We collected infant food samples from 714 households in Kisumu, Kenya, and estimated the prevalence and concentration of Enterococcus, an indicator of food hygiene conditions. In a subset of 212 households, we quantified the change in concentration in stored food between a morning and afternoon feeding time. In addition, household socioeconomic characteristics and hygiene practices of the caregivers were documented. The prevalence of Enterococcus in infant foods was 50% (95% confidence interval: 46.1 - 53.4), and the mean log10 colony-forming units (CFUs) was 1.1 (SD + 1.4). No risk factors were significantly associated with the prevalence and concentration of Enterococcus in infant foods. The mean log10 CFU of Enterococcus concentration was 0.47 in the morning and 0.73 in the afternoon foods with a 0.64 log10 mean increase in matched samples during storage. Although no factors were statistically associated with the prevalence and the concentration of Enterococcus in infant foods, household flooring type was significantly associated with an increase in concentration during storage, with finished floors leading to 1.5 times higher odds of concentration increase compared to unfinished floors. Our study revealed high prevalence but low concentration of Enterococcus in infant food in low-income Kisumu households, although concentrations increased during storage implying potential increases in risk of exposure to foodborne pathogens over a day. Further studies aiming at investigating contamination of infant foods with pathogenic organisms and identifying effective mitigation measures are required to ensure infant food safety.

Escherichia coli O157 is a Shiga toxin-producing E. coli causing disease in humans. Cattle are the primary reservoir of the pathogen. Information regarding the contribution of cattle to diarrheal illnesses in humans through consumption of contaminated beef is scarce in Ethiopia. We collected samples from 240 cattle, 127 beef, and 216 diarrheic patients in Bishoftu town in Ethiopia to assess the occurrence and determine the virulence genes, genetic relatedness, and antimicrobial resistance of E. coli O157. E. coli O157 was detected in 7.1% of the rectal content samples from cattle in slaughterhouses, in 6.3% (n = 127) of the beef samples, and in 2.8% of the diarrheic patients' stool samples. All isolates were positive for eae gene, 24 (77%) of them were positive for stx2 gene (21 stx2c and 3 stx2a), whereas stx1 gene was not detected. Molecular typing grouped the isolates into eight pulsed-field gel electrophoresis pulsotypes with three pulsotypes containing isolates from all three sources, one pulsotype containing one isolate from human origin and one isolate from beef. The remaining four pulsotypes contained isolates unique either to beef or to humans. With the exception of 1 multidrug-resistant isolate from beef, which was resistant to 8 antimicrobial drugs, the remaining 30 isolates were susceptible to the 14 antimicrobials tested. In conclusion, the finding of genetically similar isolates in cattle, beef, and humans may indicate a potential transmission of E. coli O157 from cattle to humans through beef. However, more robust studies are required to confirm this epidemiological link.

Within Ethiopia, there is a lack of information on the genetic relatedness of Salmonella from cattle, beef, and diarrheic patients and its potential transmission from cattle to humans through consumption of contaminated beef. The objective of this study was to assess the prevalence and determine the serotypes, genetic relatedness, and antimicrobial resistance of Salmonella in cattle in two local slaughterhouses, in beef at retail shops, and in diarrheic patients in the only hospital in Bishoftu, Ethiopia. Salmonella was detected in 2.5% (6/240) of cattle samples, in 8.7% (11/127) of beef samples, and in 2.3% (5/216) of the diarrheic patients. Four Salmonella serotypes: Salmonella Typhimurium, Salmonella Eastbourne, Salmonella Saintpaul, and Salmonella Cotham were identified. Salmonella Typhimurium and Salmonella Eastbourne were isolated from cattle and beef, whereas Salmonella Saintpaul and Salmonella Cotham were isolated only from diarrheic patients. Except for serotype Salmonella Saintpaul, all isolates were grouped into five pulsotypes, of which two pulsotypes contained isolates from cattle and beef. Isolates from humans represented unique pulsotypes. Among the 22 Salmonella isolates tested, 95.5% were resistant to at least 1 of the 14 antimicrobials tested. Three Salmonella isolates originating from cattle were multidrug resistant. One human isolate was susceptible to all antimicrobials tested. More specifically, resistance to ampicillin, sulfamethoxazole, tetracycline, tigecycline, and trimethoprim were observed. The most frequently observed resistance was to sulfamethoxazole (90.9%, 20/22) followed by trimethoprim (22.7%, 5/22). The study revealed considerable Salmonella contamination of beef at retail shops, antimicrobial resistance to commonly used antimicrobials, and shared genetically similar Salmonella serotypes between cattle and beef; the link with humans could not be established. Still, the findings of Salmonella in cattle and beef, the propensity of transfer of Salmonella from cattle to beef coupled with the common consumption of raw/undercooked beef are likely to pose public health risk in Ethiopia.

