We assessed risk factors for postoperative urinary retention (UR) in elderly males with femoral bone fractures: 169 Japanese males (mean age 81.95 ± 1.19 years) who had undergone hip surgery at a municipal hospital (Toyama, Japan). A multiple logistic regression analysis was used to test possible risk factors for UR: age, body mass index, serum albumin, cognitive impairment, activities of daily living (ADL), and history of diabetes mellitus (DM). UR occurred in 24 (14.2%) of the 169 patients. A multivariate logistic regression analysis with age adjustment showed that ADL (odds ratio [OR] 3.88; 95% confidence interval [CI]: 1.2-12.5, p=0.023) was significantly associated with the development of UR, and a history of DM showed marginal significance for UR occurrence (OR 0.36, 95%CI: 0.11-10, p=0.064). These results suggests that ADL is a risk factor for UR development in elderly males who have undergone surgery for femoral neck or trochanter fractures.

BACKGROUND: This study aimed to evaluate the effects of orthogeriatric co-management of hip fractures at a regional core hospital. METHODS: This study included patients with proximal hip fracture. Patients were divided into two groups, conventional multidisciplinary group I including patients attending the hospital between April 2015 and March 2016 and orthogeriatric group II including patients attending the hospital between April 2016 and March 2017, which were compared etrospectively. In the control group, the conventional multidisciplinary team treated patients as whole-body controls. In the intervention group, the newly recruited geriatricians performed physical examinations, laboratory tests, radioactive imaging, and physiological tests. Furthermore, they consulted ward pharmacists, rigorously conducted positive polypharmacy interventions , and evaluated the type and number of mediated drugs on admission. RESULTS: The number of medicated drugs significantly decreased from 6.03 ± 4.3 on admission to 5.50 ± 3.59 on discharge in group II, whereas group I did not show a significant decrease. Despite the more number of hospitalized patients in group II (166 patients) than in group I (126 patients), the recovery rate from postoperative urinary retention increased significantly from 57.8% (19/30) in group I to 84.3% (32/59) in group II (p = 0.049), while the incidence of aspiration pneumonia decreased from 7.1% (9/126) in group I to 2.49% (4/166) in group II (p = 0.08). The patients received six or more prescribed drugs on admission, and the number remained constant. However, the number of medicated drugs on discharge showed a marginally significant decrease from 6.03 ± 4.3 in group I to 5.50 ± 3.59 in group II (p < 0.05). CONCLUSIONS: Compared to the conventional multidisciplinary group, the orthogeriatric team contributed to reducing the number of multi-effect drugs and perioperative complications without negatively affecting mortality despite the increased number of patients. The in-hospital mortality rate did not change between the groups. The orthogeriatric program succeeded in preventing and treating perioperative complications.

BACKGROUND: Low polyethylene wear rate and low incidence of osteolysis have been reported after total hip arthroplasty (THA) using first-generation remelted highly cross-linked polyethylene (HXLPE). However, osteolysis has not been evaluated in long-term studies of these patients (15 years or more). The present study assessed computed tomography (CT) images to determine the incidence of osteolysis associated with HXLPE in THA during at least 15 years of follow-up. METHODS: We evaluated 105 primary THAs in 84 patients (77 women and 7 men). Mean follow-up was 15.9 years (range, 15-18 years). All THAs used a Longevity HXLPE liner and a 26-mm zirconia femoral head. Areas of osteolysis were identified from CT images using 3D and multiplanar reconstruction views. RESULTS: CT 3D multiplanar reconstruction images at 15-year follow-up showed no obvious osteolysis in the acetabulum or femur of any patient. No patients developed cup loosening or liner breakage. CONCLUSION: Our study indicates that first-generation remelted HXLPE liners do not increase the risk of osteolysis during 15-year follow-up and suggests that the wear particles from first-generation remelted HXLPE are less biologically active than those generated by conventional polyethylene devices.

