RESUMEN

PURPOSE: To evaluate pattern electroretinogram (pattern ERG) and anatomical changes in optical coherence tomography (OCT) in acute retinal vein occlusion with macular oedema after intravitreal triamcinolone acetonide (IVTA) injection. METHODS: In this prospective interventional study, 20 patients with acute retinal vein occlusion (branch and central) of less than 1 month duration were evaluated for pattern ERG, best-corrected visual acuity (BCVA), central macular thickness on OCT, and contrast sensitivity (CS) before and 1, 6 and 12 weeks after 1 mg IVTA injection. RESULTS: The amplitude of P50 wave (pattern ERG) improved from 3.01 ± 1.22 to 3.56 ± 1.29 µv, mean logMAR BCVA improved from 1.10 ± 0.60 to 0.69 ± 0.45, and CS improved from 0.45 ± 0.41 to 0.79 ± 0.29; mean central macular thickness (CMT) decreased from 515.35 ± 202.83 to 358.35 ± 135.4µ at 12 weeks. All the values were statistically significant (p value < 0.001). CONCLUSION: IVTA injection in acute retinal vein occlusion with macular oedema results in electrophysiological (pattern ERG) improvement in addition to anatomical (OCT) improvement.

RESUMEN

PURPOSE: To compare the structural and functional outcome of use of autologous heparinized whole blood before staining internal limiting membrane with brilliant blue (BB) versus conventional BB-assisted macular hole surgery. METHODS: Sixty eyes of 60 patients were randomly divided equally in Group A (BB staining using whole blood) and Group B (conventional BB staining). Clinical assessment and spectral domain optical coherence tomography was done at baseline and 3 weeks, 6 weeks, 16 weeks, and 6 months postoperatively. RESULTS: Group A eyes had a significantly higher best-corrected visual acuity as compared with Group B postoperatively (P < 0.001, <0.001, 0.004, 0.04 at 3, 6, 16 weeks, and 6 months). Inner segment/outer segment junction continuity was noted in greater number of eyes in Group A compared with Group B (P = 0.02, 0.002, 0.003, and 0.03 at 3, 6, 16 weeks, and 6 months). Eyes in Group A had significantly higher outer foveal thickness at 3 weeks (P = 0.001) and 6 weeks (P < 0.001) compared with Group B. CONCLUSION: Use of whole blood before staining internal limiting membrane with BB causes earlier and better visual rehabilitation postoperatively, which could be attributed to earlier photoreceptor regeneration as evidenced by inner segment/outer segment junction continuity and increase in outer foveal thickness.

BACKGROUND: To evaluate the pattern electroretinogram (PERG) in central serous retinopathy. METHODS: In this prospective study, 20 patients with recent onset (<6 weeks) unilateral CSR were studied with the fellow eye acting as control. BCVA, psychophysical parameters, FFA, OCT, flash and pattern ERG were evaluated. RESULTS: The mean P50 amplitude in the affected eye at presentation was 2.36 ± 0.8 µv compared to 3.24 ± 0.85 µv in the normal eye. At 12 weeks, the mean P50 amplitude in the affected eye was 3.16 ± 0.88 µv compared to 3.63 ± 0.82 µv in the normal eye. The reduction of 27 % in P50 amplitude even after recovery was statistically significant (P < 0.001). A reduction of 19.77 % in the amplitude of N95 wave in the affected eye compared to the normal eye was seen, which was statistically significant (P < 0.001). In the affected eye, there was statistically significant improvement in the psychophysical tests at 12 weeks. CONCLUSIONS: This study reveals that P50 component of the PERG is reduced in cases of CSR, indicating functional disturbance of the macular photoreceptors. A normal flash ERG indicates that pathology is electrophysiologically localised to macula without affecting general retina. Residual deficit in the PERG explains poor quality of vision in spite of normal anatomical apposition and quantitative 6/6 visual acuity.

