RATIONALE & OBJECTIVE: Although postoperative acute kidney injury (AKI) is a serious complication after cardiac surgery, preventive measures are limited. Despite the known association of preoperative low magnesium levels with cardiac surgery-related atrial fibrillation, the association between preoperative magnesium concentration and postoperative AKI has not been fully elucidated. This study evaluated the association between preoperative serum magnesium level and the development of AKI after cardiac surgery. STUDY DESIGN: Retrospective observational cohort study. SETTING & PARTICIPANTS: Patients aged≥18 years who underwent cardiac surgery at 2 South Korean tertiary hospitals between 2006 and 2020 were identified from medical records. Patients with missing information, an estimated glomerular filtration rate<15mL/min/1.73m2, receiving maintenance dialysis, or a history of AKI treated by dialysis within 1 year before surgery were excluded. EXPOSURE: Preoperative serum magnesium levels. OUTCOME: Postoperative AKI within 48 hours after surgery, defined using the Acute Kidney Injury Network (AKIN) criteria, and dialysis-treated AKI within 30 days after surgery. ANALYTICAL APPROACH: Multivariable logistic regression analysis. RESULTS: Among the 9,766 patients (median age, 64.0 years; 60.1% male), postoperative AKI and dialysis-treated AKI were observed in 40.1% and 4.3% patients, respectively. Postoperative AKI was more prevalent in patients with lower serum magnesium levels (44.9%, 41.4%, 39.4%, and 34.8% in quartiles 1-4, respectively). Multivariable logistic regression analysis revealed that the odds ratios (ORs) for postoperative AKI were progressively larger across progressively lower quartiles of serum magnesium concentration (adjusted ORs of 1.53 [95% CI, 1.33-1.76], 1.29 [95% CI, 1.12-1.48], 1.15 [95% CI, 1.01-1.31] for quartiles 1-3, respectively, relative to quartile 4, P for trend<0.001). Preoperative hypomagnesemia (serum magnesium level<1.09mg/dL) was also significantly associated with AKI (adjusted OR, 1.39 [95% CI, 1.10-1.77]) and dialysis-treated AKI (adjusted OR, 1.67 [95% CI, 1.02-2.72]). LIMITATIONS: Causality could not be evaluated in this observational study. CONCLUSIONS: Lower serum magnesium levels were associated with a higher incidence of AKI in patients undergoing cardiac surgery.

Photoplethysmography (PPG) is a simple and cost-efficient technique that effectively measures cardiovascular response by detecting blood volume changes in a noninvasive manner. A practical challenge in the use of PPGs in real-world applications is noise reduction. PPG signals are likely to be compromised by various types of noise, such as scattering or motion artifacts, and removing such compounding noises using a monotonous method is not easy. To this end, this paper proposes a neural PPG denoiser that can robustly remove multiple types of noise from a PPG signal. By casting the noise reduction problem into a signal restoration approach, we aim to achieve a solid performance in the reduction of different noise types using a single neural denoiser built upon transformer-based deep generative models. Using this proposed method, we conducted the experiments on the noise reduction of a PPG signal synthetically contaminated with five types of noise. Following this, we performed a comparative study using six different noise reduction algorithms, each of which is known to be the best model for each noise. Evaluation results of the peak signal-to-noise ratio (PSNR) show that the neural PPG denoiser is superior in three out of five noise types to the performance of conventional noise reduction algorithms. The salt-and-pepper noise type showed the best performance, with the PSNR of the neural PPG denoiser being 36.6080, and the PSNRs of the other methods were 19.8160 and 32.8234. The Poisson noise type performed the worst, showing a PSNR of 33.0090; the PSNRs of other methods were 35.1822 and 33.4795, respectively. Thereafter, an experiment to recover a signal synthesized with two or more of the five noise types was conducted. When the number of mixed noises was two, three, four, and five, the PSNRs were 29.2759, 27.8759, 26.5608, and 25.9402, respectively. Finally, an experiment to recover motion artifacts was also conducted. The synthesized motion artifact signal was created by synthesizing only a certain ratio of the total signal length. As a result of the motion artifact signal restoration, the PSNRs were 25.2872, 22.8240, 21.2901, and 19.9577 at 30%, 50%, 70%, and 90% motion artifact ratios, respectively. In the three experiments conducted, the neural PPG denoiser showed that various types of noise were effectively removed. This proposal contributes to the universal denoising of continuous PPG signals and can be further expanded to denoise continuous signals in the general domain.

