RESUMEN

This qualitative-descriptive study was carried out in a university hospital in Rio de Janeiro, Brazil, from August, 2007 to December, 2008. Nursing care was described and characterized based on the perceptions of 26 hospitalized patients. Data collection was carried out through interviews and the "Almanac" creative and sensitivity technique, followed by thematic content analysis from which the following category emerged: "Nursing care dimensions from the subjects' perspectives scientific-technical and expressive". The results show that these dimensions should be manifested in a coordinated way and integral care should be designed and constructed in the relationship with the other. The participants value and defend an alliance between these dimensions and expect nurses to integrate them into care actions. Knowledge about the patients' perceptions concerning hospital care collaborates to the promotion of the environment and favors its restoration through effective care generated in this relationship.

RESUMEN

The purpose of this qualitative research was to characterize the elements that constitute the environment of the Surgical Center and to analyze its implications for dynamic of care and nursing care. Based on the Environmental Theory's principals. Participated twelve nurses from the Surgical Center of a College Hospital in Rio de Janeiro. Data were gathered through the creativity and sensitivity technique "Map-Speaker", semi-structered interviews and participant observation, and were analyzed by thematic categories. The results showed that care can happen directly and indirectly in favor of full client recovery, counting the environment that the integrate in purpose to maintain harmonic and balanced. The nurse interventions aim to maintain the environment in favorable conditions so that a higher standard of care can be promoted.

RESUMEN

Study of qualitative nature, developed along with the nursing team and the accompanying relatives of interned customers in a public hospital at Vitória/ES. The objectives were to describe the bases that sustain the relations among those subjects; to analyze the dynamics of this relation; and to discuss its implications for the nursing care. The theoretical reference was linked to the studies of power relations in the disciplined hospital environment and the care in a humanized perspective concept. The data collection was through a participant observation and semi-structured interviews. The results pointed out that those relations don't happen lineally. At same time that conflicting experiences emerge, there is another whose position of one and other subject reveals sensibility and solidarity that implicate directly in the delivered care to the hospitalized customer.

OBJECTIVE: Understand the meanings people attribute to the use of plants in self-care practices during situations of ailment. METHOD: Qualitative, exploratory-descriptive research with ethnographic orientation, based on health anthropology with informants selected from a relations network. The sampling was done by semi-structured interviews and participant observations, from April 2015 to February 2017. RESULTS: Seventeen informants participated. The thematic axis emerged in the content analysis: meanings attributed by the informants to the use of medicinal plants. Feelings of love and happiness were unveiled, meanings of helping others, cure practices, family care through plants, comfort, the sense of welfare, among others. CONCLUSION: The importance of the nursing professional in establishing a therapeutic alliance with the subjects and social groups, aiming at the promotion of health and care completeness.

OBJECTIVE: To create a board game as a form of gerotechnology to promote active and healthy aging. METHOD: This was a qualitative, convergent care study conducted with 32 older adults. Individual interviews, creativity and sensitivity techniques, group discussions, and participant observations guided by Paulo Freire's theoretical framework of problematization were used to produce data. The participants and researchers assessed the entire production process at the end of the study, after the gerotechnology was implemented in the groups. French discourse analysis was adopted. RESULTS: Patented under registration no. BR 1020160107725, the game was created as a technological product based on the discussions with older adults, considering the conceptions, knowledge and practices shared by them about aging. Final considerations: The gerotechnology implemented in clinical nursing practices functioned as a playful resource to exercise self-determination and independence among older adults, boosting memory, self-esteem, socialization processes, exchanging experiences and shared learning.