Contaminated lake water and fish can be sources of bacterial pathogens of public health concern, including pathogenic E. coli. Within Ethiopia, specifically, Central Oromia, raw fish consumption is a common practice. Although there are few reports on occurrence of E. coli O157 in fish destined for human consumption and children under five years, information on the transmission pathways of E. coli O157 and other sorbitol non-fermenting (SN-F) E. coli from water-to-fish-to-human, and their virulence factors and antimicrobial resistant determinants along the fish supply chain is lacking. The study aimed to investigate the occurrence, molecular characteristics, and antimicrobial susceptibility of E. coli O157 and other SN-F E. coli strains in fish, lake water and humans in central Oromia, Ethiopia. A total of 750 samples (450 fish samples, 150 water samples, 150 human stool samples) were collected from five lakes and three health facilities. The samples were processed following the standard protocol recommended by European Food Safety Authority and Kirby-Bauer disc diffusion method for detection of the bacteria, and antimicrobial susceptibility tests, respectively. Molecular characterization of presumptive isolates was performed using Whole-Genome Sequencing (WGS) for serotyping, determination of virulence factors, antimicrobial resistance traits, and genetic linkage of the isolates. Overall, 3.9% (29/750) of the samples had SN-F E. coli; of which 6.7% (n = 10), 1.8% (n = 8) and 7.3% (n = 11) were retrieved from water, fish, and diarrheic human patients, respectively. The WGS confirmed that all the isolates were SN-F non-O157: H7 E. coli strains. We reported two new E. coli strains with unknown O-antigen from fish and human samples. All the strains have multiple virulence factors and one or more genes encoding for them. Genetic relatedness was observed among strains from the same sources (water, fish, and humans). Most isolates were resistant to ampicillin (100%), tetracycline (100%), cefotaxime (100%), ceftazidime (100%), meropenem (100%), nalidixic acid (93.1%) and sulfamethoxazole/trimethoprim (79.3%). Majority of the strains were resistant to chloramphenicol (58.6%) and ciprofloxacin (48.3%), while small fraction showed resistance to azithromycin (3.45%). Isolates had an overall MDR profile of 87.5%. Majority, (62.1%; n = 18) of the strains had acquired MDR traits. Genes encoding for mutational resistance and Extended-spectrum beta-lactamases (ESBL) were also detected. In conclusion, our study revealed the occurrence of virulent and MDR SN-F E. coli strains in water, fish, and humans. Although no genetic relatedness was observed among strains from various sources, the genomic clustering among strains from the same sources strongly suggests the potential risk of transmission along the supply chain at the human-fish-environment interface if strict hygienic fish production is not in place. Further robust genetic study of the new strains with unknown O-antigens, and the epidemiology of SN-F E. coli is required to elucidate the molecular profile and public health implications of the pathogens.

