RESUMO

The care of migrant patients includes initial screening and lifelong monitoring, highlighting the importance of preventing and tracking chronic, communicable and non-communicable diseases. The prevalence of hypertension, diabetes mellitus, dyslipidemia, and obesity varies by ethnicity, influenced by genetic factors, lifestyle, and socio-economic status. Preventive measures, health promotion, and risk factor identification are crucial. Chronic communicable diseases may manifest years after transmission, underscoring the necessity of primary care screening, especially for populations from endemic or high-risk areas. Imported skin lesions are a common reason for consultation among migrant and traveller patients. Their ethiology is varied, ranging from common conditions such as scabies, mycoses, and urticaria to tropical dermatoses like filariasis and leprosy.

Human schistosomiasis is the parasitic disease with the highest morbidity and mortality worldwide after malaria. It is endemic in more than 78 tropical and subtropical countries, especially in sub-Saharan Africa, and it is estimated that 236 million people are infected. It can cause serious health complications at the genitourinary and hepatosplenic level, leading to the death of 300,000 people each year. The number of imported cases in Western countries has increased in recent years due to the arrival of a significant number of migrants from endemic regions and a growing number of travelers who have visited them. On the other hand, outbreaks of autochthonous transmission have recently been reported in Corsica (France) and Almería (Spain). For all these reasons, the European health authorities have recommended serological screening for the disease in all migrants from endemic areas who have been living in Europe for less than 5 years. Since Primary Care is usually the first point of contact for these people with the Health System, doctors must know the main aspects of the disease, and be provided with the necessary means for its diagnosis and treatment. This document has been prepared by professionals belonging to five scientific societies of Primary Care (SEMFyC, SEMG, SEMERGEN), Pediatrics (SEIP) and Tropical Medicine and International Health (SEMTSI), in order to establish clear recommendations for the diagnosis and management of schistosomiasis in Primary Care.

INTRODUCTION AND OBJECTIVES: Little is known regarding the relevance of racial/ethnic background to the risk for COVID-19 infection, particularly in Europe. We evaluated the risk of COVID-19 among migrants from different areas of the world within the context of universal free access to medical care. MATERIAL AND METHODS: We conducted a population-based cohort analysis of the cumulative incidence of PCR-confirmed COVID-19 among adult residents of Alcorcon (Spain) in the first wave of the disease up to April 25, 2020. RESULTS: The crude cumulative incidence among migrants (n=20,419) was higher than among Spaniards (n=131,599): 8.81 and 6.51 and per 1,000 inhabitants, respectively (p<.001), but differed by region of origin. As per a negative binomial regression adjusted for age and sex, relative risk (RR) for COVID-19 for individuals from Europe, Asia, or North Africa was not significantly different from Spaniards. In contrast, a markedly increased risk was found for people from Sub-Saharan Africa (RR 3.66, 95% confidence interval (CI) 1.42-9.41, p=.007), the Caribbean (RR 6.35, 95% CI 3.83-10.55, p<.001), and Latin America (RR 6.92, 95% CI 4.49-10.67, p<.001). CONCLUSIONS: Migrants from Sub-Saharan Africa, the Caribbean, and Latin America exhibited increased risk for COVID-19 as compared to Spaniards or migrants from Europe, North Africa, or Asia. Our data suggest that the ethnic background may play a role in risk for COVID-19. Migrants from some areas of the world may merit closer attention for both clinical and epidemiological reasons.

Immigrant populations, especially those from endemic countries, living in the European Union (EU) suffer a disproportionate burden of HIV, delayed diagnosis and poorer access to antiretroviral treatment. While International Organisations are developing recommendations aimed at increasing the uptake of HIV testing, the feasibility and real outcomes of these measures remain unexplored. The aim of this review was, firstly to identify the recommendations of the main International Organisations (IO) on HIV testing in immigrants. Secondly, to describe the challenges for implementing and expanding HIV testing and counselling interventions targeting immigrants by interviewing key informants. The importance of HIV testing in immigrants is discussed, along with the appropriateness of universal HIV testing approaches vs most at risk targeted approaches. Also addressed is, pre- and post-HIV test counselling characteristics and community initiatives suitable to reach this population and, finally the legal issues regarding access to treatment for illegal immigrants.

