RESUMEN

Limb apraxia is evident in approximately 50% of patients after left hemisphere cerebral vascular accident (LCVA) and increases disability and caregiver dependence. Individuals with apraxia exhibit abnormalities in spatio-temporal aspects of gesture production and/or in knowledge of tool-related actions (action semantics). This preliminary study of three LCVA participants aimed to (i) explore the efficacy of a novel Action Network Treatment (ANT) that focused on improving the semantic association between tool actions and other types of tool knowledge, an intervention inspired by successful semantic network treatments in aphasia (e.g., Edmonds et al., 2009), and (ii) explore whether there are individuals with apraxia who benefit from ANT relative to a version of a comparatively well-studied existing apraxia treatment (Smania et al., 2006; Smania et al., 2000) that shapes gesture via focus on practicing the spatio-temporal aspects of gesture production (Tool Use Treatment or TUT). One participant demonstrated treatment benefits from both ANT and TUT, while another only benefited from TUT. These findings indicate that our novel semantic network strengthening approach to gesture training may be efficacious in at least some individuals with apraxia, and provide a foundation for future study of the characteristics of people with apraxia who benefit from each approach.

This study tested the hypothesis that a use-dependent learning mechanism operates at each of two stages of lexical access: retrieval of a word from semantics ("Stage 1"), followed by retrieval of the word's constituent phonemes ("Stage 2"). Two participants with aphasia were selected due to their contrasting types of naming impairment (Stage 1 versus Stage 2 difficulty). For each participant, items were assigned to naming training that involved retrieval practice (retrieval of the name from semantics) or repetition practice (hear the name and orally repeat it). Naming tests were administered one day and one week after training. The results supported the predicted training effects: (a) Because successful naming via retrieval practice requires both Stage 1 and Stage 2, this technique uses and strengthens item-specific connections in both stages. (b) Because word repetition circumvents semantically driven retrieval, this technique primarily uses and strengthens item-specific connections in Stage 2.

PURPOSE: Increasingly, mechanisms of learning are being considered during aphasia rehabilitation. Well-characterized learning mechanisms can inform "how" interventions should be administered to maximize the acquisition and retention of treatment gains. This systematic scoping review mapped hypothesized mechanisms of action (MoAs) and treatment ingredients in three learning-based approaches targeting naming in aphasia: errorless learning (ELess), errorful learning (EFul), and retrieval practice (RP). The rehabilitation treatment specification system was leveraged to describe available literature and identify knowledge gaps within a unified framework. METHOD: PubMed and CINHAL were searched for studies that compared ELess, EFul, and/or RP for naming in aphasia. Independent reviewers extracted data on proposed MoAs, treatment ingredients, and outcomes. RESULTS: Twelve studies compared ELess and EFul, six studies compared ELess and RP, and one study compared RP and EFul. Hebbian learning, gated Hebbian learning, effortful retrieval, and models of incremental learning via lexical access were proposed as MoAs. To maximize treatment outcomes within theorized MoAs, researchers manipulated study ingredients including cues, scheduling, and feedback. Outcomes in comparative effectiveness studies were examined to identify ingredients that may influence learning. Individual-level variables, such as cognitive and linguistic abilities, may affect treatment response; however, findings were inconsistent across studies. CONCLUSIONS: Significant knowledge gaps were identified and include (a) which MoAs operate during ELess, EFul, and RP; (b) which ingredients are active and engage specific MoAs; and (c) how individual-level variables may drive treatment administration. Theory-driven research can support or refute MoAs and active ingredients enabling clinicians to modify treatments within theoretical frameworks.

