RESUMEN

OBJECTIVE: This study was carried out to understand whether Q.Clear and ordered subset expectation maximization (OSEM), reconstruction algorithms used in fluorine-18-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) applications, and parameters such as time of flight (TOF) and point spread function (PSF) cause different results in semi-quantitative measurements. SUBJECTS AND METHODS: Raw PET data of 264 patients who were referred to 18F-FDG PET/CT imaging with the purpose of evaluation of known or suspicious malignant disease were reconstructed separately with Q.Clear (GE Healthcare), a BPL, an OSEM algorithm, PSF (SharpIR®) and TOF (VUE Point FX®) methods. Each patient's liver, mediastinal blood pool, metabolic tumor volume (MTV), total lesion glycolysis (TLG), and standardized uptake values (SUV) (SUVmax, SUVmean, and SUVpeak) of a total of 264 lesions selected from the patients were performed. RESULTS: ß350+ToF yielded higher measurement results than all other variables for all of the lesion SUVmax, lesion SUVmean, L/AP SUVmax, and L/AP SUVmean parameters. OSEM+ToF and OSEM+TOF+PSF algorithms yielded higher mean and median SUVmax values for the reference structures (liver and mediastinum) and for lesions SUVmax and SUVmean values were statistically significantly lower than the ß350+ToF method. The method with the lowest mean value for the L/Liver SUVmax variable was OSEM+ToF 4iter16ss (mean=1.76), while the method with the highest mean value was ß350+ToF (mean=2.26). ß350+ToF was the reconstruction method with the highest ratios for L/AP SUVmax and SUVmean for both lesions below and above 1 cm. ß350+ToF algorithm had also statistically significantly higher results for these variables compared to all other parameters in malignant lesions. CONCLUSION: When comparing 18F-FDG PET/CT images, the use of different reconstruction algorithms may lead to misleading results, especially in the evaluation of response to treatment of malignancies.

RESUMEN

A 61-year-old male patient, who had undergone radical prostatectomy, underwent 68Ga labeled prostate-specific membrane antigen (PSMA) positron emission tomography/computerized tomography (PET/CT) for evaluation of suspected biochemical recurrence of prostate cancer (PCa). PET/CT scan showed increased 68Ga PSMA expressions in hypodense mass lesions in both adrenal gland localizations. An adrenal gland tru-cut biopsy was performed for the right side, which showed poor-differentiated carcinoma metastases associated with the patient's high-grade PCa. As far as we could determine based on an extensive literature search, this is the second case in which isolated adrenal metastasis was detected by 68Ga PSMA PET/CT study in a patient with PCa.

RESUMEN

Meningiomas constitute 37% of primary central nervous system tumors and are more common in women. Also may occur with other primary malignancies, which can cause confusion with the metastasis in whole body bone scan (WBBS) imaging. A 58-year-old woman diagnosed with breast cancer was referred to the WBBS for the investigation of possible bone metastases. In the planar images, radiotracer uptake at multiple sites was detected on the anterior side of the skull base and the posterior side of the vertex of the cranium. Single photon emission computed tomography/computed tomography was performed for anatomical localization of possible metastatic lesions, and it revealed that detected accumulations of radiotracer did not belong to the bone metastases; uptakes were located at the cerebral parenchyma and the lesions in the falx cerebri. Patient history explained that she had been diagnosed with meningioma five years ago, which mimicked bone metastases in this study.

