BACKGROUND: Handgrip strength (HGS) is an important health biomarker whose low scores have been shown to be associated with the morbimortality. This study aimed to analyze the factors associated with low HGS in older people in Rio Branco, Acre, Brazil. METHODS: The study was carried out with data from the Study of Chronic Diseases (EDOC-I) - Older People, a cross-sectional household PAPI probability sample survey performed with 1016 people aged over 60 residing in Rio Branco in 2014. The low HGS was defined by the 20th percentile of the maximum HGS by sex and age group. Associations between variables of health status (psychological and physical) and low HGS, by sex, were estimated using logistic regression, expressed by adjusted ORs (aOR). RESULTS: Older individuals had lower median HGS than younger individuals (- 6.0 kg among men and - 2.6 kg among women). Women aged over 80 had, on average, the lower quintile of HGS compared to women of the previous age groups. Factors independently associated with low HGS in men and women, respectively, were low weigh in body mass index [(aOR = 2.80; 95%CI: 1.19, 6.61) and (aOR = 2.61; 95%CI: 1.46, 4.66)], anemia [(aOR = 4.15; 95%CI: 2.09, 8.21) and (aOR = 1.80; 95%CI: 1.06, 3.06)] and diabetes as a risk factor in men (aOR 1.95; 95%CI: 1.00, 3.81). There was a higher chance of low HGS in men with partners (aOR = 2.44; 95%CI: 1.32, 4.51), smokers or former smokers (aOR = 3.25; 95%CI: 1.25, 8.44), with current self-assessment of health worse than the 12 previous months (aOR = 2.21; 95%CI: 1.14, 4.30) and dependence in activities of daily living (aOR = 2.92; 95%CI: 1.35, 6.30). Only among women, there was an increased chance of low HGS associated with altered waist-to-hip ratio (aOR = 1.79; 95%CI: 1.02, 3.12), insomnia (aOR = 1.83; 95%CI: 1.10, 3.03) and physical activity from displacement/occupation (aOR = 1.75; 95%CI: 1.08, 2.84). CONCLUSION: Factors associated with low HGS are not the same between sexes, and the inclusion of HGS as a component of health assessment seems to be a promising strategy for disease prevention and health promotion.

BACKGROUND: Individuals who live in rural areas are at greater risk for brain cancer, and pesticide exposure may contribute to this increased risk. The aims of this research were to analyze the mortality trends and to estimate the age-period-cohort effects on mortality rates from brain cancer in two regions in Rio de Janeiro, Brazil. METHODS: This descriptive study examined brain cancer mortality patterns in individuals of both sexes, >19 years of age, who died between 1996 and 2010. They were residents of a rural (Serrana) or a non-rural (Metropolitan) area of Rio de Janeiro, Brazil. We estimated mortality trends using Joinpoint Regression analysis. Age-period-cohort models were estimated using Poisson regression analysis. RESULTS: The estimated annual percentage change in mortality caused by brain cancer was 3.8% in the Serrana Region (95% confidence interval (CI): 0.8-5.6) and -0.2% (95% CI: -1.2-0.7) in the Metropolitan Region. The results indicated that the relative risk was higher in the rural region for the more recent birth cohorts (1954 and later). Compared with the reference birth cohort (1945-49, Serrana Region), the relative risk was four times higher for individuals born between 1985 and 1989. CONCLUSIONS: The results of this study indicate that there is an increasing trend in brain cancer mortality rates in the rural Serrana Region in Brazil. A cohort effect occurred in the birth cohorts born in this rural area after 1954. At the ecological level, different environmental factors, especially the use of pesticides, may explain regional disparities in the mortality patterns from brain cancers.

To investigate the association between pesticide use and breast cancer. A hospital-based case-control study was conducted in Petrópolis city, Brazil. The study data were obtained through interviews, and the magnitude of the association between self-reported pesticide exposure and breast cancer was determined using unconditional logistic regression. A higher estimated risk for breast cancer was found in women exposed to pesticides for 10 or more years, where this association was not statistically significant after adjusting for potential confounders (OR = 1.40; 95% CI 0.85-2.49). A positive statistically significant association was found between breast cancer and higher educational level or previous use of hormone replacement therapy (HRT), whereas having had 2 or more pregnancies to term proved a protective factor. Further studies elucidating the contribution of pesticide exposure to the development of breast cancer are needed, given that current findings in the literature are conflicting.