BACKGROUND: In Japan, the majority of hip osteoarthritis (OA) was caused by acetabular dysplasia, and about 90 % of patients were female. The present study focused on Japanese female patients with hip OA due to acetabular dysplasia, and examined the associated factors with OA staging at diagnosis, in special reference to body weight. METHODS: Study subjects were 336 Japanese women who were newly diagnosed with hip OA caused by acetabular dysplasia at 15 hospitals in 2008. The self-administered questionnaire elicited patients' body weight at age 20 and at OA diagnosis. Four ranked OA staging according to radiographic findings of the hip joint (pre-OA, initial stage, advanced stage or terminal stage) was regarded as the outcome index. Proportional odds models in logistic regression were used to calculate odds ratios (ORs) and 95 % confidence intervals (CIs) for severer stage of OA. RESULTS: At diagnosis, 45 % of patients suffered from terminal stage of OA, whereas 13 % and 14 % were categorized into pre-OA and initial stage, respectively. After adjustment for potential confounders, weight gain since age 20 revealed the increased ORs for severer OA stage at diagnosis (OR 2.02; 95 % CI, 1.07-3.80). Other significant characteristics were age (67+ vs. 20-49 years, OR 12.4), lower education (junior high school vs. junior college or higher, OR 4.00), parity (OR 2.19), lower acetabular head index (<60.0 vs. 71.1+, OR 2.36), and longer duration since symptom onset (6.0+ vs. <1.0 year, OR 2.94). CONCLUSIONS: Weight gain since age 20 might be involved in mechanisms of OA development, which is independent of age or severity of acetabular dysplasia.

There is a rising concern about squeaking in ceramic-on-ceramic total hip arthroplasty (THA). In pin-on-disc testing of a delta-delta coupling, we reproduced squeaking and observed microcracks on worn surfaces. We used a pin-on-disc machine and made discs and pins by cutting delta ceramic to a diameter of 40 mm (D-D). Cross-linked polyethylene was used for a comparison disc (D-P). We performed the same test using another D-D coupling specimen to confirm reproducibility. Squeaking in the D-D specimen was reproduced in wet conditions, though it was not found in the D-P specimen. Fast Fourier transform analysis showed a peak frequency for squeaking of 2794 Hz. The noise occurred at about 6.6 km of sliding distance. Scanning electron microscopy revealed that the worn surface of the pin in D-D at 10.8 km of sliding distance had some microcracks. However, there was no obvious damage to the worn surface of the pin in D-P at the same sliding distance. We confirmed the reproducibility of these findings, obtaining similar results, including squeaking, from another D-D coupling specimen. Our findings show that squeaking may occur in THA using delta ceramic bearings even if implants are placed to avoid extra-articular impingement of the femoral neck. Although the clinical relevance of microcracks is unknown, they may affect long-term outcomes in THA using delta ceramic bearings.

INTRODUCTION: Rotational acetabular osteotomy (RAO) has been used successfully in patients with developmental dysplasia of the hip (DDH). However, some patients are forced to undergo total hip arthroplasty (THA) because of the progression of osteoarthritis. We evaluated the effect of previous RAO on the outcome of THA performed for degenerative arthritis secondary to DDH, comparing outcomes for patients with THA and prior RAO versus outcomes for patients with THA and no prior RAO. MATERIALS AND METHODS: At an average follow-up point of 8.2 years (range 7-11 years), we compared outcomes in dysplastic hips for 22 hips (group R) in patients who underwent THA after successful RAO with outcomes for a well-matched control group of 30 hips in patients who underwent primary THA (group C) during the same period. RESULTS: Both groups had similar midterm results. No acetabular or femoral components exhibited loosening or revision in either group. Harris hip scores (HHSs) at the most recent follow-up had not been compromised by RAO, and there were no significant differences in intraoperative blood loss and operative time between the two groups. Although there was a tendency toward superolateral placement of the acetabular component in group R, there were no significant differences in the mean steady-state linear and volumetric wear rates between the two groups. There were no infections, dislocations, intraoperative fractures, damaged nerves, or deep vein thromboses in either group. CONCLUSIONS: Our midterm results demonstrated that RAO does not lead to higher revision rates, compromised HHSs, or shortened survivorship in eventual THA for DDH.

