RESUMEN

A 43-year old woman presented with lethal purpura fulminans caused by a Streptococcus pneumoniae infection with pneumonia, bacteraemia and meningitis. The only predisposing factor identified was severe chronic alcoholism. The high morbidity and mortality rates of pneumococcal infections in alcoholics are emphasized.

RESUMEN

A case report of recurrent metabolic encephalopathy with disorders of hydro-electrolytic balance, predominantly severe hypochloremic alkalosis, resulting from self-induced vomiting and self-administered gastric tubage and lavage. Munchausen syndrome was diagnosed. Some established characteristics of this syndrome as well as some atypical features concerning patient-doctor relationship and natural history are discussed.

RESUMEN

UNLABELLED: PURPOSE OF THE STUDY AND MATERIAL: A search for sicca syndrome was performed in 54 HTLV 1 positive patients. Cases of sicca syndrome due to associated pathologies, or iatrogenic, were eliminated from this study. RESULTS: Lacrimal hyposecretion was found in 79% of the cases. Defective lacrimal quality was found in 86% of patients and a variable intensity coloration with the Fluorescine and/or with Rose Bengale was positive in 83% of cases. Histological study of the conjunctival print showed ocular dryness in 65% of the patients. Biopsy of the labial minor salivary glands showed a Gougerot-Sjögren syndrome in 71% of the cases. CONCLUSION: In our study, a sicca syndrome with varied gravity was found in 78% of cases.

RESUMEN

Three pediatric cases of vesiculopustules persisting for several months or years after eradication of Sarcoptes scabiei are reported. This particular course of scabies in infants is underrecognized although probably common. Its clinical manifestations, natural history and histologic features are identical to those seen in infantile acropustulosis, a recently individualized syndrome whose pathogenesis is unelucidated. In the light of these cases and a review of the literature, the possible relationship between scabies and infantile acropustulosis is discussed.

RESUMEN

Chlorozotocin, 120 mg/m2 every 4 weeks, was administered to 22 patients with advanced soft tissue sarcoma. All patients had received extensive prior chemotherapy and had measurable progressive disease at the time of entry in the study. No objective responses were observed. The best individual results were in three patients with disease stabilization lasting 2--4 months. Toxic effects included nausea in one half of the patients and sweating episodes in one. One patient developed temporary leukopenia and three had thrombocytopenia that lasted for 3 weeks, 4 weeks, and 4 months. Blood sugar levels were unaffected by the treatment. No renal or hepatic toxicity was recorded.
