In August 2021, two juvenile male Antarctic fur seals (Arctocephalus gazella) stranded in the southeastern Brazilian coast and were referred to rehabilitation centers. The animals presented increased body temperature, prostration, respiratory distress and despite treatment died. A necropsy following a standardized protocol was performed, and formalin-fixed tissues were processed for microscopic examination. Samples were screened for morbillivirus, herpesvirus, and Brucella spp. by molecular analyses (PCR, RT-PCR). Bacteriological culture was performed in samples collected from the lungs, trachea, and lymph nodes of both cases. The main histopathologic findings were of infectious nature, including multifocal necrotizing and fibrinous mixed interstitial pneumonia, bronchiolitis, and bronchitis, with intralesional myriad bacteria associated with vascular fibrinoid necrosis. Pseudomonas aeruginosa was isolated from tracheal and lung swabs of Case 1, and Klebsiella oxytoca was found in nostril swabs, tracheobronchial lymph nodes, and lung of Case 2. Gammaherpesvirus infection was detected in both cases, and the sequences retrieved were classified into the genus Percavirus. All tested samples were PCR-negative for Brucella spp. and morbillivirus. We hypothesize that the deficient immunological status in association with starvation predisposed the reactivation of herpesvirus and secondary bacterial co-infections. To the authors' knowledge, this is the first molecular detection of herpesvirus in an Antarctic pinniped. These findings reinforce that Otariid gammaherpesvirus circulating in the Southern Hemisphere are likely endemic in the Arctocephalus genus. This report contributes to the current knowledge of health aspects affecting wild pinnipeds, especially in the poorly studied Antarctic species.

Herpes simplex virus type 2 (HSV-2) is highly prevalent in several regions of the world and is the main pathogen causing genital herpes, which is transmitted almost exclusively through sexual contact. Systemically disseminated infections caused by HSV-2 are rare and most often seen in newborns, pregnant women, or immunocompromised populations. The virus can invade multiple organs and cause damage. In this paper, we present an extremely rare case of an immunocompetent 36-year-old male who came to our hospital with a high fever with abdominal pain and died of sepsis and multiple organ dysfunction syndrome within a short period. After the exclusion of common pathogens such as bacterial and fungal infections during hospitalization, metagenomic next generation sequencing of the patient's peripheral blood and ascites gave us the answer, and very high nucleic acid sequence counts of HSV-2 were detected in both his peripheral blood and ascites, confirming HSV-2 as the causative virus. In addition, this paper provides a brief review of the relevant literature.

Macrophage activation syndrome (MAS) is a life-threatening systemic hyperinflammatory syndrome triggered by various infections, particularly viral infections, autoimmune disorders, and malignancy. The condition is characterized by an increased production of proinflammatory cytokines resulting in a cytokine storm and has been associated with poor clinical outcomes. During the COVID-19 pandemic, patients with severe manifestations developed features similar to those of MAS, although these characteristics remained well defined within the lung. Additionally, other viral infections including EBV, the herpes family of viruses, hepatitis viruses, influenza, HIV, and hemorrhagic fevers can be complicated by MAS. The diagnosis and management of the condition remain challenging due to the lack of consensus on specific guidelines, especially among the adult population. Currently, therapeutic options primarily rely on medications that are typically used to treat primary hemophagocytic lymphohistiocytosis, such as corticosteroids and etoposide. In addition, cytokine-targeted therapies present promising treatment options. The objective of this review is to discuss the emergence of MAS in the context of viral infections including, but not limited to, its occurrence in COVID-19.

Introduction: Human parechovirus (HPeV) belongs to the Picornaviridae family and has been described in viral meningoencephalitis and sepsis like illness in infants. Until now, 16 genotypes have been recognized, the most common are HPEV 1, 2 and 3; type 3 is most severe. Aims: To estimate the frequency of HPEV etiology in viral meningoencephalitis and sepsis in infants and characterize clinical and molecular aspects of infection. Methods: Between October 2013 and March 2015 we collected CSF samples, plasma, nasopharyngeal swabs and/or stools of patients younger than two years with suspected sepsis and/or viral meningitis. Samples were obtained from laboratory storage sites and from hospitalized patients. HPeV was diagnosed by real-time polymerase chain reaction (PCR) assay against the 5'UTR region. Positive samples were genotyped by sequencing a 304pb segment in VP3/VP1 overlapping region obtained with a nested PCR. Results: Overall HPeV detection rate was 18,6% (11/59 patients), distributed in 8.7% (4/46) laboratory's samples and 53.8% (7/13) of samples from hospitalized patients; mean age was 49 days (SD?). Most common clinical signs were irritability (%), inappetance and fever (algo mas? Magnitude? %). All six samples genotyped were HPeV 3. Conclusions: HPeV should be considered as a relatively significant etiologic agent of viral meningoencephalitis and sepsis in infants.


Introducción: Parechovirus humano (HPeV) pertenece a la familia Picornaviridae; ha sido descrito en sepsis viral y meningoencefalitis en niños de dos años o menos (lactantes). Se conocen 16 genotipos, siendo los más frecuentes HPeV 1, 2 y 3; el más grave es el tipo 3. Objetivos: Estimar la frecuencia de HPeV como agente causal de meningoencefalitis o sepsis viral en lactantes; caracterizar clínica y molecularmente los HPeV encontrados. Material y Métodos: Estudio descriptivo. Se utilizaron muestras de LCR, plasma, hisopado nasofaríngeo y/o deposiciones de lactantes con sospecha de sepsis y/o meningoencefalitis viral entre octubre 2013 y marzo 2015. Se estudiaron muestras almacenadas en laboratorio y de pacientes hospitalizados. Como diagnóstico, se realizó RPC-TR en tiempo real para HPeV dirigido a sector 5'UTR. Para la caracterización molecular, se secuenció un sector de 304 pb en unión VP3/VP1 mediante una RPC-TR anidada. Resultados: Se reclutó un total de 59 pacientes con frecuencia de HPeV de 18,6% (11/59), correspondientes a 8,7% (4/46) en muestras de colección y 53,8% (7/13) en hospitalizados, edad promedio 49 días. En la presentación clínica destacó la irritabilidad, el rechazo alimentario y la fiebre. Seis casos fueron genotipificados, todos correspondieron al tipo HPeV 3. Conclusiones: HPeV debe ser considerado como agente causal en sepsis y/o meningoencefalitis en lactantes.
