OBJETIVO: Determinar o efeito da caminhada em esteira sobre a frequência cardíaca materna (FCM) e parâmetros cardiotocográficos (batimentos cardiofetais basais [BCFs], movimentos ativos fetais [MAFs], número de acelerações e desacelerações e variabilidade de curta [STV] e longa [LTV] duração da frequência cardíaca fetal) em gestantes na 36a semana. MéTODOS: Foi realizado um ensaio clínico não randomizado e aberto com 88 gestantes saudáveis submetidas a caminhada de moderada intensidade na esteira e a cardiotocografia computadoriza em 3 momentos de 20 minutos (antes, durante e após a caminhada). RESULTADOS: A média dos BCFs diminuiu durante a caminhada, retornando a níveis prévios (antes: 137 bpm; durante: 98 bpm; após: 140 bpm; p < 0,001), com bradicardia ocorrendo em 56% dos fetos nos primeiros 10 minutos do exercício, e em 47% após 20 minutos. A bradicardia fetal não foi observada em outros momentos (antes ou depois). As médias da STV e da LTV foram 7,9, 17,0 e 8,0 milissegundos (p < 0,001) e 7,6, 10,7 e 7,6 bpm (p = 0,002) antes, durante e após a caminhada, respectivamente. A média dos números dos MAFs em 1 hora foi 29,9, 22,2 e 45,5, respectivamente, nos três momentos (p < 0,001). Nas mulheres com sobrepeso/obesidade, a média da FCM foi menor (p = 0,02). Após a análise de regressão logística, duas variáveis permaneceram significativamente associadas a bradicardia: aptidão maternal na 28a semana de gravidez (efeito protetor) e peso materno (aumento do risco). CONCLUSãO: Em fetos saudáveis, o exercício físico mostrou-se seguro, uma vez que, embora os BCFs e os MAFs diminuam durante a caminhada na esteira, foi observado um aumento da SVT e da LTV.

The mean number of fetal movements in 1 hour was 29.9, 22.2 and 45.5, respectively, in the 3 periods (p < 0.001). In overweight/obese women, the mean FHR was lower (p = 0.02). Following the logistic regression analysis, two variables remained significantly associated with bradycardia: maternal fitness in the 28th week of pregnancy (protective effect) and maternal weight (increased risk). CONCLUSION: In healthy fetuses, physical exercise proved to be safe, since, although FHR and AFM decreased during treadmill walking, an increase in SVT and LTV was observed.

IMPORTANCE: Recent studies have reported an increase in the number of fetuses and neonates with microcephaly whose mothers were infected with the Zika virus (ZIKV) during pregnancy. To our knowledge, most reports to date have focused on select aspects of the maternal or fetal infection and fetal effects. OBJECTIVE: To describe the prenatal evolution and perinatal outcomes of 11 neonates who had developmental abnormalities and neurological damage associated with ZIKV infection in Brazil. DESIGN, SETTING, AND PARTICIPANTS: We observed 11 infants with congenital ZIKV infection from gestation to 6 months in the state of Paraíba, Brazil. Ten of 11 women included in this study presented with symptoms of ZIKV infection during the first half of pregnancy, and all 11 had laboratory evidence of the infection in several tissues by serology or polymerase chain reaction. Brain damage was confirmed through intrauterine ultrasonography and was complemented by magnetic resonance imaging. Histopathological analysis was performed on the placenta and brain tissue from infants who died. The ZIKV genome was investigated in several tissues and sequenced for further phylogenetic analysis. MAIN OUTCOMES AND MEASURES: Description of the major lesions caused by ZIKV congenital infection. RESULTS: Of the 11 infants, 7 (63.6%) were female, and the median (SD) maternal age at delivery was 25 (6) years. Three of 11 neonates died, giving a perinatal mortality rate of 27.3%. The median (SD) cephalic perimeter at birth was 31 (3) cm, a value lower than the limit to consider a microcephaly case. In all patients, neurological impairments were identified, including microcephaly, a reduction in cerebral volume, ventriculomegaly, cerebellar hypoplasia, lissencephaly with hydrocephalus, and fetal akinesia deformation sequence (ie, arthrogryposis). Results of limited testing for other causes of microcephaly, such as genetic disorders and viral and bacterial infections, were negative, and the ZIKV genome was found in both maternal and neonatal tissues (eg, amniotic fluid, cord blood, placenta, and brain). Phylogenetic analyses showed an intrahost virus variation with some polymorphisms in envelope genes associated with different tissues. CONCLUSIONS AND RELEVANCE: Combined findings from clinical, laboratory, imaging, and pathological examinations provided a more complete picture of the severe damage and developmental abnormalities caused by ZIKV infection than has been previously reported. The term congenital Zika syndrome is preferable to refer to these cases, as microcephaly is just one of the clinical signs of this congenital malformation disorder.