Antibiotics must be safe and effective for use in both human and veterinary medicine. However, information about the efficacy of different brands of antibiotics commonly used in veterinary practices is lacking in Ethiopia. In this study, we determined the efficacy of three brands of penicillin-streptomycin (Pen&strep, Penstrep, and Pro&strep) by performing antimicrobial susceptibility testing against Staphylococcus aureus isolated from cow milk from dairy farms in the towns of Sebata and Bishoftu, Central Ethiopia. We also assessed the knowledge, attitudes, and practices (KAP) of veterinarians regarding the quality and use of brand antibiotics and the antibiotic utilization practices of dairy farm personnel using a structured questionnaire. Of 43 S. aureus isolated and tested, 33 (77%), 10 (23%), and 1 (2%) were susceptible to brands A, B, and C, respectively. According to the respondents, all of them reported that penstrep is the most prescribed antibiotic in dairy farms (100%), followed by oxytetracycline (78%) and sulfa drugs (52%). All veterinarians perceived that antibiotics imported from Western countries have a higher efficacy than those from Eastern countries, and they preferred brand A to the other brands, witnessing its better clinical outcome. The majority (87%) and a little more than half (53%) of the respondents perceived the overuse of antibiotics in veterinary clinics and dairy farms, respectively. Our study revealed the better efficacy of brand A against S. aureus compared to the other brands. Interestingly, the veterinarians' perception of and preference toward the use of brand antibiotics agreed with the findings of our antibacterial susceptibility testing. The prudent use of brand A is critically important for sustaining effective treatment, avoiding the risk of antimicrobial resistance, and helping to address animal welfare issues.

INTRODUCTION: Global morbidity from enteric infections and diarrhoea remains high in children in low-income and middle-income countries, despite significant investment over recent decades in health systems and water and sanitation infrastructure. Other types of societal development may be required to reduce disease burden. Ecological research on the influence of household and neighbourhood societal development on pathogen transmission dynamics between humans, animals and the environment could identify more effective strategies for preventing enteric infections. METHODS AND ANALYSIS: The 'enteric pathome'-that is, the communities of viral, bacterial and parasitic pathogens transmitted from human and animal faeces through the environment is taxonomically complex in high burden settings. This integrated cohort-exposure assessment study leverages natural socioeconomic spectrums of development to study how pathome complexity is influenced by household and neighbourhood infrastructure and hygiene conditions. We are enrolling under 12-month-old children in low-income and middle-income neighbourhoods of two Kenyan cities (Nairobi and Kisumu) into a 'short-cohort' study involving repeat testing of child faeces for enteric pathogens. A mid-study exposure assessment documenting infrastructural, behavioural, spatial, climate, environmental and zoonotic factors characterises pathogen exposure pathways in household and neighbourhood settings. These data will be used to inform and validate statistical and agent-based models (ABM) that identify individual or combined intervention strategies for reducing multipathogen transmission between humans, animals and environment in urban Kenya. ETHICS AND DISSEMINATION: The protocols for human subjects' research were approved by Institutional Review Boards at the University of Iowa (ID-202004606) and AMREF Health Africa (ID-ESRC P887/2020), and a national permit was obtained from the Kenya National Commission for Science Technology and Innovation (ID# P/21/8441). The study was registered on Clinicaltrials.gov (Identifier: NCT05322655) and is in pre-results stage. Protocols for research on animals were approved by the University of Iowa Animal Care and Use Committee (ID 0042302).

Fishborne diseases are among the major causes of morbidity and mortality worldwide. Contamination of the aquatic ecosystem and unhygienic handling practices along the fish supply chain can lead to a contaminated fish. Consumption of raw or under cooked fish and fish products is a major source of fishborne infections in humans. Despite reports of fish contamination with foodborne pathogens in Ethiopia, information regarding the hygienic status of fish handling practices is limited. We assessed fish hygienic handling practices at production sites and along the fish supply chain in three towns in east Shewa zone of Oromia. Data were collected using a semi-structured questionnaire interviews and personal observations. The study consisted of purposively selected respondents comprising of 50 fishermen, 10 retailers, 20 food establishments serving fish, and 120 consumers. Descriptive statistics and Chi-square test were used to present the proportion of various actors along the fish production and supply chain and to compare the proportions of observations among the different categories respectively. We observed that the lakes were accessible to animals and exposed to chemical and microbial contaminations through rainwater run-off. Fish were processed under unhygienic practices like washing of filleted fish with lake water, indiscriminate processing at unhygienic landing sites, use of a single knife for processing all fish with infrequent washing and with no disinfection in between. Majority (70%; n = 10) of the retailers and all the food establishments transported fish in vehicles with no cold chain facilities. Good hygienic practices we observed were the use of refrigerators for storage in all retailers and 70% (n = 20) of the food establishments; 30% of retailers used vehicles with a cold chain facility for the transportation of fish. Over three-fourths (77%; n = 120) of the consumers preferred consuming raw fish; 80% of them lacked the knowledge of fishborne diseases. The study revealed a wide range of unhygienic handling practices along fish production and supply chain; lack of infrastructure for post-harvest fish handling and processing, lack of appropriate transportation facilities and presence of knowledge gaps regarding fish borne diseases.