Persistent organic pollutants (POPs) are highly lipophilic and can accumulate and biomagnify in food chains. Characterized as a public health problem, exposure to these compounds enables the development of diseases like cancer, cardiovascular disease, diabetes, and obesity. The objective of this study was to estimate the plasma levels of organochlorine pesticides and PCBs in 151 samples (97 women; 54 men) in hospitalized individuals in Petropolis, Brazil. Individuals over 18 years of age and residing for at least 2 years in the mountainous region of the State of Rio de Janeiro, Brazil, participated in a cross-sectional study. Interviews using a structured questionnaire and blood samples to estimate plasma levels of persistent organic pollutants provided data. Gas chromatography coupled with triple quadrupole mass spectrometry provided the levels of organochlorine pesticides and PCBs. Compared to data present in the literature, the concentration of POPs was lower, and individuals from 55 to 64 years of age (3.28 ng mL-1) and women (2.52 ng mL-1) presented a higher average concentration of organochlorine pesticides; men (0.05 ng mL-1) also presented a high concentration of PCBs. This is the first Brazilian study to estimate the concentration of several POPs in a hospital-based sample that includes men and women, thus contributing to the characterization of our population regarding environmental exposures relevant to health.

OBJECTIVE: To develop and establish content validation of a nutritional assessment tool for pediatric cancer patients using the Delphi method. METHODS: A literature review was performed and the nutritional assessment in pediatrics and cancer construct was discussed with experts. Forty-six nutrition experts from Brazil with experience in oncology participated in the panel. Rounds were held to discuss and judge the items that should be included in this tool. With the aid of an algorithm, it was possible to simultaneously consider the adequacy, relevance and measurement of the items. The consensus was reached with an agreement equal to or greater than 80% among the experts. RESULTS: From the 7 domains suggested by the literature, the first round generated 81 items, which were assessed for adequacy, relevance and the possibility of being measured in the routine of nutritional assessment, by specialists in the following two rounds. The percentage of specialists who responded to each round was high (above 90%) and the dropout rate between the stages was relatively low. After applying the algorithm, the ANPEDCancer tool had 36 items considered appropriate by specialists from the five different geographical regions of Brazil. CONCLUSIONS: The Delphi method allowed the construction and content validation of the nutritional assessment tool for children and adolescents with cancer, providing the first stage for use in the Brazilian hospital setting.

OBJECTIVE: To analyze polypharmacy prevalence and associated factors in older adults living in Rio Branco, Acre, Brazil, in 2014. METHODS: This was a cross-sectional population-based study using complex sampling with older adults. Polypharmacy was defined as concomitant use of five or more medications. RESULTS: Polypharmacy prevalence was 14.9% (95%CI 11.8;18.6), positively associated with females (OR=2.29 - 95%CI 1.41;3.74), white race/skin color (OR=1.61 - 95%CI 1.10;2.38), dependence (OR=1.65 - 95%CI 1.05;2.60), change in eating habits/dieting (OR=1.66 - 95%CI 1.16;2.36), hospitalization in the last 12 months (OR=1.61 - 95%CI 1.02;2.53) and presence of the following self-reported morbidities: systemic arterial hypertension (OR=2.40 - 95%CI 1.33;4.34), diabetes mellitus (OR=2.17 - 95%CI 1.23;3.84), osteoporosis (OR=2.92 - 95%CI 1.84;4.64) and heart problems (OR=2.94 - 95%CI 1.90;4.56). CONCLUSION: This study found that polypharmacy in the older adults was associated with demographic and health conditions.

The scope of this study was to establish the prevalence of CKD and associated factors among young adults (18-59 years of age) from Rio Branco in the State of Acre. It involved a population-based survey conducted in 2014, in the urban and rural areas of the municipality and CKD was defined as the glomerular filtration rate (GFR) < 60ml/min/1.73m², estimated by the CKD-EPI, and the presence of albuminuria > 29 mg/g. Association measures were estimated by logistic regression, with a confidence level of 95%. The overall prevalence of CKD was 6.2%. The presence of CKD was higher among women, aged 40 to 59 years, with non-white skin color, with lower schooling, and of sedentary disposition. There were statistically significant differences in the distribution according to the presence of CKD in the hypertension, diabetes and hospitalization variables over the past 12 months. CKD among adults was associated with the female sex (OR: 2.41, 95%CI: 1.14-5.12), diabetes (OR: 4.67, 95%CI: 1.28-17.03) and arterial hypertension (OR: 1.98; 95%CI: 1.16-3.37). CKD reveals a high prevalence and is associated with chronic diseases, calling for the need for public health measures for early detection and prevention of its progression.