Osteonecrosis is a major glucocorticoid-induced complication in the orthopedics field. Despite the extensive researches, mechanisms underlining the glucocorticoid-induced osteonecrosis are largely unknown. Here, we first provide the evidence that a combined treatment of cultured osteocytic cells with glucocorticoid and hypoxia caused necrotic cell death, which is assumed to occur in the acute bone injuries induced by glucocorticoids. We cultured MLO-Y4 murine osteocytic cells under hypoxia in the presence or absence of Dexamethasone (Dex) and examined the rates of apoptotic and necrotic cell death. Dex or hypoxia alone increased apoptotic cells, but not necrotic cells. The combination of Dex and hypoxia dramatically increased osteocytic cell death, notably necrotic cell death. The expression of Dickkopf-1 (Dkk-1), an inhibitor of Wnt/ß-catenin signal, was scarcely expressed in the control and hypoxic cells, but a dramatic increase of the Dkk-1 expression was detected in Dex-treated cells. siRNA-mediated knockdown of Dkk-1 in Dex and hypoxia-treated osteocytic cells showed the significant decreases in both apoptotic and necrotic cells. The results indicated that the combination of Dkk-1 overexpression by Dex and hypoxia causes the necrotic osteocytic cell death. The results also indicated that blocking of Dkk-1 can protect bone cells from glucocorticoid and hypoxia-induced cell injury.

INTRODUCTION: Femoral component revisions with extensively porous-coated stems have shown promising clinical results, although there are still concerns over stress-shielding. MATERIALS AND METHODS: We retrospectively reviewed data for 18 patients undergoing femoral component revisions with extensively porous-coated stems with high stiffness. The average length of follow-up was 10 years (range 6-13.7 years). RESULTS: Radiographic evidence of a bone-ingrown stem was present in 94 % of the hips, and stable fibrous fixation was present in only one hip, but no hips were considered unstable. There was mild stress-shielding in 15 hips, moderate stress-shielding in one hip, and severe stress-shielding in two hips. In one of the latter two hips, stress-shielding was progressive even 8 years after revision surgery. In 12 (92 %) of the 13 hips in which bypass fixation for diaphyseal cortical defects was used, there was good remodeling. CONCLUSIONS: We suggest that femoral component revision using extensive porous-coated stems is a useful option that may allow for healing of diaphyseal cortical defects.

The pathophysiology of rapidly destructive osteoarthritis (OA) of the hip is unknown. This study documented cases of inversion of the acetabular labrum, which has clinicoradiologic features similar to those of initial-stage rapidly destructive hip OA. Our study was based on a prospective review of data for 9 patients with rapidly destructive hip OA. Intraoperative findings showed that the anterosuperior portion of the acetabular labrum had inverted into the articular space, along with many fragments of articular cartilage, in all patients. Subchondral insufficiency fractures of the femoral heads were seen just under the inverted labra in 8 of the 9 patients. Inversion of the acetabular labrum may be involved in rapid joint-space narrowing and subchondral insufficiency fracture in rapidly destructive hip OA.

Zirconia femoral heads were introduced for total hip arthroplasty (THA) with the expectation of lower polyethylene (PE) wear and better clinical results. Because several studies reported poor survivorship of yttria-stabilized zirconia-PE THA, we investigated a new-generation yttria-stabilized zirconia head (diameter, 26 mm) manufactured by NGK Spark Plug. We retrieved six zirconia heads at revision THA after they had been in place for a mean of 8.6 years and measured their surface roughness and mean monoclinic content. Although their mean monoclinic content was higher than that of the unused head, surface roughness in the implanted heads was as low as that of the unused head, indicating that wear reduction may be possible with the selection of a suitable zirconia femoral head.