A recent discussion debates the extent of human in-migration around protected areas (PAs) in the tropics. One proposed argument is that rural migrants move to bordering areas to access conservation outreach benefits. A counter proposal maintains that PAs have largely negative effects on local populations and that outreach initiatives even if successful present insufficient benefits to drive in-migration. Using data from Tanzania, we examined merits of statistical tests and spatial methods used previously to evaluate migration near PAs and applied hierarchical modeling with appropriate controls for demographic and geographic factors to advance the debate. Areas bordering national parks in Tanzania did not have elevated rates of in-migration. Low baseline population density and high vegetation productivity with low interannual variation rather than conservation outreach explained observed migration patterns. More generally we argue that to produce results of conservation policy significance, analyses must be conducted at appropriate scales, and we caution against use of demographic data without appropriate controls when drawing conclusions about migration dynamics.

Every year, millions of migratory shorebirds fly through the East Asian-Australasian Flyway between their arctic breeding grounds and Australasia. This flyway includes numerous coastal wetlands in Asia and the Pacific that are used as stopover sites where birds rest and feed. Loss of a few important stopover sites through sea-level rise (SLR) could cause sudden population declines. We formulated and solved mathematically the problem of how to identify the most important stopover sites to minimize losses of bird populations across flyways by conserving land that facilitates upshore shifts of tidal flats in response to SLR. To guide conservation investment that minimizes losses of migratory bird populations during migration, we developed a spatially explicit flyway model coupled with a maximum flow algorithm. Migratory routes of 10 shorebird taxa were modeled in a graph theoretic framework by representing clusters of important wetlands as nodes and the number of birds flying between 2 nodes as edges. We also evaluated several resource allocation algorithms that required only partial information on flyway connectivity (node strategy, based on the impacts of SLR at nodes; habitat strategy, based on habitat change at sites; population strategy, based on population change at sites; and random investment). The resource allocation algorithms based on flyway information performed on average 15% better than simpler allocations based on patterns of habitat loss or local bird counts. The Yellow Sea region stood out as the most important priority for effective conservation of migratory shorebirds, but investment in this area alone will not ensure the persistence of species across the flyway. The spatial distribution of conservation investments differed enormously according to the severity of SLR and whether information about flyway connectivity was used to guide the prioritizations. With the rapid ongoing loss of coastal wetlands globally, our method provides insight into efficient conservation planning for migratory species.

OBJECTIVES: To know the needs and lack of information on health issues of Maghrebi women. SUBJECTS, MATERIAL AND METHOD: Design: Qualitative study technique nominal group (TGN). SUBJECTS: 14 North African women from Association for Solidarity and Rapprochement of Cultures (ASAC) from the rural clinic of La Aljorra who knew and understood Spanish, older age, and lived in the town for more than 3 years. Material and method TGN asks: «What daily health issues for you and your family worry you the most and do you need answers?¼ Individual reflection and list that were discussed among them, joining the group consensus. Subsequently, they were prioritized individually and a definitive list of their prioritized health needs was made. RESULTS: Thirty-six percent of the ASAC associates attended. At first, 17 topics were written on health needs for which they wanted a response. The final topics chosen and grouped by them were 6: 1) Contraceptive methods; 2) Relationship problems; 3) Thyroid; 4) How to cure the sciatic nerve?; 5) Anemia and 6) Stress. 70% related to the area of sexuality. CONCLUSIONS: 1) Qualitative research is an adequate consensus method to obtain information on little-known topics and directly from the person involved; 2) The Moroccan migrant woman considers information about sexuality and couple relationships very important; 3) Another pathology unknown to them and emerging in the area is thyroid; 4) Stress and its repercussions; 5) Sciatica and its consequences are also important when being agricultural workers.