O objetivo do presente estudo foi verificar a prevalência de DRC e os fatores a ela associados em adultos jovens (18-59 anos) de Rio Branco, Acre. Trata-se de um inquérito de base populacional realizado em 2014, nas zonas urbana e rural do município de Rio Branco, Acre. A DRC foi definida pela taxa de filtração glomerular (TFG) < 60 ml/min/1,73 m², estimada pela CKD-EPI, e presença de albuminúria > 29 mg/g. Medidas de associação foram estimadas por regressão logística, com grau de confiança de 95%. A prevalência geral de DRC foi de 6,2%. A presença de DRC foi maior entre as mulheres, naqueles com 40 a 59 anos, de cor da pele não-branca, com menor escolaridade e entre os sedentários. Houve diferenças estatisticamente significativas na distribuição segundo a presença de DRC nas variáveis hipertensão arterial, diabetes e internação nos últimos 12 meses. A DRC entre os adultos esteve associada ao sexo feminino (OR: 2,41; IC95%: 1,14-5,12), diabetes (OR: 4,67; IC95%: 1,28-17,03) e hipertensão arterial (OR: 1,98; IC95%: 1,16-3,37). A DRC apresenta alta prevalência e está associada às doenças crônicas, evidenciando a necessidade de medidas em saúde pública para detecção precoce e prevenção da sua progressão.

OBJECTIVE To review the main instruments of functional assessment and health status cited in the literature to evaluate Brazilian workers and verify the compatibility of their items with the core set for professional rehabilitation. METHODS A review of the literature was conducted in the main databases in search of articles that used assessment instruments in populations of workers between 2007 and 2017. Subsequently, the contents of the identified instruments were retrieved, and two evaluators analyzed their items to verify the compatibility with the categories of the core set of the International Classification of Functioning for professional rehabilitation. Cohen's kappa coefficient was used to evaluate the agreement between the evaluators. RESULTS Five specific and eight generic instruments were selected to evaluate the functioning of workers. The analysis of the items of the total instruments allowed the definition of 58 categories (64.5%) of the core set with minimal overlap: 13 (76.5%) of the body functions component, 29 (72.5%) of the activities and participation component and 16 (49%) environmental factors. CONCLUSIONS The association of several instruments requires time and makes it difficult to use the classification. The development of instruments with direct association with its categories is essential to operationalize it.

OBJECTIVE: To analyze the association between anthropometric variables and cardiovascular risk factors in adults and older adults of Rio Branco, Acre. METHODS: A population-based cross-sectional study with 641 adults and 957 older adults was conducted. The statistical analyses consisted of the distribution of anthropometric variables according to the cardiovascular risk factors by frequency and dispersion measures. Pearson's correlation coefficient and prevalence ratios (PR) were estimated with their respective 95% confidence intervals (95%CI) using the SPSS ® version 20.0. RESULTS: Moderate correlations were obtained in adult men for waist-hip ratio and total cholesterol (r = 0.486; p < 0.001) and for waist-hip and triglyceride ratios (r = 0.484; p < 0.001). The highest prevalence of hypertension and diabetes in adults were observed in men; in the older adults, the prevalence of hypertension was above 65% in both sexes. The prevalence of dyslipidemia was above 78% in obese adults and older adults. When analyzing the associations, a higher strength of association was found between arterial hypertension and waist-to-stature ratio (PR = 13.42; 95%CI 12.58-14.31) and body mass index greater than 30 kg/m 2 (PR = 6.61; 95%CI 6.34-6.89) in adult men. In the analysis of diabetes, the waist-hip ratio presented greater robustness in the association for women (PR = 7.53; 95%CI 6.92-8.20) and men (PR = 9.79; 95%CI 9.14-10.49). CONCLUSION: Anthropometric variables are important predictors of cardiovascular risk; however, their assessments should be performed independently, according to sex and age group.

ABSTRACT Workers' functioning is related to the environmental conditions that influence their activities, favoring or hindering their fulfilment. The Work Rehabilitation Questionnaire (WORQ) was based on a core set of the Classification of Functioning (ICF) to assess workers. It has a validated Portuguese version for use with active Brazilian workers but it only assesses function body limitations, activities, and participation. This study developed a questionnaire to assess environmental barriers at work following the WORQ format and tested its reliability. This study reached a consensus (10 professionals and 11 workers) to choose environmental categories in the CIF core set that originated the WORQ to be integrated into the questionnaire. This research selected elements if at least 85% of participants reported it. Categories were transformed into questions. For reliability analysis, the questionnaire was applied to a random sample of 123 active workers at a public university in Brazil. The questionnaire had 20 questions based on the ICF core set for vocational rehabilitation and WORQ. This study evaluated its reliability, finding an r=0.855 (test-retest) and Cronbach's alpha=0.936 (internal consistency). This study developed an ICF-based questionnaire to assess environmental barriers in the workplace. The analysis of psychometric characteristics showed strong test-retest reliability and the internal consistency of the instrument.