BACKGROUND: Cementless total hip arthroplasty (THA) implants using alkaline and heat treatments were developed to enhance bone bonding. Although bone-bonding ability of the alkali- and heat-treated titanium surface has been demonstrated in animal studies, it remains unknown whether it enhances or provides durable bone bonding in humans. QUESTIONS/PURPOSES: We therefore (1) determined long-term survivorship, function, and radiographic signs of failure of fixation of alkali- and heat-treated THA implants; and (2) histologically examined their bone-bonding ability in two human retrievals. METHODS: We retrospectively reviewed 58 patients who underwent 70 primary THAs, of whom 67 were available for minimum followup of 8 years (average, 10 years; range, 8-12 years). Survival rate was calculated. Hip function was evaluated using the Japan Orthopaedic Association (JOA) hip scores, and radiographic signs of implant failure were determined from anteroposterior radiographs. Two retrieved implants were investigated histologically. RESULTS: Using revision for any reason as the end point, the overall survival rate was 98% (95% confidence interval, 96%-100%) at 10 years. The patients' average JOA hip scores improved from 47 points preoperatively to 91 points at the time of the last followup. No implant had radiographic signs of loosening. Histologically we observed bone in the pores 2 weeks after implantation in one specimen and apparently direct bonding between bone and the titanium surface in its deep pores 8 years after implantation. CONCLUSIONS: Cementless THA implants with alkaline and heat treatments showed a high survival rate. Further study is required to determine whether the treatment enhances direct bone bonding.

We previously reported the results of wear comparison at a minimum of 5 years between highly cross-linked polyethylene (HXLPE) and conventional polyethylene (PE) against a zirconia femoral head. We now report the mean wear at 10 years for 52 patients (56 hips) of the original cohort of 61 patients (65 hips) who had undergone primary total hip arthroplasty at our hospital between November 1999 and August 2000. The mean steady-state linear wear rate of HXLPE was 0.045 mm/y, compared with 0.080 mm/y for conventional PE (P=.0003). The incidence of osteolysis was 25% in the conventional PE group compared with 0% in the HXLPE group. Our study demonstrated that the steady-state wear rate for HXLPE remains significantly lower than that for conventional PE against a zirconia femoral head at a mean of 10 years after implantation.

Anatomic femoral components are optimized for primary osteoarthritis, and few long-term results for their use are available for developmental dysplasia of the hip (DDH). We reviewed 135 uncemented total hip arthroplasties in which an anatomic femoral component was used in 106 patients with DDH, excluding femurs with neck-shaft angles of >160° and femurs with anteversion of >50°. The mean age of patients at surgery was 49.4 years (range, 33-66 years), and the mean duration of follow-up was 13.5 years (range, 10-18 years). No osteoporotic femurs were included in our study group. Despite 18 acetabular revisions, no femoral component was removed for any reason. No femoral loosening was seen at the final follow-up examination. An anatomic femoral component with circumferential porous coating can produce good clinical and radiologic findings in young patients with good bone quality who have DDH without excessive femoral deformity.

BACKGROUND: In cemented total hip arthroplasty (THA), a polished tapered femoral stem with a design based on the taper-slip concept enables extremely reliable and durable fixation. In contrast, cemented femoral stems made from titanium alloys are not favored because of reports describing insufficient clinical outcomes. However, we have reported excellent clinical and radiological outcomes for cemented titanium stems made using the composite-beam concept. This study examines the characteristics of cemented titanium femoral stems with a smooth surface. METHODS: The bonding strength between titanium alloys with different surface finishes and bone cement was evaluated by use of push-out and detachment tests. Torsional stability tests were performed to evaluate the initiation and propagation of disruption of the fixation of cemented stems at the cement-implant interface. The wear resistance was investigated by use of wear-friction tests performed using a multidirectional pin-on-disc machine. The bone strain loaded on to the femoral cortex was measured by use of an implanted Sawbone and analyzed by use of the finite element method. RESULTS: The push-out and detachment tests revealed increasing cement adhesion strength with increasing degree of roughness of the metal surface. The torsional stability tests indicated that a load >1,000 N led to progressive debonding between the cement and the implant with a smooth surface finish. Interestingly, wear-friction tests revealed the wear rate for polished titanium surfaces was clearly higher than for smooth surfaces. In addition, the greater elasticity of titanium stems compared with cobalt-chromium stems transmitted the external load to the proximal side of the femur more effectively. CONCLUSIONS: The smooth surface finish of the stems is an important factor for the satisfactory clinical and radiological outcomes of cemented titanium femoral stems. The greater elasticity of a titanium stem effectively transmits the external load to the medial side of the femur.

