RESUMEN

This case report compares a conventional and a digital workflow for manufacturing metal frameworks for maxillary and mandibular removable partial dentures (RPDs). Two sets of maxillary and mandible RPDs were produced. The metal framework of one set of RPDs was produced conventionally using the lost wax casting technique. Intraoralscanning and computer-aided designing (CAD) were used to fabricate the metal frameworks of the other set of RPDs using direct metal laser sintering (DMLS) technology. The accuracy of fit of the two sets of RPDs was evaluated after 3 months using replica models. Patient satisfaction was assessed. Two years later, the fit accuracy of the DMLS prosthesis and patient satisfaction were re-evaluated. The accuracy of fit in the maxillary RPD with the DLMS manufactured metal framework showed better results in all areas except areas of rests (457 vs. 421 µm) and the major connector (850 vs. 512 µm). The mandibular RPD with DLMS manufactured metal framework showed only in the areas of the reciprocal arm and major connector better fit accuracy compared to the conventional RPD. The patient satisfaction with the DLMS manufactured RPDs was rated equally to the conventional one. The use of digital technologies in manufacturing RPDs seems promising regarding accuracy and patient satisfaction.

OBJECTIVES: In-office and lab milled prostheses are the staple for indirect restorations. It is therefore critical to determine their long-term bonding durability. METHODS: CAD/ CAM blocks of two classes of restorative materials: 1) a nano-ceramic reinforced polymer matrix (NCPM) and, 2) a polymer-infiltrated ceramic network (PICN) were bonded using four different universal adhesives (UA) and silane systems. A lithium disilicate glassceramic (LDS) was used as a reference. The blocks were bisected and bonded with different UA/resin-cement pairs. Bonded blocks were then cut into 1.0x1.0x12.0 mm bar specimens for microtensile bond testing. Half the bars were subjected to bond strength testing immediately and the other half after aging by 50,000 thermal cycles between 5°C and 55°C. ANOVA and post-hoc tests were used to compare mean bond strength among groups. RESULTS: NCPM presented consistently high bond strength regardless of bonding techniques, while the bond strength of PICN and LDS were lower when bonded with UA relative to traditional silanes. The more hydrophilic UA produced higher bond strengths. DISCUSSION: Glass-ceramics exhibited lower bond strength with UA than the conventional etch-rinse-silane techniques. However, UAs preserved bonding interface in the long-term. SIGNIFICANCE: NCPM displayed superior bond strength relative to PICN and LDS regardless of the type of adhesives and bonding techniques.

BACKGROUND: Psychosis treatment guidelines recommend cognitive behaviour therapy (CBT) and family intervention (FI), for all patients with first episode psychosis (FEP), though guidance borrows heavily from literature in adults from high income countries. To our knowledge, there are few randomized controlled trials (RCTs) examining the comparative effect of these commonly endorsed psychosocial interventions in individuals with early psychosis from high-income countries and no such trials from low and middle-income countries (LMICs). The present study aims to confirm the clinical-efficacy and cost-effectiveness of delivering culturally adapted CBT (CaCBT) and culturally adapted FI (CulFI) to individuals with FEP in Pakistan. METHOD: A multi-centre, three-arm RCT of CaCBT, CulFI, and treatment as usual (TAU) for individuals with FEP (n = 390), recruited from major centres across Pakistan. Reducing overall symptoms of FEP will be the primary outcome. Additional aims will include improving patient and carer outcomes and estimating the economic impact of delivering culturally appropriate psychosocial interventions in low-resource settings. This trial will assess the clinical-efficacy and cost-effectiveness of CaCBT and CulFI compared with TAU in improving patient (positive and negative symptoms of psychosis, general psychopathology, depressive symptoms, quality of life, cognition, general functioning, and insight) and carer related outcomes (carer experience, wellbeing, illness attitudes and symptoms of depression and anxiety). CONCLUSIONS: A successful trial may inform the rapid scale up of these interventions not only in Pakistan but other low-resource settings, to improve clinical outcomes, social and occupational functioning, and quality of life in South Asian and other minority groups with FEP. This study investigated the effect of silane coupling agent and universal adhesive application on repair bond strength of resin-based composite after bur grinding. Microhybrid resin composite (Charisma Smart) blocks (N=80; 8x8x4 mm3) were prepared, aged (37°C; 1 month), roughened, etched and randomly divided into two groups. Silane was applied to half of the groups (Porcelain Primer, Bisco), before one of the following universal primers/adhesives was applied: a) Scotchbond Universal (3M), b) All-Bond Universal (Bisco), c) G-Premio Bond (GC), and d) Clearfil SE Bond (Kuraray). In each adhesive group half of the group was photo-polymerized. The blocks were repaired with the same size resin composite and segmented into beams. Half of the beams were subjected to micro-tensile bond test (1 mm/min), while the other half was aged (37°C; 6 months) prior to testing. Failure modes were analyzed using Scanning Electron Microscopy (SEM). Data were analyzed using repeated measures of ANOVA, Tukey's post-hoc, and paired t-tests (alpha=0.05). The silane application did not affect the repair bond strength regardless of photo-polymerization of the adhesive resin. The repair bond strength decreased significantly after 6 months when adhesive resin was not photopolymerized (p⟨0.05). Photo-polymerizing universal adhesives might ensure higher repair bond strength and its maintenance after aging.