RESUMEN La funcionalidad del trabajador está relacionada con las condiciones ambientales que influyen en sus actividades, favoreciendo o dificultando su desempeño. El Cuestionario de Rehabilitación Profesional (WORQ) se basa en un core set de la Clasificación de Funcionalidad (CIF) para evaluar a los trabajadores y cuenta con una versión en portugués validada para aplicar a trabajadores brasileños en actividad; sin embargo, esta herramienta está circunscrita a evaluar solo las limitaciones en las funciones corporales, las actividades y la participación. Este estudio tuvo por objetivo desarrollar un cuestionario, con base en el formato del WORQ, para evaluar las barreras en el entorno laboral y comprobar su fiabilidad. La elección de las categorías ambientales que serán incluidas en el cuestionario -y que constan en el core set de la CIF que dio origen al WORQ- se realizó por consenso (10 profesionales y 11 trabajadores). Se seleccionaron las categorías elegidas por aproximadamente el 85% de los participantes para convertirlas en preguntas. Para evaluar la fiabilidad, el cuestionario se aplicó a una muestra aleatoria de 123 trabajadores en actividad de una universidad pública de Brasil. El cuestionario constaba de un total de 20 preguntas. Su fiabilidad se evaluó con r=0,855 (test-retest) y alfa de Cronbach=0,936 (consistencia interna). Este estudio desarrolló un cuestionario basado en la CIF para evaluar las barreras ambientales en el entorno laboral que, mediante el análisis de las características psicométricas, indicó una fuerte fiabilidad test-retest y consistencia interna de esta herramienta.

O Questionário de Reabilitação Profissional (WORQ) foi baseado em um core set da Classificação de Funcionalidade (CIF) para avaliar trabalhadores, ele apresenta versão em português validada para uso com trabalhadores brasileiros ativos, porém, se restringe à avaliação de limitações em funções corporais, atividades e participação. Este estudo teve como objetivo elaborar um questionário de avaliação de barreiras ambientais no trabalho, seguindo o formato do WORQ, e testar sua confiabilidade. A escolha das categorias ambientais - constantes no core set da CIF que originou o WORQ - para integrar o questionário foi realizada em consenso (10 profissionais e 11 trabalhadores). Foram selecionadas aquelas apontadas por pelo menos 85% dos participantes e, dessa forma, transformadas em questões. Para análise da confiabilidade, o questionário foi aplicado em uma amostra aleatória de 123 trabalhadores ativos de uma universidade pública do Brasil. O questionário elaborado teve o total de 20 questões. A sua confiabilidade foi avaliada com r=0,855 (teste-reteste) e alfa de Cronbach=0,936 (consistência interna). OBJECTIVE: Describe the sampling design and other methodological aspects of the Study of Chronic Diseases (Edoc). METHODS: Edoc comprises two household surveys with distinct populations, one with adults aged 18 to 59 years (Edoc-A) and another with older adults aged 60 years or more (Edoc-I), living in Rio Branco, Acre. The selection of the participants used complex samples by clusters in two stages of selection, census enumeration areas (CEA) and household. In the first stage, common to both surveys, 40 CEAs were selected with probability proportional to size, and in the second stage, independent for each survey, households were selected with equal probability, and all the residents eligible for each survey were selected. Sampling weights were estimated by the inverse of the product of inclusion probabilities at each stage and then calibrated to produce unbiased population estimates. Interviews were held with questionnaires about socioeconomic and demographic conditions, life habits and health conditions. Anthropometric measures focused on measures of body height, girths and mass, while the vital signs analyzed were blood pressure, heart rate and respiratory rate. Blood and urine samples were collected for analysis. RESULTS: The Edoc comprised 1,701 participants, 685 of Edoc-A and 1,016 of Edoc-I. Considering the loss of information of some participants and the need of studying specific themes with production of population inferences, 16 subsamples of complete information by theme were generated and two subsamples were exclusive of Edoc-I. CONCLUSIONS: The Edoc has as important developments the analyses of epidemiological profile of the population from the capital of the state of Acre, contributing to the production of knowledge in public health with useful information for decisions in public health policies.