RESUMEN

OBJECTIVE: To investigate the relationship between transient oxidative stress and the development of osteonecrosis in a rat model. METHODS: A total of 160 male Wistar rats (24 weeks old) were injected only once with the pro-oxidant DL-buthionine-(S,R)-sulfoximine (BSO) (500 mg/kg given intraperitoneally) and were killed 12 hours (group A), 1 day (group B), 3 days (group C), 4 days (group D), 5 days (group E), 7 days (group F), or 14 days (group G) after administration (n = 20 per group). Twenty untreated rats were used as a control group (group N). Femurs were examined histopathologically for the presence of osteonecrosis, and reduced glutathione (GSH) in liver tissue was measured as an index of oxidative stress. RESULTS: GSH decreased rapidly after BSO administration. Significant decreases were noted in groups A and B as compared to group N (P < 0.0001 and P = 0.0007, respectively), confirming the development of transient extreme oxidative stress soon after BSO administration. The histopathologic study revealed osteonecrosis in 10% of the rats in group E, 35% of the rats in group F, and 40% of the rats in group G. CONCLUSION: Transient extreme oxidative stress was confirmed to induce osteonecrosis in this model. Since preparation of this model is extremely simple and because rats are well suited to genetic studies, this model may be of use in elucidating the pathophysiology of femoral head osteonecrosis in future studies.

BACKGROUND: The Japanese Orthopaedic Association Hip Score is widely used in Japan, but this tool is designed to reflect the viewpoint of health-care providers rather than that of patients. In gauging the effect of medical therapies in addition to clinical results, it is necessary to assess quality of life (QOL) from the viewpoint of patients. However, there is no tool evaluating QOL for Japanese patients with hip-joint disease. METHODS: With the aim of more accurately classifying QOL for Japanese patients with hip-joint disease, we prepared a questionnaire with 58 items for the survey derived from 464 opinions obtained from approximately 100 Japanese patients with hip-joint disease and previously devised evaluation criteria. In the survey, we collected information on 501 cases, and 402 were subjected to factor analysis. From this, we formulated three categories-movement, mental, and pain-each comprising 7 items, for a total of 21 items to be used as evaluation criteria for hip-joint function. RESULTS: The Cronbach's α coefficients for the three categories were 0.93, 0.93, and 0.95, respectively, indicating the high reliability of the evaluation criteria. The 21 items included some related to the Asian lifestyle, such as use of a Japanese-style toilet and rising from the floor, which are not included in other evaluation tools. CONCLUSIONS: This self-administered questionnaire may become a useful tool in the evaluation of not only Japanese patients, but also of members of other ethnic groups who engage in deep flexion of the hip joint during daily activities.

Highly cross-linked polyethylene (HXLPE) was developed to reduce wear of articular bearing surface in total hip arthroplasty patients. Several studies have shown reduced wear of HXLPE compared with conventional polyethylene; however, these studies had used HXLPE in combination with a Co-Cr metal head. The purpose of this study was to compare the 5-year in vivo wear of HXLPE with that of conventional PE using a zirconia femoral head. Forty-five hips with a Trilogy HXLPE (Zimmer, Warsaw, Ind) were matched and compared with a control group of 20 conventional Trilogy PE hips. The 2-dimensional linear wear rate was significantly less in the HXLPE group between 1 and 5 years postoperation (P < .001). The results show that HXLPE reduces short-term polyethylene wear against not only a Co-Cr head but also a zirconia head.