Prior research suggests that cognitive control can assist the comprehension of sentences that create conflict between interpretations, at least under some circumstances. However, the mixed pattern of results suggests that cognitive control may not always be necessary for accurate comprehension. We tested whether cognitive control recruitment for language processing is systematically variable, depending on the type of sentential ambiguity or conflict, individual differences in cognitive control, and task demands. Participants completed two sessions in a web-based experiment. The first session tested conflict modulation using interleaved Stroop and sentence comprehension trials. Critical sentences contained syntax-semantics or phrase-attachment conflict. In the second session, participants completed three cognitive control and three working memory tasks. Exploratory factor analysis was used to index individual differences in a cognitive control factor and a working memory factor. At the group level, there were no significant conflict modulation effects for either syntax-semantics or phrase-attachment conflict. At the individual differences level, the cognitive control factor correlated with offline comprehension accuracy but not online processing measures for both types of conflict. Together, the results suggest that the role of cognitive control in sentence processing may vary according to task demands. When overt decisions are required, individual differences in cognitive control may matter such that better cognitive control results in better language comprehension performance. The results add to the mixed evidence on conflict modulation and raise questions about the situations under which cognitive control influences online processing.

Aphasia is a common consequence of stroke with severe impacts on employability, social interactions and quality of life. Producing discourse-relevant information in a real-world setting is the most important aspect of recovery because it is critical to successful communication. This study sought to identify the lesion correlates of impaired production of relevant information in spoken discourse in a large, unselected sample of participants with post-stroke aphasia. Spoken discourse (n = 80) and structural brain scans (n = 66) from participants with aphasia following left hemisphere stroke were analysed. Each participant provided 10 samples of spoken discourse elicited in three different genres, and 'correct information unit' analysis was used to quantify the informativeness of speech samples. The lesion correlates were identified using multivariate lesion-symptom mapping, voxel-wise disconnection and tract-wise analyses. Amount and speed of relevant information were highly correlated across different genres and with total lesion size. The analyses of lesion correlates converged on the same pattern: impaired production of relevant information was associated with damage to anterior dorsal white matter pathways, specifically the arcuate fasciculus, frontal aslant tract and superior longitudinal fasciculus. Damage to these pathways may be a useful biomarker for impaired informative spoken discourse and informs development of neurorehabilitation strategies.

Agrammatic or asyntactic comprehension is a common language impairment in aphasia. We considered three possible hypotheses about the underlying cause of this deficit, namely problems in syntactic processing, over-reliance on semantics, and a deficit in cognitive control. We tested four individuals showing asyntactic comprehension on their comprehension of syntax-semantics conflict sentences (e.g., The robber handcuffed the cop), where semantic cues pushed towards a different interpretation from syntax. Two of the four participants performed above chance on such sentences indicating that not all agrammatic individuals are impaired in structure-based interpretation. We collected additional eyetracking measures from the other two participants, who performed at chance on the conflict sentences. These measures suggested distinct underlying processing profiles in the two individuals. Cognitive assessments further suggested that one participant might have performed poorly due to a linguistic cognitive control impairment while the other had difficulty due to over-reliance on semantics. Together, the results highlight the importance of multimodal measures for teasing apart aphasic individuals' underlying deficits. They corroborate findings from neurotypical adults by showing that semantics can strongly influence comprehension and that cognitive control could be relevant for choosing between competing sentence interpretations. They extend previous findings by demonstrating variability between individuals with aphasia-cognitive control might be especially relevant for patients who are not overly reliant on semantics. Clinically, the identification of distinct underlying problems in different individuals suggests that different treatment paths might be warranted for cases who might look similar on behavioral assessments.