PURPOSE: To evaluate whether metabolic and volumetric data from 68 Ga-PSMA PET/CT performed during staging of de-novo high-volume mCSPC patients who received docetaxel could be used to predict survival. METHODS: Forty-two de-novo high-volume mCSPC patients, who received ADT + Docetaxel and underwent 68 Ga-PSMA PET/CT for staging, were included in the study. The association between patients' pathological data, all PSA measurements, treatments they received, the data obtained from 68 Ga-PSMA PET/CT and progression-free and overall survival were examined. RESULTS: In the multivariate analysis, PSMA-TV (primary) and PSMA-TV (WB) variables were shown to be independent negative predictors of overall survival. For the threshold value of 19.91 cm 3 obtained for PSMA-TV (primary), HR was calculated as 6.31, the 95% confidence interval (CI): 1.01-39.18, P = 0.048. For the threshold value of 1226.5 cm 3 obtained for PSMA-TV (WB) variable, HR was calculated as 58.62, the 95% CI: 2.55-1344.43, P = 0.011. In our study, SUVmax (WB) variable was found to be an independent and negative predictor of progression-free survival. For the determined threshold value of 17.74, HR was calculated as 16.24, 95% CI: 1.18-22.76, P = 0.037. CONCLUSION: Metabolic and volumetric data obtained from 68 Ga-PSMA PET/CT can be used to predict survival in de-novo high-volume mCSPC. Our results show that in ADT + Docetaxel receiving patients, a subgroup with higher PSMA-TV (WB) values have a significantly worse prognosis. This situation suggests that the high-volume disease definition in the literature may be insufficient for this group, and that 68 Ga-PSMA PET/CT can play an essential role in demonstrating the heterogeneity within the group.

Objectives: The aim of this study was to compare the treatment responses after ablation with 30-50 mCi radioactive iodine (RAI) and 100 mCi RAI in patients with differentiated thyroid cancer (DTC) who were in the low-risk group according to 2015 American Thyroid Associations Classification (ATA 2015) criteria. Methods: Between February 2016 and August 2018, 100 patients who received RAI treatment in our clinic after total thyroidectomy and who were in the low-risk group DTC were included in this retrospective study. These patients were divided into 2 groups: low-activity (30-50 mCi) (group 1) and high-activity (100 mCi) (group 2). While 54 patients were treated with low activity, 46 patients received high activity RAI. The 2 groups were compared according to the 1st- and 3rd-year treatment response status. Results: According to the first-year follow-up, 15 patients were accepted as indeterminate response and 85 patients as excellent response. Three (5.5%) of the patients who were accepted as indeterminate response were in group 1 and 12 (26%) were in group 2. According to the third year follow-up, 1 patient in group 1 and 3 patients in group 2 were accepted as indeterminate response. No biochemical incomplete response or recurrent disease was detected. In the chi-square analysis performed to investigate the relationship between the first-year treatment response and RAI activities, a significant relationship was found (p=0.004). In the Mann-Whitney U test performed to investigate the parameters that may be effective in the treatment response, only the preablative serum thyroglobulin value was shown to have a significant difference between the two groups (p=0.01). In the long-term follow-up of the patients, based on the third year treatment response data, chi-square analysis was performed to evaluate the two groups in terms of treatment responses, and no statistically significant relationship was found (p=0.73). Conclusion: Ablation with 30-50 mCi can be safely applied in DTC patients who are in the ATA 2015 low-risk group and are planned for RAI ablation treatment.

Objectives: The present study aimed to identify the prevalence of focal uptake in the colon on 18fluorine-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) studies performed for the evaluation of malignancies other than colon, to detect the rate of malignancy in incidental focal 18F-FDG avid colonic lesions and to investigate if any possible role of maximum standardized uptake value (SUVmax) values in the discrimination of malignant lesions from premalignant and benign ones exist. Methods: We retrospectively reviewed the files of 8,017 patients with known or suspected malignancy, who underwent whole-body 18F-FDG PET/CT at our institution during the period November 2017 to November 2019. Patients showing a single site of focally increased colonic 18F-FDG uptake that was more intense compared to liver uptake on 18F-FDG PET studies and referred to colonoscopy were enrolled in the study. Results: Fifty two patients (83.8%) had at least 1 corresponding lesion on colonoscopy, whereas in 10 patients no lesion was detected. Subsequent histopathological examinations revealed no corresponding lesion in 13 (13.7%), a benign lesion in 18 (18.9%), hyperplastic polyp in 10 (10.5%), low-grade polyp in 16 (16.8%), high-grade polyp in 29 (30.5%) and malignant lesion in 9 (9.5%) of the focal 18F-FDG uptake sites. According to histopathology results, statistically no significant difference was found between the SUVmax measurements of malignant and benign cases (p>0.05) but the average SUVmax measurements of malignant cases were found to be significantly higher than lower + high-grade cases (p<0.05) and hyperplastic polyp cases (p<0.01). Conclusion: In conclusion, any unexpected focal 18F-FDG uptake in 18F-FDG PET/CT studies is suspicious for malignancy and should be clarified by colonoscopy. The intensity of 18F-FDG uptake does not preclude the application of colonoscopy and histopathological verification of the lesion if there is any.