Hand grip strength (HGS) is recognized as an important health indicator, but validated reference values that can be applied to the evaluation of individuals in different populations are still lacking. This work aimed to identify correlations between HGS and anthropometric variables and to establish HGS reference values for adult and elderly populations. This is a population-based cross-sectional study considering the subsets of individuals with healthy right or left upper limbs from a sample of 1,609 adults and elderly residents in Rio Branco, Acre, Brazil. Descriptive statistics of anthropometric measures and HGS values at maximum performance based on three measurements of the two hands were obtained, and Pearson correlations between these variables were applied. Percentile distributions were estimated for right and left HGS by sex and age group. Men presented, in general, a maximum HGS 57% higher than women (43.4 kg vs. 27.6 kg), and also higher HGS levels in the different age groups. In both sexes, the highest HGS values were observed in the age group of 30 to 39 years (men, 46.9 kg; women, 29.4 kg), with a subsequent decline. HGS presented a negative correlation with age and a weak to moderate positive correlation with anthropometric variables, among men and women. The median HGS of men was reduced by about 46% between the ages of 30 and 39 years and 80 years and over (right hand, 46.4 to 23.7 kg; left hand, 42.2 to 23.5 kg) and by about 44% in women (right hand, 29.0 to 16.4 kg, left hand, 27.3 to 15.2 kg). The values identified are a reference for HGS behavior among healthy adults and seniors, although they do not discriminate individuals with specific health conditions. They can be used in rehabilitation programs and subsidize future studies aimed at exploring their potential application in the evaluation of the health condition of adults and elderly individuals.

OBJECTIVE: To verify the prevalence of chronic kidney disease and the factors associated to it in older adults (≥ 60 years). METHODS: This is a population-based research conducted in 2014, involving 1,016 older adults living in urban and rural areas of the municipality of Rio Branco, Acre. Chronic kidney disease was defined by glomerular filtration rate < 60 ml/min/1.73 m 2 , estimated by the equations of the Chronic Kidney Disease Epidemiology Collaboration, and the presence of albuminuria > 29 mg/g. Association measure were estimated by gross and adjusted odds ratio (OR), with a confidence level of 95% (95%CI). RESULTS: The overall prevalence of chronic kidney disease was 21.4% in older adults, with the associated factors age, diabetes (OR = 3.39; 95%CI 2.13-5.40), metabolic syndrome (OR = 2.49; 95%CI 1.71-3.63), self-assessment of poor health (OR = 1.79; 95%CI 1.10-2.91), arterial hypertension (OR = 1.82; 95%CI 1.04-3.19) and obesity (OR = 1.69; 95%CI 1.02-2.80). CONCLUSIONS: The prevalence of chronic kidney disease was high in older adults, being associated with age, self-assessment of health as bad or very bad, obesity, diabetes and metabolic syndrome.

Introdução: analisar a prevalência e os fatores associados à doença renal crônica em pessoas com hipertensão arterial (HAS) cadastrados na Estratégia de Saúde da Família em Rio Branco, Acre. Método: Trata-se de um estudo transversal realizado com hipertensos cadastrados nas Estratégias de Saúde da Família (ESF), na zona urbana de Rio Branco, Acre, em 2019. A DRC foi definida por TFG < 60 ml/1,72m², estimada a partir da fórmula Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), ou presença de albuminúria > 29 mg/g. Medidas de associação foram estimadas por regressão logística, com grau de confiança de 95%. Resultados: A prevalência de DRC foi de 27,3% em pessoas com HAS. Observou-se associação estatisticamente significativa nos pacientes hipertensos entre DRC e escolaridade (ensino fundamental ­ OR: 1,56; IC95%:1,09; 2,23; sem escolaridade ­ OR:1,87; IC95%: 1,14; 3,07), não controle da pressão arterial (OR: 2,39; IC95%: 1,45; 3,94), sedentarismo (OR: 1,63; IC95%: 1,02; 2,60). Conclusão: A DRC apresenta alta prevalência nos hipertensos, evidenciando a necessidade de medidas em saúde pública para prevenção de complicações mediante melhor controle da doença e mudanças no estilo de vida.


Introduction: To analyze the prevalence and factors associated with chronic kidney disease in people with hypertension (SAH) registered in the Family Health Strategy in Rio Branco, Acre. Method: This is a cross-sectional study carried out with hypertensive patients registered in the Family Health Strategies (ESF), in the urban area of Rio Branco, Acre, in 2019. CKD was defined by GFR < 60 ml/1.72m², estimated from the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) formula, or presence of albuminuria > 29 mg/g. Association measures were estimated using logistic regression, with a 95% confidence level. Results: The prevalence of CKD was 27.3% in people with hypertension. A statistically significant association was observed in hypertensive patients between CKD and education (primary education ­ OR: 1.56; 95% CI: 1.09; 2.23; no education ­ OR: 1.87; 95% CI: 1.14; 3.07), lack of blood pressure control (OR: 2.39; 95% CI: 1.45; 3.94), sedentary lifestyle (OR: 1.63; 95% CI: 1.02; 2.60). Conclusion: CKD has a high prevalence in hypertensive patients, highlighting the need for public health measures to prevent complications through better control of the disease and changes in lifestyle.