PURPOSE: to evaluate the effect of maternal, socioeconomic and obstetric variables, as well the presence of artery incisions in the 20th and 24th weeks on the fetal weight estimated at the end of pregnancy (36th week) in pregnant women attended by Programa Saúde da Família, in an inland town of the northeast of Brazil. METHODS: a longitudinal study including 137 pregnant women, who have been followed up every four weeks in order to assess clinical, socioeconomic and obstetric conditions, including their weight. The uterine arteries were evaluated by Doppler in the 20th and 24th weeks, the fetal weight and the amniotic fluid index (AFI), determined in the 36th week. The initial maternal nutritional state has been determined by the body mass index (BMI), the pregnant women being classified as low weight, eutrophic, over weight and obese. Weight gain during gestation has been evaluated, according to the initial nutritional state, being classified at the end of the second and third trimester as insufficient, adequate and excessive weight gain. Analysis of variance was performed to evaluate the association of the fetal weight in the 36th week with the predictor variables, adjusted by multiple linear regression. RESULTS: an association between the fetal weight estimated in the 36th week and the mother's age (p=0.02), mother's job (p=0.02), initial nutritional state (p=0.04), weight gain in the second trimester (p=0.01), presence of incisions in the uterine arteries (p=0.02), and AFI (p=0.007) has been observed. The main factors associated to the fetal weight estimated in the 36th week, after the multiple regression analysis were: BMI at the pregnancy onset, weight gain in the second trimester, AFI and tabagism. CONCLUSIONS: in the present study, the fetal weight is positively associated with the initial maternal nutritional state, the weight gain in the second trimester and the volume of amniotic fluid, and negatively, to tabagism.

OBJETIVO: avaliar o efeito de variáveis maternas, socioeconômicas e obstétricas, assim como a presença de incisuras na 20ª e na 24ª semana, sobre o peso fetal estimado no final da gravidez (36ª semana) em gestantes atendidas pelo Programa Saúde da Família em uma cidade do interior do Nordeste do Brasil. MÉTODOS: estudo longitudinal incluindo 137 gestantes. As gestantes foram acompanhadas a cada quatro semanas para aferição das condições clínicas, socioeconômicas e obstétricas, incluindo o peso materno. As artérias uterinas foram avaliadas pelo Doppler na 20ª e 24ª semana, o peso fetal e o índice de líquido amniótico (ILA) foram determinados na 36ª semana. O estado nutricional materno inicial foi determinado pelo índice de massa corpórea (IMC), classificando-se as gestantes como com baixo peso, eutróficas, com sobrepeso e obesas. O ganho ponderal durante a gestação foi avaliado de acordo com o estado nutricional inicial, sendo ao final do segundo e terceiro trimestre classificado em ganho ponderal insuficiente, adequado e excessivo. Foi realizada análise de variância para avaliar a associação do peso fetal estimado na 36ª semana com as variáveis preditoras, ajustada por regressão linear múltipla. RESULTADOS: observou-se associação entre peso fetal estimado na 36ª semana e idade da mãe (p=0,02), trabalho materno (p=0,02), estado nutricional inicial (p=0,04), ganho ponderal no segundo trimestre (p=0,01), presença de incisuras nas artérias uterinas (p=0,02) e ILA (p=0,007). Os principais fatores associados ao peso fetal estimado na 36ª semana, após a análise de regressão múltipla, foram: IMC no início da gravidez, ganho ponderal no segundo trimestre, ILA e tabagismo. CONCLUSÕES: o peso fetal no presente estudo associou-se positivamente ao estado nutricional materno inicial, ao ganho ponderal no segundo trimestre, ao volume do líquido amniótico e negativamente ao hábito de tabagismo.