Two different restoration materials, an alkasite-based resin composite and a resinmodified glass ionomer cement were used to assess restoration of non-carious cervical dental lesions. This split mouth randomized controlled trial included 40 patients. After randomization both sides of the dental arch were restored with either an alkasite-based (Cention N, Ivoclar Vivadent) or a resin-modified glass ionomer cement (Voco GmbH) restoration. The placed fillings were evaluated by blinded additional operators 1, 6 months and 1 year after to the USPHS criteria (retention, marginal integrity and discoloration, anatomical form and secondary caries). Data were analyzed using Kendall's Coefficient of Concordance test and Chi-square tests using SPSS software (SPSS Inc., Version 20) (P=0.05). As for retention and anatomic form both materials performed similar after one month. However, the retention and anatomic form for alkasite based restorative Cention showed significantly better results after 6 months (p=0.013/p=0.003) and one year (p=0.026/p=0.008). The resin modified glass ionomer restoration showed higher discoloration after 6 months (p=0.025) and one year (p=0.018), while Cention performed better regarding marginal integrity at all time intervals. No secondary caries occurred. Alkasite based restorative materials displayed superior technical, mechanical and aesthetical performance in a follow-up period of one year and can therefore be recommended as an alternative to resin-modified glass ionomer cements.

This study evaluated the effect of the thickness and translucency of lithium disilicatebased glass ceramics on resin composite substrates on color change and masking effect. Laminate veneers were fabricated using IPS e.max CAD (A1) blocks with two different light transmittance values (High translucent [HT], Low translucent [LT]). Slices of two different thicknesses (0.3 mm, 0.5 mm) were obtained (n=10) and laminate veneers were cemented on the resin composite substrates of two different shades (A2, A3.5). The color change (ΔE values) was evaluated with the CIELab color system using a spectrophotometer, while the masking effect was calculated. The data were analyzed using independent-samples t-test and two-way analysis of variance. The ceramic thickness and translucency had a significant effect on final color and masking. When HT was used, and the laminate veneer thickness decreased (0.3 mm), the masking effect in ΔE values were lower (p⟨0.05). The ΔE values (⟩3.7) were clinically unacceptable. With the increase in thickness, translucency of porcelain laminate veneers decreases showing better color masking ability. Veneer thickness seems to be more effective on the restoration's masking ability than the shade of the substrate and translucency. Cinically, in case a 0.5-mm or thinner laminate veneer is planned, tooth color, resin cement and ceramic type should be considered.

