RESUMEN

A case of pancreatic mucinous cystadenocarcinoma (PMC) with two pseudosarcomatous mural nodules (PMN) is described. These nodules have not been previously described in this type of tumor. In ovarian mucinous tumors (OMT), similar nodules have been reported, the nature of which has been discussed in detail. Here the similarity between the tumor described here and ovarian tumors is stressed. The immunohistochemical study carried out disclosed in the nodules strong positive staining for vimentin and moderate positivity for keratin and epithelial membrane antigen. These findings, along with histologic details, favor the epithelial nature of the nodules. It was concluded that the nodules are foci of anaplastic carcinoma with high proliferative cell rate, which could explain the coexpression of vimentin and keratin.

RESUMEN

We present the clinical and pathologic features of gallbladder carcinomas in a review of surgical and autopsy cases from the Hospital "Juan Canalejo", La Coruña. Out of 713 cholecystectomies performed in the last 30 months we found 13 primary (frequency 1.8%) and 3 metastatic tumors. All primary tumors were carcinomas, most of them well differentiated. The age ranged from 60 to 91 years (median of 73.6) and the female to male ratio was 2.2 to 1. The clinical diagnosis of carcinoma previous to the laparotomy was established only in one case. Cholelithiasis was associated in 11 cases. The staging of the tumors varied considerably, but it was rather advanced. In the review of 1,617 adult autopsies done in the last 14 years, 11 cases had a primary gallbladder carcinoma (frequency 0.61%). The median age was 70.4 years with extremes from 56 to 84. Female to male ratio was 1.2:1 and carcinomatosis, usually from unknown primary, was the form of presentation. Only in two cases the gallbladder had been suspected as the primary site of the tumor. Cholelithiasis was present in 6 cases. Both groups are compared in their clinical and pathologic features.