Oxygen saturation (SPO2) is an important indicator of health, and is usually measured by placing a pulse oximeter in contact with a finger or earlobe. However, this method has a problem in that the skin and the sensor must be in contact, and an additional light source is required. To solve these problems, we propose a non-contact oxygen saturation measurement technique that uses a single RGB camera in an ambient light environment. Utilizing the fact that oxygenated and deoxygenated hemoglobin have opposite absorption coefficients at green and red wavelengths, the color space of photoplethysmographic (PPG) signals recorded from the faces of study participants were converted to the YCgCr color space. Substituting the peaks and valleys extracted from the converted Cg and Cr PPG signals into the Beer-Lambert law yields the SPO2 via a linear equation. When the non-contact SPO2 measurement value was evaluated based on the reference SPO2 measured with a pulse oximeter, the mean absolute error was 0.537, the root mean square error was 0.692, the Pearson correlation coefficient was 0.86, the cosine similarity was 0.99, and the intraclass correlation coefficient was 0.922. These results confirm the feasibility of non-contact SPO2 measurements.

Pulse rate variability (PRV) refers to the change in the interval between pulses in the blood volume pulse (BVP) signal acquired using photoplethysmography (PPG). PRV is an indicator of the health status of an individual's autonomic nervous system. A representative method for measuring BVP is contact PPG (CPPG). CPPG may cause discomfort to a user, because the sensor is attached to the finger for measurements. In contrast, noncontact remote PPG (RPPG) extracts BVP signals from face data using a camera without the need for a sensor. However, because the existing RPPG is a technology that extracts a single pulse rate rather than a continuous BVP signal, it is difficult to extract additional health status indicators. Therefore, in this study, PRV analysis is performed using lab-based RPPG technology that can yield continuous BVP signals. In addition, we intended to confirm that the analysis of PRV via RPPG can be performed with the same quality as analysis via CPPG. The experimental results confirmed that the temporal and frequency parameters of PRV extracted from RPPG and CPPG were similar. In terms of correlation, the PRVs of RPPG and CPPG yielded correlation coefficients between 0.98 and 1.0.

Photoplethysmography (PPG) is an optical measurement technique that detects changes in blood volume in the microvascular layer caused by the pressure generated by the heartbeat. To solve the inconvenience of contact PPG measurement, a remote PPG technology that can measure PPG in a non-contact way using a camera was developed. However, the remote PPG signal has a smaller pulsation component than the contact PPG signal, and its shape is blurred, so only heart rate information can be obtained. In this study, we intend to restore the remote PPG to the level of the contact PPG, to not only measure heart rate, but to also obtain morphological information. Three models were used for training: support vector regression (SVR), a simple three-layer deep learning model, and SVR + deep learning model. Cosine similarity and Pearson correlation coefficients were used to evaluate the similarity of signals before and after restoration. The cosine similarity before restoration was 0.921, and after restoration, the SVR, deep learning model, and SVR + deep learning model were 0.975, 0.975, and 0.977, respectively. The Pearson correlation coefficient was 0.778 before restoration and 0.936, 0.933, and 0.939, respectively, after restoration.

The purpose of this study is to provide essential data for the establishment of education and policy for the formation of healthy lifestyles of adolescents in the future by analyzing the patterns of changes in society due to the prolonged COVID-19 in the physical activities, sleeping habits, obesity, and mental health of Korean adolescents. To this end, a total of 147,346 adolescents were selected and analyzed according to the purpose of the study in the 2018 (14th), 2019 (15th), and 2020 (16th) raw data of the "Youth Health Behavior Online Survey," an annual national approval statistical survey conducted by a Korean government agency. The study examined changes in the physical activity, obesity, sleep, and mental health of Korean adolescents due to COVID-19. The physical activity rate of Korean adolescents in 2019 decreased by 5.3% from 2018. In addition, the physical activity rate in 2020 decreased by 2.1% compared to 2019. It was found that physical activity steadily decreased (p < 0.001). The obesity rate increased by 0.9% in 2019 compared to 2018 and by 1.8% in 2020 compared to 2019. Although the obesity rate steadily increased, it was found that it was accelerated due to COVID-19 (p < 0.001). Looking at the subjective sleep satisfaction rate of Korean adolescents, in 2019, it was 0.1% lower than in 2018, while in 2020, when COVID-19 began, it increased by 3.5% compared to 2019. It was found that satisfaction with sleep increased after COVID-19. Finally, the mental health characteristics of Korean adolescents by year were divided into stress and depression. Stress decreased by 1% compared to 2019 and 2018 and by 6.2% compared to 2020 and 2019. Depression increased by 1% in 2019 compared to 2018 and decreased by 3.4% in 2020 compared to 2019. In other words, stress and depression decreased after COVID-19. In 2020, when COVID-19 occurred, it was confirmed that there was a change in the health behavior of adolescents compared to 2018 and 2019. Therefore, active responses from schools, families, and communities are required to foster healthy lifestyle habits in social changes such as COVID-19.