RESUMEN

OBJECTIVES: to evaluate the efficacy of a laser acupuncture protocol developed and applied by nurses in arterial hypertension patients. METHOD: randomized, multicenter, triple-blind and two-armed clinical trial. The sample consisted of 102 participants, 51 per arm, both sexes, aged between 30 and 75 years, undergoing drug therapy for a year or more, with difficulty to control blood pressure, maintaining regular measures >140x90 mmHg. Participants underwent six standard or simulated laser-acupuncture sessions, for 24 minutes, within a period of six weeks. Descriptive analyzes expressed as frequencies of occurrences, means and medians were used, and analysis of the association between variables was performed using Student's t-test and Anova, using Statistica® software, version 12.0. The significance level was set at 5% (alpha=0.05). The comparison between blood pressure measurements was performed using Student's t-test for paired samples and Anova for repeated measures. RESULTS: a significant reduction in systolic (p<0.001) and diastolic (p<0.001) blood pressure was observed among participants in the intervention arm, which was not observed in the simulation arm. CONCLUSION: the results have demonstrated the efficacy of the protocol. Reduction and control of blood pressure have been demonstrated, indicating the possibility of using this technology for the care of patientes with essential systemic arterial hypertension. RESUMEN

This qualitative research aimed to analyze the biomedical influence in the context of nurses' formation and professional activities; to reflect about ethical and legal implications of using medicinal plants in nursing care; to argue about the need to configure and delimit this resource as a legitimate instrument for expanding nursing care practice. We used the creative-sensible method and developed dynamics with a group of nurses to produce data, analyzed in the categories "the biomedical influence in the academic-professional orientation of nursing" and "ethical and legal implications in the applicability of medicinal plants in care". Results indicated the need to advance in studies on the nursing diagnosis that implies the prescription of medicinal plants, so that the client is well taken care of by nurses, thus legitimizing it as an extension of their professional practice. Not as an exclusive territory, but as shared and interdisciplinary health care action.

RESUMEN

Qualitative study, discusses the attention and the physical presence as the fundamental care in the dialogical relation between the client living with AIDS and the hospital nursing, using concepts of expressive activities and human care, according to Watson and dialogue, according to Freire. Five persons participated with internment experience, under ambulatory follow up in a hospital of Rio de Janeiro city. It used the triangulation techniques: almanac, semi-structured interview and participant observation. It showed the importance of the valorization of that is necessary and of interest of the client from the attention and the physical presence as essential requirements in the human care, expressed in the dialogical experience and in the participation of the client in the whole care process.

RESUMEN

OBJECTIVES: describe care needs and demands that mark the discursive practices of ostomized clients and family members and discuss guidelines for a comprehensive care program to ostomized clients and their families, organized by macrosociological categories. METHOD: Creative and Sensitive, involving 17 ostomized subjects and family members at a municipal outpatient clinic. The ethical aspects were complied with. A characterization form was used, as well as Creativity and Sensitivity Dynamics: "speaking map", "body-knowledge" and "calendar". Critical Discourse Analysis was applied. RESULTS: the health needs and care demands of the ostomized patients and their family members, in their multiple dimensions, were constituted in the home and community, outpatient and social context, implying new orientations for nursing care. The unveiling of the data brought elements that constituted guidelines, in a macrosociological approach, to achieve the expanded integrality of nursing care. CONCLUSION: the ostomized clients are unique in their genre/peculiar from Latin sui generis, calling for strategies that respond to and distinguish their specificities. Elaborating a Public Health Policy that improves and reorganizes the care demands, taking into account these individual biopsychosocial and spiritual aspects, is a possible and irrevocable target in the attempt to achieve better conditions of health and wellbeing.