OBJECTIVE: To characterise social determinants of health, mental health problems and potentially problematic symptoms in the adult population displaced by internal armed conflict in Colombia. METHODS: Cross-sectional descriptive study with a random sample of 98 adults forcefully displaced to Soacha, Colombia, due to internal armed conflict. The Self Report Questionnaire to detect potentially problematic mental health problems and symptoms, and a structured questionnaire on social determinants of health were applied. RESULTS: The median age was 38 [interquartile range, 28-46] years, and women predominated (69.39%). The median time since displacement was 36 [16-48] months, and time since settlement in Soacha, 48 [5-48] months. 86.32% survived on less than the minimum wage per month and 93.87% did not have an employment contract. 42.86% and 7.14% reported being owners of their homes before and after displacement, respectively. Upon arriving in Soacha, 79.60% went to primary support networks and 3% to institutions. Before displacement, 16.33% lacked health insurance and 27.55% afterwards. Regarding mental health problems; there were possible depressive or anxious disorders in 57.29%; possible psychosis in 36.73%; and potentially problematic symptoms in 91.66%, being more prevalent and serious in women (p = 0.0025). CONCLUSIONS: A deterioration in living conditions and a higher prevalence of potentially problematic mental health problems and symptoms was reported in displaced adult populations settled in Soacha compared to other regions of the country. Analyses with complementary perspectives are required to evaluate these differences.

Background: Filipino migrant workers in Macao are vulnerable to posttraumatic stress disorder (PTSD) symptoms and addictive behaviours due to trauma histories, postmigration stressors, and access to alcohol and gambling venues. While PTSD addiction comorbidity is well-established in the existing literature, such research among migrant workers is lacking.Objective: The current study investigated differential relations between PTSD symptoms and addictive behaviours in a polytrauma exposed sample of Filipino domestic workers in Macao (SAR), China.Methods: Data were collected from 1375 Filipino migrant workers; data from a subsample of 1200 participants who reported an index traumatic event and PTSD symptoms were used in the analyses. Participants responded to the PTSD Checklist for DSM-5, gambling disorder symptoms checklist from DSM-5, and The Alcohol Use Disorders Identification Test. We estimated a regularized partial correlation network structure of PTSD symptoms and addictive behaviours employing graphical LASSO and extended Bayesian information criterion.Results: PTSD symptoms of arousal and negative emotions had bridge connections with gambling disorder symptoms; while PTSD symptoms of arousal, restricted affect, negative emotions, and emotional reactivity had bridge connections with alcohol misuse.Conclusions: PTSD's arousal and negative emotion symptoms were common in the networks of PTSD and addictive behaviours, while PTSD's restricted affect and emotional reactivity symptoms were unique to the network of PTSD and alcohol misuse. Treatment of the comorbidity of PTSD and addictive behaviours may yield optimal effects when tailored to these symptoms.


A network analytic study was conducted to explore patterns of comorbidity among international migrants.Post-traumatic stress disorder symptoms are associated with addiction symptoms among female Filipino migrant workers in Macau.Negative emotions symptoms of post-traumatic stress disorder are associated with both gambling disorder and alcohol misuse.Restricted affect and emotional reactivity are associated with alcohol misuse only.

