Suitability of Wrist External Fixator for Treatment of Mandibular Fracture.

The mandibular fracture is one of the most commonly fractures of the facial bones. In high-energy traumas, a comminuted mandible fracture may occur. The closed reduction with external fixator is an interesting alternative for these types of fractures. The aim of this article is to report the case of a patient with comminuted fracture in the right mandibular body, whose surgical treatment was the noninvasive reduction of fracture and stabilization using a Colles' wrist external fixator. After 1 year of follow-up, the patient is without complaints, with satisfactory dental occlusion, adequate contour, and mandibular alignment. External fixation with wrist orthopedic fixators adapted to the mandible provides many advantages owing to its versatility and simplicity of use. Therefore, this method should be remembered as a valid and affordable option for the treatment of complex lesions when indicated correctly.

Tratamento de fratura complexa de mandíbula por abordagem transcervical: relato de caso / Mandible complex fracture treatment through transcervical approach: case report

A fratura de mandíbula está entre as mais frequentes fraturas dos ossos da face, com um aumento significativo de casos nos últimos anos. A deficiência no exame físico e o tratamento inadequado podem levar à deformidade estética ou funcional permanente. O tratamento dessas injúrias visa efetuar uma redução anatômica e fixação dos fragmentos ósseos com o objetivo de restaurar a função e forma, reduzindo as complicações. Como opções de tratamento, incluem-se o conservador e as técnicas abertas para a redução e a fixação. O método aberto para fraturas complexas de mandíbula com múltiplos fragmentos ósseos pode ser por abordagens intraoral ou extraoral. O presente artigo relata o caso de um paciente do sexo masculino, 52 anos de idade vítima de agressão física que compareceu ao serviço de urgência de Cirurgia e Traumatologia Bucomaxilofacial apresentando trauma em face. Após o exame clínico e tomográfico, teve-se como diagnóstico a fratura múltipla de mandíbula, sendo o tratamento cirúrgico adotado com abordagem transcervical, abrangendo as regiões submandibulares bilateralmente e submentoniana com simplificação dos fragmentos com placas do sistema 2.0mm e fixação com placa do sistema 2.4mm, do tipo locking. The mandible fracture is among the most common of the bones of the face, with a significant increase in cases in recent years. Deficiency in physical examination and inadequate treatment can lead to aestheticor permanent functional deformity. The treatment of these injuries aims to make an anatomic reduction and fixation of bone fragments in order to restore form and function, reducing complications. The treatment options include conservative and open techniques for reduction and fixation. The open method for complex fractures of the jaw with multiple bone fragments can be through intra-oral or extra-oral approaches. This article reports a case of a male patient, 52 years old, physical aggression victim, who attended the urgency department of Oral and Maxillofacial Surgerywith trauma in the face. After clinical and CT examination had up to diagnosis of multiple jaw fractures, and surgical treatment with transcervical approach covering bilateral submandibulars and submental regions with simplification of the fragments with 2.0mm system plates and fixation with2.4mm system plates, locking type. Reconstrução de lesão em lábio superior por mordedura animal em criança / Reconstruction for injury in upper lip biting in animal in child

Os ferimentos ocasionados por mordeduras animais são comuns e de importância na saúde pública, tendo consequências, como desfiguração, infecção e, até mesmo, a morte em casos mais graves. A conduta frente a essas lesões é controversa, principalmente do ponto de vista do fechamento primário da ferida da profilaxia de doenças infecto-contagiosas, originadas a partir do contato da saliva do animal com a ferida. O atendimento desses doentes na urgência consiste no controle da infecção, reabilitação funcional e, consequentemente, estética, a fim de minimizar danos psíquicos e possibilitar retorno ao convívio social. O presente trabalho relata o caso de ferimento em lábio superior de uma criança decorrente de agressão por mordida canina. A reconstrução foi realizada através da rotação de retalho rombóide da mucosa oral bem como do avanço retilíneo do retalho cutâneo associado aos triângulos de Descarga. Além disso, foi instituída uma antibioticoterapia específica. Injuries caused by dog bites are common and have importance in public health, with consequences such as disfigurement, infection and even death in severe cases. The correct approach to these lesions is controversial, especially in primary closure of the point of view of the wound and prophylaxis of infectious diseases originating from the animal's saliva contact with the wound. The care of these patients in the emergency consists in controlling infection, functional rehabilitation and consequently aesthetics in order to minimize possible psychological damage and return to social life. This paper reports the injury case in upper lip of a child resulting from aggression by canine bite. The reconstruction was carried out by retail rhomboid oral mucosa rotation and straight forward skin flap associated with the discharge of triangles. Moreover, a particular antibiotic was introduced. Os ferimentos ocasionados por mordedura humana são geralmente tidos como portadores de maior incidência de complicações infecciosas, pois possuem alto índice de contaminação. O tratamento dessas lesões é controverso, principalmente no que se refere à sutura primária e à profilaxia de doenças infecto-contagiosas originadas a partir do contato da saliva do agressor com a ferida. O atendimento desses doentes na urgência consiste no controle da infecção, reabilitação funcional e consequentemente estética, a fim de minimizar danos psíquicos e possibilitar retorno ao convívio social. O presente trabalho relata um caso de uma extensa avulsão de lábio inferior decorrente de agressão por mordedura humana. A reconstrução inicial foi possível através de avanço de retalhos laterais, associada à antibioticoterapia específica. Desta forma foi obtido um resultado estético e funcional satisfatório, sem complicações infecciosas pós operatórias ou deiscências de suturas, a continência oral foi preservada, bem como a sensibilidade e a mobilidade labial... The treatment of these lesions is controversial, especially with regard to primary suturing and disease prophylaxis. Infectious-contagious caused by contact between the aggressor's saliva and the wound. the service of these patients in the emergency room consists of infection control, functional rehabilitation and, consequently, aesthetics, in order to minimize psychic damage and enable a return to social life. The present work reports a case of an extensive avulsion of the lower lip resulting from human bite aggression. The initial reconstruction was possible through the advancement of lateral flaps, associated with specific antibiotic therapy. In this way, a satisfactory aesthetic and functional result was obtained, without complications.
