RESUMEN

OBJECTIVE: To determine the prevalence of head injuries (HIs), posttraumatic stress disorder (PTSD), and depressive symptoms in law enforcement officers (LEOs) and (2) the association between HIs and psychological health conditions. SETTING: County-level survey administered via Research Electronic Data Capture. PARTICIPANTS: A total of 381 LEOs completed the survey (age = 43 ± 11 years; 40 [11%] females; time as LEO = 1-50 years, median = 15 years). DESIGN: Cross-sectional study. MAIN MEASURES: We examined the prevalence of HIs (the Ohio State University Traumatic Brain Injury Identification Method), PTSD (PTSD Checklist-Civilian [PCL-C]), and depressive symptoms (Patient Health Questionnaire-9 [PHQ-9]). We used Mann-Whitney U and chi-square analyses to compare PTSD and depressive symptoms between those with and without a HI history. RESULTS: There were 282 (74%) participants who reported a lifetime history of 1 or more HIs; 116 (30%) sustained 1 or more HIs on the job. PCL-C scores ranged 17 to 85 (median = 27); 33 (10%) participants met or exceeded the clinical cutoff score of 50 to indicate a positive PTSD screening. Participants with a HI history (median = 29) had higher PCL-C scores than those with no HI history (median = 24; P < .001), but the proportion of participants who met the clinical cutoff for PTSD was not different between those with (n = 28, 11%) and without (n = 5, 5%) a HI history (X2 = 2.52, P = .112, odds ratio = 2.18; 95% confidence interval, 0.82-5.83). PHQ-9 scores ranged 0 to 20 (median = 3); 124 (36%) participants reported mild or greater depressive symptoms. Participants with a HI history (median = 3) had higher depressive symptoms than those with no HI history (median = 2; P = .012). The proportion of participants with mild or greater depressive symptoms was higher among those with a HI history (n = 99, 39%) than without (n = 25, 27%; X2 = 4.34, odds ratio = 1.74; 95% confidence interval, 1.03-2.93). CONCLUSION: HIs are prevalent in LEOs, which may have consequences for their performance, well-being, and career longevity. PTSD and depressive symptoms are higher in those with a HI history, suggesting LEOs need better traumatic brain injuries and mental health resources.

Countermovement jump force-time curve analyses: reliability and comparability across force plate systems. J Strength Cond Res 38(1): 30-37, 2024-Considering the growing prevalence of commercial force plates providing automated force-time analyses, understanding levels of agreement across force plate systems is warranted. Countermovement jump (CMJ) metrics across Vald ForceDecks (FD), Hawkin Dynamics (HD), and Sparta Science (SS) force plate systems were compared. Twenty-two subjects completed CMJ testing (∼128 comparisons) on each force plate system separately with rest between jumps. Baseline testing occurred 3 times and demonstrated poor test-retest reliability for modified reactive strength index (mRSI) and rate of force development (RFD). ForceDecks and HD comparisons yielded acceptable agreement for concentric/propulsive relative force and net impulse, jump height, eccentric/braking RFD, and mRSI, but systematic and proportionate bias existed for RFD. Sparta Science jump height and reactive strength index (RSI) demonstrated systematic overestimations compared with HD and FD, but jump height had acceptable agreement according to concordance correlation coefficients (CCC = 0.92-0.95). Agreement between SS load (eccentric RFD) and HD braking RFD was acceptable (CCC = 0.91), whereas agreement between SS load and FD deceleration RFD was considered acceptable (CCC = 0.81-0.87) but demonstrated systematic and proportionate bias. ForceDecks (CCC = 0.89) and HD (CCC = 0.85) average relative concentric/propulsive force yielded acceptable agreement with SS explode (average relative concentric force), but SS explode demonstrated systematically lower values than FD and HD. Sparta Science drive (concentric impulse) yielded acceptable agreement with HD relative propulsive impulse (CCC = 0.85), but not FD concentric impulse. Human performance practitioners need to be aware of inconsistencies among testing procedures and analyses across force plate systems, such as differences in metric definitions and units of measurement, before making comparisons across systems.