Introducción: Se analizó la prevalencia y los factores asociados a la enfermedad renal crónica (ERC) en personas con hipertensión arterial (HTA) inscritas en la Estrategia de Salud Familiar en Rio Branco, Acre. Método: Estudio transversal con hipertensos inscritos en las Estrategias de Salud Familiar (ESF) en la zona urbana de Rio Branco, Acre, en 2019. La ERC se definió por un FG <60 ml/1,72m², estimado a partir de la fórmula de Colaboración de Epidemiología de Enfermedades Renales Crónicas (CKD-EPI), o la presencia de albuminuria >29 mg/g. Se estimaron medidas de asociación mediante regresión logística, con un nivel de confianza del 95%. Resultados: La prevalencia de ERC fue del 27,3% en personas con HTA. Se observó una asociación estadísticamente significativa entre la ERC y la escolaridad (educación primaria ­ OR: 1,56; IC95%: 1,09; 2,23; sin escolaridad ­ OR: 1,87; IC95%: 1,14; 3,07), falta de control de la presión arterial (OR: 2,39; IC95%: 1,45; 3,94), y sedentarismo (OR: 1,63; IC95%: 1,02; 2,60) en pacientes hipertensos. Conclusión: La ERC presenta una alta prevalencia en pacientes hipertensos, lo que destaca la necesidad de medidas de salud pública para prevenir complicaciones mediante un mejor control de la enfermedad y cambios en el estilo de vida.

OBJECTIVE: to analyze the validity of self-reported systemic arterial hypertension (SAH) in the adult population of Rio Branco, Acre, Brazil. METHODS: this was a study of diagnostic accuracy with 576 adults aged 18 to 59 years; accuracy, sensitivity, specificity and positive and negative predictive values were calculated in order to build the Receiver Operating Characteristic (ROC) curve. RESULTS: the prevalence rates of measured SAH (gold standard) and self-reported SAH were 19.6% and 16.6%, respectively; self-reported SAH showed 53.7% sensitivity and 92.4% specificity; sensitivity ranged from 29.9% in individuals under 40 years of age, to levels above 70.0% among obese individuals and those who reported having dyslipidemia; specificity varied from 70.0% in those who self-reported diabetes mellitus, to 95.3% in underweight patients; the area under the ROC hypertension analysis curve was 0.77 (95%CI 0.72;0.81). CONCLUSION: self-reported SAH in adults 40 years old and over was found to be accurate for use in studies in Rio Branco.

OBJECTIVE: to analyze the temporal trend of leukemia mortality in children and adolescents under 20 years old, in Brazilian state capitals and Federal District, between 1980 and 2015. METHODS: this was a descriptive time series study; age-standardized leukemia mortality rates were calculated; the Prais-Winsten regression model was used. RESULTS: leukemia mortality rates fell from 2.73 to 1.58 per 100,000 inhabitants aged under 20 years in the period studied (annual percentage variation of -7.06%: 95%CI -8.61;5.49), with differences between capitals; there was a reduction in magnitude in all age groups - 0-4 years (9.17%: 95%CI -11.58;-6.69), 5-9 years (-9,27%: 95%CI -11.90;-6.56), 10-14 years (-4.86%: 95%CI -6.69;-3.00) and 15-19 years (-3.77%: 95%CI -5.22;-2.31). CONCLUSION: there was a falling trend in pediatric leukemia mortality rates, although it was unequal between the capitals, indicating the need for measures to minimize regional differences.

ABSTRACT Objective: To develop and establish content validation of a nutritional assessment tool for pediatric cancer patients using the Delphi method. Methods: A literature review was performed and the nutritional assessment in pediatrics and cancer construct was discussed with experts. Forty-six nutrition experts from Brazil with experience in oncology participated in the panel. Rounds were held to discuss and judge the items that should be included in this tool. With the aid of an algorithm, it was possible to simultaneously consider the adequacy, relevance and measurement of the items. The consensus was reached with an agreement equal to or greater than 80% among the experts. Results: From the 7 domains suggested by the literature, the first round generated 81 items, which were assessed for adequacy, relevance and the possibility of being measured in the routine of nutritional assessment, by specialists in the following two rounds. The percentage of specialists who responded to each round was high (above 90%) and the dropout rate between the stages was relatively low. After applying the algorithm, the ANPEDCancer tool had 36 items considered appropriate by specialists from the five different geographical regions of Brazil. Conclusions: The Delphi method allowed the construction and content validation of the nutritional assessment tool for children and adolescents with cancer, providing the first stage for use in the Brazilian hospital setting.