PURPOSE: This study was done to determine whether interruption of metformin before (18)F-FDG PET/CT imaging could prevent the increased (18)F-FDG uptake in the intestine caused by this drug. METHODS: Included in the study were 41 patients with known type 2 diabetes mellitus who were referred to our department for evaluation of various neoplastic diseases. Patients underwent two (18)F-FDG PET/CT scans, the first while they were on metformin and the second after they had stopped metformin. They stopped metformin and did not take any other oral antidiabetic medication starting 3 days before the second study and their blood glucose level was regulated with insulin when necessary to keep it within the range 5.55-8.33 mmol/l. FDG uptake was graded visually according to a four-point scale and semiquantitatively by recording the maximum standardized uptake value (SUVmax) in different bowel segments. A paired-samples t-test method was used to determine whether there was a significant difference between SUVmax measurements and visual analysis scores of the metabolic activity of the bowel in the PET/CT scans before and after stopping metformin. RESULTS: Diffuse and intense (18)F-FDG uptake was observed in bowel segments of patients, and the activity in the colon was significantly decreased both visually and semiquantitatively in PET/CT scans performed after patients stopped metformin (p<0.05). There was a statistically significant decrease in activity in the small intestine on visual analysis (p<0.05), but semiquantitative measurements did not show a significant decrease in the SUVmax values in the duodenum or jejunum (p>0.05). CONCLUSION: Metformin causes an increase in (18)F-FDG uptake in the bowel and stopping metformin before PET/CT study significantly decreased this unwanted uptake, especially in the colon, facilitating the interpretation of images obtained from the abdomen and preventing the obliteration of lesions.

This study was performed because the efficacy of technetium-99m methoxyisobutyl isonitrile scintimammography ((99m)Tc-MIBI-SM) in the evaluation of the breast lesions and axillary lymph node involvement in comparison with X-rays mammography (XRM), ultrasonography (US) and magnetic resonance imaging (MRI) has not been fully investigated. Forty six female patients were included in this study, with suspicious lesions detected in their breasts by palpation, by imaging modalities or clinically. All patients underwent (99m)Tc-MIBI-SM, US and MRI for the evaluation of breast lesions. All patients according to clinical situation and imaging studies underwent fine needle aspiration, mass extirpation, core biopsy, modified radical mastectomy or partial lumpectomy in order to confirm the nature of the lesions. Our results showed that (99m)Tc-MIBI-SM detected 15 of 16, US 11 of 16, XRM 13 of 16 and MRI 13 of 16 malignant lesions. Sensitivities were 93%, 68%, 81% and 81%, respectively. Among these cases there were 4 of 15 false positive (FP) results on SM whereas 4 of 11 on US, 11 of 13 on XRM and 8 of 13 on MRI. The specificities of the above modalities were 86%, 87%, 63% and 73%, respectively. The sensitivities of the above imaging modalities for the detection of axillary lymph node metastases were 55%, 55%, 11% and 77% for SM, US, XRM and MRI, respectively. The sensitivity and specificity of SM for the palpable lesions (n=31) were 100% and 84% and for the nonpalpable lesions (n=15) were 75% and 90%, respectively. In conclusion, although the number of patients studied was small it is the opinion of the authors that (99m)Tc-MIBI-SM has a much better sensitivity and less FP results in detecting malignant breast lesions than the other 3 modalities and also better sensitivity than XRM and MRI. Thus (99m)Tc-MIBI-SM can be included in the diagnostic algorithms for detecting malignancy in breast tumors.