Cognitive rehabilitation research is increasingly exploring errorless learning interventions, which prioritise the avoidance of errors during treatment. The errorless learning approach was originally developed for patients with severe anterograde amnesia, who were deemed to be at particular risk for error learning. Errorless learning has since been investigated in other memory-impaired populations (e.g., Alzheimer's disease) and acquired aphasia. In typical errorless training, target information is presented to the participant for study or immediate reproduction, a method that prevents participants from attempting to retrieve target information from long-term memory (i.e., retrieval practice). However, assuring error elimination by preventing difficult (and error-permitting) retrieval practice is a potential major drawback of the errorless approach. This review begins with discussion of research in the psychology of learning and memory that demonstrates the importance of difficult (and potentially errorful) retrieval practice for robust learning and prolonged performance gains. We then review treatment research comparing errorless and errorful methods in amnesia and aphasia, where only the latter provides (difficult) retrieval practice opportunities. In each clinical domain we find the advantage of the errorless approach is limited and may be offset by the therapeutic potential of retrieval practice. Gaps in current knowledge are identified that preclude strong conclusions regarding a preference for errorless treatments over methods that prioritise difficult retrieval practice. We offer recommendations for future research aimed at a strong test of errorless learning treatments, which involves direct comparison with methods where retrieval practice effects are maximised for long-term gains.

OBJECTIVE: Little research has addressed the treatment of lexical-semantic comprehension deficits (i.e., difficulty retrieving the meanings of words) in people with aphasia (PWA). Research suggests that practice retrieving names for depicted objects from long-term memory (production-based retrieval practice) more strongly benefits word retrieval for production in PWA compared to errorless learning (i.e., word repetition), which eschews retrieval practice. This study assessed whether production-based and comprehension-based retrieval practice enhance performance on errorful word-comprehension items in PWA measured relative to nonretrieval forms of training and untrained control items. METHOD: In a within-participant group study of PWA, errorful comprehension items were assigned to (a) a production-based training module (retrieval practice vs. errorless learning); (b) a comprehension-based training module (a receptive form of retrieval practice vs. restudy). Each module comprised one training session and a 1-day and 1-week comprehension posttest on the module's trained items and an untrained item set. RESULTS: The comprehension module conditions produced similar and superior posttest performance relative to untrained items. Both production module conditions improved posttest performance relative to untrained items, with retrieval practice conferring more durable learning and generalization indicative of refinement of semantic representations compared to errorless learning. CONCLUSIONS: Results suggest comprehension- and production-based forms of training are both beneficial for improving lexical-semantic deficits in aphasia, with production-based retrieval practice conferring additional benefits to the targeted deficit compared to errorless learning. Future studies should examine these learning factors in schedules of training more commensurate with clinical practice and in other neurological populations (e.g., semantic dementia). Word retrieval difficulty (lexical access deficit) is prevalent in aphasia. Studies have shown that practice retrieving names from long-term memory (retrieval practice) improves future name retrieval for production in people with aphasia (PWA), particularly when retrieval is effortful. To explicate such effects, this study examined a potential role for semantic competition in the learning mechanism(s) underlying effortful retrieval practice effects in lexical access in 6 PWA. Items were trained in a blocked-cyclic naming task, in which repeating sets of pictures drawn from semantically-related versus unrelated categories underwent retrieval practice with feedback. Naming accuracy was lower for the related items at training, but next-day accuracy did not differ between the conditions. However, greater semantic-relatedness of an item to its set in the related condition was associated with lower accuracy at training but higher accuracy at test. Relevance to theories of lexical access and implications for naming treatment in aphasia are discussed.

The present study examined spontaneous detection and repair of naming errors in people with aphasia to advance a theoretical understanding of how monitoring impacts learning in lexical access. Prior work in aphasia has found that spontaneous repair, but not mere detection without repair, of semantic naming errors leads to improved naming on those same items in the future when other factors are accounted for. The present study sought to replicate this finding in a new, larger sample of participants and to examine the critical role of self-generated repair in this monitoring learning effect. Twenty-four participants with chronic aphasia with naming impairment provided naming responses to a 660-item corpus of common, everyday objects at two timepoints. At the first timepoint, a randomly selected subset of trials ended in experimenter-provided corrective feedback. Each naming trial was coded for accuracy, error type, and for any monitoring behavior that occurred, specifically detection with repair (i.e., correction), detection without repair, and no detection. Focusing on semantic errors, the original monitoring learning effect was replicated, with enhanced accuracy at a future timepoint when the first trial with that item involved detection with repair, compared to error trials that were not detected. This enhanced accuracy resulted from learning that arose from the first trial rather than the presence of repair simply signifying easier items. A second analysis compared learning from trials of self-corrected errors to that of trials ending in feedback that were detected but not self-corrected and found enhanced learning after self-generated repair. Implications for theories of lexical access and monitoring are discussed.

