RESUMEN

BACKGROUND: Neurological prognosis after cardiac arrest remains ill-defined. Plasma brain natriuretic peptide (BNP) may relate to poor neurological prognosis in brain-injury patients, though it has not been well studied in survivors of cardiac arrest. METHODS: We performed a retrospective review and examined the association of BNP with mortality and neurological outcomes at discharge in a cohort of cardiac arrest survivors enrolled from January 2012 to December 2016 at the Wake Forest Baptist Hospital, in North Carolina. Cerebral performance category (CPC) and modified Rankin scales were calculated from the chart based on neurological evaluation performed at the time of discharge. The cohort was subdivided into quartiles based on their BNP levels after which multivariable adjusted logistic regression models were applied to assess for an association between BNP and poor neurological outcomes as defined by a CPC of 3 to 4 and a modified Rankin scale of 4 to 5. RESULTS: Of the 657 patients included in the study, 254 patients survived until discharge. Among these, poor neurological status was observed in 101 (39.8%) patients that had a CPC score of 3 to 4 and 97 patients (38.2%) that had a modified Rankin scale of 4 to 5. Mean BNP levels were higher in patients with poor neurological status compared to those with good neurological status at discharge (P = .03 for CPC 3-4 and P = .02 for modified Rankin score 4-5). BNP levels however, did not vary significantly between patients that survived and those that expired (P = .22). BNP did emerge as a significant discriminator between patients with severe neurological disability at discharge when compared to those without. The area under the curve for BNP predicting a modified Rankin score of 4 to 5 was 0.800 (95% confidence interval [CI] 0.756-0.844, P < .001) and for predicting CPC 3 to 4 was 0.797 (95% CI 0.756-0.838, P < .001). BNP was able to significantly improve the net reclassification index and integrated discriminatory increment (P < .05). BNP was not associated with long-term all-cause mortality (P > .05). CONCLUSIONS: In survivors of either inpatient or out-of-hospital cardiac arrest, increased BNP levels measured at the time of arrest predicted severe neurological disability at discharge. We did not observe an independent association between BNP levels and long-term all-cause mortality. BNP may be a useful biomarker for predicting adverse neurological outcomes in survivors of cardiac arrest.

A new methodology involving the use of Bovine Serum Albumin (BSA) as a probe and liquid crystal (LC) as a signal reporter for the detection of heavy metal ions in water at neutral pH was developed. BSA acted as a multi-dentate ligand for the detection of multiple metal ions. The LC sensor was fabricated by immobilizing 3 µg mL-1 BSA solution on dimethyloctadecyl-[3-(trimethoxysilyl)propyl]ammonium chloride (DMOAP)-coated glass slides. In the absence of heavy metal ions, a dark optical image was observed, while in their presence, a dark optical image turned to bright. The optical response was characterized by using a polarized optical microscope (POM). The BSA based LC sensor selectively detected toxic metal ions as compared to s block metal ions and ammonium ions in water. Moreover, the limit of detection was found to be very low (i.e., 1 nM) for the developed new biosensor in comparison to reported biosensors.

BACKGROUND: Bone marrow stem cell treatment has been proven a promising therapeutic strategy and showed significant results given the strong immune modulating properties. We have investigated the safety and efficacy of autologous bone marrow stem cell transplantation through liver puncture in two patients with recently diagnosed type 1 diabetes mellitus. MATERIAL AND METHODS: The procedure was approved by the Institutional Ethics Committee. In 2011, in three young patients, type 1 diabetes mellitus diagnosis was confirmed, with the presence of positive antibodies and ketoacidosis. Two patients was treated with autologous bone marrow stem cell stimulated with filgrastim and transplantation, through liver puncture, as immune modulators. One patients was treated with conventional treatment and participate in this experiment as a control group. The families of the patients signed the informed consent. No specific statistical analysis was performed. The patients had less than 8 years old, diagnosis of type 1 diabetes for less than 60 days, body mass index less than 22 kg/m2, normal complete blood count, coagulation and renal function, no lesions in target organs, glycosylated hemoglobin (HbA1c) level less than 13.70%, c-peptide level less than 0.67 ng/ml, positive results of Islets Cells Antibody (ICA), Glutamic Acid Decarboxylase (GAD) and insulin antibody. RESULTS: In two patients treated, the follow up at 12 months showed negative value in ICA, GAD and anti insulin antibody levels, with an increased levels of c peptide and decreased levels of blood glucose and HbA1c. CONCLUSIONS: Treatment with autologous bone marrow stem cells is easy and effective as it reversed the production and effect of anti pancreatic islet antibody and significantly resulted in an increased c-peptide concentration.

