OBJECTIVES: Fibula free flaps (FFF) are the standard approach to mandibular reconstruction after partial resection, with the goal of restoring aesthetics and masticatory function. The graft position affects both and must be carefully selected. Correlations between sagittal positioning and upper airway anatomy are known from orthognathic surgery. This study aims to evaluate changes in mandibular position and upper airway anatomy after reconstructive surgery with FFF and corresponding correlations. MATERIALS AND METHODS: Mandibular position after reconstruction was evaluated using three-dimensional datasets of pre- and postoperative computed tomography scans of patients treated between 2020 and 2022. Three-dimensional measurements were performed on both condyles and the symphyseal region. Changes in upper airway volume and minimum cross-sectional area (minCSA) were analysed. Intra-rater reliability was assessed. Correlations between changes in upper airway anatomy and sagittal mandibular position were tested. RESULTS: The analysis included 35 patients. Intra-rater reliability was good to excellent. Condylar deviations and rotations were mostly rated as small. Changes in symphyseal position were considerably greater. Median airway volume decreased in the oropharynx and hypopharynx. Posterior deviation of the symphysis was associated with a decreasing minCSA in the hypopharynx and vice versa. CONCLUSIONS: The overall accuracy of mandibular reconstructions with FFF is high, but there is room for optimization. The focus of research should be extended from masticatory to respiratory rehabilitation. CLINICAL RELEVANCE: Effects on respiratory function should be considered prior to graft positioning. The clinical relevance of upper airway changes within the complex rehabilitation of reconstructive surgery patients needs to be further investigated.

BACKGROUND: Unilateral cleft lip surgery is a complex procedure, and the outcome depends highly on the surgeon's experience. Digital simulations and low-fidelity models seem inadequate for effective surgical education and training. There are only few realistic models for haptic simulation of cleft surgery, which are all based on synthetic materials that are costly and complex to produce. Hence, they are not fully available to train and educate surgical trainees. This study aims to develop an inexpensive, widely available, high-fidelity, ex vivo model of a unilateral cleft lip using a porcine snout disc. METHODS: A foil template was manufactured combining anatomical landmarks of the porcine snout disc and the anatomical situation of a child with a unilateral cleft. This template was used to create an ex vivo model of a unilateral cleft lip from the snout disc. Millard II technique was applied on the model to proof its suitability. The individual steps of the surgical cleft closure were photo-documented and three-dimensional scans of the model were analysed digitally. Sixteen surgical trainees were instructed to create a unilateral cleft model and perform a unilateral lip plasty. Their self-assessment was evaluated by means of a questionnaire. RESULTS: The porcine snout disc proved highly suitable to serve as a simulation model for unilateral cleft lip surgery. Millard II technique was successfully performed as we were able to perform all steps of unilateral cleft surgery, including muscle suturing. The developed foil-template is reusable on any porcine snout disc. The creation of the ex vivo model is simple and inexpensive. Self-assessment of the participants showed a strong increase in comprehension and an eagerness to use the model for surgical training. CONCLUSIONS: A porcine snout disc ex vivo model of unilateral cleft lips was developed successfully. It shows many advantages, including a haptic close to human tissue, multiple layers, low cost, and wide and rapid availability. It is therefore very suitable for teaching and training beginners in cleft surgery and subsequently improving surgical skills and knowledge. Further research is needed to finally assess the ex vivo model's value in different stages of the curriculum of surgical residency.