This study aims to examine the effects of acute whole-body photobiomodulation (wbPBM), applied pre-exercise, on bouts of anaerobic cycling (Wingate) performances. Forty-eight healthy, active males and females participated in this single-blind, randomized, crossover study. Participants visited the laboratory three times to complete repeat (4 ×) Wingate testing, with one week between each visit. All participants completed baseline testing during their first visit and randomly received either the wbPBM or placebo condition before testing on the second visit, followed by the opposite condition on the third visit. There were no significant condition × time interactions for any variable (peak power, average power, power decrement, lactate, heart rate, ratings of perceived exertion, heart rate variability (HRV), root-mean square of differences between R-R intervals (rMSSD), power in the high-frequency range (HF) average, power in the low-frequency range (LF) average, total power, LF/HF, or power in the very-low-frequency range average). A main condition effect was only noted for heart rate, where peak heart rate was significantly higher for wbPBM (145, 141-148 bpm) than placebo (143, 139-146 bpm; p = 0.006) and baseline testing (143, 140-146; p = 0.049) throughout the entire testing session (i.e., collapsed across all timepoints). Furthermore, HRV (rMSSD) the following morning after testing was significantly higher for the wbPBM session compared to placebo (p = 0.043). There were no differences in perceived recovery (p = 0.713) or stress (p = 0.978) scores between wbPBM and placebo. Implementing 20 min of wbPBM immediately prior to maximal bouts of anaerobic cycling did not improve performance (i.e., power output) or physiological responses (e.g., lactate). However, wbPBM elicited the ability to work at a higher heart rate throughout testing and seemed to enhance recovery through improved HRV the following morning.

RESUMEN

microRNAs (miRNAs) are ~21-22 nucleotide (nt) RNAs that mediate broad post-transcriptional regulatory networks. However, genetic analyses have shown that the phenotypic consequences of deleting individual miRNAs are generally far less overt compared to their misexpression. This suggests that miRNA deregulation may have broader phenotypic impacts during disease situations. We explored this concept in the Drosophila eye, by screening for miRNAs whose misexpression could modify the activity of pro-apoptotic factors. Via unbiased and comprehensive in vivo phenotypic assays, we identify an unexpectedly large set of miRNA hits that can suppress the action of pro-apoptotic genes hid and grim. We utilize secondary assays to validate that a subset of these miRNAs can inhibit irradiation-induced cell death. Since cancer cells might seek to evade apoptosis pathways, we modeled this situation by asking whether activation of anti-apoptotic miRNAs could serve as "second hits". Indeed, while clones of the lethal giant larvae (lgl) tumor suppressor are normally eliminated during larval development, we find that diverse anti-apoptotic miRNAs mediate the survival of lgl mutant clones in third instar larvae. Notably, while certain anti-apoptotic miRNAs can target apoptotic factors, most of our screen hits lack obvious targets in the core apoptosis machinery. These data highlight how a genetic approach can reveal distinct and powerful activities of miRNAs in vivo, including unexpected functional synergies during disease or cancer-relevant settings.

