PURPOSE: Numerous biomarkers of diabetic kidney disease (DKD) are associated with renal prognosis but head-to-head comparisons are lacking. This study aimed to examine the association of soluble tumor necrosis factor receptor type 1 (sTNFR1), fibroblast growth factor 21 (FGF-21), endocan, N-terminal pro-brain natriuretic peptide (NT-pro-BNP), and renal outcomes of patients with or without clinical signs of DKD. METHODS: A total of 312 patients were enrolled in a prospective observational study that excluded individuals with estimated glomerular filtration rates (eGFR) < 30 mL/min/1.73 m2. Composite renal outcomes included either a > 30% decline in eGFR and worsening albuminuria or both from consecutive tests of blood/urine during a 3.5-year follow-up period. RESULTS: Higher sTNFR1 and FGF-21, rather than endocan and NT-pro-BNP, levels were associated with renal outcomes but the significance was lost after adjusting for confounders. However, sTNFR1 levels ≥ 9.79 pg/dL or FGF-21 levels ≥ 1.40 pg/dL were associated with renal outcomes after adjusting for the confounders (hazard ration [HR] 2.76, 95% confidence interval [CI] 1.36-5.60, p = 0.005 for sTNFR1 level; HR 1.95, 95% CI 1.03-3.69, p = 0.03 for FGF-21 level). The combination of both levels exhibited even better association with renal outcomes than did either one alone (adjusted HR 4.45, 95% CI 1.86-10.65, p = 0.001). The results were consistent among patients with preserved renal function and normoalbuminuria. CONCLUSION: Both sTNFR1 and FGF-21 levels were associated with renal outcomes of in patients with type 2 diabetes, and the combination of the abovementioned markers exhibits better predictability.

RESUMEN

A number of clinical guidelines on nutrition therapy in cancer patients have been published by national and international societies; however, most of the reviewed data focused on gastrointestinal cancer or non-cancerous abdominal surgery. To collate the corresponding data for esophageal cancer (EC), a consensus panel was convened to aid specialists from different disciplines, who are involved in the clinical nutrition care of EC patients. The literature was searched using MEDLINE, Embase, the Cochrane Central Register of Controlled Trials, and the ISI Web of Knowledge. We searched for the best evidence pertaining to nutrition therapy in the case of EC. The panel summarized the findings in 3 sections of this consensus statement, based on which, after the diagnosis of EC, an initial distinction is made between the patients, as follows: (1) Assessment; (2) Therapy in patients with resectable disease; patients receiving chemotherapy or chemoradiotherapy prior to resection, and patients with unresectable disease, requiring chemoradiotherapy or palliative therapy; and (3) Formula. The resulting consensus statement reflects the opinions of a multidisciplinary group of experts, and a review of the current literature, and outlines the essential aspects of nutrition therapy in the case of EC. The statements are: Patients with EC are among one of the highest risk to have malnutrition. Patient generated suggestive global assessment is correlated with performance status and prognosis. Nutrition assessment for patients with EC at the diagnosis, prior to definitive therapy and change of treatment strategy are suggested and the timing interval can be two weeks during the treatment period, and one month while the patient is stable. Patients identified as high risk of malnutrition should be considered for preoperative nutritional support (tube feeding) for at least 7-10 days. Various routes for tube feedings are available after esophagectomy with similar nutrition support benefits. Limited intrathoracic anastomotic leakage postesophagectomy can be managed with intravenous antibiotics and self-expanding metal stent (SEMS) or jejunal tube. Enteral nutrition in patients receiving preoperative chemotherapy or chemoradiation provides benefits of maintaining weight, decreasing toxicity, and preventing treatment interruption. Tube feeding or SEMS can offer nutrition support in patients with unresectable esophageal cancer, but SEMS is not recommended for those with neoadjuvant chemoradiation before surgery. Enteral immunonutrition may preserve lean body mass and attenuates stress response after esophagectomy. Administration of glutamine may decrease the severity of chemotherapy induced mucositis. Enteral immunonutrition achieves greater nutrition status or maintains immune functions during concurrent chemoradiation.

