RESUMEN

AIM: To investigate clients' perspectives about outcomes of a telehealth residential unit (RU) program for families experiencing complex early parenting issues, and to explore facilitators and barriers to positive client outcomes. DESIGN: Qualitative study using semi-structured interviews. METHODS: Semi-structured interviews were conducted with mothers (n = 18) admitted to a telehealth RU program. Interview transcripts were analysed using thematic analysis. RESULTS: Mothers reported short-term improvements in their child's presenting issues (e.g. feeding to sleep, night-time waking, co-sleeping), increased confidence and increased partner involvement. According to participants, program outcomes were facilitated by a positive parent-clinician relationship, the accessibility of clinicians and being able to take part in the program from their own home. Barriers included difficulties with technical equipment and connecting with the clinician overnight, and challenges with implementing strategies in the longer term. CONCLUSION: This nurse-led telehealth program was viewed positively by parents and the study identified a number of areas for improvement. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: Telehealth early parenting programs provide an important way for parents to receive support with early child sleep, settling and feeding issues. Clinicians working in this area should focus on the development of positive parent-nurse relationships, enhancing communication and availability for parents during overnight periods and supporting parents to develop early parenting skills that will be applicable across the early childhood period. IMPACT: The study is the first to address client experiences of a telehealth RU program. Facilitators and barriers identified will inform service improvements to the program going forward, and similar telehealth programs for families; to ensure benefits and service outcomes are maximised for parents for such a crucial service. REPORTING METHOD: The Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines for qualitative research were followed.

The rheological and mechanical properties of mixed κ/ι-carrageenan - LM pectin gels were determined, and the potential of these gels for the formation of beads using the extrusion method and for the encapsulation of Lacticaseibacillus rhamnosus ATCC 53103 (LGG) was evaluated. Self-standing gels were obtained with all formulations evaluated. Carrageenan-rich gels, with carrageenan fraction (XC) ≥ 0.75, exhibited the highest storage modulus, but they were also brittle, while pectin-rich gels (XC ≤ 0.25) presented the highest hardness and cohesiveness. Pectin-rich formulations formed beads with the smallest initial diameter (2.40-2.45 mm), and the addition of carrageenan produced significantly more spherical beads compared to pure-pectin ones. As pectin-rich beads were the formulations that resisted simulated gastrointestinal conditions, these were selected for the encapsulation of LGG. These beads showed high encapsulation yields (87-96 %), and the percentage reduction of CFU/g during storage and simulated gastrointestinal conditions was not significantly different among formulations, the latter being significantly lower for encapsulated cells (8.64-15.03 %) compared to free cells (71.20 %). These results indicate that carrageenan-pectin gel beads with XC ≤ 0.25 were successful in encapsulating probiotic bacteria, and this capacity was related to the rheological and mechanical properties of the gels.

RESUMEN

This study investigated how wheelchair-using individuals with paraplegia and chronic pain make sense of the factors associated with quality of life based on interviews using photo-elicitation and interpretative phenomenological analysis. Three superordinate themes emerged in the analysis: experiencing quality of life through the perception of self and identity, interpersonal relationships as facilitators and barriers to quality of life and life in a wheelchair: pain experience and management. Quality of life for those living with paraplegia and chronic pain is experienced as a complex interaction across several life domains. The use of photographs may improve the communication of pain-related experiences and understanding by healthcare staff.

RESUMEN

In the midst of the multifarious healthcare of the 21st century an Anglican clergyman from the 18th century named John Wesley can serve as a valuable resource for contemporary Christians seeking to faithfully live a life of well-being which incorporates different forms of medicine. In order to explore Wesley's contributions to integrative care this essay will first situate Wesley in his 18th century context-a time period not completely unlike our own in which medicine was also experiencing dramatic shifts in practice and philosophy. In order to demonstrate his integration of the medical knowledge of his day the essay will look at Wesley's own medical practices and his relationships to physicians as forging a "middle way" between physic and empiricism. The essay will examine Wesley's theology as a practical piety which is grounded in a holistic sotieriology that sustains an integrative medicine (caring for body, mind, and spirit). Finally, the essay concludes with possibilities for integrated medicine in our own day as informed by a Wesleyan "way" of holistic practice.

