RESUMO

Most tumours of the tracheobronchial tree are malignant. Benign tumours such as hamartoma are infrequent and generally intra-parenchymal. Here we present a case of a 65-year-old male patient who presented with a purely endobronchial lobulated mass lesion in the left main bronchus. This central airway obstruction was managed by a complete endobronchial resection using an electrocautery snare and cryo-recanalisation techniques. A diagnosis of endobronchial chondroid hamartoma was made on histopathological examination. Such endobronchial lesions are rare and constitute less than 2% of all hamartomas.

A 34-year-old pregnant woman in the 34th week of gestation with uncontrolled asthma was admitted because of asthma exacerbation. Although she received bronchodilators and systemic corticosteroids, respiratory failure rapidly progressed. Chest computed tomography revealed a mass occluding approximately 80% of the tracheal lumen. After urgent Caesarean section, endobronchial resection was performed. The pathological findings of the resected tumor were compatible with tracheal glomus tumor. Tracheal tumors are often misdiagnosed as asthma, but its complication with asthma is rare. Even if the diagnosis of asthma is definitive, clinicians should consider coexisting diseases, including tracheal tumors, when asthma control is poor.

RESUMO

Tracheal papilloma is a rare benign lung tumour. It often recurs and sometimes transforms into a malignant tumour. While historically radical resection has been the primary treatment, endobronchial resection, a less invasive procedure to manage the airway using a flexible and rigid bronchoscope, was effectively used in our patient.

OBJECTIVES: Thoracic surgeons often encounter patients with obstruction of the central airways requiring bronchoscopy to provide an open airway. The endobronchial shaver (Richard Wolf GmbH,Germany) is one of the new devices available for treatment of central airway obstruction. We report a tracheal tumor managed with this device. METHODS: We report the successful use of endobronchial shaver via rigid bronchoscopy to resecte a complex stenosis 77-year-old male patient. Patient admitted to our clinic for progressive dyspnea and stridor. The patient's medical history was cervical collar incision with resection and anastomosis 10 years ago. A mass causing a obstruction of 70% of the tracheal lumen was detected with fiberoptic bronchoscopy. So, we decided to use endobronchial shaver for immediate effect. RESULTS: Trachea was successfully recanalized with Endobronchial shaver (Richard Wolf GmbH, Germany). The patient was successfully extubated, and his dyspnea was resolved. Histopathologic examination of debris was revealed a tracheal pleomorphic adenoma. Postoperative course was uneventful and there was no evidence of recurrent or stenosis in 5 months of follow-up. CONCLUSION: The Endobronchial shaver is an excellent instrument to manage intrluminal complex stenoses including tumor and granulation tissue with advantages including efficient removal of mass without a need for separate suctioning, coagulation,and limitation in oxygenation.

RESUMO

This report highlights the usefulness of bronchoscopy in case of recurrent pneumonia with the same localization even in CF patients where the presence of bronchiectasis as promoting factor of infections could delay the diagnosis.