OBJETIVO: Evaluar la efectividad de la acupuntura-laser aplicada al cuidado de enfermería de personas con diabetes mellitus tipo II. MÉTODOS: Ensayo clínico aleatorizado, triple ciego, con 42 participantes diabéticos tipo II, ambos sexos, con edades comprendidas entre 30 y 75 años, con hipoglucemiantes orales y dificultades para mantener la glucemia posprandial ≤ 180mg/ dl. Los participantes fueron asignados aleatoriamente a dos grupos diferentes y sometidos a láser-acupuntura estándar, brazo experimental o brazo de control simulado. Se utilizaron análisis descriptivos y de asociación entre variables, con un nivel de significancia del 5% (p <0,05). Para comparar los resultados de glucosa en sangre, se utilizó la prueba t de Student para muestras pareadas y Anova para mediciones repetidas. RESULTADOS: Se observó una disminución significativa de la glucemia capilar en los participantes del brazo experimental, fenómeno no verificado en los del brazo control. CONCLUSIONES: Se infiere la plausibilidad de utilizar la técnica como tecnología para el cuidado de enfermería de personas con diabetes mellitus tipo II. Registro Brasileño de Ensayos Clínicos. Descriptive and association analyses between variables were used, with a significance level of 5% (p <0,05). To compare blood glucose results, Student's t test was used for paired samples and Anova for repeated measurements. RESULTS: A significant decrease in capillary glycaemia was observed in the participants in the experimental arm, a phenomenon not verified in those in the control arm. MÉTODOS: Ensaio clínico randomizado, triplo cego, com 42 participantes diabéticos do tipo II, ambos os sexos, idades entre 30 a 75 anos, em uso de hipoglicemiantes orais e dificuldades para manter a glicemia pós prandial ≤ 180mg/dl. Os participantes foram distribuídos aleatoriamente em dois grupos distintos e submetidos a laser-acupuntura padrão, braço experimental, ou simulada, braço controle. Empregaram-se análises descritivas e de associação entre variáveis, com nível de significância de 5% (p < 0,05). Para comparação entre os resultados da glicemia empregou-se teste t de Student para amostras pareadas e Anova para medidas repetidas. RESULTADOS: Observou-se diminuição significativa na glicemia capilar dos participantes do braço experimental, fenômeno não verificado naqueles do braço controle. RESUMO Objetivo: Descrever o conhecimento e experiências de idosos sobre a cirurgia de catarata e os cuidados pré e pós-operatórios. Método: Estudo qualitativo-descritivo feito com 24 idosos em hospital privado no Rio de Janeiro e em domicílio. Realizaram-se entrevistas individuais, seguidas de discussão com cada participante, entre setembro e dezembro de 2017. Aplicou-se análise de conteúdo temática na organização dos dados. Resultados: Categorias de análise: concepções sobre a doença e a cirurgia, fontes de acesso à informação, a cultura do silêncio no contexto do cuidado em saúde, e expectativas dos idosos sobre a cirurgia e seus resultados. Os idosos apresentaram lacunas sobre o que sabem sobre a cirurgia e os cuidados pré e pós-operatórios; trouxeram situações que reiteram, por vezes, a cultura do silêncio na relação com os profissionais de saúde e fizeram proposições sobre o que consideravam importante na educação em saúde sobre o tema. Conclusões: A partir da reunião com os idosos, foi possível evidenciar não apenas seus conhecimentos sobre a patologia enfrentada e o procedimento cirúrgico de catarata, como também avaliar, de forma integral, o processo e as mudanças necessárias no âmbito da promoção da saúde.


RESUMEN Objetivo: describir el conocimiento y las experiencias de personas mayores sobre la cirugía de cataratas y los cuidados prey postoperatorios. Método: estudio cualitativo-descriptivo llevado a cabo con 24 ancianos en hospital privado en Rio de Janeiro-Brasil y en domicilio. Se realizaron entrevistas individuales, seguidas de discusión con cada participante, entre septiembre y diciembre de 2017. Se aplicó el análisis de contenido temático en la organización de los datos. Resultados: categorías de análisis: concepciones sobre la enfermedad y la cirugía, fuentes de acceso a la información, la cultura del silencio en el contexto del cuidado en salud, y expectativas de las personas mayores sobre la cirugía y sus resultados. Los ancianos presentaron lagunas sobre lo que saben respecto a la cirugía y los cuidados pre y post operatorios; trajeron situaciones que reiteran, aveces, la cultura del silencio en la relación con los profesionales de salude hicieron proposiciones sobre qué consideraban importante en la educación en salud sobre el tema. Conclusiones: a partir de la reunión con los ancianos, fue posible evidenciar no solo sus conocimientos sobre la patología enfrentada yel procedimiento quirúrgico de cataratas, sino también evaluar, de forma integral, el proceso y los cambios necesarios en el ámbito de la promoción de la salud.