J Strength Cond Res 36(9): 2387-2402, 2022-With the growing prevalence of commercial force plate solutions providing automated force-time curve analysis, it is critical to understand the level of agreement across techniques. Thus, this study directly compared commercial and custom software analyses across force-time curves. Twenty-four male and female subjects completed 6 trials of countermovement, squat, and drop jumps, and isometric mid-thigh pulls on the same force plate. Vertical ground reaction forces were analyzed by automated software from Vald Performance, Hawkin Dynamics, and custom MATLAB scripts. Trials were visually assessed to verify proper landmark identifications. Systematic and proportional bias among analyses were compared via least products regressions, Bland-Altman plots, and percent error. Hawkin Dynamics had subtle differences in analysis procedures and demonstrated low percent errors across all tests (<3% error), despite demonstrating systematic and proportional bias for several metrics. ForceDecks demonstrated larger percent differences and greater biases for several metrics. These errors likely result from different identification of movement initiation, system weight, and integration techniques, which causes error to subsequent landmark identifications (e.g., braking/propulsive phases) and respective force-time metrics. Many metrics were in agreement between devices, such as isometric mid-thigh pull peak force consistently within 1 N across analyses, but some metrics are difficult and incomparable across software analyses (i.e., rate of force development). Overall, many metrics were in agreement across each commercial software and custom MATLAB analyses after visually confirming landmarks. However, because of inconsistencies, it is important to only compare metrics that are in agreement across software analyses when absolutely necessary.

Using random forest regression to determine influential force-time metrics for countermovement jump height: a technical report. J Strength Cond Res 36(1): 277-283, 2022-The purpose of this study was to indicate the most influential force-time metrics on countermovement jump (CMJ) height using multiple statistical procedures. Eighty-two National Collegiate Athletic Association Division I American football players performed 2 maximal-effort, no arm-swing, CMJs on force plates. The average absolute and relative (i.e., power/body mass) metrics were included as predictor variables, whereas jump height was the dependent variable within regression models (p < 0.05). Best subsets regression (8 metrics, R2 = 0.95) included less metrics compared with stepwise regression (18 metrics, R2 = 0.96), while explaining similar overall variance in jump height (p = 0.083). Random forest regression (RFR) models included 8 metrics, explained ∼93% of jump height variance, and were not significantly different than best subsets regression models (p > 0.05). Players achieved higher CMJs by attaining a deeper, faster, and more forceful countermovement with lower eccentric-to-concentric force ratios. An additional RFR was conducted on metrics scaled to body mass and revealed relative mean and peak concentric power to be the most influential. For exploratory purposes, additional RFR were run for each positional group and suggested that the most influential variables may differ across positions. Thus, developing power output capabilities and providing coaching to improve technique during the countermovement may maximize jump height capabilities. Scientists and practitioners may use best subsets or RFR analyses to help identify which force-time metrics are of interest to reduce the selectable number of multicollinear force-time metrics to monitor. These results may inform their training programs to maximize individual performance capabilities.

J Strength Cond Res 36(2): 411-419, 2022-This study aimed to reduce the dimensionality of countermovement jump (CMJ) force-time characteristics and evaluate differences among positional groups (skills, hybrid, linemen, and specialists) within National Collegiate Athletic Association (NCAA) division I American football. Eighty-two football athletes performed 2 maximal effort, no arm-swing, CMJs on force plates. The average absolute and relative (e.g., power/body mass) metrics were analyzed using analysis of variance and principal component analysis procedures (p < 0.05). Linemen had the heaviest body mass and produced greater absolute forces than hybrid and skills but had lower propulsive abilities demonstrated by longer propulsive phase durations and greater eccentric to concentric mean force ratios. Skills and hybrid produced the most relative concentric and eccentric forces and power, as well as modified reactive strength indexes (RSIMOD). Skills (46.7 ± 4.6 cm) achieved the highest jump height compared with hybrid (42.8 ± 5.5 cm), specialists (38.7 ± 4.0 cm), and linemen (34.1 ± 5.3 cm). Four principal components explained 89.5% of the variance in force-time metrics. Dimensions were described as the (a) explosive transferability to concentric power (RSIMOD, concentric power, and eccentric to concentric forces) (b) powerful eccentric loading (eccentric power and velocity), (c) countermovement strategy (depth and duration), and (d) jump height and power. The many positional differences in CMJ force-time characteristics may inform strength and conditioning program designs tailored to each position and identify important explanatory metrics to routinely monitor by position. The overwhelming number of force-time metrics to select from may be reduced using principal component analysis methods, although practitioners should still consider the various metric's applicability and reliability.

