RESUMEN

PURPOSE: To determine the association between isolates in the middle ear (ME) and nasopharynx of patients with chronic otitis media in Ilorin, north-central Nigeria. METHODS: An ethically approved case control study was carried out in the Ear, Nose, and Throat clinic amongst consenting cases using normal subjects as controls. A microbiology investigation form giving the results for otoscopy, aspirate and swabs was filled out for both the ME and nasopharynx. The experimental procedure was carried out and bacteria were identified according to colony characteristics, morphological appearance, Gram-staining, and standard biochemical testing. Data obtained were analysed with SPSS version 16.0 and Epi Info 3.5.1 using the mean, standard deviation and chi-square results. RESULT: A total of 140 cases and 70 controls, were recruited. The Gram stain reaction of the ME aspirates were positive in 28.6% and negative in 71.4% of cases. Nasopharyngeal swabs revealed 64.3% Gram positive and 35.7% negative organisms. Overall, there was no relationship between the ME and nasopharyngeal isolates amongst cases, with a P value of 0.000. However, there was a relationship amongst the isolate from the nasopharynx of cases and controls, with the exception of Klebsiella pneumoniae, at P < 0.009. CONCLUSION: There was no relationship amongst the bacterial isolate from the ME and nasopharyngeal specimen of patients with otitis media.

OBJECTIVES: The study intends to report the profiles of anaerobic bacteria isolated and attempts to evaluate the impact of immunocompromised status of patients on the disease. STUDY DESIGN: A cross-sectional prospective study. SETTING: ENT clinic, University of Ilorin Teaching Hospital, Nigeria; a 450-bed tertiary health facility. SUBJECTS: 104 consecutive consenting participants with clinical evidence of CSOM ≥12 weeks. METHODS: A hospital-based study conducted over a seven (7) month period. The middle ear aspirate was obtained with the aid of a micropipette and sent immediately for Microscopy, Culture and Sensitivity of both anaerobic and aerobic bacterial isolates. RESULTS: A total of 11 patients with chronically discharging ears, of the 104 studied had anaerobic bacteria cultured from their middle ear aspirates. Age ranged from 4 to 50 years with a male:female ratio of 1:1.8. A total of 32 isolates (11 anaerobic and 21 aerobic) were recovered. All anaerobic organisms were mixed and included both Peptostreptococcus and Bacteroides. Metronidazole had 100% sensitivity to gram negative and 62.5% to gram positive anaerobic organisms tested. Five (5) patients had immunocompromised states of 11 patients with anaerobic bacteria isolated in middle ear aspirates (group I), while 13 immunocompromised patients of the remaining 93 patients with no anaerobic bacteria isolated (group II). The proportion of anaerobic isolates in group I is higher (p < 0.05) than in group II. CONCLUSION: Immunocompromised state and age appear to play key role in presence of the anaerobic organisms recovered from the middle ear of the patients studied.

In this retrospective study, we determined the incidence, serovars, and antibiogram of Vibrio cholerae isolated from 102 clinical stool samples collected from rice water diarrheic patients during an outbreak (May - July 2017) in Ilorin metropolis, Nigeria. The culture positive rate of the V. cholerae isolates was 41.2%, with 41 and 1 isolates from O1 (Inaba) and non-O1/O139 serogroups, respectively. The isolates were the most susceptible to ciprofloxacin (76.2%) followed by amoxicillin-clavulanate (71.4%). However, all isolates were resistant to ampicillin and tetracycline. In conclusion, V. cholerae O1 was the predominant circulating serogroup exhibiting multi-drug resistance during the outbreak.

BACKGROUND: Drug-resistant tuberculosis (TB) is a significant public health problem. Greater than 90% of rifampicin (RIF)-resistant isolates were also isoniazid resistant, and hence, rifampicin resistance (RR) is frequently used as a surrogate for multidrug-resistant TB. SETTING: This study was conducted at Infectious Disease Hospital Kano in North-Western Nigeria. OBJECTIVES: The aim of this study was to determine the prevalence of RR among patients previously treated for pulmonary TB (PTB). MATERIALS AND METHODS: A total of 120 patients previously treated for PTB with current clinical features of PTB were recruited into this study. Relevant clinical information were obtained using a questionnaire. The sputum was collected and analyzed by the Gene Xpert MTB/RIF® machine to detect RR tuberculosis infection and blood screened for HIV infection. RESULTS: The mean (±standard deviation) age of the participants was 35.9 ± 14.3 years and they comprised 73 (60.8%) males and 47 (39.2%) females. HIV-seropositive rate was 11.7% among the participants. Of the 120 participants, PTB was detected in 35 (29.2%) of the participants by Gene Xpert MTB/RIF and 29 of them were cases of relapse. Five patients (4.2%) had RR tuberculosis and 80% of them were below the age of 45 years. CONCLUSION: The prevalence of RR is not high among previously treated PTB patients in this study when compared with other previous studies. This finding is a window for evaluating the efficacy of current interventions in the region and evidence for the consolidation of existing control policies.

