RESUMEN

Neutrophilic panniculitis encompasses a heterogeneous group of diseases histopathologically characterized by an inflammatory infiltrate in the subcutaneous fat mainly composed of mature neutrophils. This group of panniculitides includes alpha(1)-antitrypsin deficiency, infectious panniculitis, factitious panniculitis, subcutaneous Sweet syndrome, neutrophilic/pustular panniculitis associated with rheumatoid arthritis, erythema nodosum-like lesions of Behçet disease, bowel bypass panniculitis, and iatrogenic panniculitis. This article reviews subcutaneous Sweet syndrome, which is a rare idiopathic panniculitis characterized by a dense neutrophilic infiltrate in the subcutis and is often related to hematologic malignancies. The relationship of subcutaneous Sweet syndrome and erythema nodosum is discussed as well as the differential diagnosis with other neutrophilic panniculitis.

Alopecia is one of the clinical manifestations of secondary syphilis. It is uncommon for hair loss to be the sole or predominant manifestation, as hair loss is the chief clinical and histologic differential diagnosis of alopecia areata. The main difference between these two entities is the detection of Treponema pallidum in syphilis. We present the case of a 24-year-old Hispanic man, human immunodeficiency virus seropositive in treatment, with tiny patches of non-cicatricial alopecia in the parieto-occipital regions of his scalp. The patient denied previous history of genital or other skin lesions. A biopsy from an alopecic patch was performed which showed an inflammatory non-scarring alopecia with a discrete lymphocytic type inflammatory infiltrate localized in the peribulbar region. There was lymphocyte exocytosis into the matrix, associated with vacuolar degeneration, and scattered apoptotic cells were observed. Plasma cells were scattered. Immunohistochemical studies showed the presence of T. pallidum limited to the peribulbar region and penetrating into the follicle matrix. To the authors' knowledge, this is the first time that spirochetes have been shown in the hair follicle in alopecia syphilitica, suggesting that the spirochetes may be pathogenetic and responsible for the alopecia.

RESUMEN

Epidermal growth factor receptor (EGFR) inhibitors are a new group of drugs used in the treatment of several malignancies. Three molecules are approved at the moment: gefitinib and erlotinib for the treatment of non-small-cell lung cancer, and cetuximab for colorectal cancer. These drugs originate cutaneous side effects with a high frequency: acneiform rashes, paronychia and generalized xerodermia. In this paper we review these common side effects and how to manage them.

RESUMEN

Spiny keratoderma of the palms is an infrequent entity of unknown etiology. Most of the cases described are acquired, but there are also family cases. This dermatosis is characterized by the appearance of hyperkeratotic, generally asymptomatic, projections on the palms and/or soles, measuring only a few millimeters. In the histological study, parakeratotic columns of cornoid lamellae were observed on an epidermis with a thin or absent granular layer. Its association with malignant tumor pathologies of different types has been described. We present a new case of this entity, which had the peculiarity of a relationship of the parakeratotic column with the acrosyringium in some histological sections, a finding typical of a porokeratotic eccrine ostial or dermal duct nevus.

Las vasculitis son un grupo de enfermedades que se caracterizan por presentar inflamación de los vasos sanguíneos y pueden afectar a cualquier tipo de vaso del organismo. La púrpura de Schönlein-Henoch es un tipo de vasculitis secundaria a hipersensibilidad, que afecta a los vasos de pequeño tamaño. Debido a la rareza de la asociación, se presenta el caso de un paciente con púrpura cutánea y afectación intestinal


Vasculitides are a group of diseases characterized by inflammation of the blood vessels and can affect any type of blood vessel in the body. Schönlein-Henoch purpura is a type of vasculitis secondary to hypersensitivity that affects the small blood vessels. Due to the rarity of this association, we present the case of a male patient with cutaneous purpura and intestinal involvement

Los inhibidores del receptor del factor de crecimiento epidérmico (EGFR) son una nueva clase de fármacos para el tratamiento de diversas neoplasias. En la actualidad existen tres fármacos aprobados: gefitinib y erlotinib para el carcinoma de pulmón no de células pequeñas, y cetuximab para el cáncer de colon. Estas sustancias producen con mucha frecuencia efectos adversos cutáneos entre los que se encuentran entre otros las erupciones acneiformes, las paroniquias, la xerodermia generalizada. En el presente trabajo se realiza una revisión de estos efectos adversos así como de su tratamiento