RESUMEN

The incidence of endophthalmitis after pars plana vitrectomy is low. Silicone oil is a tamponading agent which has anti-microbial activity. Post-operative endophthalmitis following vitrectomy with silicone oil tamponade has been rarely reported. We describe the case of a young male who underwent pars plana vitrectomy with silicone oil for retinal detachment with a giant retinal tear. He developed a clinical picture suggestive of endophthalmitis on the first post-operative day, and vitreous culture grew multidrug-resistant Pseudomonas aeruginosa. He was treated with intravitreal piperacillin/tazobactam, along with appropriate surgical management. This was followed by resolution of the infection with a remarkable improvement in visual acuity. This is the first case of multidrug-resistant Pseudomonas aeruginosa endophthalmitis following pars plana vitrectomy with silicone oil treated successfully with piperacillin/tazobactam.

RESUMEN

Retinal astrocytic hamartomas are benign tumors of the retinal nerve fiber layer (NFL) that may be found in association with tuberous sclerosis (TS). We describe the spectral-domain optical coherence tomography (SD-OCT) features of type 1 astrocytic hamartomas in a child with TS that may aid in the diagnosis and follow-up of these lesions. Gradual, dome-shaped hyper-reflective elevations of the retinal NFL were seen on SD-OCT with subtle shadowing of the underlying tissues. In addition, the vitreous cortex showed focal adhesions to the surface of the tumor.

RESUMEN

Rehabilitation of a post-exenterated orbital defect is a necessity, to restore a patient's esthetic appearance and help overcome the associated psychosocial stigma. An implant supported prosthesis enjoys a better patient acceptance due to its superior retention and stability. This clinical report highlights the challenges faced in planning, designing and placement of maxillofacial implants in the supra-orbital rim of an exenterated socket post-radiotherapy along with the management of the unexpected complications which developed subsequently. Administration of hyperbaric oxygen therapy, fabrication of a digitally designed surgical guide to ensure predictable implant placement, selection of surface treated implants for better biomechanical anchorage, and a gentler surgical technique for recovery of irradiated hard and soft tissues were measures undertaken during the treatment phase. An effort has been made to point-out the fact that despite the various approaches adopted in an irradiated patient, success of implant placement in such a situation remains a challenge.

RESUMEN

The authors report an unusual case of Marcus Gunn syndrome in an adult patient, characterized by classical Marcus Gunn on one side and inverse Marcus Gunn on the other side. The patient exhibited a "see-saw" kind of eyelid movement on opening and closing the mouth and on moving the jaw from side to side. This clinical entity has been reported very rarely. A similar but etiologically different acquired condition seen commonly in adults, Marin Amat syndrome, is also discussed.

A 58-year-old male presented with decreased visual acuity from persistent diffuse diabetic macular edema despite multiple interventions including laser photocoagulation and intravitreal triamcinolone acetonide injections. Following treatment with 1.25 mg of intravitreal bevacizumab, reduction in macular leakage was shown by fluorescein angiography and resolution of edema was shown by optical coherence tomography (OCT); however, visual acuity deteriorated and fluorescein angiograms demonstrated progressive enlargement of the foveal avascular zone (FAZ), which had shown no abnormalities before the injection. Despite normalisation of macular thickness on OCT and cessation of leakage on angiography, visual acuity remained at the same level until 6 months of follow-up. Intravitreal bevacizumab must be used with caution in patients with chronic, persistent diabetic macular edema. Disruption of the fragile fine foveal network of capillaries with induction of macular ischemia may result following treatment even if there are no pre-existing defects in the FAZ and may be responsible for non-improvement or decrease in visual acuity.

RESUMEN

An 18-year-old male presented to the ophthalmology department with bilateral total subluxated cataract. On systemic examination, he was found to have grossly short stature, multiple severe bony deformities, hypogonadism and partial adontia. Detailed work-up revealed a biochemical and radiologic picture consistent with osteomalacia, which had resulted from malabsorption secondary to celiac disease, confirmed histopathologically. To the best of our knowledge, this is the first reported case of bilateral cataract as the presenting feature of celiac disease. Malabsorption leading to hypocalcemia along with chronic diarrhea itself may have contributed to the development of bilateral cataract.