INTRODUCTION: Newborn screening is important for early diagnosis and effective treatment of inborn errors of metabolism (IEM). In response to a 2008 coroners' report of a 14-year-old boy who died of an undiagnosed IEM, the OPathPaed service model was proposed. In the present study, we investigated the feasibility of the OPathPaed model for delivering expanded newborn screening in Hong Kong. In addition, health care professionals were surveyed on their knowledge and opinions of newborn screening for IEM. METHODS: The present prospective study involving three regional hospitals was conducted in phases, from 1 October 2012 to 31 August 2014. The 10 steps of the OPathPaed model were evaluated: parental education, consent, sampling, sample dispatch, dried blood spot preparation and testing, reporting, recall and counselling, confirmation test, treatment and monitoring, and cost-benefit analysis. A fully automated online extraction system for dried blood spot analysis was also evaluated. A questionnaire was distributed to 430 health care professionals by convenience sampling. RESULTS: In total, 2440 neonates were recruited for newborn screening; no true-positive cases were found. Completed questionnaires were received from 210 respondents. Health care professionals supported implementation of an expanded newborn screening for IEM. In addition, there is a substantial need of more education for health care professionals. The majority of respondents supported implementing the expanded newborn screening for IEM immediately or within 3 years. CONCLUSION: The feasibility of OPathPaed model has been confirmed. It is significant and timely that when this pilot study was completed, a government-led initiative to study the feasibility of newborn screening for IEM in the public health care system on a larger scale was announced in the Hong Kong Special Administrative Region Chief Executive Policy Address of 2015.

AIM: Dental restorations have long been used for the management of early childhood caries, but there is a need to have an evidence based approach when selecting the most appropriate restorative intervention to treat dentin caries in preschool children. This systematic review aimed to assess the effectiveness of restorative treatments of dentin caries in primary teeth in preschool children. DESIGN: A systematic search of the main electronic databases (Pubmed, Cochrane Collaboration, EMBASE) was conducted to identify peer reviewed papers published in English in the years 1947-2014. Search keywords and MeSH headings were "dental caries", "primary dentition" and "dental restoration". The inclusion criteria were clinical studies conducted in children under 6 years old, and reported findings on the longevity or failure of restorations in primary teeth. Retrieved papers were read by two reviewers independently to assess suitability for inclusion, and the final decision was made by consensus. The quality of the included studies was assessed and data were extracted for analysis. RESULTS: The search identified 348 papers for screening. Among these, 218 papers did not satisfy the study inclusion criteria. Consequently, 130 full papers were retrieved and reviewed. Finally, 9 papers were included. Most of the trials were assessed as having high risk of bias. Five included studies that compared the success rates of restorations with different filling materials and liner materials. Two studies showed clinical advantages of using minimally invasive approaches in caries removal and cavity preparation. The other two trials showed low success rates of interim GI restorations done in a field setting, compared to the high caries arrest rates of silver diammine fluoride application. CONCLUSION: Within the limitation of this systematic review, there is insufficient evidence to make recommendations regarding which material and technique is the most appropriate for restorative treatment in young children. Minimally invasive approaches are advantageous in operative caries management in primary teeth in preschool children. More well-designed randomised controlled trials are required to confirm these findings.

RESUMEN

We report a high-efficiency hard-X-ray resonator with inclined-incidence geometry. A beam incident at 36.87° with respect to [3 1 0] excites Bragg back diffraction along (12 4 0) at 14.4388 keV for resonance in a Si-based resonator to produce intense resonance fringes. The experimental results showed the visibility enhanced by nearly 30 times compared with normal incidence. Also numerical calculations of the inclined-incidence resonator demonstrate ultrahigh efficiency and extremely narrow resolving power (sub-meV) with low background. This geometry surpasses the intrinsic limits of normal-incidence crystal-based resonators and enables ultrahigh-resolution X-ray optics for X-ray diffraction, spectroscopy, and imaging applications.