Purpose: This study aimed to examine the relationship between smartphone dependency (SD) and mental health (MH) in adolescents in order to develop and implement plans pertaining to SD control. Methods: Raw data from the 16th Online Adolescent Health Behavior Survey in 2020 were analyzed. A total of 482 respondents were selected as study subjects based on their experience of smartphone overdependence (SO), specifically, 241 participants whose score for SO was 37 or higher (Group 2) and age- and gender-matched 241 participants whose score was lower than 10 (Group 1). Results: Frequency analysis, cross-tab analysis (χ2 test), and multinomial logistic regression were performed Analysis shows that the MH affecting the increase in SO is the subjective perception of happiness, subjective perception of stress, sadness and despair, and experience of Loneliness. But, the variable affecting the reduction is the subjective evaluation of sleep quality. The likelihood of SO increased as adolescents felt unhappier [Exp (ß) = 2.408] and more stressed [Exp (ß) = 4.453] and more often felt lonely [Exp (ß) = 8.149], but the likelihood decreased as they had neither sufficient nor insufficient sleep duration [Exp (ß) = 0.344]. The findings suggest that it is necessary to develop aggressive measures for the prevention and management of MH in adolescents showing SO because mental health is closely linked to SD. In developing the measures, realistic approaches to widely pervasive SO among adolescents should be explored by taking into account MH factors, that is, predictors of SO, and the characteristics of youths, such that they can self-control smartphone use and form desirable life habits.

BACKGROUND: The coronavirus disease 2019 (COVID-19) has rapidly turned into a public health emergency worldwide; however, the risk factors for infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) have not been well-described. We aimed to identify the clinical risk factors for SARS-CoV-2 infections in Korea, where social distancing and face masks have been strongly recommended. METHODS: The data of individuals who underwent the reverse transcription polymerase chain reaction test for SARS-CoV-2 between January 3 and May 31, 2020 were retrieved from the Health Insurance Review and Assessment Service dataset. We used multivariable logistic regression models to identify the risk factors for SARS-CoV-2 infections in the population. RESULTS: We retrieved the results of 219,729 SARS-CoV-2 tests, of which 7,333 were positive results. In the multivariable analysis, female sex was associated with a higher risk of testing positive for SARS-CoV-2 [odds ratio (OR) =1.30, 95% confidence interval (CI): 1.24-1.37, P<0.0001]. Additionally, populations living in areas that had large outbreaks of COVID-19 were at an increased risk of testing positive for SARS-CoV-2 (OR =6.87, 95% CI: 6.55-7.21, P<0.0001). The odds of a positive test were greater for the Medical Aid beneficiaries (OR =1.99, 95% CI: 1.82-2.18, P<0.0001) than for the National Health Insurance beneficiaries. Individuals with diabetes mellitus (DM) were more likely to test positive (OR =1.15, 95% CI: 1.07-1.24, P=0.0002). CONCLUSIONS: Women, individuals living in areas with large outbreaks of COVID-19, Medical Aid beneficiaries, and individuals with DM might have greater risks of contracting SARS-CoV-2 infections despite practicing social distancing and using face masks.

Background/Aims: Surgical resection, transplantation, and radiofrequency ablation (RFA) are generally accepted as amenable treatments for small hepatocellular carcinoma (HCC). Recently drug-eluting beads (DEB) which had several treatment advantages were introduced for transarterial chemoembolization (TACE). The aim of this study was to evaluate feasibility and safety of DEB-TACE compared with RFA for the treatment of single small HCC. Methods: In this pilot non-randomized trial, we assessed retrospective data of 40 patients who underwent DEB-TACE (n=21) or RFA (n=19) for single small (≤3 centimeter in greatest dimension) HCC. The primary outcomes were tumor response and time to recurrence. The secondary outcome was treatment-related complications. Results: Complete response rate to DEB-TACE and RFA after first follow-up assessment was 90.5% and 94.7%, respectively (P=1.000). During mean follow-up of 87.6 months (95% confidence interval, 74.4-102), 7 patients experienced local recurrence. The 6- and 12-month cumulative local recurrence rate was 5.0% and 21.8% in DEB-TACE vs. 11.1% and 17.0% in RFA group (P=0.877). A total 14 distant intrahepatic recurrences were developed and 12- and 24-month cumulative distant intrahepatic recurrence rate was 20.6% and 42.7% in DEB-TACE vs. 17.2% and 36.3% in RFA group (P=0.844). Two patients experienced gangrenous cholecystitis after DEB-TACE requiring cholecystectomy as treatment-related adverse event. Conclusions: Tumor response and recurrence rate after single session of DEB-TACE or RFA were similar. DEB-TACE could be applied selectively in patients with a single small HCC if the other therapeutic modality is unfeasible.