To evaluate the role of intravitreal bevacizumab in the treatment of Coats' disease diagnosed in adulthood. Prospective, interventional case series. Three patients with Coats' disease diagnosed in adulthood were managed with a single intravitreal injection of bevacizumab (1.25 mg) with peripheral laser photocoagulation 3 weeks later. All three patients had exudation at the macula (Stage 2B) along with peripheral retinal telangiectasia and aneurysms. They were followed up for 9 months. An appreciable reduction in the exudation at the macula and macular edema was observed in all cases following intravitreal bevacizumab therapy. In all patients, the visual acuity improved, and no signs of recurrence were observed at the final follow-up at 9 months. Intravitreal bevacizumab injections may be effective as an adjunctive treatment for adult-onset Coats' disease with foveal exudation along with laser photocoagulation to the peripheral retinal vascular abnormalities. Intravitreal bevacizumab was well tolerated by our patients, and no adverse effects were observed.

RESUMEN

An unusual case of traumatic maculopathy following blunt trauma with a sugarcane stick is reported in a 12-year-old boy. An area of parafoveal exudates associated with decrease in visual acuity was observed in the affected eye of the patient. Baseline parameters related to the best-corrected visual acuity, visual fields, fundus photographs, fluorescein angiography, and macular optical coherence tomography were performed and the patient was observed for 6 months with conservative management. There was complete resolution of the exudates with commensurate increase in the best-corrected visual acuity over this period. This unusual presentation of traumatic maculopathy is discussed along with the role of optical coherence tomography in such cases.

RESUMEN

Two siblings who were born out of consanguineous marriage in an Indian family and had typical clinical manifestations of Sjögren-Larsson syndrome were found to have normal electroretinogram and abnormal visual evoked potential results. Abnormal visual evoked potential results along with the presence of distinctive foveal and parafoveal deposits, which could have a prognostic bearing on the visual acuity of the patients, emphasize the importance of a complete ophthalmological and electrophysiological evaluation in all patients with Sjögren-Larsson syndrome.

BACKGROUND: The purpose of this study was to evaluate subthreshold diode micropulse (SDM) laser as a treatment modality in acute central serous chorioretinopathy (CSC) and compare it with the current standard of care (observation). METHODS: A randomised controlled trial was conducted on 68 eyes (34 eyes in SDM laser group and 34 eyes in observation group) with acute CSC, with a single angiographic leak and duration of complaints less than two months. Detailed history, examination and investigations were performed at the baseline and at regular intervals until six months. RESULTS: Eyes in the laser group had significantly higher best-corrected visual acuity at two weeks (p = 0.002), four weeks (p < 0.001), eight weeks (p < 0.001), 16 weeks (p = 0.042) and six months (p = 0.008), and higher contrast sensitivity at eight weeks (p = 0.008), 16 weeks (p < 0.001) and six months (p < 0.001). A recurrent/persistent neurosensory detachment was observed at the end of six months in 11.76 per cent of SDM laser treated eyes versus 29.41 per cent of eyes in the observation group (p = 0.036). CONCLUSION: SDM laser produces faster and superior visual rehabilitation without any adverse effects. It also reduces the chances of CSC going into chronicity and recurrence compared to the current standard of care (observation).

PURPOSE: The purpose of the study was to describe macular changes in treatment-naïve eyes with Eales disease using spectral domain optical coherence tomography (SD-OCT). METHODS: A cross-sectional study was done on 79 eyes of 66 patients with Eales disease. Best-corrected visual acuity (BCVA), slit-lamp biomicroscopy (SLB), indirect ophthalmoscopy, fundus fluorescein angiography (FFA), and quantitative (central macular thickness [CMT]) and qualitative analysis on SD-OCT were performed. RESULTS: Forty-six (58.2%) eyes had macular involvement as assessed with SD-OCT, while in 33 (41.8%) eyes, macula was not affected. Macular edema was the most common feature when macula was affected followed by epiretinal membrane. Mean CMT was higher (315.3 ± 102.3 µm) in eyes with macular involvement than those without it (243.8 ± 19.3 µm). Eyes with active vasculitis involving larger vessels and neovascularization had greater chance of macular involvement. SLB and FFA alone missed 28.3% and 50% eyes with macular abnormalities on SD-OCT, respectively. CONCLUSION: While the clinical description of Eales disease points mainly to a peripheral location, macular involvement can be commonly picked up when SD-OCT is used. Macular involvement when present is associated with a poorer BCVA.