This study aimed to evaluate the fluoride uptake of dentine treated with a 38 % silver diamine fluoride (SDF) solution and laser irradiation at sub-ablative energy levels. Fifteen human dentine slices were prepared and divided into four samples each. Four types of laser were chosen: CO2 (10,600 nm), Er:YAG (2,940 nm), Nd:YAG (1,064 nm) and Diode (810 nm). First, the four samples from 12 of the dentine slices were treated with SDF, and then irradiated by one of the four types of laser at three different settings. One sample was untreated and acted as a control. The setting that rendered the highest fluoride uptake was selected. Second, the remaining dentine slices were treated with SDF and irradiated by the four lasers with the selected settings. Fluoride uptake was assessed using Energy Dispersive X-ray Spectrometry at the dentine surface and up to 20 µm below the surface. The selected settings were CO2 irradiation at 1.0 W for 1 s, Er:YAG irradiation at 0.5 W for 20 s, Nd:YAG irradiation at 2.0 W for 1 s and diode irradiation at 3.0 W for 3 s. The fluoride content (weight %) at the dentine surface following CO2, Er:YAG, Nd:YAG and diode irradiation was 6.91 ± 3.15, 4.09 ± 1.19, 3.35 ± 2.29 and 1.73 ± 1.04, respectively. CO2 and Er:YAG irradiation resulted in higher fluoride uptake than Nd:YAG and diode irradiation at all levels (p < 0.05). CO2 laser and Er:YAG laser irradiation rendered higher fluoride uptake in the SDF-treated dentine than Nd:YAG laser and diode laser irradiation.

To evaluate the preventive effect of Er:YAG laser (EYL) irradiation followed by silver diamine fluoride (SDF) application on dentine with cariogenic biofilm challenge. Twenty-four dentine slices were prepared from extracted sound human third molars. Each slice was cut into four parts for SDF application, followed by EYL irradiation (group SL), SDF application (group S), EYL irradiation (group L) and water (group W). The specimens were subjected to cariogenic biofilm challenge for 12 h, followed by immersion in a buffered remineralising solution containing calcium chloride and sodium hypophosphate for 12 h at 37 °C. Surface morphological changes in the specimens were examined using scanning electronic microscopy. Elemental analysis was performed using energy-dispersive X-ray spectrometry. Micro-mechanical properties were investigated by nano-indentation. The specimen surfaces of groups SL and L showed laser melting contours with narrowed dentinal orifices. Group S showed a partial tubular occlusion. A porous surface was observed in group W, indicating demineralisation. The mean (SD) fluoride weight percentages were 3.93 (0.91), 3.10 (0.61), 0.17 (0.09) and 0.32 (0.07) in groups SL, S, L and W, respectively, (p < 0.001; SL, S > L, W). The mean (SD) micro-hardness values in GPa were 1.84 (0.22), 0.49 (0.13), 0.41 (0.11) and 0.30 (0.06) in groups SL, S, L and W, respectively, (p < 0.001; SL > S > L, W). The mean (SD) elastic moduli in GPa were 75.1 (7.2), 20.0 (1.3), 24.3 (5.2) and 20.2 (2.8) in groups SL, S, L and W, respectively, (p < 0.001; SL > S, L, W). SDF application followed by EYL irradiation on a dentine surface increased its resistance to cariogenic biofilm challenge.

OBJECTIVE: The objectives were to design and fabricate an oligopeptide that simulates dentine matrix protein 1 (DMP1) to study its ability to bind to dentine collagen fibrils and induce biomimetic mineralization for the management of dentine hypersensitivity. MATERIALS AND METHODS: A novel oligopeptide was developed by connecting the collagen-binding domain of DMP1 to the hydrophilic C-terminal of amelogenin. Fluorescein isothiocyanate-coupled oligopeptide was applied to the completely demineralized dentine collagen and examined using fluorescent microscopy. The nucleation and growth of hydroxyapatite were initiated by immersing oligopeptide into calcium chloride and sodium hydrogen phosphate solutions. Scanning electron microscopy (SEM), transmission electron microscopy, and selected area electron diffraction (SAED) were used to examine the formation. Dentine slices were acid-etched, coated with oligopeptide, and immersed into a metastable calcium phosphate solution. Dentine mineralization was evaluated by SEM, X-ray diffraction (XRD), and Fourier transform infrared spectroscopy (FTIR). RESULTS: Fluorescent dentine collagen was identified in the specimens. The nucleation and growth of crystals were detected after immersing the oligopeptide into calcium chloride and sodium hydrogen phosphate solutions. Under SEM, crystals were observed covering the oligopeptide-coated dentine surface, within the demineralized dentine collagen matrix and occluding dentinal tubules. SAED, XRD, and FTIR confirmed that the crystals were hydroxyapatite. CONCLUSION: A novel oligopeptide-simulating DMP1 was developed, that can bind to collagen fibrils, initiate mineralization, and induce biomimetic mineralization of dentine. CLINICAL RELEVANCE: Biomimetic mineralization of dentine facilitated by this oligopeptide is a potential therapeutic technique for the management of dentine hypersensitivity.