DNA aptamers that bind biomolecular targets are of interest as the recognition element in colorimetric sensors based on gold nanoparticles (AuNP), where sensor functionality is related to changes in AuNP colloidal stability upon target binding. In order to understand the role of target binding on DNA-AuNP colloidal stability, we have used high-resolution NMR to characterize the interactions of the 36 nucleotide cocaine-binding aptamer (MN4) and related aptamers with AuNPs, cocaine, and cocaine metabolites. Changes in the aptamer imino proton NMR spectra with low (20 nM) concentrations of AuNP show that the aptamers undergo fast-exchange adsorption on the nanoparticle surface. An analysis of the spectral changes and the comparison with modified MN4 aptamers shows that the AuNP binding domain is localized on stem two of the three-stemmed aptamer. The identification of an AuNP recognition domain allows for the incorporation of AuNP binding functionality into a wide variety of aptamers. AuNP-induced spectral changes are not observed for the aptamer-AuNP mixtures in the presence of cocaine, demonstrating that aptamer absorption on the AuNP surface is modulated by aptamer-target interactions. The data also show that the DNA-AuNP interactions and sensor functionality are critically dependent on aptamer folding.

BACKGROUND: Azathioprine and 6-mercaptopurine (thiopurines) are common adjunct treatments for inflammatory bowel disease (IBD). Although thiopurine therapy in organ transplant recipients is known to increase nonmelanoma skin cancers (NMSCs), dermatologic literature yields less data regarding NMSC risk of thiopurine use in IBD. OBJECTIVE: The aim of this study was to systematically review current literature on NMSC risk in patients with IBD using thiopurine therapy. METHODS: Systematic review of PubMed was performed with keywords "inflammatory bowel disease," "ulcerative colitis," "Crohn's disease," "thiopurine," "azathioprine," "6-mercaptopurine," "skin cancer," "non-melanoma," "squamous cell carcinoma," and "basal cell carcinoma." All available publication years were included. Publications were evaluated using PRISMA guidelines. RESULTS: The systematic review yielded 67 articles; 18 met final inclusion criteria. LIMITATIONS: Heterogeneity of study designs limited direct comparisons of thiopurine exposure and NMSC risk. CONCLUSION: Patients with IBD using thiopurines seem to have a moderately increased risk of NMSC that is proportional to therapy duration. Risk of NMSC seems to decrease or return to baseline after discontinuing therapy, although additional data are needed to support this trend. Younger patients with IBD using thiopurines seem to be at greater risk of NMSC. Appreciating NMSC risk in patients with IBD undergoing thiopurine therapy should help direct skin cancer screening recommendations and sun protective measures.

Neurotransmitters detection is critical to understanding communication between the brain and peripheral tissue. Serotonin is a key neurotransmitter linked to a number of conditions, but a full understanding of its role in disease is still lacking. The development of fast and selective serotonin detection platforms will provide researchers with tools to monitor serotonin in individuals before and after treatment for the condition of interest. Aptamer-gold nanoparticles conjugates that responded colorimetrically to serotonin with minimal response to its metabolite and other neurotransmitters were designed by simply adsorbing the DNA on the surface of AuNPs. A plasmonic assay for serotonin detection was designed with a response to biologically relevant serotonin levels. Importantly, the assay performance was not compromised when tested in filtered spiked fetal bovine serum as a mimic of biofluids. This work shows that these simple and stable Apt-AuNP conjugates are promising tools to develop fast assays for point-of-care and personalized diagnostics applications.