BACKGROUND: Migrants, a population vulnerable to communicable diseases, face multiple barriers in access to immunization programs. Individual studies suggest that they suffer immunization inequity compared to non-migrants, but the gap in vaccination has not been quantified. This systematic review assessed quantitatively the level of vaccination coverage among migrants, in comparison with non-migrants, collating the published literature. METHODS: Review protocol was prospectively registered (PROSPERO CRD42021228061). A literature search without language restrictions was conducted in PubMed, Scopus and Web of Science, from database inception to February 2021. This review included observational studies that provided the vaccination rates among migrant and non-migrant groups. Study quality was assessed using Newcastle-Ottawa scale. Data were synthesized pooling data from individual studies to generate summary odds ratio (OR) with 95% confidence interval (CI) using random effects model, assessing heterogeneity with I2 statistic and publication bias with funnel asymmetry analysis. FINDINGS: There were 44 relevant studies (7,937,996 participants). Overall risk of bias was low in 13 (30%), moderate in 22 (50%) and high in 9 (20%) studies. Point estimates of individual ORs showed lower vaccination coverage among migrants in 36 of 39 meta-analyzable studies. Overall, the odds of vaccination coverage among migrants were lower compared to non-migrants (7,375,184 participants; summary OR 0.50; 95% CI 0.37-0.66; I2 99.9%). There was no funnel asymmetry. INTERPRETATION: Migrants are half as often vaccinated compared to non-migrants. Public health prevention programs need to prioritize vaccination equity, not just to protect migrants but also to protect the host communities.

INTRODUCTION: In 2019, 9,828 new cases of HIV and 5,825 of AIDS were detected in Mexico, the migrant population is considered highly vulnerable, which is presumed to be due to unfavorable conditions in their transit through the country, an important variable in the context of sexual health is the perception that they may have about being or not at risk. A study of perception of contracting HIV can have important implications for health. OBJECTIVE: To determine the association between the perception of risk for HIV and safe sex in migrants from the northern border of Mexico. METHODS: The design was predictive correlational and cross-sectional, sample of 302 migrants from two border cities of Tamaulipas, systematic random probability sampling. RESULTS: It was found that about 90% were men with a mean age of (M = 33,25, SD = 9,62) years, in general, migrants have a low perception of risk for HIV and this was correlated with safe sex (rs = -,134, p =,020), furthermore, a regression analysis showed that the perception of risk for HIV has a predictive effect on safe sex (ß = -, 143, F = 6,27 [gl=1,300], p = 0,013), CONCLUSION: Migrants are a group that requires interest in the field of research related to sexuality, despite being a vulnerable group and identified as at risk, their perception of acquiring HIV is low, this variable should be considered of importance in future research in the sexual field of migrants, because it can be used in conjunction with others as a model or as a mediator.

RESUMO

In this article, we explore relationships between risk and emotions among Bolivian women living with Chagas disease, and the implications of this for their diagnosis and treatment in Catalonia, Spain. Here, risk is a social phenomenon, while emotions are conceived as embedded in the sociocultural and relational world. Emotions play key risk-related roles as both a cause and consequence of Chagas disease, are the basis of health practices, and allow us to link risk to wider social inequalities. The way we conceive emotions is crucial both theorically and practically.

RESUMO

Mexican immigrants are affected by Chagas disease (CD) in California. It is through the representation of Chagas as a rare disease that participants make sense of the disease. A positive diagnosis has meant the disruption of patients' sense of normality and self-image, as well as their memories of homeland both reproducing and challenging hegemonic and stigmatized ideas of the disease associated with rurality and poverty. Access to treatment and medical care was the major coping mechanism. Health programs on CD should consider the emotional and social impact of the disease on people's self-perceptions to develop better medical care and prevention.

There are still few publications that analyse the effects on migrants or ethnic minorities of COVID-19 or of measures taken to curb this pandemic, although early studies point to a greater impact on black, asian and ethnic minority populations in the UK or on migrants in Mexico. In addition to barriers to access to information and health services, we consider it a priority to focus on their living conditions, particularly those in situations of vulnerability or social exclusion. People who are unemployed or with precarious jobs, without social benefits, in overcrowded conditions, may be more at risk of infection and not receiving adequate treatment. Confinement has predictably more negative impact on migrants in irregular administrative situations, victims of gender-based violence and those unable to comply with physical estrangement measures, such as refugees in camps or migrants under-living and settlements, without adequate hygienic conditions. Recommendations such as suspending deportations, extending or facilitating residence and work permits, closing detention centres for foreign persons, evacuating those in prisons and refugee camps or settlements have been applied unequally in different countries. Only a strong political commitment to global health equity can ensure the health of migrant populations and ethnic minorities, as well as their access to protection measures, information, medical testing and health services.Keywords: Migrants, COVID-19, Minority Groups, Vulnerable Populations, Social Determinants of Health.