BACKGROUND: Situated and sociocultural perspectives on learning indicate that the design of complex tasks supported by educational technologies holds potential for dental education in moving novices towards closer approximation of the clinical outcomes of their expert mentors. A cross-faculty-, student-centred, web-based project in operative dentistry was established within the Universitas 21 (U21) network of higher education institutions to support university goals for internationalisation in clinical learning by enabling distributed interactions across sites and institutions. This paper aims to present evaluation of one dental faculty's project experience of curriculum redesign for deeper student learning. METHODS: A mixed-method case study approach was utilised. Three cohorts of second-year students from a 5-year bachelor of dental surgery (BDS) programme were invited to participate in annual surveys and focus group interviews on project completion. Survey data were analysed for differences between years using multivariate logistical regression analysis. Thematic analysis of questionnaire open responses and interview transcripts was conducted. RESULTS: Multivariate logistic regression analysis noted significant differences across items over time indicating learning improvements, attainment of university aims and the positive influence of redesign. Students perceived the enquiry-based project as stimulating and motivating, and building confidence in operative techniques. Institutional goals for greater understanding of others and lifelong learning showed improvement over time. Despite positive scores, students indicated global citizenship and intercultural understanding were conceptually challenging. CONCLUSIONS: Establishment of online student learning communities through a blended approach to learning stimulated motivation and intellectual engagement, thereby supporting a situated approach to cognition. Sociocultural perspectives indicate that novice-expert interactions supported student development of professional identities.

AIM: To study the students' satisfaction with the week-long summer programme and the importance of common influencing factors (IFs) for choosing dentistry as their career. METHODS: Anonymous questionnaire was given to all 214 participants in July of 2011 and 2012. Demographic information including gender, age and education level was collected. The students were asked about their satisfaction with the programme with separate ratings for learning experiences, including hands-on workshops (HOW); clinic observations (CO); problem-based learning tutorials (PBL); and lectures (L). They also rated the relative importance of the ten common IFs. The Friedman test was used to study the order of their preferences of the programme's activities. The Chi-square test was used to study the influence of their demographic factors on the importance of the IFs. RESULTS: A total of 208 students returned their questionnaires. The majority were below the age of 18 (81%), and 44% were studying in an international school. Most of the students (96%) were satisfied with the programme overall. They liked the HOWs and COs more than the PBL tutorials and Ls. 'Altruism' and 'medical/health care career' were the two most important IFs overall. 'Altruism' and 'past experience with dentist' were considered more important by those aged 18 or above. 'Past experience with dentist' and 'working with hands' were considered more important by the international school students. CONCLUSIONS: Most participants were satisfied with the summer programme. They preferred practical, skill-based activities to knowledge-based activities. The importance of some IFs was associated with age and education system.

Cancer patients with terminal stage peritoneal carcinomatosis are often unable to eat, rendering total parenteral nutrition (TPN) as the only option to avoid starvation. In this retrospective study, we reviewed the medical records of 46 patients with peritoneal carcinomatosis and compared them to the records of 51 patients who had gastrointestinal malignancy without evidence of peritoneal carcinomatosis. The factors evaluated include demographic data, cause of primary malignancy, ascites formation, anthropometric measurements, laboratory tests, and outcome measurements as well as factors associated with greater than 90-day survival. In-hospital mortality was observed in 31 of the 46 patients with peritoneal carcinomatosis, with a median survival time of 40 days (4-148 days) for all 46 patients. The median duration of TPN administration in the peritoneal carcinomatosis group was 24.1 ± 27.4 days (3-68 days). Severe infection related to TPN application was seen in 5/46 (10.7%) patients with peritoneal carcinomatosis and 6/51 (9.8%) patients without peritoneal carcinomatosis. The length of survival varied widely among terminal patients with peritoneal carcinomatosis. The average survival time in peritoneal carcinomatosis patients receiving TPN was short, indicating that the nutrition support of TPN was relatively suboptimal. Ascites was not a prognostic factor for peritoneal carcinomatosis, while body mass index was a predictor for 90-day survival.