ABSTRACT Aim: To describe the knowledge and experiences of the elderly about cataract surgery and pre- and post-operative care. Method: This qualitative-descriptive study was conducted with 24 elderly people in a private hospital in Rio de Janeiro and at their homes. Individual interviews were held, followed by a discussion with each participant, between September and December 2017. Thematic content analysis was applied in the organization of the data. Results: Categories of Analysis: concepts of the disease and surgery, sources of access to information, the culture of silence in the context of health care, and expectations of the elderly about surgery and its results. The elderly had gaps in their knowledge about the surgery and pre- and post-operative care; spoke about situations that sometimes reiterated the culture of silence in their relationships with health professionals and made proposals about aspect of the topic that they considered important in health education. Conclusions: Based on meetings with the elderly, it was possible to show not only their knowledge about the pathology they faced and surgical procedure [for the treatment of cataracts], but also to fully assess the process and the changes required in the scope of health promotion.

RESUMEN

A qualitative research carried out with relatives who take care of elderly people with Alzheimer disease, it aimed at describing the experiences of these family members in living with the elderly and analyzing aspects that interfere with the transformations of family dynamics since the onset of the disease. The concept of dialogical education was applied to produce data in the dynamics of creativity and sensitivity Subjects shared the reality of living with the diseased person in the most diverse situations of life, their feelings, their anguish, interpersonal relationship, their expectations and family members' strategies in home co-existence. It was commented that the different stages of the diseased person and his/her family members, respectively, even though not always coinciding, are interpenetrated, thus generating transformations and re-configuration in family dynamics.

RESUMEN

The study focuses conversation as care that integrates other elements of hospital nursing. It had as its objective to analyze conversation as an essential stage of the dialogue between the patient and the nursing team. Five people participated who had already been hospitalized and that were now under clinical monitoring due to learning to live with AIDS. The information was obtained by means of the "Almanac" technique of sensitivity and creativity, semi-structured interview, and by active observation. The study contributed to reinforce the relational nature of the nursing care and the position of the patient as an object of this care who expresses his/her requests and wants in the process of dialoguing between this person and the nursing staff, expressed by conversation, which is viewed as a significant factor in nursing care.

RESUMEN

This research aims to understand the care practices of health professionals who assist the elderly Kaingang. It is a qualitative study, supported in ethnography, conducted by ten professionals working in primary health care in the indigenous land of Faxinal, Paraná, Brazil. The data was collected from November 2010 to February 2012 by participant observation and interviews, and analyzed based on the Transcultural Care Theory. Was identified the preoccupation of the carers practices with the medication and immunization, as well as traditional medical care. To achieve these, care professionals had strategies that implemented maintenance of older people in care. We conclude that cultural values and integrate scientific need assistance to improve the health of elderly indigenous.

Método: Investigación cualitativa, convergente asistencial, con 32 ancianos. Datos generados por entrevistas individuales, técnicas de creatividad y sensibilidad, discusión grupal y observación participante, orientada por referencial teórico de problematización de Paulo Freire. La evaluación de los participantes e investigadora sobre todo el proceso de producción ocurrió en última etapa posterior a implementación de la tecnología gerontológica en los grupos. Se adoptó análisis del discurso francés. Resultados: El juego, patentado bajo registro BR 1020160107725, fue elaborado como producto tecnológico partiendo de discusiones mantenidas con los ancianos, considerando compartir concepciones, saberes y prácticas sobre envejecimiento. Consideraciones finales: La tecnología gerontológica implementada en la clínica de atención de enfermería actuó como elemento lúdico para ejercicio de autodeterminación e independencia del anciano, potenciador de memoria, autoestima, procesos de socialización, intercambio de experiencias y aprendizaje compartido.

Os dados foram produzidos por entrevistas individuais, técnicas de criatividade e sensibilidade, discussão em grupo e observação participante norteadas pelo referencial teórico da problematização de Paulo Freire. A avaliação pelos participantes e pela pesquisadora de todo o processo de produção ocorreu na última etapa, após a implementação da gerontotecnologia nos grupos. Adotou-se análise de discurso francesa. Resultados: o jogo, patenteado sob o registro BR 1020160107725, foi elaborado como produto tecnológico a partir das discussões havidas com os idosos, considerando-se o compartilhamento de suas concepções, seus saberes e suas práticas sobre o envelhecimento. Considerações finais: a gerontotecnologia implementada na clínica do cuidado de enfermagem agiu como elemento lúdico no exercício da autodeterminação e independência do idoso, como potencializador de memória, autoestima, processos de socialização, trocas de experiências e aprendizagem compartilhada.