RESUMEN

A "congenital retinal macrovessel" (CRM) is an aberrant retinal vessel (frequently a vein) that traverses the central macula and supplies or drains both above and below the horizontal raphe. It is an uncommon entity that is usually disclosed on routine examination and may rarely cause a compromised visual acuity. We describe the case of an adolescent girl who underwent vitrectomy for vitreous hemorrhage and was found to have a CRM with vascular abnormalities. To our knowledge, this is the first report of vitreous hemorrhage secondary to a CRM.

Retinal neovascularization in Diabetes mellitus (DM) is commonly seen at the optic disc and mid-periphery. It is rare at the fovea as the fovea is an avascular zone and there are very few case reports of foveal neovascularization in Type 1 DM. We report a case of unilateral foveal neovascularization in Type 2 DM.

PURPOSE: To report a case of exudative retinal detachment as the presenting feature of tuberous sclerosis complex. METHODS: A 14-year-old girl presented with loss of vision in the right eye for 1 month. Visual acuity was no perception of light in the right eye and 20/20 in the left eye. Clinical examination, fundus fluorescein angiography, spectral domain optical coherence tomography, and ultrasound B-scan was performed along with complete systemic evaluation. RESULTS: On examination, the right eye had an exudative retinal detachment and the left eye had multiple lesions suggestive of retinal astrocytic hamartomas. Ultrasonography of the right eye revealed a total retinal detachment with subretinal exudates and an acoustically solid mass lesion in the inferonasal quadrant, whereas that of the left eye detected a small mass in inferonasal quadrant. Fundus fluorescein angiography of the left eye revealed staining in the late phase in the lesions. Spectral domain optical coherence tomography taken through the lesion demonstrated an elevated lesion with high reflectivity arising from inner retinal layers and causing backshadowing. On systemic examination, she had multiple skin colored bumps on cheeks and nose and multiple hypomelanotic macules on lower legs. Contrast-enhanced magnetic resonance imaging brain revealed features suggestive of multiple cortical tubers. Contrast-enhanced magnetic resonance imaging orbits showed a T2-hypointense nodule in the right globe medially with intense postcontrast enhancement, and contrast-enhanced magnetic resonance imaging abdomen detected multiple renal cysts suggestive of angiomyolipoma. CONCLUSION: Retinal astrocytic hamartomas in association with tuberous sclerosis complex is considered to be a relatively stationary lesion that has little potential for aggressive behavior. RESUMEN

PURPOSE: To describe an unusual retinal manifestation of dengue fever in an endemic region. CASE REPORT: A 35 year old male presenting with acute onset decreased vision in his right eye, was found to have a massive retinal pigment epithelial detachment (PED) extending up to the vascular arcades. He had been diagnosed with acute hypokalemic quadriparesis in dengue fever in the preceding week, which had resolved following treatment. The patient was managed conservatively. At three months follow up, there was spontaneous flattening of the PEDs with improvement in visual acuity. CONCLUSION: Dengue fever complicated by acute hypokalemic quadriparesis can be associated with PED, which can be large. The condition resolves spontaneously and bears a good prognosis.

RESUMEN

Neurofibromatosis type 1 (NF-1) is an autosomal dominantly inherited disease characterized by café-au-lait spots, neurofibromas, axillary freckling, Lisch nodules of iris, gliomas and various systemic vascular ischemic manifestations mainly in the aorta, brain and kidney. Retinal vascular manifestations in patients with NF-1 are usually representative of retinal capillary hemangiomatosis. Few cases of NF-1 with retinal vascular occlusive disease have been described. We describe a young Indian woman with NF-1 with unilateral peripheral retinal ischemia but no vascular abnormality at the posterior pole.