BACKGROUND: Bilateral cleft lip surgery is very challenging and requires a high level of skill, knowledge and experience. Existing high-fidelity simulation models that can be used by novice cleft surgeons to gain experience and expand their knowledge are rare and expensive. In this study, we developed a bilateral cleft lip model using porcine snout discs, which are available anywhere and inexpensive. METHODS: Anatomic reference points of a patient with a bilateral cleft lip were superimposed with landmarks of the porcine snout disc on a foil template. The template was used to construct an ex vivo bilateral cleft lip model. Surgery was performed on the model according to Millard and the surgical steps were photodocumented analogous to two clinical cases of bilateral cleft lip surgery. The suitability of the model was further tested by twelve participants and evaluated using self-assessment questionnaires. RESULTS: The bilateral cleft lip ex vivo model made of a porcine snout disc proved to be a suitable model with very low cost and ease of fabrication, as the template is reusable on any snout disc. The Millard procedure was successfully performed and the surgical steps of the lip plasty were simulated close to the clinical situation. Regarding the nasal reconstruction, the model lacks three-dimensionality. As a training model, it enhanced the participants comprehension of cleft surgery as well as their surgical skills. All participants rated the model as valuable for teaching and training. CONCLUSIONS: The porcine snout discs can be used as a useful ex vivo model for bilateral cleft lip surgery with limitations in the construction of the nose, which cannot be realistically performed with the model due to anatomical differences with humans. Benefits include a realistic tissue feel, the simulation of a multi-layered lip construction, a wide and rapid availability and low cost. This allows the model to be used by novice surgeons also in low-income countries. It is therefore useful as a training model for gaining experience, but also as a model for refining, testing and evaluating surgical techniques for bilateral lip plasty.

BACKGROUND: African swine fever (ASF) has been present in Lithuania since 2014. The disease affects mainly the wild boar population. Thus, hunters play a key role in the performance of disease surveillance and control measures. We used participatory methods to gain insight into the knowledge of hunters and to include their perceptions in the design and the implementation of surveillance and control measures to increase their effectiveness. RESULTS: The willingness and the interest of hunters to participate was high, but only eight focus group meetings with 33 hunters could be held due to the COVID-19 pandemic. The overall knowledge of Lithuanian hunters regarding ASF, investigated by semi-structured interviews, was sufficient to understand their part in ASF control and surveillance. However, their knowledge did not necessarily lead to an increased acceptance of some ASF control measures, like the targeted hunting of female wild boar. Participating hunters showed a good understanding of the processes of the surveillance system. Their trust in the performance within this system was highest towards the hunters themselves, thus emphasizing the importance of acknowledging their role in the system. Hunters refused measures including the reduction of hunting activities. They feared a complete elimination of the wild boar population, which in turn demonstrates the necessity to increase professional information exchange. CONCLUSIONS: The perceptions of Lithuanian hunters regarding ASF surveillance and control in wild boar resembled those obtained in neighboring countries. It is imperative to communicate the results with decision-makers, to consider the views of hunters, when designing or adapting measures to control ASF in wild boar and to communicate with hunters on these measures and their justification.

The first cases of African swine fever (ASF) were detected in the Lithuanian wild boar population in 2014. Since then, the disease spread slowly through the whole country, affecting both, wild boar and domestic pigs. In the other Baltic states, which both are also affected by ASF since 2014, the recent course of ASF prevalence suggests that the countries might be well under way of disease elimination. In contrast, in Lithuania the epidemic seems to be still in full progress. In the present study, we aimed to extend a previous prevalence study in Lithuania. Looking at ASF virus (ASFV) and seroprevalence estimates of wild boar in all months of 2018 and in all affected municipalities in Lithuania, the course of ASF was evaluated on a temporal and spatial scale. A non-spatial beta-binomial model was used to correct for under- or overestimation of the average prevalence estimates. Within 2018 no big differences between the prevalence estimates were seen over time. Despite of the lower sample size, highest ASFV prevalence estimates were found in dead wild boar, suggesting higher detection rates through passive surveillance than through active surveillance. Accordingly, with the maximum prevalence of 87.5% in May 2018, the ASFV prevalence estimates were very high in wild boar found dead. The number of samples originating from hunted animals (active surveillance) predominated clearly. However, the ASFV prevalence in those animals was lower with a maximum value of 2.1%, emphasizing the high value of passive surveillance. A slight increase of the seroprevalence in hunted wild boar could be seen over time. In the center of Lithuania, a cluster of municipalities with high ASFV and seroprevalence estimates was found. The results of the study indicate that ASFV is still circulating within the Lithuanian wild boar population, constituting a permanent risk of disease transmission into domestic pig holdings. However, additional, more recent data analyses are necessary to re-evaluate the course of ASF in Lithuania and thus, to be able to make a statement about the stage of the ASF epidemic in the country. This is of huge importance for Lithuania for evaluating control measures and their efficacy, but also for neighbouring countries to assess the risk of disease spread from Lithuania.

