RESUMEN

The targeted delivery of drugs using wireless navigable magnetic robots allows the delivery of drug molecules to be controlled non only in time but also in space, improving medical outcomes. The main disadvantages behind their use lies in the low amount of drug that can be transported and the single nature of drug that can be loaded (hydrophilic or hydrophobic). These considerations limit their use in co-delivery systems, now recognized to be very promising for many different pathologies. A magnetic bijel-like structure is developed to load and release different types of molecules (hydrophilic and hydrophobic). In this work, the use of ε-caprolactone is explored, which can polymerize, forming hydrophobic domains (oil phase). After mixing with iron oxide nanoparticles (NPs), the water dispersion creates a magnetic biphasic porous structure without phase separation. The resulting device shows good performance both in magnetic actuation and as a drug delivery system.

INTRODUCTION: There are no standard therapies for chronic lymphocytic leukemia (CLL)-associated immune thrombocytopenia (IT) so far. PATIENTS AND METHODS: We report the results of therapy with single agent rituximab in 21 patients with CLL-associated IT. The mean age at CLL and IT diagnosis was 64 and 68 yr, respectively. IT developed at a mean time of 44 months from the diagnosis of CLL. In four cases, IT was diagnosed at the same time as CLL. For three patients, IT was considered fludarabine-related and two patients showed autoimmune hemolysis also. All patients but one received steroids as first-line treatment for IT. Some patients received intravenous high-dose Ig, vincristine, and Cytoxan also, without beneficial effect. After a mean time of 43 d from the diagnosis of IT, all patients were scheduled to receive rituximab at a dosage of 375 mg/mq/weekly. RESULTS: Eighteen (86%) patients completed the scheduled four cycles of rituximab. Irrelevant first infusion side effects were seen only in one patient. Twelve (57%) patients showed a complete response (CR), six (29%) patients a partial response (PR), and three (14%) patients did not respond. In responding patients, the mean duration of response was 21 months (4-49 months). At a mean follow-up of 28 months, 14 (66%) patients were still alive, 10 (48%) of them in CR and three (14%) in PR. CONCLUSIONS: This retrospective analysis prove that rituximab is an effective and well-tolerated alternative treatment for CLL-associated IT.

AC133 is a molecule whose expression in the human hematopoietic system is restricted to a subset of CD34+ progenitor/stem cells with long-term repopulating ability. The antigenic features of these cells, like CD34+ cells, are described heterogeneous. The immunophenotypic profile of AC133+ cells, detected by means of dual-color flow cytometry, in bone marrow (BM), cytokine-mobilized peripheral blood (PB) and umbilical cord blood (UCB) was evaluated. The highest percentage of AC133+ cells was detected in mobilized PB despite not significantly different from that found in BM, but both are higher than that found in UCB. In addition, the highest percentage of CD34, HLA-DR and CD33 co-expressing AC133+ cells was observed in mobilized PB. Furthermore, UCB was found to be enriched in CD7+ and CD19+ cells and BM was found to be enriched in AC133+ cells co-expressing CDw90 and CD71. Our data confirm the immunophenotypic heterogeneity of cells expressing AC133 antigen, a promising new stem cell marker to be increasingly used as additional target for alternative identification and separation of early hematopoietic cells.

RESUMEN

Normal promyelocytes express CD45RO, while acute promyelocytic leukemia (APL) blasts express CD45RA, both isoforms of common leukocyte antigen with mutually exclusive expression. Here we report a patient with accumulation of promyelocytes in the bone marrow and the diagnostic strategy is described along with the ability to quickly discriminate between normal and abnormal cells using the flow cytometric detection of expression of RA/RO isoforms of CD45.

Complementary and alternative medicine (CAM) is common in patients with cancer and its use is steadily increasing over time. We performed a multicenter survey in which the use of CAM in 442 Italian patients with chronic lymphocytic leukemia (CLL), the commonest form of leukemia in Western countries, was assessed. Data were collected by means of a face-to-face standardized questionnaire with several items. Mean age was 69 years; 258 patients (58%) were male and 184 (42%) female. Seventy-three patients (16.5%) were found to be CAM users. The most common CAM therapies were green tea, aloe formulations and high dose vitamins. Predictors of CAM use were female gender, younger age, higher education level, internet availability and newspaper reading. The reasons for CAM popularity among these patients are complex. Given the number of patients combining therapy with CAM and its possible drug interactions, doctor interest as well as patient education about CAM should be improved.
