RESUMEN

BACKGROUND: Otitis externa is associated with a lack of bacterial/fungal diversity in atopic dermatitis. Clinical experience has shown that use of topical corticosteroids in the ear canal (EC) can prevent otitis. No data are available on the impact of this treatment on the EC microbiota. HYPOTHESIS/OBJECTIVES: To observe the bacterial/fungal diversity in the EC and the clinical effect of topical corticosteroids administered over a four week period in atopic dogs without active otitis. ANIMALS: Ten atopic dogs without active otitis. METHODS AND MATERIALS: Mometasone was applied in the right EC, while the left was used as control. A clinical and cytological evaluation of the EC was performed. Swabs of each EC were analysed using next-generation sequencing methods. RESULTS: At the beginning of the trial, variations in microbiota and mycobiota were observed between dogs and also within individuals. Statistically, no significant difference was observed in alpha and beta diversity between the treated and the untreated group over time. Clinically, right and left EC diversities were no different at Day (D)28 (P = 0.28). A significant difference was noted between D0 and D28 for the treated ears (P = 0.012) and not for the untreated ears (P = 0.63). No cytological evidence of microbes was found for treated ECs at D28. CONCLUSIONS AND CLINICAL RELEVANCE: These data suggest that the use of topical corticosteroids as proactive treatment is unlikely to increase the risk of secondary microbial overgrowth. The positive clinical effect of this proactive treatment seems to be supported through cytological and otoscopic improvement.

PURPOSE: To determine the frequency, characteristics, and resource use related to the emergency department (ED) encounters resulting from electric scooter use in a major metropolitan area. METHODS: This Institutional Review Board-approved study was conducted at a four-hospital healthcare system. Our clinical data warehouse was retrospectively searched from 5/3/2018 (the date electric scooters were introduced) through 8/15/2019 for various forms of the word "scooter" in triage notes. Demographic variables, arrival mode, length of stay, disposition, and resource utilization, including diagnostic radiology, were extracted. RESULTS: Over the 471-day study window, 293 unique patients presented with e-scooter injuries (0.62 mean ED visits/day). When broken down into 8-h periods, there was a significant increase (p = 0.048) from Friday after 5 PM through Sunday night. Thirty-two percent of patients arrived at the ED during (newly enacted at the time of study) nighttime e-scooter ban hours (9 PM-4 AM). There was a range of one to nine diagnostic radiology examinations per patient, with 100% (293) of patients receiving at least one diagnostic radiology examination. A total of 710 diagnostic radiology examinations were performed on the entire cohort (mean 2.4 per patient): 77.2% (548) were radiographs and 22.1% (157) were computed tomography (CT). CONCLUSION: ED visits resulting from electric scooter injuries are common and increasing. E-scooter injury patients disproportionately present on evenings and weekends, possibly exacerbating already busy periods in the ED. Healthcare resource needs and availability should be considered when developing policy about electric scooter use and distribution, particularly involving emergency care providers in close proximity to e-scooter distribution centers.

Otitis media can be a consequence of chronic otitis externa and could represent a perpetuating factor. While the microbiota of the EEC in healthy dogs and in the presence of otitis externa has been described, only sparse information is available concerning the normal microbiota of the middle ear. The objective was to compare the tympanic bulla (TB) with the external ear canal (EEC) microbiota in healthy dogs. Six healthy experimental Beagle dogs were selected based on the absence of otitis externa, negative cytology and bacterial culture from the TB. Samples from the EEC and TB were collected directly after death using a total ear canal ablation and lateral bulla osteotomy. The hypervariable segment V1-V3 of the 16S rDNA was amplified and sequenced with a MiSeq Illumina. The sequences were analyzed by the Mothur software using the SILVA database. No significant differences between the EEC and TB microbiota for the Chao1 richness index (p = 0.6544), the Simpson evenness index (p = 0.4328) and the reciprocal Simpson alpha diversity (p = 0.4313) were noted (Kruskal-Wallis test). A significant difference (p = 0.009) for the Chao1 richness index between the right and left EEC was observed. The microbiota profile was similar in the EEC and the TB of the Beagles.

Sensory and autonomic neuropathy was diagnosed in a five-month-old Border Collie puppy, who presented with progressive self-mutilation, proprioceptive ataxia and urinary incontinence. In the Border Collie, sensory neuropathy is different from what is observed in acral mutilation syndrome, as the genetic mutation is linked to an inversion disrupting the FAM134B gene. Diagnosis was based on history, clinical signs and genetic testing. The prognosis of sensory neuropathies is poor and no curative treatment is available. In the present case, oclacitinib was started for symptomatic treatment of the self-mutilation. A good control of the self-mutilation was quickly observed with an improvement in quality of life for five months. Unfortunately, progression of neurological signs with severe proprioceptive deficits, ataxia, muscular atrophy and urinary/fecal incontinence was observed. Five months after diagnosis, the owner elected for euthanasia.

A discrepancy between cytology and bacterial culture methods is sometimes observed in canine otitis externa. The objective of this study was to compare results from cytology, bacterial culture and 16S amplicon profiling. Twenty samples from 16 dogs with chronic suppurative otitis externa were collected. A direct cytological evaluation was carried out during the consultations. Aerobic bacterial culture and susceptibility were performed by an external laboratory used in routine practice. For 16S amplicon profiling, DNA was extracted and the hypervariable segment V1−V3 of the 16S rDNA was amplified and then sequenced with a MiSeq Illumina sequence carried out by the Mothur software using the SILVA database. A good correlation between cytology and bacterial culture was observed in 60% of the samples. Some bacterial species revealed by bacterial culture were present with low relative abundance (<10%) in 16S amplicon profiling. Some bacterial species revealed by the 16S amplicon profiling analysis were not identified with culture; most of the time, the offending species was a Corynebacterium. To conclude, a careful interpretation of the results of bacterial culture should be made and always be in agreement with the cytology. The 16S amplicon profiling method appears to be a more sensitive method for detecting strains present in suppurative otitis but does not provide information on bacterial susceptibility.

OBJECTIVE: To determine the degree of fluctuation in tracheal dimensions between forced inspiration and passive expiration in healthy dogs of various sizes. ANIMALS: 10 client-owned dogs with no evidence of respiratory disease or tracheal collapse. PROCEDURES: Anesthetized dogs underwent a computed tomographic examination during forced inspiration and induced but passive expiration to assess tracheal dimensions. Tracheal height, width, and cross-sectional area were measured at inspiration and expiration, and percentage change in dimension was calculated for each variable. RESULTS: Measurements were acquired in 10 dogs that ranged in body weight from 3.5 to 47.8 kg. Tracheal cross-sectional area at inspiration and expiration was associated with body weight at all 3 tracheal regions. The percentage change in tracheal height and cross-sectional area was associated with body weight in the cervical but not the thoracic-inlet or thoracic regions. The tracheal cross-sectional area changed by as much as 24.2% (mean, 5.5%), 20.0% (mean, 6.0%), and 18.6% (mean, 6.0%) in the cervical, thoracic-inlet, and thoracic regions, respectively. CONCLUSIONS AND CLINICAL RELEVANCE: The change in tracheal cross-sectional area from inspiration to expiration was as great as 24% in healthy dogs, and the area was associated with body weight. Respiratory fluctuations appeared to result in changes in tracheal dimension during respiration similar to those reported for humans.