The chemomechanical cleaning of the root canal system is performed using manual and rotary files. This study aimed to compare the effect of ProTaper, Reciproc and HyFlex rotary files considering 2.5% sodium hypochlorite and 2% chlorhexidine (CHX) as irrigating solutions on apical extrusion of intracanal debris. Single-canal mandibular premolars (N=165) were evaluated. The teeth were divided into 6 experimental groups (N=25) for root canal instrumentation using 3 file systems, (Reciproc, HyFlex and ProTaper) and two irrigation solutions, namely 2.5% sodium hypochlorite or 2% CHX. Fifteen teeth were also assigned into three control groups (n=5) with saline as irrigation solution. Eppendorf tubes were weighed and the teeth were placed inside. The canals were instrumented and the tubes weighed to determine the dry weight of extruded debris. Data were analyzed using one-way ANOVA and Bonferroni test (α=0.05). The mean amount of extruded debris using the sodium hypochlorite solution was significantly higher than other irrigating solutions (p⟨0.001). The mean amount of extruded debris using the Reciproc file system was significantly higher compared to ProTaper and HyFlex (p⟨0.001). The use of the sodium hypochlorite solution, single-file systems and reciprocal movement are associated with higher debris extrusion and might be associated with higher risk of postoperative pain.

To assess the clinical periodontal status of abutment teeth in regards of different design components of distal-extension removable partial dentures. Subjects (N=100) with acrylic or cobalt-chromium distal-extension removable partial dentures were enrolled and their periodontal parameters plaque and gingival indices [PI, GI], probing depths [PD], clinical attachment loss [CAL] and a mobility index [MI] evaluated. Denture base type, major connector, occlusal rests position, design of direct retainers, retention, stability and denture wearing habits were further evaluated. Acrylic RPDs were associated with higher mean±SE PI [1.70±0.74], GI [1.76±0.55], PD scores [2.47±1.02 mm] and CAL values [4.46±2.11 mm] compared to CO-CR RPDs [p⟨0.05]. For abutments, the PI [1.6±0.83], GI [1.72±0.57], PD [2.32±1.03] and CAL [4.26±2.08] were higher than their non-abutments counterparts [p⟨0.05]. CAL scores were found to be significantly higher for mandibular abutments compared to maxillary ones [P=0.002]. The highest PI [1.83±1.10] and GI [2.00±0.00] scores were associated with lingual bars and horse-shoe connectors, respectively. Full palatal coverage and lingual plates were associated with the highest PD [2.80±0.48] and CAL [4.70±0.37] scores. Acrylic RPDs, type of major connector, wrought wire clasps and distal occlusal rests may be considered as risk factors for periodontal disease progression in distal-extension removable partial denture wearers.

Background: Simple and accurate clinical indicators to detect metabolic abnormalities might be helpful for early management and lowering the risk of future consequences like cardiovascular disease and type 2 diabetes mellitus. Aim: The visceral adiposity index (VAI), lipid accumulation product (LAP), and atherogenic index of plasma (AIP) have been proposed as reliable, straightforward clinical markers and indications of metabolic syndrome (MetS). This study aimed to see how well these obesity and lipid-related indicators will predict MetS in adult Sudanese patients. Subjects and Methods: This community hospital-based case-control study included 420 middle-aged people (154 men and 266 women). Anthropometric measurements, weight (kilogram), height (meters), and waist circumference (WC) were evaluated, and the body mass index (BMI) and waist-to-height ratio (WHtR) were calculated. Fasting blood samples were collected for glycated hemoglobin (HbA1c) and lipid profile assessment. VAI, LAP, and AIP were calculated. Results: Significantly higher means of BMI, WC, WHtR, HbA1c, triglycerides (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), triglycerides/high-density lipoprotein cholesterol (TG/HDL-C) ratio, LAP, VAI, AIP, and significantly decrease in high-density lipoprotein cholesterol (HDL-C) were seen among MetS when compared with non-MetS group. LAP had a significant proportion with BMI, WC, WHtR, TG, TG/HDL-C, VAI, and AIP, and it is inversely related to HDL-C in the MetS group. On ROC analysis, LAP had the largest operating characteristic curves (AUC) for both gender 0.970 (0.948-0.993) for men and 0.964 (0.945-0.982) for women, followed by WC, and VAI, while BMI showed the lowest AUCs for men and women. In multiple regression analyses, AIP values increased significantly with LDL-C, DBP, HbA1c, LAP, and VAI. Conclusion: The LAP was considerably higher in middle-aged people with MetS in both gender and was considered the best diagnostic performance.