BACKGROUND: We conducted a nationwide epidemiologic study regarding hip osteoarthritis (OA) in Japan, and a previous report found these patients to be unique in comparison to Caucasians. This report focused on the data regarding each hip joint, and the involvement of acetabular dysplasia with hip OA was analyzed. METHODS: Seven hundred twenty OA hips were examined. Sixty-five joints with osteonecrosis of the femoral head and 215 non-OA contralateral joints of the unilateral patients were examined as controls. The revised system of stage classification for hip OA of the Japanese Orthopedic Association (JOA) was used according to the reproducibility in order to ensure reliable data from the multiple institutions. The acetabular dysplasia indexes were also chosen according to the reproducibility and measured in the radiograph of bilateral hip joints. The clinical score was assessed using the JOA scoring system. The relative risk of the grade of acetabular dysplasia indexes for hip OA was calculated as the odds ratio and the 95% confidence interval. RESULTS: The stage of the OA joints deteriorated with increasing age. The clinical scores also decreased. The grade of the acetabular dysplasia indexes of the OA joints was significantly higher than that of the control joints. Each index of acetabular dysplasia demonstrated significantly increased odds ratios for hip OA. Among the OA joints, the deterioration of the OA stage was found to be significantly associated with an increasing grade of acetabular dysplasia. The odds ratio for OA deterioration in the acetabular dysplasia index was also obtained. The joints of females tended to have a higher grade and prevalence of acetabular dysplasia than those of males. CONCLUSIONS: These findings confirmed a high prevalence of acetabular dysplasia in hip OA joints in Japan. Acetabular dysplasia was one of the most important factors associated with hip OA.

INTRODUCTION: The results of isolated acetabular revision performed in 35 patients (36 hips) were monitored from 3 to 10 years. All femoral components were well fixed and not removed or revised during index surgery. METHOD: All revision acetabular implants were cementless, using a porous-coated hemispheric cup with or without bone graft. There were no cases of femoral component radiographic or clinical failure. For some cases, we performed bone grafting to focal osteolysis of the proximal femur around the cementless stem. RESULTS: Bone incorporation occurred in 12 hips (overall 14). The mean pre- and postoperative Harris Hip Scores were 49 and 80, respectively. The findings suggest that isolated acetabular revision using a cementless porous-coated hemispheric cup can be performed without removing or revising a stable, well-fixed, uncemented femoral component if there is no concern about dislocation. CONCLUSION: We demonstrated that isolated acetabular revision is feasible and that grafting to the osteolytic femoral defects is a worthwhile procedure to restore bone stock.

BACKGROUND: We focused on vitamins with marked antioxidant potency to see whether their use might prevent the development of steroid-induced osteonecrosis. METHODS: Fifteen Japanese white rabbits weighing about 3.5 kg were injected once into the right gluteal muscle with methylpred-nisolone (MPSL) 40 mg/kg (S group). In addition, 10 other rabbits received consecutive daily intravenous injections of vitamin E 50 mg/kg, starting from the day of MPSL administration (E group). All animals were killed 2 weeks after MPSL administration, and femurs were extracted and stained with H&E. Blood levels of reduced glutathione (GSH) were also measured. RESULTS: In the S group the osteonecrosis development rate was 93%, in contrast to 0% in the E group (P < 0.01). Also, GSH levels in the S group abruptly decreased from the first day after MPSL administration, whereas in the E group, the decline in GSH levels was significantly suppressed on days 1 and 3 after MPSL administration (P< 0.05). CONCLUSIONS: Vitamin E administration significantly inhibited steroid-induced oxidative stress. The results of this study suggest that the administration of vitamin E may be a novel and simple method to prevent the development of steroid-induced osteonecrosis.