In this work, we developed an assay to determine if an arbitrary white powder is a controlled substance, given the plasmonic response of aptamer-gold nanoparticle conjugates (Apt-AuNPs). Toward this end, we designed Apt-AuNPs with specific a response to common controlled substances without cross reactivity to chemicals typically used as fillers in street formulations. Plasmonic sensor variation was shown to produce unique data fingerprints for each chemical analyzed, supporting the application of multivariate statistical techniques to annotate unknown samples by chemical similarity. Importantly, the assay takes less than fifteen minutes to run, and requires only a few micrograms of the material, making the proposed assay easily deployable in field operations.

miR-124 is conserved in sequence and neuronal expression across the animal kingdom and is predicted to have hundreds of mRNA targets. Diverse defects in neural development and function were reported from miR-124 antisense studies in vertebrates, but a nematode knockout of mir-124 surprisingly lacked detectable phenotypes. To provide genetic insight from Drosophila, we deleted its single mir-124 locus and found that it is dispensable for gross aspects of neural specification and differentiation. On the other hand, we detected a variety of mutant phenotypes that were rescuable by a mir-124 genomic transgene, including short lifespan, increased dendrite variation, impaired larval locomotion, and aberrant synaptic release at the NMJ. These phenotypes reflect extensive requirements of miR-124 even under optimal culture conditions. Comparison of the transcriptomes of cells from wild-type and mir-124 mutant animals, purified on the basis of mir-124 promoter activity, revealed broad upregulation of direct miR-124 targets. However, in contrast to the proposed mutual exclusion model for miR-124 function, its functional targets were relatively highly expressed in miR-124-expressing cells and were not enriched in genes annotated with epidermal expression. A notable aspect of the direct miR-124 network was coordinate targeting of five positive components in the retrograde BMP signaling pathway, whose activation in neurons increases synaptic release at the NMJ, similar to mir-124 mutants. Derepression of the direct miR-124 target network also had many secondary effects, including over-activity of other post-transcriptional repressors and a net incomplete transition from a neuroblast to a neuronal gene expression signature. Altogether, these studies demonstrate complex consequences of miR-124 loss on neural gene expression and neurophysiology.

The utilization of the plasmonic response of aptamer-gold nanoparticle conjugates (Apt-AuNPs) to design cross-reactive arrays for fingerprint identification of small molecular targets was demonstrated for the first time. Four aptamers with different structural features previously selected to bind different targets were used in combination with AuNPs by adsorbing the DNA on the AuNPs surface. The optimized response of the Apt-AuNPs to the analytes showed that, depending on the specific aptamer used, target binding by the aptamer could result in an increase or decrease of Apt-AuNPs stability. These Apt-AuNPs showed the ability to recognize different analytes with different affinities, generating fingerprints that allowed unambiguous analyte identification with response times in less than fifteen minutes. Importantly, it was observed that it was not necessary to select an aptamer per analyte of interest to generate differentiable signatures, but a subset of aptamers could be used to identify a larger number of analytes. The data was analyzed using principal component analysis, showing efficient clustering of the different datasets for qualitative and quantitative identification. This work opens the door to using these Apt-AuNPs in point of care diagnostics applications where fast sensors with easy to read outputs are needed.

Pentacene-based organic thin-film transistors (OTFTs) were fabricated on several types of flexible substrate: commercial photo paper, ultra-smooth specialty paper and ultra-thin (100 µM) flexible glass. The transistors were fabricated entirely through dry-step processing. The transconductance and field-effect mobility of OTFTs on photo paper reached values of ∼0.52 mS m(-1) and ∼ 0.1 cm(2) V (-1) s(-1), respectively. Preliminary results on the lifetime of OTFTs on photo paper yielded stable transconductance and mobility values over a period of more than 250 h. The comparable characteristics of OTFTs fabricated on widely available, low cost paper and high quality expensive liquid crystal display glass indicate the potential importance of cellulose-based electronic devices.

