Escala Visual Analógica (EVA) e dados sociodemográficos. A correlação entre a satisfação dos pacientes e outras variáveis numéricas foi determinada através do teste de Spearman. Os testes de Kruskal-Wallis e Mann-Whitney foram usados para os escores de satisfação dos pacientes e as variáveis categóricas independents (p≤0,05). O escore mediano do DPFC foi de 2,9. Todos os participantes estavam muito satisfeitos considerando a escuta ativa do dentista (Q3) e as recomendações sobre o tratamento dentário (Q16). O escore mais baixo foi relacionado aos hábitos familiares e história médica relacionada a saúde bucal (Q9:2.0). Em relação as percepções a saúde bucal, a maioria dos participantes demonstrou satisfação com a mastigação, sorrisco e estética dental. Ter menos de 40 anos influenciou a satisfação com a saúde bucal quando a escala EVA foi utilizada. A cor da pele (p=0,001 e p=0,005) e a relação conjugal (p=0,001 e p=0,006) também demonstraram uma significância estatística positiva entre o DPFC e a escala EVA, respectivamente. Foi observado um alto nível de satisfação dos pacientes em relação a habilidade de comunicação do dentista no hospital analisado. Idade, cor da pele e a relação conjugal dos pacientes interferiu na satisfação relacionada a comunicação do dentista-paciente e a percepção de saúde bucal.(AU)

RESUMO

In recent important days, social media have been communication tools for the new Guidelines for the Dental Clinic that are being used and disseminated by the Ministry of Health (MS) through the Secretariat of Primary Health Care (APS). This year, more precisely in May, the Guideline for clinical dental practice in Primary Health Care: treatment in pregnant women was published. The document was previously made available, still in 2021, in public consultation, and various sectors of society and the community were aware of their considerations and suggestions. This is a relevant strategy for validating the material, as it provides opportunities for the professional health teams to look at the use of the future Guideline. For those who are unaware, the General Coordination of Oral Health of the Ministry of Health, in partnership with the GODeC (Global Observatory for Dental Care) initiative of the Federal University of Pelotas, will make available in the year 2022, 22 clinical guidelines for the qualification of the caregiver. in health oral supply in Primary Health Care by the Unified Health System (SUS)