We investigated the influence of phonological neighbourhood density (PND) on the performance of aphasic speakers whose naming impairments differentially implicate phonological or semantic stages of lexical access. A word comes from a dense phonological neighbourhood if many words sound like it. Limited evidence suggests that higher density facilitates naming in aphasic speakers, as it does in healthy speakers. Using well-controlled stimuli, Experiment 1 confirmed the influence of PND on accuracy and phonological error rates in two aphasic speakers with phonological processing deficits. In Experiments 2 and 3, we extended the investigation to an aphasic speaker who is prone to semantic errors, indicating a semantic deficit and/or a deficit in the mapping from semantics to words. This individual had higher accuracy, and fewer semantic errors, in naming targets from high- than from low-density neighbourhoods. It is argued that the Results provide strong support for interactive approaches to lexical access, where reverberatory feedback between word- and phoneme-level lexical representations not only facilitates phonological level processes but also privileges the selection of a target word over its semantic competitors.

This study examined how the distribution and amount of practice affect word retrieval in aphasia as well as how such factors relate to the efficiency of learning. The central hypothesis was that factors that enhance the learning of new knowledge also enhance persistent access to existing, but inconsistently available, word representations. The study evaluated the impact of learning principles on word retrieval by manipulating the timing and amount of retrievals for items presented for naming. Nine people with chronic aphasia with naming impairment completed the experiment. Training materials involved proper noun entities assigned to six conditions formed by crossing a 2-level factor of spacing of sessions, i.e., intersession interval (1 day versus 7 days between sessions) with a 3-level factor of number of correct retrievals per item per session, i.e., criterion level (Criterion-1, Criterion-2, and Criterion-4). Each intersession interval condition comprised three training sessions and a one-month retention test. Increasing the criterion level enhanced naming performance after short (1 day, 7 days) and long (one month) retention intervals, but these advantages came at the cost of many additional training trials. In most cases, later naming success was superior when the same number of correct retrievals of an item was distributed across multiple sessions rather than administered within one session. The substantial advantages for across-session spacing were gained at little cost in terms of additional training trials. At one-month retention, naming accuracy was numerically but not significantly higher in the 7-day versus 1-day intersession interval condition. Implications for theories of lexical access and naming treatment in aphasia are discussed.

It is uncontroversial in psychological research that different schedules of practice, which govern the distribution of practice over time, can promote radically different outcomes in terms of gains in performance and the durability of learning. In contrast, in speech-language treatment research, there is a critical need for well-controlled studies examining the impact of the distribution of treatment on efficacy (for reviews, see Cherney, 2012; Warren, Fey, & Yoder, 2007). In this paper, we enumerate key findings from psychological research on learning and memory regarding how different schedules of practice differentially confer durable learning. We review existing studies of aphasia treatment with a focus on naming impairment that have examined how the distribution of practice affects treatment efficacy. We close by discussing potential productive lines of research to elaborate the clinical applicability of distributed practice principles to language treatment.

A widespread theoretical assumption is that many processes involved in text comprehension are automatic, with automaticity typically defined in terms of properties (e.g., speed, effort). In contrast, the authors advocate for conceptualization of automaticity in terms of underlying cognitive mechanisms and evaluate one prominent account, the memory-based processing account, which states that one mechanism underlying automatization involves a shift from algorithm-based interpretation of stimuli to retrieval of prior interpretations of those stimuli. During practice, participants repeatedly read short stories containing novel conceptual combinations that were disambiguated with either their dominant or subordinate meaning. During transfer, the combinations were embedded in new sentences that either preserved or changed the disambiguated meaning. The primary dependent variable was reading time in the disambiguating region of target sentences. Supporting the memory-based processing account, speed-ups with practice were larger for repeated versus unrepeated items of the same type, reading times for subordinate versus dominant meanings of the combinations converged on later trials, and practiced meanings were retrieved when items appeared in a transfer context.