Hong Kong, a Special Administrative Region of People's Republic of China, is a metropolitan city in Asia with a population of over 7 million people. This paper reflects the current oral health and dental care situations in Hong Kong. Water fluoridation was commenced in 1961, with a current level at 0.5 ppm. And there has continuously been lower caries prevalence thereafter. Dental care is mainly provided by private practitioners. The School Dental Care Service, run by the Department of Health, provides dental care to enrolled primary school children through treatments by dental therapists. An Oral Health Education Unit is set up to promote dental health among the public, particularly preschool children. Government dentists serve mainly civil servants and their dependents. Limited emergency dental care is available to the public at designated government clinics for pain relief, most commonly in the form of extractions. There are about 2200 registered dentists and the dentist to population ratio is about 1:3200. Amongst the dental team, dental hygienists are trained in limited numbers. There are only less than 320 dental hygienists registered, working under the supervision of dentists. The Faculty of Dentistry of the University of Hong Kong has been providing 5-year undergraduate training in dentistry since 1981, and this is lengthened to 6 years from 2012 onwards. Specialty training requires at least a further 6 years. There are 8 specialties, which are Community Dentistry, Endodontics, Family Dentistry, Oral & Maxillofacial Surgery, Oral Rehabilitation, Orthodontics, Pediatric Dentistry, and Periodontics.

Periodontal disease is a significant global health burden affecting half of the world's population. Given that plaque and inflammation control are essential to the attainment of periodontal health, recent trends in preventive dentistry have focused on the use of behavioral models to understand patient psychology and promote self-care and treatment compliance. In addition to their uses in classifying, explaining and predicting oral hygiene practices, behavioral models have been adopted in the design of oral hygiene interventions from individual to population levels. Despite the growing focus on behavioral modification in dentistry, the currently available evidence in the field of periodontology is scarce, and interventions have primarily measured changes in patient beliefs or performance in oral hygiene behaviors. Few studies have measured their impact on clinical outcomes, such as plaque levels, gingival bleeding and periodontal pocket reduction, which serve as indicators of the patient's disease status and quality of oral self-care. The present narrative review aims to summarize selected literature on the use of behavioral models to improve periodontal outcomes. A search was performed on existing behavioral models used to guide dental interventions to identify their use in interventions measuring periodontal parameters. The main models were identified and subsequently grouped by their underlying theoretical area of focus: patient beliefs (health belief model and cognitive behavioral principles); stages of readiness to change (precaution adoption process model and transtheoretical model); planning behavioral change (health action process approach model, theory of planned behavior and client self-care commitment model); and self-monitoring (self-regulation theory). Key constructs of each model and the findings of associated interventions were described. The COM-B model, a newer behavioral change system that has been increasingly used to guide interventions and policy changes, is discussed with reference to its use in oral health settings. Within the limitations of the available evidence, interventions addressing patient beliefs, motivation, intention and self-regulation could lead to improved outcomes in periodontal health. Direct comparisons between interventions could not be made due to differences in protocol design, research populations and follow-up periods. The conclusions of this review assist clinicians with implementing psychological interventions for oral hygiene promotion and highlight the need for additional studies on the clinical effects of behavioral model-based interventions.

Integration of smoking cessation program into routine oral health care has been advocated by World Health Organization since it brings extensive benefits to oral health. By tobacco cessation, patients are less prone to progression of periodontal disease, have less future tooth loss, have reduced risks of oral mucosal lesions and head and neck cancers. Evidence indicates that dentists are in a favorable position to deliver effective smoking cessation advice to improve patients' oral health. This article aims to present the current situation of smoking cessation in dental setting, including dental management of smoking patients, perceptions of dentists and dental students towards smoking cessation, challenges dental professionals face when carrying out cessation interventions. Patients' perspectives are also evaluated to provide a clearer picture of smoking cessation practice in the dental field. Review of past surveys show most patients welcome smoking cessation advice from dental practitioners. Meanwhile dentists may have wrong assumption that patients would disapprove them if they advise patient to quit smoking. On top of that, main obstacles identified are lack of training, inadequate treatment time and insufficient knowledge towards smoking cessation guidelines and referral routes. With regard to the potential barriers, evidence demonstrates that more trainings on smoking cessation strategies are needed. Future research in this aspect is also indicated to further foster the practice of smoking cessation counselling in dental setting.