RESUMEN

The first-known aptamer for the stress biomarker cortisol was selected using a tunable stringency magnetic bead selection strategy. The capture DNA probe immobilized on the beads was systematically lengthened to increase the number of bases bound to the complementary pool primer regions following selection enrichment. This resulted in a single sequence (15-1) dominating the final round 15 pool, where the same sequence was the second-highest copy number candidate in the enriched pool with the shorter capture DNA probe (round 13). A thorough analysis of the next-generation sequencing results showed that a high copy number may only correlate with enhanced affinity under certain stringency and enrichment conditions, in contrast with prior published reports. Aptamer 15-1 demonstrated enhanced binding to cortisol (K(d) = 6.9 ± 2.8 µM by equilibrium dialysis; 16.1 ± 0.6 µM by microscale thermophoresis) when compared with the top sequence from round 13 and the negative control progesterone. Whereas most aptamer selections terminate at the selection round demonstrating the highest enrichment, this work shows that extending the selection with higher stringency conditions leads to lower amounts eluted by the target but higher copy numbers of a sequence with enhanced binding. The structure-switching aptamer was applied to a gold nanoparticle assay in buffer and was shown to discriminate between cortisol and two other stress biomarkers, norepinephrine and epinephrine, and a structurally analogous biomarker of liver dysfunction, cholic acid. We believe this approach enhances aptamer selection and serves as proof-of-principle work toward development of point-of-care diagnostics for medical, combat, or bioterrorism targets.

In 2007, Petrella et al described a series of patients with clonally restricted, well-differentiated, nonepidermotropic, CD8-dominant lymphocytic infiltrates localized to the facial area. The clinical course described was indolent. A CD8 variant of primary cutaneous pleomorphic T-cell lymphoma has been proposed; however, there are many dissimilar features. The authors encountered 2 patients with CD8⁺ indolent lymphoid proliferation of the face, localized to the eyelid. Both patients were males in their 30s presenting with localized lesions of the eyelids. The biopsies showed an effacing lymphocytic infiltrate that spanned the sampled dermis. The lymphocytes were well differentiated, exhibiting mild nuclear contour irregularity. The infiltrate was predominated by CD8⁺ lymphocytes demonstrating TIA expression. There was a minimal B-cell component in 1 case, whereas another showed a significant degree of B-cell hyperplasia. They both underwent complete excision without recurrence. CD8⁺ indolent lymphoid proliferation has a reproducible clinical and morphologic presentation that warrants categorization as a distinct form of indolent lymphoproliferative disease, preferentially involving older adults with the most common site being the ear, recognizing that the nose, acral surfaces, and eyelids can also be involved.

Athletic trainers are increasingly utilized in non-traditional settings, such as in law enforcement, where they can contribute to healthcare management, including concussion management of law enforcement officers (LEOs). Despite the prevalence of concussions among LEOs, there is a notable gap in concussion management guidelines for this population. LEOs may lack the education and resources necessary for concussion recognition and proper management. Drawing on advancements in concussion management in athletes and military personnel, here we present a comprehensive framework for concussion management in LEOs encompassing concussion education, a graduated return to duty (RTD) protocol, and considerations for implementation and documentation specific to law enforcement. We also present several barriers and facilitators to implementation. Due to job requirements, it is critical for law enforcement organizations and their medical providers to adopt a concussion management strategy. Without proper concussion management, LEOs may risk subsequent injury and/or suffer from prolonged recovery and adverse long-term outcomes.