Dual-core yarns, containing two filaments within the core of the yarn, have gained increasing commercial and research interest recently, especially in denim manufacturing. The use of multi-components in dual-core yarns allows for tailoring the properties of the yarn and denim fabric. The type of filaments and fibers and their surface characteristics play a role in fiber-to-fiber cohesion within yarn structure. However, little has been reported regarding the effect of different filaments on the properties of dual-core yarns. The objective of this study was to investigate the effect of three different filaments, T400, polyester flat (PET flat) and polyester textured (PET textured) as well as two yarn structures, siro versus non-siro, on tensile, elastic and other properties of dual-core yarns at same twist level and linear density of the yarn. The results showed that the siro spun dual-core yarn containing T400 exhibited 25% higher tenacity compared with yarns containing other filaments. However, the plastic deformation of the yarn containing PET flat filament, having a higher initial modulus, was at a relatively lower level compared with T400 and PET textured. Overall, the siro yarn structure showed lower imperfections and higher tenacity compared with the non-siro yarn structure. The dual-core yarn containing T400 showed a higher level of moisture wicking compared with other filaments which can add to the comfort properties but a similar hairiness level. The findings of this study suggest that the use of a filament with a higher initial modulus can improve the stretch and recovery behavior of the dual-core yarns.

Evidence to support clinical decision making must be based on safety data that have been captured, analysed and interpreted in a robust and reliable way. Randomised real-world evidence (RRWE) studies provide the opportunity to evaluate the use of medicines in patients and settings representative of routine clinical practice. However, elements that underpin the design of RRWE studies can have a significant impact upon the analysis, interpretation and implications of safety data. In this narrative review, we use data from the Salford Lung Study; two prospective, 12-month, open-label, parallel-group, phase III randomised controlled trials conducted in primary care in the UK; to highlight the importance of capturing treatment modifications when attempting to evaluate safety events according to actual treatment exposure. We demonstrate that analysing safety data by actual treatment received (i.e. accounting for the treatment modifications that occur routinely in the primary care setting) provides additional insight beyond analysing according to randomised treatment strategy only. It is therefore proposed that understanding of safety data from RRWE trials can be optimised by analysing both by randomised group and by actual treatment received.

The present research work elucidates the photocatalytic effect of titania on liquid crystal based sensor used to detect sodium arsenate in water. The titania nanoparticles with an average size of 18-20 nm were synthesized by hydrothermal method and calcined up to 500 °C to obtain pure anatase phase. The catalytic activity of titania nanoparticles was enhanced by surface modification with (3-aminopropyl) trimethoxysilane. The sensing experiment was performed under visible light irradiation while keeping the concentration of substrate, analyte, the thickness of the liquid crystal layer, temperature and pH values constant. In addition, span 80 (a non-ionic surfactant) was used to obtain homeotropic alignment of liquid crystals. The TiO2 nanoparticles gave dark to bright texture within 53 min while the functionalized titania responded in less than 25 min under similar sensing condition. The detection results showed that the functionalized titania nanoparticles possess greater photocatalytic activity under visible light and enhanced the sensing response than unfunctionalized titania nanoparticles.
