RESUMO

Medical personnel who perform dermabrasions are exposed to airborne blood and tissue fragments. The safety or hazards of exposure to such aerosols have not been adequately studied. Using scanning electron microscopy, the air density and size distribution of particles produced during dermabrasion were analyzed. Such particles are of sufficient size to allow for access to and retention by mucosal and pulmonary surfaces. Transmission electron microscopy reveals amorphous particles without discernible cell membranes. Commonly used personnel protection standards do not prevent respiration of these particulates. Mathematical estimation of particle size production allows extrapolation of these data to other rotary instrument applications.

RESUMO

The subcutaneous island pedicle flap is a useful and reliably successful means of closing small- to medium-sized cutaneous excisional defects. It is especially useful in areas where primary closure could result in distortion of critical features. The technique is conceptually straightforward and offers advantages over skin grafting and transposition flaps. We describe our experience with 60 consecutive, successful subcutaneous island pedicle flaps.

Sun exposure is linked to visible signs of skin aging, skin cancer, photodermatoses, exacerbation of systemic disease and photoallergic, as well as phototoxic, drug eruptions. Sunscreens vary considerably in their ability to protect patients from exposure to ultraviolet light and its effects. Inappropriate choice and use of sunscreen products can lead to worse problems than using no sunscreen at all. Controversies about sunscreen include adequate level of sun protection factor, appropriate age of users, and whether use of sunscreen products can prevent skin cancer. Instructing patients in how to select and use sunscreen can help prevent or mitigate a variety of cutaneous and systemic diseases.

It has long been held that embryologic fusion planes influence the spread of skin cancer. Embryologic fusion planes have been implicated in the depth of invasion, horizontal spread, and recurrence of cutaneous carcinoma. However, these structures have never been studied in detail. A review of the surgical literature reveals considerable confusion regarding the exact nature, location, and tumor interactions of these fusion planes. We review the gross and microscopic development of sites of embryologic fusion. We examine histologic sections through fusion sites in normally developed adult and fetal fresh cadaver specimens. Our studies, supported by our review of developmental anatomy, indicate that fusion planes do not persist as identifiable anatomic structures that would influence tumor spread.

RESUMO

Pemphigus is an autoimmune bullous dermatosis in which intercellular antibodies are associated with loss of intercellular cohesion. Pemphigus vegetans, a rare variant of pemphigus, is manifested by vegetating plaques, usually in intertriginous areas of patients who probably have an increased resistance to the disease. We report a case of chronic balanitis owing to pemphigus vegetans, and discuss proposed pathogenic mechanisms and treatment.