Retrieval practice and spacing are two factors shown to enhance learning in basic psychological research. The present study investigated the clinical applicability of these factors to naming treatment in aphasia. Prior studies have shown that naming treatment that provides retrieval practice (i.e., practice retrieving names for objects from semantic memory) improves later naming performance in people with aphasia (PWA) more so than repetition training. Repetition training is a common form of naming treatment that can support errorless production of names for objects, but it does not provide retrieval practice. Prior work has also demonstrated enhanced naming treatment benefit in PWA when an item's training trials are separated by multiple intervening trials (i.e., spacing) compared to only one intervening trial (i.e., massing). However, in those studies, items were only trained in one session. Also, the effects of the learning factors were probed after one day and one week. The goal of the present study was to examine the effects of retrieval practice and spacing in a more clinically-inspired schedule of delivery and to assess the effects of the learning factors at retention intervals of greater functional significance. Matched sets of errorful items for each of four PWA were presented for multiple trials of retrieval practice or repetition in a spaced or massed schedule in each of multiple training sessions. Mixed regression analyses revealed that retrieval practice outperformed repetition, and spacing outperformed massing, at an initial post-treatment test administered after one week. Furthermore, the advantage for retrieval practice over repetition persisted at a follow-up test administered after one month. The potential clinical relevance of retrieval practice and spacing for multi-session interventions in speech-language treatment is discussed.

Naming pictures from the same semantic category hinders subsequent naming from that category (i.e., semantic interference), irrespective of the number of intervening different-category exemplars named. Persistent semantic interference has been well documented in chronometric studies, and has been attributed to experience-driven adjustments in the strength of connections between semantic and lexical representations. However, whether parallel effects exist in speech error data remains unclear. In the current study, people with aphasia, a speaker population prone to naming errors, provided naming responses to a large picture corpus presented in random order that comprised multiple exemplars drawn from several different categories. We found persistent semantic interference in the task in semantic error rates specifically, and that semantic similarity between consecutive related exemplars modulated the effect. The results provide further evidence for the presumed lexical-semantic locus and mechanism(s) underlying semantic interference.

Purpose: The purpose of this study was to determine how 2 methods known to improve naming impairment in aphasia (i.e., retrieval practice and errorless learning) affect lexical access. We hypothesized that instances of naming during retrieval practice use and strengthen item-specific connections in each of 2 stages of lexical access: Stage 1, meaning-to-word connections, and Stage 2, word-to-phonology connections. In contrast, errorless learning prioritizes opportunities for repeating words, which we expect to primarily strengthen item-specific connections in Stage 2 because repetition circumvents the need for semantically driven word retrieval. Method: We tested the outcomes of retrieval practice versus errorless learning training for items that were selected because the naming errors they elicited suggested weakened connections at Stage 1 or at Stage 2 of lexical access for each of 10 individuals with chronic aphasia. Each participant's Stage 1 items and Stage 2 items were divided evenly between the 2 training conditions. Naming tests were administered 1 day and 1 week after training to assess retention of training gains. We also examined whether the participants' pretraining naming error profiles were associated with the relative efficacy of retrieval practice versus errorless learning. Results: The posttraining naming tests showed an advantage of retrieval practice over errorless learning for Stage 1 items and an advantage of errorless learning over retrieval practice for Stage 2 items. In addition, greater percentages of phonological error naming responses prior to training were associated with greater posttraining accuracy in the errorless learning condition relative to the retrieval practice condition. Conclusions: The findings suggest that the advantage of retrieval practice for naming impairment in aphasia largely results from greater strengthening of practiced semantic-lexical connections compared with errorless learning, which prioritizes repetition and, therefore, mainly confers strengthening of practiced lexical-phonological connections. Understanding how specific training conditions improve naming can help predict the relative efficacy of each method for individuals with aphasia.