OBJECTIVE: To describe the different perceptions about health rights for migrant population in Chile published on Twitter, concerning the first liver transplant carried out in Chile to a foreign national woman from Haiti, in September 2018. METHOD: Qualitative study, case analysis. The case corresponded to the first emergency liver transplant in a migrant woman in Chile. Opinions expressed on Twitter regarding this case were collected between September 29 and November 17 (n=339). Thematic analysis was performed using NVivo12 software, with codes defined conforming to the objective. RESULTS: According to the perceptions raised on Twitter, the right to access health services of the migrant population in Chile should be limited, and priority should be given to nationals. These opinions coexist with viewing health as a human right. There are also feelings of racism and discrimination towards this group. CONCLUSIONS: In Chile, there are different perceptions of what should grant rights of access to migrant health services. This situation can generate a worsening of stigmatization and vulnerability faced by migrants and a barrier to the policy's implementation, further exacerbating the presence of health inequities.

This study aims to analyze the feasibility of building an evaluative model for the management of the Tuberculosis Prevention and Control Program in the State of Roraima, located on the border between Brazil and Venezuela. This is an evaluability assessment, a type of study used as a pre-evaluation of the development and implementation stages of a program, as well as throughout its execution. The study was developed in stages comprising the: (i) definition of the intervention to be analyzed and its objectives and goals; (ii) construction of the intervention logical model; (iii) screening of parties interested in the evaluation; (iv) definition of the evaluative questions; and (v) design of the evaluation matrix. Four priority components were defined for the evaluation: management of the organization and implementation of tuberculosis (TB) prevention and control policy; epidemiological surveillance management; care network management; and management of expected/achieved results. In this model, and based on theoretical references, we defined the necessary resources, activities, outputs, outcomes, and the expected impact for each of the policy management components. The management of the TB control program is feasible for evaluation based on the design of its components, the definition of structure and process indicators, and relevant results for the analysis of the management of TB prevention and control actions, as well as its influence on compliance with the agreed indicators and targets aiming at eradicating the disease by 2035.


Objetivou-se examinar a viabilidade da construção de um modelo avaliativo para a gestão do Programa de Prevenção e Controle da Tuberculose no Estado de Roraima, localizado na fronteira entre Brasil e Venezuela. Trata-se de um estudo de avaliabilidade, um tipo de estudo utilizado como pré-avaliação na fase de desenvolvimento e implementação de um programa, bem como ao longo de sua execução. O estudo foi desenvolvido em etapas: (i) definição da intervenção a ser analisada e seus objetivos e metas; (ii) construção do modelo lógico da intervenção; (iii) mapeamento dos interessados na avaliação; (iv) definição das questões avaliativas e (v) delineamento da matriz de avaliação. Foram definidos quatro componentes prioritários para avaliação: gestão da organização e implementação da política de prevenção e controle da tuberculose (TB), gestão da vigilância epidemiológica, gestão da rede de atenção à saúde e gestão dos resultados esperados/obtidos. Nesse modelo, e com base em referenciais teóricos, definimos os recursos, atividades, produtos, resultados e o impacto esperado para cada um dos componentes de gestão de políticas. A gestão do programa de controle da TB é passível de avaliação a partir do delineamento de seus componentes e da definição de indicadores de estrutura e processo, assim como de resultados relevantes e sua influência para o cumprimento das metas pactuadas, visando erradicar a doença até 2035.


