RESUMEN

Background: Doxorubicin is a highly effective anti-cancer drug that causes left ventricular (LV) dysfunction and induces late-onset cardiomyopathy. However, an effective and clinically applicable preventive treatment is yet to be discovered. Objective: Cardiac-Extracorporeal shockwave therapy (C-ESWT) has been suggested to treat inflammatory and ischemic diseases and protect cardiomyocytes from doxorubicin-induced cardiomyopathy. This study aims to assess the safety and efficacy of C-ESWT in the prevention of subclinical cardiotoxicity. Methods: We enrolled 64 breast cancer patients. C-ESWT group 33 patients were treated with our C-ESWT (200 shots/spot at 0.09 mJ/mm2 for 20 spots, 3 times every six weeks). The efficacy endpoints were the difference in left ventricular global longitudinal strain (LVGLS) change by 2D speckle tracking echocardiography and chemotherapy-related cardiac dysfunction (CTRCD). Echocardiography was performed on the baseline line and every 4 cycles of chemotherapy, followed by a follow-up 3,6 months after chemotherapy to compare the incidence of cardiomyopathy of subclinical LV dysfunction due to chemotherapy between the two groups. Results: Participants averaged 50 ± 9 years in age, 100% female. In the results of follow-up 6 months after the end of chemotherapy, there was a significant difference in delta LVGLS between the C-ESWT group and the control group (LVGLS; -1.1 ± 10.9% vs. -11.5 ± 11.6% p-value; <0.001). A total of 23% (15 patients) of patients developed CTRCD (Control group; 13 vs. C-ESWT group; (2). C-ESWT was performed safely without any serious adverse events. Conclusion: In this prospective study, C-ESWT established efficacy in preventing subclinical cardiotoxicity, especially in breast cancer patients using doxorubicin chemotherapy, and the safety of C-ESWT. A photovoltaic-electrolysis-PEM hybrid model was developed for a feasibility study, and simulations of several scenarios in Korea were performed. The solar irradiance was derived from the University of Arizona solar irradiance based on satellite-Korea Institute of Energy Research model which provides the satellite imagery over the Korean peninsula every 15 min. In Korea, the annual average solar irradiance is 1,310 kWh m-2 with a maximum of 1,440 kWh m-2 in 2017. Electricity load and solar irradiance information were used to test the performance model of the photovoltaic-electrolysis-PEM hybrid system for baseload and several peak load shave runs. When the baseload was set at 4200 MW, the total capacity of the Photovoltaic plants was 58.5 GWp. In contrast, the hybrid system reduced the peak load more efficiently during daytime. In particular, the capacity factor of the Proton Exchange Membrane system increased in winter because the solar irradiance is relatively weak in that season. These results provide useful insights for the development of control logic models for the PV-electrolysis-PEM system in micro-grid setups.

With the increased utilization of robot thyroidectomy in recent years, surgical proficiency is the paramount consideration. However, there is no single perfect or ideal method for measuring surgical proficiency. In this study, we evaluated the learning curve of robotic thyroidectomy using various parameters. A total of 172 robotic total thyroidectomies were performed by a single surgeon between March 2014 and February 2018. Cumulative summation analysis revealed that it took 50 cases for the surgeon to significantly improve the operation time. Mean operation time was significantly shorter in the group that included the 51st to the 172nd case, than in the group that included only the first 50 cases (132.8 ± 27.7 min vs. 166.9 ± 29.5 min; p < 0.001). On the other hand, the surgeon was competent after the 75th case when postoperative transient hypoparathyroidism was used as the outcome measure. The incidence of hypoparathyroidism gradually decreased from 52.0%, for the first 75 cases, to 40.2% after the 76th case. These results indicated that the criteria used to assess proficiency greatly influenced the interpretation of the learning curve. Incorporation of the operation time, complications, and oncologic outcomes should be considered in learning curve assessment.

The vehicular ad hoc network (VANET) has been identified as one of the most promising technologies for managing future intelligent transportation systems. This paper proposes a distributed transmission power adjustment algorithm for communication congestion control and awareness enhancement to address communication congestion problems that can arise in VANETs. The objective of the proposed algorithm is to provide maximum awareness of surrounding vehicles' status while maintaining a communications channel load below the allowed threshold. The proposed algorithm accomplishes this by adjusting the transmission range of each vehicle in the network progressively and gradually, while monitoring the communications channel load of each vehicle. By changing the transmission range of a vehicle little by little according to the communications channel load of its neighboring vehicles, the algorithm finds the optimal transmission range that provides maximum awareness without resulting in communications congestion. In addition, the proposed algorithm appropriately controls the channel load in a fair manner without sacrificing awareness of specific vehicles in the congested situation. This allows nearby vehicles to obtain more peripheral information to help them stay away from potential hazards and maintain safety. The proposed algorithm was implemented in a simulation environment, and its performance was validated in various traffic scenarios. The simulation results show that the proposed algorithm can deal with communication congestion by controlling the transmission power fairly to a target threshold in various traffic situations.
