RESUMEN
The aim of this study was to estimate the prevalence and risk factors for self-reported diabetes mellitus (DM) in the adult population of the Central-West region of Brazil. In 2013, a cross-sectional study using the data from the National Health Survey and comprising 7519 individuals aged ≥18 years from the Central-West region was conducted. Participants were interviewed at their homes about sociodemographic data and risk factors for DM. To verify the risk factors with DM, the Poisson regression model was used. The analyses were performed for the total sample and stratified according to sex. The prevalence of DM was 6.5% (95% confidence interval [95% CI], 5.7-7.3). The diagnosis of self-reported DM was 4.3% in men and 7.5% in women. In the global sample, it was found that age between 40-59 years and ≥60 years, previous smoking (former smoker), self-reported hypertension, self-reported dyslipidemia, overweight, and obesity were independently associated with self-reported DM. In men, risk factors were: Age ≥ 60 years, self-reported hypertension, self-reported dyslipidemia, and obesity. In women, risk factors were: Age 30-39 years, 40-59 years, and ≥60 years, previous smoking (former smoker), self-reported hypertension, self-reported dyslipidemia, overweight, and obesity. Conclusion: The prevalence of DM was 6.5%. DM was associated with advanced age; previous smoking (former smoker), hypertension, dyslipidemia, overweight, and obesity. Some differences in risk factors between men and women were noted.

INTRODUCTION: Unspecified stroke (UnST) is of great importance in mortality statistics, as it is the fourth leading cause of death in Brazil. The objective of this study was to identify the profile of reclassified causes of death after investigation of deaths caused by UnST in Brazil. METHODS: All deaths registered as UnST in 2017 in the Mortality Information System (SIM) were considered as garbage codes. The specific causes, detected after investigation in 60 selected cities, were analyzed by age and sex. RESULTS: Of the total deaths due to UnST identified in these 60 cities (n = 11,289), 25.8% were investigated. Of these, 56.3% were reclassified to ischemic stroke, 12.7% to hemorrhagic stroke, and 23.3% to other specific causes, such as diabetes and chronic kidney disease, in both sexes. DISCUSSION: The higher proportion of deaths due to ischemic stroke in comparison to hemorrhagic stroke was expected. However, the detection of other specific causes outside the stroke group indicates possible quality problems in the filling of death certificate (DC). CONCLUSION: The investigations allowed the identification of subgroups of deaths due to stroke. In addition to the research, however, it is important to conduct physician training in the adequate filling in of the DC, in order to improve estimates of specific stroke mortality, and to enable appropriate targeting of health actions and services.

Além da investigação, entretanto, é importante realizar capacitação com médicos para o preenchimento adequado da DO, a fim de melhorar as estimativas da mortalidade por AVC específico e possibilitar direcionamento adequado das ações e dos serviços de saúde.

Objetivou-se analisar a acessibilidade aos serviços de sauÌde na AtençaÌo BaÌsica no estado de GoiaÌs. Estudo descritivo de corte transversal com base nos dados secundaÌrios proveniente do Programa Nacional de Melhoria do Acesso e da Qualidade da AtençaÌo BaÌsica. A amostra do estudo foi constituiÌda por profissionais de sauÌde das 1.216 UBS. Verificou- se que, a sala de acolhimento estaÌ ausente em 68,5% das unidades de sauÌde, o que prejudica, consideravelmente, os profissionais na tomada de decisaÌo imediata. A auseÌncia de consultoÌrios cliÌnicos em 2% dos locais dificulta a acessibilidade aos serviços de sauÌde na AtençaÌo BaÌsica em GoiaÌs. Com relaçaÌo ao horaÌrio e turno de funcionamento, verificou-se que 86% das unidades atendem cinco dias na semana em turnos de oito horas, naÌo favorecendo a acessibilidade do usuaÌrio. Com este estudo constata-se a deficieÌncia da acessibilidade aos serviços de sauÌde e a necessidade de maiores investimentos no fortalecimento da AtençaÌo BaÌsica.
The objective of this study was to evaluate accessibility to primary health care services in the state of GoiaÌs. A descriptive cross-sectional study was conducted based on secondary data from the National Program to Improve Access to and Quality of Primary Health Care. The study sample was composed of health professionals from 1,216 primary health care units. Results showed that 68.5% of the health units miss a screening room, thus considerably damaging prompt decision-making by professionals. The lack of medical offices in 2% of the sites hinders the primary health care services accessibility in GoiaÌs. As regards opening hours and work shifts, 86% of the units are open five days a week in eight- hour shifts, which does not favor accessibility for users. This study confirms the lack of accessibility to health services and the need for additional investments to strengthen primary health care.