El objetivo fue analizar la viabilidad de elaborar un modelo de evaluación para la gestión del Programa de Prevención y Control de la Tuberculosis en el Estado de Roraima, que está en la frontera entre Brasil y Venezuela. Se trata de un estudio de evaluabilidad, un modelo de estudio que se utiliza como evaluación previa en la fase de desarrollo e implementación de un programa, así como a lo largo de su ejecución. El desarrollo del estudio se realizó en etapas: (i) definir la intervención a analizar y sus objetivos y metas; (ii) construir el modelo lógico de la intervención; (iii) mapear los interesados en la evaluación; (iv) definir las preguntas de la evaluación; y (v) trazar la matriz de evaluación. Se definieron cuatro componentes prioritarios para la evaluación: la gestión de la organización e implementación de la política de prevención y control de la tuberculosis (TB), la gestión de la vigilancia epidemiológica, la gestión de la red de atención a la salud y la gestión de los resultados esperados/obtenidos. En este modelo, y basándonos en referentes teóricos, definimos los recursos, actividades, productos, resultados y el impacto esperado para cada uno de los componentes de la gestión de políticas. La gestión del programa de control de la TB puede evaluarse a partir del diseño de sus componentes y de la definición de indicadores de estructura y proceso, así como de resultados relevantes y su influencia para el cumplimiento de las metas pactadas, con el fin de erradicar la enfermedad para 2035.

La migración internacional de odontólogos es un fenómeno en aumento, pero poco estudiado. Objetivo: Reconocer el abordaje en la literatura científica del fenómeno de migración de odontólogos en diferentes regiones del mundo. Método: Se realizó una revisión sistemática exploratoria en bases de datos de Pubmed, Medline, Scopus, Lilacs y Web of Science en español e inglés. Se analizaron objetivos, metodologías, problemas, temas, principales hallazgos y las discusiones. Resultados 25 estudios seleccionados utilizaron abordajes cualitativos, cuantitativos y mixtos. Se enfocaron en América del norte, Europa, Oceanía y Asia orientados a mejorar políticas migratorias, y ajustar demanda - oferta. Los temas abordados fueron: flujo migratorio, datos demográficos, evaluación y cualificación; aspectos de la experiencia migratoria; y condiciones políticas de la migración. Se trató la migración sur-norte o entre países centrales, pero no la migración sur-sur, ni en Latinoamérica. Se identifica que la migración de odontólogos tiene repercusiones en los sistemas de salud, equidad de la atención; debilidades en la regulación, sistemas de información y evaluación y cualificación de los migrantes; los motivos de migración pueden ser personales, profesionales y sociopolíticos; las condiciones de trabajo, adaptación cultural y experiencias de la migración dependen del proceso y condiciones migratorias, personales y profesionales. Conclusión: Es necesario realizar investigaciones de la migración sur-sur, mejorar la información sobre la migración en los países de salida. Los países de la periferia deben articular sus esfuerzos con los países receptores para entender mejor el fenómeno, no producir inequidades y garantizar un mejor flujo y satisfacción laboral de los odontólogos migrantes.

Objetivo: Reconhecer a abordagem na literatura científica do fenômeno da migração de dentistas em diferentes regiões do mundo. Método: Foi realizada uma revisão sistemática exploratória nas bases de dados Pubmed, Medline, Scopus, Lilacs e Web of Science em espanhol e inglês. Foram analisados ​​objetivos, metodologias, problemas, temas, principais achados e discussões. Resultados 25 estudos selecionados utilizaram abordagens qualitativas, quantitativas e mistas. Centraram-se na América do Norte, Europa, Oceânia e Ásia, com o objetivo de melhorar as políticas de migração e ajustar a procura-oferta. Os temas abordados foram: fluxo migratório, dados demográficos, avaliação e qualificação; aspectos da experiência migratória; e condições políticas da migração. A migração Sul-Norte ou entre países centrais foi discutida, mas não a migração Sul-Sul, nem na América Latina. Identifica-se que a migração de dentistas repercute nos sistemas de saúde, na equidade do atendimento; fragilidades na regulação, nos sistemas de informação e na avaliação e qualificação dos migrantes; As razões da migração podem ser pessoais, profissionais e sociopolíticas; As condições de trabalho, a adaptação cultural e as experiências migratórias dependem do processo e das condições migratórias pessoais e profissionais. Conclusão É necessário realizar pesquisas sobre a migração sul-sul, melhorar a informação sobre a migração nos países de partida. Os países periféricos devem articular os seus esforços com os países receptores para melhor compreender o fenómeno, não produzir desigualdades e garantir um melhor fluxo e satisfação profissional dos dentistas migrantes.