Consumption of raw or undercooked milk and meat is a major source of zoonoses. Information on the public risk perceptions and protective behaviors are essential for prevention and control of these diseases. However, such information is limited in Ethiopia. We assessed the risk perceptions and protective behaviors of the community toward zoonoses associated with consumption of raw meat and milk by employing the basic constructs of food safety health belief model. We collected data from 210 respondents (105 from each urban and peri-urban) using a face-to-face questionnaire interview. Majority of the respondents (96.2%) had knowledge about zoonoses. Despite their knowledge, 91% and 94.3% had raw meat and raw milk consumption habits, respectively. The odds of raw meat consumption was significantly higher in males (AOR = 3.90; CI = 1.28-11.86) and raw milk consumption was higher in females (AOR = 3.82; CI = 0.78-18.65). About 65% of respondents, obtained meat from backyard slaughtering. Self-owned and smallholder dairy farms are the major sources of milk for peri-urban and urban dwellers, respectively. While 46.7% of the respondents reported that community members are the primary sources of information, only 2.4% reported animal health professionals as their primary source of information. More than half of the respondents either moderately or strongly agreed that consumption of raw meat and raw milk can expose them to zoonoses. Urban dwellers had perceived more seriousness of consuming raw animal products. Peri-urban residents had significantly lower intention than urban ones to implement protective behaviors such as stopping consuming raw meat (P = 0.017) and milk (P = 0.043). We noted that lack of access to refrigerator and pasteurized milk were the perceived barriers for protection against zoonoses among the peri-urban dwellers. There was significant difference in perceived benefits of avoiding consumption of raw meat (P = 0.005) and milk (P = 0.001) between urban and peri-urban residents. Our study showed that irrespective of knowledge about zoonoses, consumptions of raw meat and raw milk remained common practices among the respondents. Public health education on the risk of consumption of raw milk and meat and the significance of protective behaviors using a one-health approach is critically needed to ensure meat and milk safety.

Background: In veterinary medicine, three Staphylococcus species are of particular importance as primary causes of specific diseases; S. aureus (mastitis in ruminants, equine botryomycosis, and bumble foot in poultry), S. hycus (porcine exudative epidermitis), and S. intermedius (canine pyoderma). The disease conditions caused by Staphylococcus in poultry vary with site, route, and predisposing factors include wounds as a result of fighting/cannibalism, immunosuppression based on virus infection or parasite infestation, and bad husbandry conditions (overcrowding). The objectives of this study were to isolate and identify Staphylococcus spp from chicken and chicken litter and personnel at chicken farm and to determine the antimicrobial susceptibility profile of the isolates. Methods: A cross-sectional study was conducted on apparently healthy chickens, farm personnel, and chicken litter at poultry farms in Addis Ababa, Ethiopia. A total of 222 samples consisting of 101 cloacal swabs, 90 tracheal swabs, 17 pooled litter swabs, 7 nasal swabs, and 7 pooled hand and boot swabs were collected from six farms and examined for the presence of Staphylococcus species. Antimicrobial resistance against 10 antimicrobial agents was also conducted following recommended standard procedures. Results: Overall proportion of Staphylococcus was 64/222 (28.83%). Of the isolates, 40/64 (62.5%), 11/64 (17.2%), 3/64 (4.7%), and 10/64 (15.6%), were S. aureus, S. hycus, S. intermedius, and coagulase-negative staphylococci (CNS), respectively. Only one isolate of S. aureus was susceptible to all antimicrobials tested. Of the 10 antibiotics tested, the isolates demonstrated highest resistance against Penicillin G (96.9%) followed by Tetracycline (78.1%), and Amoxicillin and Erythromycin at the same level (65.6%). Conversely, the isolates were highly susceptible to Ciprofloxacin (95.3%) followed by Sulphamethoxazole-trimethoprim (85.9%). Out of 64 isolates, 61/64 (95.3%) were resistant to three or more antimicrobials tested. Of the isolates, 38/40 (95%) S. aureus, 10/11 (90.9%) S. hycus, 3/3 (100%) S. intermedius, and 10/10 (100%) CNS showed multidrug resistance. Conclusion: This study showed a considerable proportion of Staphylococcus spp in chicken litter and farm workers with a potential source of resistant Staphylococcus species, and more importantly multidrug resistance strains. Further studies on molecular characterization of the isolates will be essential to identify the resistant genes and establish epidemiological links in the transmission dynamics of resistant Staphylococcus species between poultry and humans.