Chemiluminescence is a fascinating phenomenon that evolved in nature and has been harnessed by chemists in diverse ways to improve life. This Account tells the story of our research group's efforts to formulate and manifest spiroadamantane 1,2-dioxetanes with triggerable chemiluminescence for imaging and monitoring important reactive analytes in living cells, animals, and human clinical samples. Analytes like reactive sulfur, oxygen and nitrogen species, as well as pH and hypoxia can be indicators of cellular function or dysfunction and are often implicated in the causes and effects of disease. We begin with a foundation in binding-based and activity-based fluorescence imaging that has provided transformative tools for understanding biological systems. The intense light sources required for fluorescence excitation, however, introduce autofluorescence and light scattering that reduces sensitivity and complicates in vivo imaging. Our work and the work of our collaborators were the first to demonstrate that spiroadamantane 1,2-dioxetanes had sufficient brightness and biological compatibility for in vivo imaging of enzyme activity and reactive analytes like hydrogen sulfide (H2S) inside of living mice. This launched an era of renewed interest in 1,2-dioxetanes that has resulted in a plethora of new chemiluminescence imaging agents developed by groups around the world. Our own research group focused its efforts on reactive sulfur, oxygen, and nitrogen species, pH, and hypoxia, resulting in a large family of bright chemiluminescent 1,2-dioxetanes validated for cell monitoring and in vivo imaging. These chemiluminescent probes feature low background and high sensitivity that have been proven quite useful for studying signaling, for example, the generation of peroxynitrite (ONOO-) in cellular models of immune function and phagocytosis. This high sensitivity has also enabled real-time quantitative reporting of oxygen-dependent enzyme activity and hypoxia in living cells and tumor xenograft models. We reported some of the first ratiometric chemiluminescent 1,2-dioxetane systems for imaging pH and have introduced a powerful kinetics-based approach for quantification of reactive species like azanone (nitroxyl, HNO) and enzyme activity in living cells. These tools have been applied to untangle complex signaling pathways of peroxynitrite production in radiation therapy and as substrates in a split esterase system to provide an enzyme/substrate pair to rival luciferase/luciferin. Furthermore, we have pushed chemiluminescence toward commercialization and clinical translation by demonstrating the ability to monitor airway hydrogen peroxide in the exhaled breath of asthma patients using transiently produced chemiluminescent 1,2-dioxetanedione intermediates. This body of work shows the powerful possibilities that can emerge when working at the interface of light and chemistry, and we hope that it will inspire future scientists to seek out ever brighter and more illuminating ideas.

Porcelains and glass-ceramics have been used to produce CAD-milled veneers and crowns for zirconia copings and implant-abutments. This study evaluated the bondstrength of a polymer-infiltrated-ceramic-network to zirconia using two adhesive cement systems: Panavia 21 and Multilink Automix. Lithium disilicate and feldspathic porcelain were also tested as reference CAD-On materials. Long beams (3x6x40 mm³) of zirconia and short beams (3x6x15 mm³) of the CAD-On materials were prepared. Zirconia and each CAD-On material were bonded in a crossbeam arrangement and subjected to a modified tensile bond-strength test. Half of the samples in each group (n=10) were tested 5 days after bonding (baseline) and the remaining (n=10) underwent aging (50,000 thermocycles at 5°C and 55°C) prior to bond-strength testing. The effects of material, cement, and aging on the tensile bond-strength were tested using a three-way ANOVA. The reference lithium disilicate/Multilink system showed no significant differences in bond strength compared to polymer-infiltrated-ceramic-network and porcelain. The long-term retention of polymer-infiltrated-ceramic-network was not statistically different compared to the baseline values and the two reference materials. With comparable bond strength between all materials, polymer-infiltrated-ceramic-network is the favorable choice for CAD-On to zirconia copings and implant-abutments due to its superior resistance to fatigue fracture relative to porcelain.

Chemiluminescence imaging of bioanalytes using spiroadamantane 1,2-dioxetanes has gained significant attention due to improved signal-to-noise ratios and imaging depth compared to excitation-based probes, as well as their modifiable scaffolds that offer analyte-specific responses and tunable emissive properties. Among several strategies employed to amplify signals under aqueous conditions and to shift the emission into the bio-relevant red region, energy transfer to an adjacent fluorophore is a popular and effective method. This Minireview highlights spiroadamantane 1,2-dioxetane-based probes that operate via an energy transfer mechanism to detect bioanalytes both in vitro and in vivo. Probes that display both non-covalent and covalent interactions with fluorophores, as well as their applications in imaging specific analytes will be discussed.