BACKGROUND: Performance of Genotype MTBDRplus assay against Lowenstein Jensen (LJ) proportion method of Drug Susceptibility Testing (DST) in detection of resistance among clinical mycobacteria isolates to rifampicin (RMP) and isoniazid (INH) was evaluated in Ilorin, Nigeria. METHODS: This retrospective study characterized one hundred mycobacteria isolates from pulmonary TB patients, stored on LJ medium and subcultured unto fresh LJ slants before being genotyped using MTBDRplus assay. DST was performed on the isolates against RMP, INH, Ethambutol and Streptomycin. RESULTS: Genotype MTBDRplus identified 97% and 3% of the 100 isolates as Mycobacterium tuberculosis Complex (MTBC) and Non-Tuberculous Mycobacteria (NTM) respectively. Fourteen of the isolates, (14%) were resistant to RMP while 86% were sensitive by the genotypic method. Three of these 14 RMP-resistant isolates were NTMs while 11 were MTBC. Twelve (12%) of the 100 isolates were resistant to INH. Three INH-resistant isolates were NTMs, and 9 were MTBC. Phenotypically and genotypically, the 3 NTMs were resistant to RMP and INH and ten of the 97 MTBC strains were RMP-resistant. One RMP-phenotypically-sensitive strain was genotypically resistant to RMP. Six of the MTBC isolates were resistant to both RMP and INH by both methods. Most mutations occurred in the S-531L and S315T1 codons of rpoB and KatG genes of RMP and INH, respectively. CONCLUSION: The high specificity and positive predictive values recorded by MTBDRplus in our study make it suitable for use in the programmatic management of drug-resistant TB in resource-limited settings.

BACKGROUND: Mentally ill persons are vulnerable to sexually transmitted infections including hepatitis B and C because of their high level risky behaviors. This study is aimed at establishing the sero-prevalence of hepatitis B and C among the mentally ill individuals (MII) attending psychiatric clinic of the University of Ilorin Teaching Hospital (UITH), Nigeria since it has not been documented. METHODS: A total of 350 MII were recruited. HBsAg testing was by immunoassay test strip (Grand Medical Diagnostic R USA) while hepatitis C was tested by commercially prepared kits from ACON, R USA. Healthy adults who presented as donors in the blood bank of the hospital were used as controls. RESULTS: A total of 700 participants including 350 MII and 350 blood donors (BDs) were recruited for the study. The mean ages of MII and control participants were 36.5 ± 12.3 and 31.4 ± 8.3, respectively.The sero-prevalence of hepatitis B and C among patients with mental illness was 10.0 and 12.6%, respectively, as compared to 10.9% and 1.1% of the blood donors. There was a significant difference in the prevalence of HCV among mentally ill when compared with the blood donors (P = 0.001, χ2 = 33.97; OR (CI) =12.44 (5.33-29.03). CONCLUSION: Mentally ill patients attending UITH were significantly infected with hepatitis C virus. There is need for interventional measures to reduce the prevalence of hepatitis C among the mentally ill population such as health education and early screening of mentally ill in our setting.

BACKGROUND: Microsporidiasis, which is of great concern for immunocompromised patients, is poorly studied in developing countries. OBJECTIVES: A study was carried out amongst HIV-positive hospital patients and HIV-negative hospital controls in Ilorin, Nigeria, between January 2009 and July 2010 to determine the prevalence and intensity of Microsporidium spores and the complications associated with their presence. METHOD: Stool samples from 750 HIV-positive patients and 375 HIV-negative patients were studied using the Chromotrope-2R staining technique. Determination of CD4+ count was performed on the Partec Cyflow SL-3 CD4/8 instrument. Intensity of spores was determined by counting the total number of the spores in a 10 µl stained smear of stool. Images were captured with Phenix Microimage Analysis Software and data obtained were analysed using the Statistical Package for the Social Sciences. RESULTS: The prevalence of Microsporidium isolates amongst the HIV-positive hospital patients was significantly higher (42.4%) than amongst the HIV-negative controls (19.2%) (p < 0.05). The intensity of microsporidial spores amongst HIV-positive hospital patients was also significantly higher than amongst the controls (p < 0.05). However, the difference in the intensity of spores amongst HIV-positive patients who were on antiretroviral therapy (n = 411) and those who were not (n = 339) was not significant (p = 0.236). Microsporidiasis in HIV infection infection was common amongst patients with with low CD4+ counts, diarrhoea, body rashes and cough. CONCLUSION: Both the prevalence and intensity of Microsporidiasis are high amongst HIV-positive hospital patients; campaigns to promote awareness, prevention and control are required. Laboratory testing for microsporidia in HIV patients should be performed routinely so as to identify the organism for prompt medical attention.

OBJECTIVE: To determine the prevalence of asymptomatic bacteriuria, bacteriology and sensitivity pattern in Ilorin using the gold standard of urine culture. METHODS: A prospective study was carried out from 1st July to 31st October 2007, at the University of Ilorin Teaching Hospital (UITH) on 125 consenting asymptomatic pregnant women. A structured proforma was used to collect information from the women and a midstream urine specimen collected for bacteriological culture. RESULTS: Of the 125 pregnant women, 50 had bacteriuria on urine culture giving a prevalence of 40%. The mean age of the women was 28.5 years with a standard deviation of 4.95. The age ranged between 14 and 40 years. Staphylococcus aureus was the commonest pathogen isolated (72%), followed by Proteus spp (14%). Most of the organisms showed good sensitivity to Nitrofurantoin and gentamicin. CONCLUSION: The prevalence of asymptomatic bacteriuria in Ilorin is high and routine urine culture is advocated for all pregnant women at booking.