Understanding the potential drivers of microbial meat contamination along the entire meat supply chain is needed to identify targets for interventions to reduce the number of meatborne bacterial outbreaks. We assessed the hygienic practices in cattle slaughterhouses (28 employees) and retail shops (127 employees) through face-to-face interviews and direct personal observations. At the slaughterhouses, stunning, de-hiding and evisceration in vertical position, carcass washing and separate storage of offal were the identified good practices. Lack of hot water baths, absence of a chilling room, infrequent hand washing, insufficiently trained staff and irregular medical check-up were practices that lead to unhygienic handling of carcasses. At the retail shops, cleaning equipment using soap and hot water (81%), storing unsold meat in refrigerators (92%), concrete floors and white painted walls and ceilings were good practices. Adjacently displaying offal and meat (39%), lack of a cold chain, wrapping meat with plastic bags and newspapers, using a plastic or wooden cutting board (57%), infrequent washing of equipment and floors, and inadequately trained employees were practices that could result in unhygienic handling of beef. Our study identified unhygienic practices both at the slaughterhouses and retail shops that can predispose the public to meatborne infections, which could be improved through training and implementation of quality control systems.

BACKGROUND: Bovine cysticercosis is a worldwide zoonotic disease that affects cattle caused by the larval stage of the tapeworm Taenia saginata, the adult parasite that causes taeniasis in humans. Although bovine cysticercosis, and the associated human taeniasis, is controlled in developed countries, it is one of the neglected tropical diseases. Like other parts of Ethiopia, raw or undercooked beef consumption is common in the rural community of Yem district with no meat inspection services. METHODS: We conducted an abattoir survey to estimate the prevalence of bovine cysticercosis in cattle and a questionnaire survey to determine the level of historical human taeniasis infection in Yem district of Ethiopia. RESULTS: Bovine cysticercosis was detected in 3.1% of 485 cattle examined. Although animal level prevalence was low, a subset of positive animals had a higher rate of cysticercosis infection widely distributed in various parts of the body. Over two-fifths (40%) of the cysts were found in the tongue making it an important organ to look for during meat inspection. Over half of the cysts were viable indicating their potential to infect people. History of taeniasis was reported by 71.5% of 151 residents interviewed during the study. Raw meat consumption was very common (89.4%) among the residents; people who consumed raw meat were 25 times more likely to report taeniasis than those who did not. In addition, traditional treatments using herbs and chemicals of unknown efficacy and risk were very common. CONCLUSIONS: Bovine cysticercosis and human taeniasis are important in this rural community. Providing meat inspection services and public health education on intervention measures such as proper cooking of meat, access to latrines and clean water are effective strategies that can break the lifecycle of the parasite and ensure beef safety and public health.