Chemiluminescent iridium-based sensors which demonstrate oxygen dependent responses have been developed. The molecular probes, named IrCL-1, IrCL-2 and IrCL-3 consist of oxygen-sensitive iridium complexes attached to a spiroadamantane 1,2 dioxetane and operate via energy transfer from the chemiexcited benzoate to the corresponding iridium(III) complex. Complexing the iridium(III) center with π-extended ligands results in emission in the biologically relevant, near-infrared (NIR) region. All probes demonstrate varying oxygen tolerance, with IrCL-1 being the most oxygen sensitive. These probes have been further utilized for in vitro ratiometric imaging of oxygen, as well as for intraperitoneal, intramuscular and intratumoral imaging in live mice. To our knowledge, these are the first iridium-based chemiluminescent probes that have been employed for in vitro ratiometric oxygen sensing, and for in vivo tumor imaging.

OBJECTIVE: Increasing evidence has shown that the risks associated with surgical revascularization for intermittent claudication outweigh the benefits. The aim of our study was to quantify the cost of care associated with perioperative complications after elective lower extremity bypass (LEB) in patients presenting with intermittent claudication. METHODS: All patients undergoing first-time LEB for claudication in the Healthcare Database (2009-2015) were included. The primary outcome was in-hospital postoperative complications, including major adverse limb events (MALE), major adverse cardiac events (MACE), acute kidney injury, and wound complications. The overall crude hospital costs are reported, and a generalized linear model with log link and inverse Gaussian distribution was used to calculate the predicted hospital costs for specific complications. RESULTS: Overall, 7154 patients had undergone elective LEB for claudication during the study period. The median age was 66 years (interquartile range, 59-73 years), 67.5% were male, and 75.3% were white. Two thirds of patients (61.2%) had Medicare insurance, followed by private insurance (26.9%), Medicaid (7.7%), and other insurance (4.2%). In-hospital complications occurred in 8.5% of patients, including acute kidney injury in 3.0%, MALE in 2.8%, wound complications in 2.3%, and MACE in 1.0%. The overall median crude hospital cost was $11,783 (interquartile range, $8911-$15,767) per patient. The incremental increase in cost associated with a postoperative complication was significant, ranging from $6183 (95% confidence interval, $4604-$7762) for MALE to $10,485 (95% confidence interval, $6529-$14,441) for MACE after risk adjustment. CONCLUSIONS: Postoperative complications after elective LEB for claudication are not uncommon and increase the in-hospital costs by 46% to 78% depending on the complication. Surgical revascularization for claudication should be used sparingly in carefully selected patients.

An attractive catalytic pathway for the conversion of water to oxygen would involve two metal oxide centers combining in a constructive sense to make O═O. This prospect makes the study of certain dinuclear transition metal complexes particularly attractive. In this work, we describe the design and synthesis of two symmetrical bis-tridentate polypyridine ligands 6 and 12 that bind two RuII centers at a separation of 3.6 Šin 7 and 5.7 Šin 13. In the presence of CeIV at pH = 1, these systems oxidize water with the system having the more proximal metals being more reactive. In the case of the more proximal metal centers, the bridging ligand is a 3,6-disubstituted pyridazine which, under the influence of CeIV, cleaves into two [Ru(bpc)(pic)2CH3CN]+ fragments (14) which then function as the actual catalyst (bpc = 2,2'-bipyridine-6-carboxylate, pic = 4-methylpyridine). The second dinuclear catalyst contains a central pyrimidine ring which is less sensitive to oxidative decay and hence less reactive. Caution is advised in the use of CeIV as a sacrificial electron acceptor due to unexpected oxidative decay of the catalyst.