BACKGROUND: To examine the association of microalbuminuria (MAU) with the carotid intima-media thickness (CIMT) in Chinese Type 2 diabetic subjects. MATERIALS AND METHODS: Two hundred and thirty-nine patients (64±13 yr, 154 males) were divided into 2 groups: one with MAU (no.=119) or one without (no.=120). We recorded clinical and biochemical data as well as CIMT and ankle-brachial index (ABI). RESULTS: The patients with MAU had had diabetes mellitus (DM) longer, had higher blood pressure (BP). They also had lower estimated glomerular filtration rate (eGFR) and higher levels of circulating glucose, glycated hemoglobin, high sensitivity C-reactive protein than those without. Lower mean ABI was found in those with MAU, however, they did not have higher mean CIMT (0.72±0.15 vs 0.71±0.16 mm, p=0.525). In patients without MAU, CIMT correlated with age, DM duration, systolic BP, eGFR, albumin- to-creatinine ratio, and ABI. However, in those with MAU, CIMT correlated only with age and eGFR. Multivariate regression analysis revealed that mean CIMT correlated only with age for patients without MAU, but correlated with age and body mass index for those with MAU. Dividing the patients into 5 age groups, we found that the older the patient, the higher the mean CIMT with no group differences between those with and without MAU in both genders. However, patients with eGFR below 60 ml/min/1.73 m(2) had higher mean CIMT than those above (0.75±0.16 vs 0.69±0.14 mm, p=0.005). CONCLUSIONS: Type 2 diabetic patients with MAU were not associated with higher CIMT. Conversely, those with deterioration of renal function were more likely associated.

AIM: Food allergy is common in children and adults, and could be potentially fatal in minor groups. It is important for physicians to identify the prevalence of food allergies and to recognise common food allergens to make precise diagnosis and choose correct therapeutic approaches. METHODS: We used a nationwide, cross-sectional, random questionnaire-based survey to estimate the self-reported and expert-screened prevalence of food allergies and to identify the common food allergens in Taiwan. In this study, the perceptional diagnosis of food allergies was screened by physicians according to descriptions of convincing symptoms and medical recordings; in the meantime, non-allergic adverse reactions to foods, including food intolerance or food avoidance, were clarified. RESULTS: A total of 30 018 individuals who met the inclusion criteria was evaluated, and 6.95% of them were diagnosed as victims of food allergies. The prevalence was 3.44% in children under 3 years of age, 7.65% in children aged 4-18 years and 6.40% in adults respectively. About 77.33% of the food allergy population had experienced recurrent allergic attacks. Systemic reactions happened about 4.89% in food allergies group. The most commonly reported food allergen in Taiwan is seafood, including shrimp, crab, fish and mollusc. In addition, mango, milk, peanuts and eggs were also important food allergens in the general population; while milk, shellfish, peanuts and eggs were common in children. CONCLUSIONS: Less than 10% of the Taiwan population suffers from food allergy with different allergic symptoms to variable food allergens in different age groups.

AIM: To evaluate the fracture resistance and fracture patterns of root filled maxillary first premolar teeth (with mesio-occlusal cavity preparation) restored with several composite restoration designs. METHODOLOGY: One hundred extracted sound human maxillary first premolars were randomly divided into five groups. Group 1 remained untreated (negative control). Conventional root canal treatment with additional mesial-occlusal cavity preparation was carried out on teeth in groups 2-5. In group 2, the teeth were restored intra-coronally with direct composite resin (positive control). In group 3, the palatal cusps of the teeth were reduced, and the cavities were restored with composite resin covering the palatal cusp (partial coverage). In group 4, the buccal and palatal cusps along with the distal marginal ridges were reduced; the cavities and cusps were restored with composite resin (conventional full coverage). In group 5, the buccal and palatal cusps were reduced but the distal marginal ridges were conserved. The cavities and the cusps were restored with composite resin (modified full coverage). All teeth were subjected to a progressive compressive loading parallel to their longitudinal axis until fracture. Fracture resistance was analysed using the one-way ANOVA and Fisher's LSD test. Fracture patterns were analyzed with chi-square test. The significance level was set at 0.05. RESULTS: The fracture resistance (mean ± SD) of groups 1-5 was 1131 ± 207N, 904 ± 184N, 927 ± 224N, 1095 ± 289N and 1085 ± 243N, respectively (groups 1, 4, 5 > groups 2, 3; P = 0.004). Cusp fractures were recorded as the fracture pattern in 20 (100%), 19 (95%), 16 (80%), 8 (40%) and 12 (60%) premolars in groups 1-5, respectively (groups 1, 2 > groups 4, 5; group 3 > group 4; P < 0.001). CONCLUSIONS: When direct composite resin was used to restore root filled maxillary first premolar teeth involving a proximal surface, those restored with full-coverage designs had greater fracture resistance.