RESUMO Objetivo: Desenvolver e estabelecer a construção, a validação de conteúdo e a elaboração de um plano de ação para cada diagnóstico nutricional de um instrumento de avaliação nutricional para pacientes pediátricos com câncer por meio do método Delphi. Métodos: Foi realizada uma revisão da literatura e discutido com especialistas o constructo avaliação nutricional em pediatria em câncer. Participaram do painel 46 especialistas nutricionistas do Brasil com experiência em oncologia. Rodadas ocorreram, para discutir e julgar os itens que deveriam constar do instrumento. Com o auxílio de um algoritmo, foi possível ponderar simultaneamente a adequação, relevância e mensuração dos itens. O consenso foi atingido com concordância igual ou superior a 80% entre os especialistas. Resultados: Com base nos sete domínios sugeridos pela literatura científica, a primeira rodada gerou 81 itens, que foram avaliados quanto à adequação, relevância e possibilidade de ser medido na rotina de avaliação nutricional pelos especialistas nas duas rodadas seguintes. O percentual de especialistas que responderam a cada rodada foi alto (acima de 90%), e a desistência entre as etapas, relativamente baixa. Após aplicação do algoritmo, o instrumento ANPEDCancer contou com 36 itens considerados apropriados pelos especialistas das cinco distintas regiões geográficas do Brasil. Conclusões: O método Delphi permitiu a construção e validação de conteúdo do instrumento de avaliação nutricional para crianças e adolescentes com câncer, fornecendo uma primeira versão a ser aplicada e avaliada no cenário hospitalar brasileiro.

Resumo Objetivo Estimar a prevalência da síndrome de fragilidade e sua associação com variáveis socioeconômicas, demográficas e de saúde, em idosos atendidos em duas Unidades de Saúde (US) no município de Rio Branco, Acre, no período de outubro de 2016 a junho de 2017. Método A prevalência de síndrome de fragilidade foi medida pela Edmonton Frail Scale (EFS), investigada em uma amostra calculada de 298 pessoas idosas, selecionadas aleatoriamente por meio de sorteio. Utilizou-se a Regressão de Poisson, com variância robusta e intervalos de confiança de 95%, para estimar as razões de prevalência e definir o modelo ajustado. Todas as análises levaram em consideração os pesos. Resultados Verificou-se que 35,1% da amostra apresentou fragilidade. A prevalência de fragilidade foi associada a ter 75 anos ou mais, inatividade física, risco nutricional, deficit cognitivo, percepção negativa da saúde, usar 5 ou mais medicamentos e ter/histórico de câncer, queda no último ano, morar sozinho, segurança de bairro insatisfatória e ser da etnia/cor não branca. Conclusão Verificou-se o perfil de alerta para rastreio da fragilidade, que poderá auxiliar na prática clínica dos profissionais das US da população de estudo e, ainda, considera a necessidade de implantação e fortalecimento de programas de atenção à saúde da pessoa idosa e atuação de matriciamento e/ou grupos de apoio multiprofissional à Saúde da Família.


Abstract Objective To estimate the prevalence of frailty syndrome and its association with socioeconomic, demographic and health variables, in elderly people treated at two Health Units in the city of Rio Branco, Acre, from October 2016 to June 2017. Method The prevalence of frailty was measured using the Edmonton Frail Scale (EFS), and associations were tested with selected variables. Poisson regression, with robust variance and 95% confidence intervals, was used to estimate the prevalence ratios and define the adjusted model. All analyzes took into account the sample weights and were performed using SPSS version 20. Results It was found that 35.1% of the sample showed fragility. The prevalence of frailty was associated with being 75 years old or more, physical inactivity, nutritional risk, cognitive deficit, negative health perception, using 5 or more medications and having/history of cancer, falls in past year, living alone, unsatisfactory neighborhood safety and being of ethnicity/non-white color. Conclusion The alert profile for screening for frailty was verified, which may assist in the clinical practice of FHS professionals in the study population, and also considers the need to implement and strengthen eldely's health care programs and performance of the Family Health Support Centers.