An important question within psycholinguistic research is whether grammatical features, such as number values on nouns, are probabilistic or discrete. Similarly, researchers have debated whether grammatical specifications are only set for individual lexical items, or whether certain types of noun phrases (NPs) also obtain number valuations at the phrasal level. Through a corpus analysis and an oral production task, we show that conjoined NPs can take both singular and plural verb agreement and that notional number (i.e., the numerosity of the referent of the subject noun phrase) plays an important role in agreement with conjoined NPs. In two written production tasks, we show that participants who are exposed to plural (versus singular or unmarked) agreement with conjoined NPs in a biasing story are more likely to produce plural agreement with conjoined NPs on a subsequent production task. This suggests that, in addition to their sensitivity to notional information, conjoined NPs have probabilistic grammatical specifications that reflect their distributional properties in language. These results provide important evidence that grammatical number reflects language experience, and that this language experience impacts agreement at the phrasal level, and not just the lexical level.

This study examined the timing of spontaneous self-monitoring in the naming responses of people with aphasia. Twelve people with aphasia completed a 615-item naming test twice, in separate sessions. Naming attempts were scored for accuracy and error type, and verbalizations indicating detection were coded as negation (e.g., "no, not that") or repair attempts (i.e., a changed naming attempt). Focusing on phonological and semantic errors, we measured the timing of the errors and of the utterances that provided evidence of detection. The effects of error type and detection response type on error-to-detection latencies were analyzed using mixed-effects regression modeling. We first asked whether phonological errors and semantic errors differed in the timing of the detection process or repair planning. Results suggested that the two error types primarily differed with respect to repair planning. Specifically, repair attempts for phonological errors were initiated more quickly than repair attempts for semantic errors. We next asked whether this difference between the error types could be attributed to the tendency for phonological errors to have a high degree of phonological similarity with the subsequent repair attempts, thereby speeding the programming of the repairs. Results showed that greater phonological similarity between the error and the repair was associated with faster repair times for both error types, providing evidence of error-to-repair priming in spontaneous self-monitoring. When controlling for phonological overlap, significant effects of error type and repair accuracy on repair times were also found. These effects indicated that correct repairs of phonological errors were initiated particularly quickly, whereas repairs of semantic errors were initiated relatively slowly, regardless of their accuracy. We discuss the implications of these findings for theoretical accounts of self-monitoring and the role of speech error repair in learning.

This study examined spontaneous self-monitoring of picture naming in people with aphasia. Of primary interest was whether spontaneous detection or repair of an error constitutes an error signal or other feedback that tunes the production system to the desired outcome. In other words, do acts of monitoring cause adaptive change in the language system? A second possibility, not incompatible with the first, is that monitoring is indicative of an item's representational strength, and strength is a causal factor in language change. Twelve PWA performed a 615-item naming test twice, in separate sessions, without extrinsic feedback. At each timepoint, we scored the first complete response for accuracy and error type and the remainder of the trial for verbalizations consistent with detection (e.g., "no, not that") and successful repair (i.e., correction). Data analysis centered on: (a) how often an item that was misnamed at one timepoint changed to correct at the other timepoint, as a function of monitoring; and (b) how monitoring impacted change scores in the Forward (Time 1 to Time 2) compared to Backward (Time 2 to Time 1) direction. The Strength hypothesis predicts significant effects of monitoring in both directions. The Learning hypothesis predicts greater effects in the Forward direction. These predictions were evaluated for three types of errors--Semantic errors, Phonological errors, and Fragments--using mixed-effects regression modeling with crossed random effects. Support for the Strength hypothesis was found for all three error types. Support for the Learning hypothesis was found for Semantic errors. All effects were due to error repair, not error detection. We discuss the theoretical and clinical implications of these novel findings.