RESUMEN

A 54-year-old woman underwent octreotide scintigraphy for evaluation of neuroendocrine tumor recurrence. The images demonstrated unusual uptake in gallbladder area in addition to physiologic uptake at other tissues. Whole-body planar and SPECT imaging were repeated after fatty meal ingestion at 28 hours in order to figure out whether this activity was physiologic or not. Since the unusual uptake in the gallbladder was still detected at these images, additional images were obtained 72 hours after radionuclide injection. The activity in the gallbladder disappeared at these images revealing the physiologic nature of this unusual accumulation.

Adrenal schwannoma is an extremely uncommon cause of incidentaloma. It originates from neural sheath Schwann cells of the adrenal gland. We report the case of a left adrenal schwannoma incidentally discovered in a 32-year-old woman during examination of bloated feeling and stomach ache. The patient was incidentally found to have a left adrenal mass of 9 cm on abdominal ultrasonography. Computed tomography (CT) of the abdomen and [(18)F] fluorodeoxyglucose positron emission tomography (PET) were also performed. Metabolic evaluation was unremarkable. Due to the large size of the tumor, left adrenalectomy was performed. The postoperative course was uneventful. Histological examination established the diagnosis of schwannoma. This diagnosis was supported by immunohistochemistry of S-100 and vimentin positivity. In conclusion, adrenal schwannoma is an extremely rare entity and can grow considerably in size. The present case report emphasizes that clinicians should be aware of the possibility of retroperitoneal schwannoma. Total excision of benign schwannoma is associated with a favorable outcome. To our knowledge, there are case reports of schwannoma with CT and magnetic resonance imaging findings in the literature, although this is the first schwannoma case with PET-CT imaging.

PURPOSE: We aimed to investigate whether administration of benzodiazepines decreases the efficacy of fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography PET/CT) ((18)F-FDG-PET/CT) dual-phase imaging. MATERIALS AND METHODS: Eighteen patients with malignant tumors who were administered 0.5 mg alprazolam before undergoing (18)F-FDG-PET/CT scan (group A) and 21 patients with malignant tumors who were not administered alprazolam before (18)F-FDG-PET/CT scan (group B) were included in this study. Forty lesions from the 18 patients in group A and 66 lesions from the 21 patients in group B were evaluated. Initial "early" whole-body imaging commenced 60 ± 5 minutes after injection of (18)F-FDG and delayed scan was obtained 120 ± 10 minutes after the injection. Maximum standardized uptake values (SUVs) were obtained by drawing three-dimensional regions of interest (ROIs) around each lesion on the early study and the corresponding lesion on the delayed study. RESULTS: The average SUVmax in lesions in group A (mean ± S.D.) was 10.2 ± 6.4 on early examination (SUVmax E) and 12.6 ± 7.6 on delayed examination (SUVmax D). There was a significant difference between these two time points (P < 0.05). Similarly, for the lesions in group B, the average uptake values were 9.3 ± 5.2 (SUVmax E) and 11.2 ± 6.5 (SUVmax D). The increase in these values was significant as it was in group A (P < 0.05). Differences between groups A and B for the variables SUVmax E, SUVmax D were not significant statistically (P > 0.05). CONCLUSION: Benzodiazepines do not adversely affect the efficacy of the dual-phase FDG-PET imaging technique.