Resumo Introdução: o Questionário de Reabilitação para o Trabalho (WORQ) foi criado para avaliação da funcionalidade de trabalhadores em reabilitação. Por ser abrangente, supõe-se que sua utilização é útil para avaliar trabalhadores ativos. Objetivo: avaliar a validade de constructo e a confiabilidade da versão autorrespondida do WORQ traduzida para o português brasileiro, para uso com trabalhadores ativos. Métodos: aplicou-se o WORQ em uma amostra aleatória de trabalhadores da Universidade Federal do Acre, no Brasil. Para validação de constructo, foram levantadas hipóteses e feitas correlações entre o WORQ e os instrumentos: WHOQOL-BREF, WHODAS, PHQ-9 e GAD-7. Para confiabilidade, foi utilizada a metodologia teste-reteste em subamostra (n=50) e posterior cálculo do coeficiente de Spearman. Para consistência interna, foi calculado o alfa de Cronbach. Resultados: participaram 241 trabalhadores, 50,2% do sexo feminino. O WORQ apresentou forte correlação com o WHODAS (r=0,782), moderada com o PHQ-9 e com a GAD-7 (r=0,68 e r=0,675) e moderada correlação inversa com o WHOQOL (r=-0,671). Foi obtido alfa de Cronbach de 0,95 e coeficiente de correlação de Spearman de 0,86. Conclusão: O WORQ apresentou evidências sugestivas de validade de constructo e de confiabilidade para uso com trabalhadores ativos, podendo sua utilização ser sugerida na triagem de incapacidades nesses trabalhadores.


Abstract Introduction: the Work Rehabilitation Questionnaire (WORQ) was created to assess the functioning and disability of workers undergoing rehabilitation. Since it is comprehensive, it is assumed that its use for a population of active workers can be useful. Objective: to evaluate the construct validity and the reliability of the self-reported WORQ translation to Brazilian Portuguese, in active workers population. Methods: we applied the WORQ to a random sample of workers from the Federal University of Acre, Brazil. For construct validation, we raised hypotheses and analised correlations between the WORQ and the instruments: WHOQOL-BREF, WHODAS, PHQ-9, and GAD-7. For assess reliability, we used the test-retest methodology in a sub-sample (n=50), followed by the calculation of the Spearman coefficient. For internal consistency, we calculated the Cronbach's alpha. Results: 241 workers participated, 50.2% female. The WORQ showed a strong correlation with WHODAS (r=0.782), moderate with PHQ-9 and GAD-7 (r=0.68 and r=0.675) and a moderate inverse correlation with WHOQOL (r=−0.671). We calculated a Cronbach´s' alpha of 0.95, and a Spearman's correlation coefficient of 0.86. Conclusion: the WORQ presented suggestive evidence of construct validity and reliability for use with active workers. Its use in screening for disabilities in these workers may be suggested.

This study examined the mortality trend due to chronic renal failure (CRF) and verified the underlying and associated causes for this trend in the capital of the state of Acre in the Brazilian Amazon. This ecological study used data provided by DATASUS related to mortality due to CRF, which occurred between 1986 and 2012 for male and female residents of the city of Rio Branco, Acre, Brazil. The estimated annual percentage chance (EAPC) was calculated by using Poisson log-linear regression and utilizing the Joinpoint program. The results showed that the adjusted mortality rates due to CRF, with correction, ranged from 15.4 per 100,000 inhabitants in 1986 to 4.0 per 100,000 inhabitants in 2012. The EAPC was -3.5% from 1986- 2012. Deaths by CRF presented associated causes such as respiratory diseases, pneumonia and pulmonary edema, septicemias and poorly defined signs and symptoms. When CRF was analyzed as an associated cause of death, the main primary causes of death were hypertensive diseases and diabetes. Thus, there was a decrease in mortality due to CRF as an underlying cause during the period studied; however, preventive and heath care measures should be maintained.


O presente estudo buscou observar a tendência de mortalidade por insuficiência renal crônica (IRC) e verificar as causas básicas e associadas na capital do Acre, Amazônia brasileira. Para tanto, foi realizado um estudo ecológico com dados de óbitos por IRC do DATASUS ocorridos entre 1986 e 2012, em ambos os sexos de residentes em Rio Branco, Acre. Estimou-se a variação anual percentual (Estimated Annual Percentage Chance ­ EAPC) com a técnica de regressão log-linear de Poisson do programa Joinpoint. Os resultados demonstraram que as taxas de mortalidade ajustadas da IRC com correção variaram de 15,4 por 100.000 hab., em 1986, para 4,0 por 100.000 hab., em 2012. A EAPC foi de -3,5%, de 1986 a 2012. Os óbitos pela IRC apresentaram como causas associadas às doenças respiratórias, pneumonia e edema pulmonar, às septicemias e aos sinais e sintomas mal definidos. Quando analisada a IRC como causa associada, as principais causas básicas do óbito foram as doenças hipertensivas e o diabetes. Assim, houve redução da mortalidade por insuficiência renal crônica como causa básica no período observado, contudo medidas de prevenção e assistência em saúde devem ser mantidas.