CONTEXT: Health care providers (HCP) experience high stress and burnout rates. Mindfulness Based Interventions (MBI) with biofeedback may help improve resiliency but require further research. DESIGN AND STUDY PARTICIPANTS: Aims were to evaluate changes in sleep patterns, nocturnal physiology, stress, mood disturbances, and perceived experience with biofeedback during the Mindfulness in Motion (MIM) intervention. Data from 66 HCP were included after removing those below 75 % compliance with wearable sensors and wellness surveys. Participants were enrolled in MIM, including eight weekly one-hour virtually delivered synchronous group meetings and ∼10 min of mindfulness home practice at least 3 times per week using a mobile application. Participants wore wearable sensors to monitor sleep and nocturnal physiology and completed short daily stress and mood disturbances. RESULTS: According to mixed effect models, no sleep nor physiological metrics changed across MIM (p > 0.05). More time was spent in bed after MIM sessions (8.33±1.03 h) compared to night before (8.05±0.93 h; p = 0.040). Heart rate variability was lower nights after MIM (33.00±15.59 ms) compared to nights before (34.50±17.04 ms; p = 0.004) but was not clinically meaningful (effect= 0.033). Significant reductions were noted in perceived stress at weeks 3 through 8 compared to Baseline and lower Total Mood Disturbance at weeks 3, 5, 6, and 8 compared to Baseline (p < 0.001). CONCLUSIONS: Participating in the MIM with mobile applications and wearable sensors reduced perceived stress and mood disturbances but did not induce physiological changes. Additional research is warranted to further evaluate objective physiological outcomes while controlling for confounding variables (e.g., alcohol, medications).

Monitoring aspects of human performance during various activities has recently become a highly investigated research area. Many new commercial products are available now to monitor human physical activity or responses while performing activities ranging from playing sports, to driving, and even sleeping. However, monitoring cognitive performance biomarkers, such as neuropeptides, is still an emerging field due to the complicated sample collection and processing, as well as the need for a clinical lab to perform analysis. Enzyme-linked immunosorbent assays (ELISAs) provide specific detection of biomolecules with high sensitivity (picomolar concentrations). Even with the advantage of high sensitivity, most ELISAs need to be performed in a laboratory setting and require around 6 h to complete. Transitioning this assay to a platform where it reduces cost, shortens assay time, and is able to be performed outside a lab is invaluable. Recently developed paper diagnostics provide an inexpensive platform on which to perform ELISAs; however, the major limiting factor for moving out of the laboratory environment is the measurement and analysis instrumentation. Using something as simple as a digital camera or camera-enabled Windows- or Android-based tablets, we are able to image paper-based ELISAs (P-ELISAs), perform image analysis, and produce response curves with high correlation to target biomolecule concentration in the 10 pM range. Neuropeptide Y detection was performed. Additionally, silver enhancement of Au NPs conjugated with IgG antibodies showed a concentration-dependent response to IgG, thus eliminating the need for an enzyme-substrate system. Automated image analysis and quantification of antigen concentrations are able to be performed on Windows- and Android-based mobile platforms.

Mycosis fungoides (MF) is the most common of the family of cutaneous T-cell lymphomas, accounting for 65% of all cases of cutaneous T-cell lymphomas. The classic phenotypic profile is one defined by CD4+ T cells showing a reduction in the expression of CD7 and CD62L. There are 3 previous reports describing CD20 expression in MF. The cell surface antigen CD20 is a transmembrane glycosylated phosphoprotein expressed in the early stages of B-cell development before differentiation into plasma cells. Two male patients, aged 14 and 44 years, presented with persistent truncal plaques up to 8 cm of 1 and 4 years duration, respectively. A third patient, an 80-year-old female, presented with a 1-year history of progressive nodules involving the head and neck area. Cases 1 and 2 both responded to topical treatment modalities. The biopsies in cases 1 and 2 showed features typical of plaque stage MF, whereas case 3 was compatible with follicular MF with tumor stage transformation. Phenotypically, the aberrant cell populace demonstrated a CD4+, CD7-, and CD62L- phenotype; at variance with classic MF was the expression of CD20. Although there were a few PAX5-positive staining cells, definitive colocalization studies were negative. Other B-cell markers and heavy chain immunoglobulin rearrangement were not detected. There are a growing number of reports describing T-cell lymphomas and leukemias with CD20 expression. Of the 6 CD20+ MF cases reported in the literature to date, 3 have been associated with a more aggressive clinical course; all but one case have occurred in males.