UNLABELLED: A 55-year-old patient, who had undergone excisional biopsy of upper lip two years ago and diagnosed to have squamous cell carcinoma, was referred to us for evaluation with Fluorine-18 fluorodeoxyglucose (F-18 FDG) positron emission tomography (PET)/CT (F-18 FDG-PET/CT) scan. F-18 FDG-PET/CT scan was performed and the maximum intensity projection images (MIP) showed unusual FDG uptake at both ears. Histopathological examination of the biopsy specimen obtained from the ears revealed discoid lupus erythematosus (DLE). CONFLICT OF INTEREST: None declared.

In this study, we investigated the efficacy of 99mTc-DTPA scintigraphic analysis of GFR with the Gates method in comparison with the measurement of plasma urea and creatinine, in the detection of nephrotoxicity occurred in patients treated with cisplatin.Twenty-six male patients with a mean age of 26.73±6.39 years (age range 15-42) who had seminomatous and nonseminomatous testicular carcinoma were included in our study. The patients received cisplatin with a dose of 20 mg/m2 per day for five consecutive days repeated every 21 days. Before starting chemotherapy, immediately after the end of four cycles of chemotherapy and 7 months after the beginning of chemotherapy, plasma urea and creatinine levels were measured and simultaneously scintigraphic GFR estimation using 99 mTc-DTPA with the Gates method was performed. In the measurements done immediately after the chemotherapy, in 18 of the 26 patients GFR levels decreased, in 4 of the 8 remaining patients GFR did not change, and in 4 patients there was an increase in the GFR levels. The changes in the averages of the plasma urea and creatinine levels between measurements done before and after the chemotherapy were not statistically significant. The decrease in the average of the GFR values immediately after chemotherapy, in comparison to the average of GFR values measured before chemotherapy, was found to be statistically significant with paired sample t test analysis (P<0.009 with 95% CI). We concluded that scintigraphic GFR measurement using the Gates method with 99mTc-DTPA is a suitable method in the diagnosis of nephrotoxicity occuring due to cisplatine.

PURPOSE: We evaluated the efficacy of fluorine-18 fluorodeoxyglucose (F-18 FDG) positron emission tomography (PET)/computerized tomography (CT; F-18 FDG-PET/CT) in the detection of renal cell carcinoma (RCC) in patients with indeterminate renal masses. MATERIALS AND METHODS: Between December 2008 and June 2010, 18 patients with suspicious primary renal masses detected by conventional imaging underwent FDG PET/CT imaging. All patients underwent nephrectomy or surgical resection of the renal mass and the final diagnoses were based on histopathology. RESULTS: Fifteen patients had RCC (14 clear-cell RCC, one papillary RCC). Three renal tumors were benign, corresponding to two renal cortical cysts and one oncocytoma. FDG PET/CT accurately detected seven malignant lesions and yielded false-negative results in eight patients. FDG PET/CT was true negative in two patients with a renal cortical cyst and false positive in a patient with oncocytoma. PET showed a sensitivity of 46.6%, specificity of 66.6%, and accuracy of 50% for primary RCC tumors. The median size of visualized tumors was greater than the median size of nonvisualized tumors, and the average Fuhrman grade of the patients with FDG-positive malignant lesions were higher than that of the patients with FDG-negative lesions. In malignant tumors, the change between early and delayed imaging for average standardized uptake values and maximum SUVs were not statistically significant. CONCLUSION: FDG PET/CT is not a reliable modality in the diagnosis of RCC with its low sensitivity, but it is effective in the detection of distant metastases and can be used as a complementary tool when conventional imaging studies yield equivocal results.