Density functional theory (DFT) calculations on Fe2S2(CO)6-2n(PMe3)2n for n = 0, 1, and 2 reveal that the most electron-rich derivatives (n = 2) exist as diferrous disulfides lacking an S-S bond. The thermal interconversion of the FeII2(S)2 and FeI2(S2) valence isomers is symmetry-forbidden. Related electron-rich diiron complexes [Fe2S2(CN)2(CO)4]2- of an uncertain structure are implicated in the biosynthesis of [FeFe]-hydrogenases. Several efforts to synthesize electron-rich derivatives of Fe2(µ-S2)(CO)6 (1) are described. First, salts of iron persulfido cyanides [Fe2(µ-S2)(CO)5(CN)]- and [Fe2(µ-S2)(CN)(CO)4(PPh3)]- were prepared by the reactions of NaN(tms)2 with 1 and Fe2(µ-S2)(CO)5(PPh3), respectively. Alternative approaches to electron-rich diiron disulfides targeted Fe2(µ-S2)(CO)4(diphosphine). Whereas the preparation of Fe2(µ-S2)(CO)4(dppbz) was straightforward, that of Fe2(µ-S2)(CO)4(dppv) required an indirect route involving the oxidation of Fe2(µ-SH)2(CO)4(dppv) (dppbz = C6H4-1,2-(PPh2)2, dppv = cis-C2H2(PPh2)2). DFT calculations indicate that the oxidation of Fe2(µ-SH)2(CO)4(dppv) produces singlet diferrous disulfide Fe2(µ-S)2(CO)4(dppv), which is sufficiently long-lived as to be trapped by ethylene. The reaction of 1 and dppv mainly afforded Fe2(µ-SCH=CHPPh2)(µ-SPPh2)(CO)5, implicating a S-centered reaction.

Scientific data analysing color masking abilities of chairside CAD/CAM materials is lacking. The purpose of this in-vitro study was to evaluate the thickness and shade influence of three materials on their optical behaviour. Three materials: a) LD: Lithium disilicate glass ceramic (Emax, Ivoclar Vivadent), b) LDS: Lithium-disilicate-strengthened aluminosilicate glass ceramic (N!ce, Straumann) and c) RNC: Resin Nanoceramic (Lava Ultimate, 3M ESPE) were polished in different shades (A1,A2,A3) and thicknesses (0.1- 1.2mm). Specimens (N=108; n=36 per group) (12x12x1mm3) were positioned on resin composite base (Clearfil AP-X, Kuraray) in shade A3. Spectrophotometric measurements were performed and the parameters thickness, shade and material were analysed using three-way ANOVA, and pairwise T-tests (P-values ⟨ 0.05). Both the shade (p⟨0.001) and the interaction of material in correlation to thickness (p⟨0.001) were significant. Shade value A1-A3 comparisons were significant A1 vs. A2 (p=0.045); A2 vs. A3 (p=0.002); A1 vs. A3 (p⟨0.001)). A significant correlation of the material and thickness was observed when comparing LD and LDS (p=0.007) at the thickness of 0.1-0.4 mm. Masking abilities were influenced by material and thickness choice. Reinforced glass ceramics showed the best results in the smallest thickness tested (0.1-0.4 mm). LDS could be considered as an advantageous alternative in minimal-invasive cases.

Scientific evidence regarding conditioning of different ceramic and hybrid materials and their bonding on titanium abutments is lacking. Titanium disks (Tritan) (N=450, n=15) were randomly cemented onto five different ceramic and hybrid materials, namely 1. Zenostar T, 2. Lava Ultimate, 3. IPS e.max CAD, 4. Vita Enamic multicolor and 5. G-ceram using three different cements, Panavia 21, TheraCem and Multilink hybrid abutment. Half of all specimens were thermocycled (5000 cycles, 5-55°C), while the other half were kept dry. Macro shear bond testing was conducted using a universal testing machine. Failure types were classified using a digital microscope. Data was statistically analyzed with three-way ANOVA and Tukey HSD post hoc tests. Both the ceramic (P⟨0.0001) and cement type(P⟨0.0001) significantly affected the shear bond strength(MPa), while thermocycling did not (P⟩0.05). The incidence of cohesive (50.34%) and adhesive failures (49.66%) were not significantly different. As for implant superstructures, when ceramics are bonded to titanium bases, the ceramic and cement type both have an impact on the bond strengths along with the conditioning and bonding protocols for each substrate. An equal affinity of the cements tested to the ceramic, hybrid materials and to titanium can be assumed. Combination of zirconia and TheraCem can be recommended for clinical use.