The international migration of dentists is a growing but little studied phenomenon. Objective: Recognize the approach in scientific literature to the phenomenon of migration of dentists in different regions of the world. Method: An exploratory systematic review was carried out in PubMed, Medline, Scopus, Lilacs and Web of Science databases in Spanish and English. Objectives, methodologies, problems, themes, main findings and discussions were analyzed. Results 25 selected studies used qualitative, quantitative and mixed approaches. They focused on North America, Europe, Oceania and Asia aimed at improving migration policies and adjusting demand-supply. The topics addressed were: migratory flow, demographic data, evaluation and qualification; aspects of the migratory experience; and political conditions of migration. South-North migration or between central countries was discussed, but not south-south migration, nor in Latin America. It is identified that the migration of dentists has repercussions on health systems, equity of care; weaknesses in regulation, information systems and evaluation and qualification of migrants; The reasons for migration can be personal, professional and sociopolitical; Working conditions, cultural adaptation, and migration experiences depend on the personal and professional migratory process and conditions. Conclusion: It is necessary to carry out research on south-south migration, improve information on migration in the countries of departure. Periphery countries must articulate their efforts with destination countries to better understand the phenomenon, not produce inequalities and guarantee a better flow and job satisfaction of migrant dentists.

INTRODUCCIÓN: En las cuatro décadas de la epidemia por VIH, se han observado avances notables que han contribuido a una disminución progresiva en la incidencia de nuevas infecciones y en la mortalidad a nivel mundial. Sin embargo, no ha ocurrido lo mismo en Latinoamérica y en Chile. OBJETIVO: Esta revisión tiene como objetivo conocer la epidemiología actual a nivel global, latinoamericano y chileno. METODOLOGÍA: Se analizaron los informes epidemiológicos oficiales de infección por VIH emitidos por organismos nacionales e internacionales, más los estudios epidemiológicos nacionales. RESULTADOS: Se estima que 39 millones de personas viven con VIH en el mundo; no obstante, la mortalidad y la incidencia de nuevos casos han disminuido de forma notoria durante las últimas dos décadas, asociado a una expansión en al acceso a terapia antirretroviral en forma global. A diferencia del resto del mundo, América Latina presenta una tendencia de alza en las nuevas infecciones y Chile registra un aumento de 35% en nuevos casos durante los últimos 10 años, coincidiendo con un aumento en los flujos de migrantes que ha afectado a la región. Algunas estrategias preventivas como la profilaxis pre exposición se han implementa-do a un ritmo lento, tanto a nivel mundial, como latinoamericano. CONCLUSIONES: La epidemiología del VIH presenta características propias regionales y nacionales. En particular en Chile, diversos factores incluyendo déficit en políticas públicas de prevención y los recientes flujos migratorios han modelado nuestra actual epidemia. El desafío presente debe contemplar los esfuerzos multisectoriales para lograr los objetivos de ONUSIDA en esta década.