RESUMEN

We are delighted to introduce the Socius Gender and Politics Special Collection. The need for a special collection on gender and politics has never been timelier, evidenced both by gains in women's political participation and leadership around the world and the 2016 presidential election in the United States. Here, we introduce the timeliness of the topic with a review of the remarkable progress women have made in political representation over the past 150 years, a reiteration of the importance of women's political representation to democracy, and a brief discussion of the 2016 U.S. election cycle. We then introduce the eight papers that make up the special collection, emphasizing their contributions to ongoing debates in the field.

The aims of the present study were to characterize for the first time the carrageenan extracted from cystocarpic stage of S. crispata collected in the Patagonian coast of Argentina, and to prepare interpolyelectrolytic complexes (IPECs) between the polysaccharide extracted from cystocarpic stage of Sarcothalia crispata and Gigartina skottsbergii thalli, and basic butylated methacrylate copolymer (Eudragit E), in order to test their potential for the controlled release of ibuprofen as model drug. The structural determination revealed that the polysaccharides extracted from S. crispata and G. skottsbergii were mainly constituted by κ-carrageenan, particularly in the case of G. skottsbergii; however, significant amounts of ι- and ν-carrageenan were also detected in both polygalactans. The differences in diad composition and possibly in their distribution along the polysaccharide chain of both carrageenans would favor a different arrangement in the resulting IPEC structure. The smaller pores observed by scanning electron microscopy in the IPEC of S. crispata suggest that the kinks in the polysaccharide backbone are evenly distributed, resulting in a slower ibuprofen release compared to the IPEC of G. skottsbergii.

RESUMEN

The global proliferation of quotas for women over the past 30 years is both remarkable and consequential. Targeting decision-making positions historically resistant to women's equal inclusion, the adoption of electoral and corporate board quotas has at times been controversial. After adoption, quotas have influenced women's numbers, the performance and outcomes of decision-making bodies, and broader public attitudes. In this review, we distinguish among types of electoral and corporate quotas, trace arguments for and against the adoption of quotas, and review research on factors that influence quota adoption across time and space. After outlining the methodological difficulties in demonstrating an impact of gender quotas, we review research that is able to isolate an impact of quotas in politics and business. We conclude by providing several suggestions to ensure that future research continues to advance our understanding of the form, spread, and impact of gender quotas.

RESUMEN

Researchers who study violence against women often face problems when trying to understand the causes of individual changes in the context of group differences, targeted interventions, and institutional shifts. The authors explore these problems through research on the connections among women's earnings, welfare, and protection orders. The authors use multigroup, piecewise, latent growth curve models to explore differences in the initial earnings and earnings changes for two groups: welfare recipients who have and who have not petitioned for a restraining order. The authors further examine these differences in the context of institutional change, specifically the implementation of the Personal Responsibility Act of 1996.

The expansion of women's political representation ranks among the most significant trends in American politics of the last 100 years. In this paper, we develop two longitudinal theories to explain patterns of growth and change in women's state legislative representation over time. Gender salience suggests that years in which women's absence from politics is problematized (e.g., 1992-the Year of the Woman) will demonstrate higher levels of growth. Political climate suggests that periods in which domestic issues are stressed (e.g., the 1990s) will produce higher levels of growth than periods in which international issues are stressed (e.g., post 9/11). Combinations of these two theories create four possible trajectories of growth in women's representation that may be observed over time. We use latent growth curve models to assess the four theoretical trajectories, using data on women's state legislative representation from 1982 to 2006. We find that while women achieved fleeting success in the Year of the Woman, further gains were limited in the remainder of the 1990s and average growth stalled completely after 2001. Our results show futher that gender salience and, to a lesser extent political, climate matter to growth and change in women's political power over time.