RESUMEN

UNLABELLED: A 64-year-old male patient with small cell lung cancer underwent Fluorine-18 fluorodeoxyglucose (F 18 FDG) positron emission tomography (PET)/CT scan which revealed multiple F 18 FDG uptake in the spine, both humeri, ribs, pelvis and proximal long bones. There was no obvious lytic or sclerotic bone destruction accompanying these lesions on CT component of the study. After the patient received six courses of chemotherapy a repeat F 18 FDG-PET/CT was performed for evaluation of therapy response. The PET/CT showed the presence of multiple sclerotic lesions on CT without FDG uptake, corresponding to the bone lesions on the previous PET/CT scan. A concomitant Tc 99m Methylene diphosphonate (Tc 99m MDP) bone scintigraphy (BS) revealed no pathologically increased Tc 99m MDP uptake in the skeletal system. The FDG avid lesions in the skeletal system, which were not sclerotic initially, were transformed into FDG non-avid sclerotic lesions after chemotherapy. This was attributed to the direct effect of previous successful therapy for bone metastases, leading to the transformation of metabolically active disease, into blastic metabolically inactive metastases. In conclusion, a F 18 FDG negative bone lesion, which is sclerotic on CT, may represent post-treatment osteoblastic change rather than active tumor and BS might play a role in the discrimination of these two situations. RESUMEN

Yersiniae enterocolitica, a gram negative rod-like organism, causes terminal ileitis and mesenteric adenitis in adolescents and adults. Some forms present with liver and spleen abscesses and have worse prognosis. We report a type 1 diabetic patient with a liver abscess mimicking metastatic liver disease who was successfully treated with percutaneous drainage and antibiotic administration; culture from blood was positive for Yersinia enterocolitica, but drainage material from the liver abscess did not yield a positive result for Yersinia enterocolitica. Although the prognosis is not good in such cases, with high mortality rates, our patient recovered from the disease with appropriate treatment.

RESUMEN

A patient who had undergone left radical nephrectomy 11 years ago for renal cell carcinoma (RCC) was referred to our clinic for restaging. Fluorine-18 fluorodeoxyglucose (F18-FDG) positron emission tomography/computed tomography (F18-FDG PET/CT) showed hypometabolic area in left frontal region of the brain and increased FDG uptake in the subcutaneous fatty tissues of the right thigh. Histopathological examination of the biopsy material from the left frontal region and right gluteal region revealed metastasis of clear cell type RCC. Seven months later, a magnetic resonance imaging (MRI) of right cruris showed a contrast-enhancing lesion with a diameter of 3.5 cm, located at the subcutaneous area of posterior part of right cruris. A concomitant F18-FDG PET/CT detected an increased FDG uptake focus in the proximal third of right cruris adjacent to the muscle planes and this finding was consistent with metastasis of RCC.

OBJECTIVE: We aimed to assess the role of Magnetic Resonance Imaging (MRI) and X-Ray in the evaluation of response to radiosynovectomy (RS) in patients with hemophilic arthropathy. MATERIAL AND METHODS: Eleven patients who suffered from hemophilic arthropathy with a mean age of 11.7 (range between 7-15) were included in this study. 148-185 MBq Yttrium 90 silicate (Y-90) was administered intraarticularly to ten knee joints and one patient was treated with intraarticular 74 MBq Rhenium 186 (Re-186) injection into his ankle. Before radiosynovectomy, plain anteroposterior and lateral X-rays of the target joints were obtained by standard technique. The follow-up MRI and X-ray studies of the patients were done 6 months after RS. Pettersson hemophilic arthropathy scales were utilized to stage the condition of the joints on plain X-ray and classification of the investigated joints on MRI were done according to Denver score. The clinical assessment of the efficacy of the RS was made with the comparison of the average bleedings before and after the intervention. RESULTS: During the 6-month follow-up period after RS, an improvement in number of hemarthrosis 75% or greater compared with the prior six months occurred in six joints (54.5%). The Pettersson scores worsened in 1/11 (9%), remained unchanged in 9/11 (81.8%), and improved in 1/11 (9%) joints. At the 6-month follow-up, the MRI score worsened in one (9%) and was unchanged in 10/11 joints (90.9%). CONCLUSION: MRI is a more sensitive tool than plain radiography for evaluating and follow-up of joint disease in persons with hemophilia, but both methods don't show correlation with the therapeutic response Conflict of interest:None declared.