RESUMEN

Quantitative research aimed to verify the relevance of Action Control Theory for the identification of risk for poor performance in the planning of preventive care for pressure ulcers. The action - state orientation deals with individual differences and the ability to regulate emotions, cognitions and attitudes to complete the intentional actions. The instruments used were the Student Assessment Instrument and HAKEMP 90, derived from this theory, in 46 undergraduate nursing students. The analysis showed high sensitivity (0.84) for the diagnosis of risk for poor performance and high specificity (0.90) for detecting the absence of risk in the care planning. The results suggest the HAKEMP 90 as a diagnostic tool for identifying essential elements of nursing education as active learning, regulation of cognitive processes and relevance of basic nursing care in hospital.

RESUMEN

This is a qualitative, descriptive and exploratory research, aiming to describe the set of ideas and knowledge that guide the use of complementary and integrative health practices (CIHP) by nurses, and analyze the challenges facing the applicability of these therapies by nurses in the hospital setting. The subjects were nurses in public hospital institutions of Rio de Janeiro. Data collection was done through semi-structured interviews and a technique of creativity and sensibility called Almanac. The results showed that the hegemony of the biomedical model in the formation and performance of health professionals hinders implementation of CIHP in hospital. However, as this model does not account for the complexity of the human being emerges the possibility of sharing with CIHP in care aiming, therefore, at comprehensiveness of the human being.

ABSTRACT Objectives: to evaluate the efficacy of a laser acupuncture protocol developed and applied by nurses in arterial hypertension patients. Method: randomized, multicenter, triple-blind and two-armed clinical trial. The sample consisted of 102 participants, 51 per arm, both sexes, aged between 30 and 75 years, undergoing drug therapy for a year or more, with difficulty to control blood pressure, maintaining regular measures >140x90 mmHg. Participants underwent six standard or simulated laser-acupuncture sessions, for 24 minutes, within a period of six weeks. Descriptive analyzes expressed as frequencies of occurrences, means and medians were used, and analysis of the association between variables was performed using Student's t-test and Anova, using Statistica® software, version 12.0. The significance level was set at 5% (alpha=0.05). The comparison between blood pressure measurements was performed using Student's t-test for paired samples and Anova for repeated measures. Results: a significant reduction in systolic (p<0.001) and diastolic (p<0.001) blood pressure was observed among participants in the intervention arm, which was not observed in the simulation arm. Conclusion: the results have demonstrated the efficacy of the protocol. Reduction and control of blood pressure have been demonstrated, indicating the possibility of using this technology for the care of patientes with essential systemic arterial hypertension. RESUMO Objetivos: avaliar a eficácia de um protocolo para laser-acupuntura, desenvolvido e aplicado por enfermeiros em pacientes com hipertensão arterial. Método: ensaio clínico randomizado, multicêntrico, triplo-cego, dois braços. A amostra foi composta por 102 participantes, 51 por braço, ambos os sexos, idade entre 30 e 75 anos, em tratamento medicamentoso há um ano ou mais, com dificuldades para controle da pressão arterial, mantendo medidas regulares >140x90 mmHg. Os participantes foram submetidos a seis sessões de laser-acupuntura, padrão ou simulada, com duração de 24 minutos, no decorrer de seis semanas. Empregaram-se análises descritivas por frequências de ocorrências, médias e medianas, e de associação entre variáveis por teste t de Student e Anova, empregando o software Statistica®, versão 12.0. O nível de significância adotado foi de 5% (alpha=0,05). A comparação entre resultados da pressão arterial foi pelo teste t de Student para amostras pareadas e Anova para medidas repetidas. Resultados: observou-se redução significativa da pressão arterial sistólica (p<0,001) e diastólica (p<0,001) dos participantes do braço intervenção, evento não verificado no braço simulado. Conclusão: pelos resultados constatou-se eficácia do protocolo. Houve redução e controle da pressão arterial, indicando seu emprego como possibilidade de tecnologia para o cuidado de pessoas com hipertensão arterial sistêmica primária. RESUMEN Objetivos: evaluar la eficacia de un protocolo de láser-acupuntura, desarrollado y aplicado por enfermeros en pacientes con hipertensión arterial. Método: ensayo clínico aleatorizado, multicéntrico, triple-ciego, de dos brazos. La muestra quedó conformada por 102 participantes, 51 por brazo, ambos sexos, con edades comprendidas entre los 30 y los 75 años, sometidos a tratamiento con medicamentos durante un año o más, con dificultades para controlar la presión arterial, manteniendo medidas regulares >140x90 mmHg. Los participantes se sometieron a seis sesiones de láser-acupuntura, estándar o simulada, durante 24 minutos, a lo largo de seis semanas. Se utilizaron análisis descriptivos expresados como frecuencias de ocurrencias, medias y medianas, y el análisis de la asociación entre variables se realizó mediante la prueba t de Student y Anova, utilizándose el software Statistica®, versión 12.0. El nivel de significación se fijó en un 5% (alfa=0,05). La comparación entre los resultados de la presión arterial se realizó utilizándose la prueba t de Student para muestras pareadas y Anova, para medidas repetidas. Resultados: se observó una reducción significativa de la presión arterial sistólica (p<0,001) y diastólica (p<0,001) entre los participantes en el brazo intervención, lo que no se observó en el brazo simulado. Conclusión: los resultados han comprobado la eficacia del protocolo. Se ha demostrado la reducción y el control de la presión arterial, lo que indica la posibilidad de utilizar esta tecnología para la atención de pacientes con hipertensión arterial sistémica primaria. RESUMEN