RESUMEN

Bovine tuberculosis (BTB) is a zoonotic bacterial infection caused by Mycobacterium bovis and is characterized by the development of granulomatous lesions in the lymph nodes, lungs and other tissues. It poses serious public health impacts and food security challenges to the agricultural sector in terms of dairy and meat productions. In Ethiopia, BTB has been considered as a priority disease because of its high prevalence in urban and peri-urban dairy farms. However, there has not been any national control program in the country. Thus, in order to initiate BTB control program in the country, information on control options is needed to tailor the best option for the Ethiopian situation. The objective of this study was to identify, evaluate and rank various BTB control options in Ethiopia using a multi-criteria decision analysis based on preference ranking organization method for enrichment evaluations (PROMETHEE) approach while accounting for the stakeholders' preferences. Control options were evaluated under two scenarios: with (scenario 1) and without (scenario 2) bacillus Calmette-Guérin (BCG) vaccination. Nine potential control options were identified that include combinations of three control options (1) test and slaughter with or without government support, (2) test and segregation, and (3) BCG vaccination. Under scenario 1, BCG vaccination, BCG vaccination and test and slaughter with partial compensation by government, and BCG vaccination and test and slaughter with full compensation by government were the top three ranked control options. Under scenario 2, test and slaughter with full compensation by government was the preferred control option, followed by test and segregation supported by test and slaughter with full government compensation, and test and slaughter with half compensation by government. Irrespective of the variability in the weighting by the stakeholders, the sensitivity analysis showed the robustness of the ranking method. In conclusion, the study demonstrated that BCG vaccination, and test and slaughter with full compensation by government were the two most preferred control options under scenarios 1 and 2, respectively. National level discussions were strongly recommended for further concretization and implementation of these control measures.

Dogs are reservoirs of many zoonotic diseases. In Ethiopia, the majority of owned dogs are semi-stray, freely roaming in the community. Studies reporting dog borne zoonotic diseases are scarce in Ethiopia. This study was conducted to assess Dipylidium caninum infection in dogs and in children with gastrointestinal complaints in Bishoftu Town, Oromia. We collected 384 fecal samples from dogs presented to veterinary teaching hospital and 259 stool samples from children presented to Bishoftu Hospital for clinical examination. Samples were first macroscopically examined for the presence of proglotids, followed by microscopic examination for the presence of eggs with the direct smear following flotation technique. The prevalence of D. caninum was 21% (95% CI: 16.6-24.9) in dogs. Although not statistically significant (p > 0.05), higher prevalence was detected in adult (11.9%), local breed (17.7%), and male (12.6%) dogs compared to young (8.59%), exotic breed (2.86%), and females (7.81%), respectively. Dipylidium caninum was detected in a stool sample obtained from a three year-old child (0.4%, 1/259). This study showed that the prevalence of D. caninum in the dogs is high while it is rare in children. Although the prevalence in children is negligible in this study, the high proportion of infected dogs can pose a significant risk of infection in the general human population. Public health risk can be reduced by eliminating the semi-roaming of owned dogs and proper management of dogs with regular deworming and prevention of environmental contamination with dog feces. Similarly, raising public awareness about dog borne zoonoses and avoiding contact with dog feces are important.

Salmonellosis is a leading cause of foodborne illnesses in humans with cattle being one of the reservoirs for Salmonella. We estimated a pooled prevalence of Salmonella in apparently healthy cattle and examined serotype diversity through systematic review and meta-analysis of studies published between 2000 and 2017. Peer reviewed publications reporting the prevalence of Salmonella in cattle were searched through five electronic databases (PubMed, Google scholar, Agricola, Scopus, CAB direct) and through manual search. We obtained 71 publications with 75 datasets consisting a total of 52,766 animals examined and 5,010 Salmonella positive cattle from 29 countries in six continents (except from Antarctica). Pooled prevalence of Salmonella in cattle was 9% (95% confidence interval: 7-11%). Significantly high heterogeneity (I 2 = 98.7%, P < 0.01) was observed among all studies as well as within continents. Prevalence varied from 2% (Europe) to 16% (North America). Overall, 143 different serotypes were reported with the most diverse serotypes being reported from Africa (76 different serotypes) followed by North America (49 serotypes). The 10 most frequently reported serotypes (Montevideo, Typhimurium, Kentucky, Meleagridis, Anatum, Cerro, Mbandaka, Muenster, Newport, and Senftenberg) accounted for 65% of the isolates for which specific serotype information was reported. Salmonella Montevideo and S. Dublin are the most frequently reported serotypes in North America and Europe, respectively, while S. Typhimurium was the most frequent in Africa, Asia and Australasia. Our results indicated variability both in the prevalence and serotype diversity of Salmonella in cattle across continents. Although all Salmonella serotypes are potentially pathogenic to humans, five (Montevideo, Typhimurium, Anatum, Mbandaka, and Newport) of the top 10 serotypes identified in this study are among the serotypes most commonly associated with clinical illnesses in humans.