BACKGROUND: Over the course of the last four decades of global HIV epidemic, significant improvements have contributed to gradually reduce the frequency of new infections and global mortality rates. However, in Latin America particularly in Chile, new infections continue increasing. AIM: This review aims to comprehend the_epidemiology today on a worldwide, Latin American, and Chilean scale. METHODS: National epidemiology studies and official HIV reports from international and national organizations were reviewed. RESULTS: It is estimated that 39 million people live with HIV worldwide; however, mortality and the incidence of new cases have decreased markedly over the last two decades, associated with an expansion in access to antiretroviral therapy globally. In contrast to the rest of the world, Latin America shows an upward trend in new infections, with Chile registering a 35% increase in new cases over the last 10 years, coinciding with an increase in migratory flows that has occurred throughout the region. Some preventive strategies, such as pre-exposure prophylaxis, have been implemented at a slow pace, both globally and in Latin America. CONCLUSIONS: The epidemiology of HIV has regional and national characteristics. Specifically in Chile, several factors, including deficits in public prevention policies and recent migratory flows, have shaped our current epidemic. The present challenge must contemplate multisectoral efforts to achieve the UNAIDS objectives during this decade.

Este ensaio emerge a partir de uma aposta científica que se propõe a produzir reflexões teorizadas sobre as práticas assistenciais de acolhimento produzidas pelos profissionais da saúde com mulheres migrantes venezuelanas na Atenção Básica. É um ensaio teórico-reflexivo. Salienta-se que, neste tipo de estudo, a produção de reflexões corre pela compreensão de um dado objeto sem que nele haja um sistema de compreensão rígido. As reflexões foram agrupadas em um núcleo teórico-reflexivo, intitulado: fundamentos para pensar o acolhimento à mulher migrante venezuelana na Atenção Básica. Nele, são encontrados conceitos e reflexões a respeito da Atenção Básica, migração, processos migratórios envolvendo mulheres venezuelanas e estratégias de acolhimento às suas necessidades em Unidades Básicas de Saúde. As (in)conclusões reflexivas para intensificar investigações perpassam por leituras do cotidiano no trabalho em saúde. Nesse sentido, é fundamental observar três rotas conceituais e reflexivas que fundamentalmente são necessárias para cuidar das migrantes venezuelanas de forma acolhedora na Atenção Básica ­ a saber: escuta qualificada, vínculo e clínica ampliada do sujeito ­ como forma de promover uma assistência alinhada aos princípios e diretrizes da política nacional de humanização.


This essay emerges from a scientific proposal to reflect on embracement care practices aimed at Venezuelan immigrant women in primary care. As a theoretical-reflective essay, the reflections produced on a given object disregard any rigid understanding system. Concepts and reflections on primary care, migration, migratory processes involving Venezuelan women, and embracement strategies were grouped into a theoretical-reflective nucleus entitled "foundations for discussing the embracement of Venezuelan immigrant women in primary care." Reflective (in)conclusions used to intensify investigations involve readings of everyday healthcare work. In this regard, three conceptual and reflective routes are essential to embrace Venezuelan immigrant women in in primary care­qualified listening, bond, and expanded clinical approach­to promote care aligned with the principles and guidelines of the national humanization policy.


Este ensayo propone producir reflexiones teorizadas sobre las prácticas asistenciales de acogida de los profesionales de la salud a las mujeres migrantes venezolanas en la atención primaria. Se trata de un ensayo teórico-reflexivo. En este tipo de estudio se destaca que la producción de reflexiones parte de la comprensión de un objeto dado sin que haya un sistema de comprensión rígido. Las reflexiones se agruparon en un núcleo teórico-reflexivo, titulado fundamentos para pensar en la acogida a la mujer migrante venezolana en la atención primaria. Se encuentran conceptos y reflexiones en torno a la atención primaria, migración, procesos migratorios que involucran a mujeres venezolanas y estrategias para acoger sus necesidades en unidades básicas de salud. Las (in)conclusiones reflexivas para intensificar las investigaciones pasan por lecturas de lo cotidiano en el trabajo en salud. En este sentido, es fundamental observar tres rutas conceptuales y reflexivas que fundamentalmente son necesarias para cuidar a la mujer migrante venezolana de forma acogedora en la atención primaria: escucha calificada, vínculo y clínica ampliada del sujeto; como forma de promover una asistencia alineada con los principios y directrices de la política nacional de humanización.