This report is a reflection that marks a change of perspective in the care relation between nurse and client, in the implementation context of the educative process. It emerged a Shared Care Plan as an educational-caring proposal, in the convergence among theorists Paulo Freire and Leininger, regarding the dialogical pedagogy and nursing cultural care. With regard to the elements considered essential to the care, learning together allows the unveiling of a peculiar reality of possibilities for integration and transformation of the reality revealed, by choice of the person. Autonomy planned becomes real, so that customers no longer carry fragmented practices, stemming from traditional pedagogy. The stand-alone client reaches, then, the fullness of the action.

Apresentar os resultados obtidos com o uso da laser-acupuntura como tecnologia aplicada ao cuidado de enfermagem a pessoas hipertensas em tratamento medicamentoso e com dificuldades para o controle da pressão arterial. Método: relato de casos clínicos acompanhados por enfermeiros no âmbito da atenção primária à saúde, articulados a projeto de investigação aprovado por Comitê de Ética em Pesquisa. Resultados: observou-se controle da pressão arterial nas pacientes submetidas à técnica proposta, com redução significativa dos níveis tensionais sistólicos e diastólicos entre a primeira e última intervenção. Conclusão: a laser-acupuntura auxiliou no controle agudo da pressão arterial das participantes do estudo, anunciando-se como possibilidade terapêutica no âmbito do cuidado de enfermagem às pessoas hipertensas.


To present the results obtained with the use of laser-acupuncture as a technology applied to nursing care for hypertensive individual sunder medication treatment and with difficulties in controlling blood pressure. Method: Case reports accompanied by nurses in primary healthcare, articulated research project approved by the Research Ethics Committee. Results: Blood pressure control was observed in patients submittedto the proposed technique with a significant reduction in systolic and diastolic blood pressure levels between the first and last interventions. Conclusion: Laser-acupuncture helped in the acute control of the blood pressure of the study participants, announcing it self as a therapeutic possibility within the scope of nursing care for hypertensive individuals.


Presentar los resultados de la acupuntura láser como tecnología aplicada en la atención de enfermería al paciente hipertenso en tratamiento medicamentoso y con dificultades para controlar la presión arterial. Método: Informe de casos clínicos acompañados por enfermeros en la atención primaria de la salud, articulados a un proyecto de investigación aprobado por el Comité de Ética en Investigación. Resultados: se observó el control dela presión arterial en las pacientes sometidas a la técnica propuesta, con reducción significativa de los niveles tensionales sistólicos y diastólicos entrela primera y la última intervención. Conclusión: la acupuntura láser ayudó en el control agudo de la presión arterial de las participantes del estudio yse revela como alternativa terapéutica en la atención de enfermería al paciente hipertenso.