To evaluate the antibacterial efficacy of HybenX, an endodontic irrigant against Enterococcus faecalis biofilm and determine its efficacy in removal of smear layer. Forty root canals were inoculated with E. faecalis for three weeks, divided into 4 groups and irrigated with: Group1: 5 ml 2.5% sodium hypochlorite; Group2: 5 ml 2% chlorhexidine gluconate; Group3: 5 ml HybenX; Group4: 5 ml distilled water (control). Dentin shavings were collected and plated on agar plates, followed by colony forming unit (CFU) determination. For smear layer removal examination, 30 single rooted teeth were instrumented and divided into 3 groups and treated with: GroupI: 5 ml of 17% EDTA; GroupII:5 ml of HybenX; and GroupIII: 5 ml of distilled water. Samples were then subjected to SEM examination. All irrigants showed a significant reduction in CFUs compared to the control group (P⟨ .001), but none compared to each other. Regarding smear layer removal in the apical third, EDTA removed smear layer more effectively than HybenX (P = .014). HybenX demonstrated good antimicrobial efficacy against E. faecalis biofilm and removed the smear layer effectively in coronal and middle third of the root canal system. HybenX can be considered as a promising irrigating agent in root canal treatment of infected teeth.

BACKGROUND: The aim of this study was to provide a nationwide, all-payer, real-world cost analysis of endovascular aortic aneurysm repair (EVAR) versus open aortic aneurysm repair (OAR) in patients with nonruptured abdominal aortic aneurysms (non-rAAA). METHODS: All non-rAAA patients registered between July 2009 and March 2015 in the Premier Healthcare Database were analyzed. The Student t-test and the χ2 test were used for continuous and categorical variables, respectively; median value comparisons were done with the Wilcoxon-Mann-Whitney rank-sum test. The in-hospital absolute mean total cost (sum of fixed cost and variable cost) and subcategories were analyzed after adjustment for inflation at July 2015. Fixed costs included all overhead costs while variables costs included in-hospital services including procedures, room and board, services provided by hospital staff, and pharmacy costs. Total cost was stratified based on admission type (emergency vs nonemergency), 75th percentile of length of hospital stay among individual procedures (expected vs extended stay), mortality, and complications. Student t-test and Fisher's analysis of variance were used for comparing mean cost. Year-wise comparison of mean cost was done with analysis of variance to look for a trend over time. RESULTS: Our study cohort included 38,809 non-rAAA patients (33,171 EVAR and 5638 OAR). The mean total cost of index admission was lower in EVAR in comparison with OAR ($32,052 vs $36,091; P < .001), with lower fixed costs ($11,309 vs $16,818; P < .001) and higher variable costs ($20,743 vs $19,272; P < .001). Cost of pharmacy, labor, operating room, room and board and other costs were significantly higher with OAR, whereas the supply cost was higher with EVAR. The expected hospital length of stay of patients who underwent EVAR was associated with a higher total cost ($27,271 vs $25,680; P < .001) and a higher variable cost ($18,186 vs $13,671; P < .001) than OAR patients. However, the extended hospital stay of patients who underwent EVAR had lower costs in all categories compared with the extended length of stay of those who underwent OAR. Mortality associated with EVAR was costlier than OAR associated mortality (mean $72,483 vs $59,804; P = .017). From 2009 to 2014, the mean total cost of EVAR increased significantly by 18.5% ($28,745 vs $34,049; P < .001) owing to a 7.8% increase in fixed costs ($10,931 vs $11,789; P < .001) and a 25.0% increase in variable costs ($17,804 vs $22,257; P < .001). The mean total cost OAR remained stable over time. CONCLUSIONS: Overall hospitalization costs associated with EVAR of non-rAAA was lower than the hospitalization cost of OAR. Interestingly, we found that, among patients who had an expected hospital length of stay, the hospitalization cost after OAR was significantly lower than after EVAR. The average hospitalization cost of OAR was stable during the 5 years study period, whereas the hospitalization cost of EVAR increased significantly over time. Further studies are required to identify reasons for increased costs associated with EVAR.