For the global pig industry, classical (CSF) and African swine fever (ASF) outbreaks are a constantly feared threat. Except for Sardinia, ASF was eradicated in Europe in the late 1990s, which led to a research focus on CSF because this disease continued to be present. However, ASF remerged in eastern Europe in 2007 and the interest in the disease, its control and epidemiology increased tremendously. The similar names and the same susceptible species suggest a similarity of the two viral diseases, a related biological behaviour and, correspondingly, similar epidemiological features. However, there are several essential differences between both diseases, which need to be considered for the design of control or preventive measures. In the present review, we aimed to collate differences and similarities of the two diseases that impact epidemiology and thus the necessary control actions. Our objective was to discuss critically, if and to which extent the current knowledge can be transferred from one disease to the other and where new findings should lead to a critical review of measures relating to the prevention, control and surveillance of ASF and CSF. Another intention was to identify research gaps, which need to be closed to increase the chances of a successful eradication of ASF and therefore for a decrease of the economic threat for pig holdings and the international trade.

UNLABELLED: The association of organisms to their environments is a key issue in exploring biodiversity patterns. This knowledge has traditionally been scattered, but textual descriptions of taxa and their habitats are now being consolidated in centralized resources. However, structured annotations are needed to facilitate large-scale analyses. Therefore, we developed ENVIRONMENTS, a fast dictionary-based tagger capable of identifying Environment Ontology (ENVO) terms in text. We evaluate the accuracy of the tagger on a new manually curated corpus of 600 Encyclopedia of Life (EOL) species pages. We use the tagger to associate taxa with environments by tagging EOL text content monthly, and integrate the results into the EOL to disseminate them to a broad audience of users. BACKGROUND: Surveillance measures can only be effective if key players in the system accept them. Acceptability, which describes the willingness of persons to contribute, is often analyzed using participatory methods. Participatory epidemiology enables the active involvement of key players in the assessment of epidemiological issues. In the present study, we used a participatory method recently developed by CIRAD (Centre de Coopération Internationale en Recherche Agronomique pour le Développement) to evaluate the functionality and acceptability of Classical Swine Fever (CSF) surveillance in wild boar in Germany, which is highly dependent on the participation of hunters. The acceptability of alternative surveillance strategies was also analyzed. By conducting focus group discussions, potential vulnerabilities in the system were detected and feasible alternative surveillance strategies identified. RESULTS: Trust in the current surveillance system is high, whereas the acceptability of the operation of the system is medium. Analysis of the acceptability of alternative surveillance strategies showed how risk-based surveillance approaches can be combined to develop strategies that have sufficient support and functionality. Furthermore, some surveillance strategies were clearly rejected by the hunters. Thus, the implementation of such strategies may be difficult. CONCLUSIONS: Participatory methods can be used to evaluate the functionality and acceptability of existing surveillance plans for CSF among hunters and to optimize plans regarding their chances of successful implementation.

African swine fever (ASF) has posed a significant threat to Ukrainian pig farming since its identification in 2012. In this study, recognising the pivotal role of pigkeepers in disease control, we conducted ten focus groups involving 52 smallholders across eight regions in Ukraine. Using participatory methods, we revealed their awareness of ASF signs, transmission routes, preventive measures, and the perceptions of stakeholders involved in ASF control. Furthermore, we identified the smallholders' acceptance of eradication and restriction measures, the perceived impact of zoning consequences, and their main sources of ASF information. Smallholders identified fever and skin haemorrhage as the most indicative signs of ASF and highlighted rodents as a primary transmission concern. Disinfection was seen as the most effective measure for preventing the introduction of ASF. Pigkeepers who perceived their stakeholder role in ASF control showed more trust in themselves and veterinarians than in central veterinary authorities. Farm-level ASF eradication measures were generally accepted; however, culling within the protection zone was least accepted, with economic losses listed as the most impactful consequence for pigkeepers. For ASF information, pigkeepers favour web searches and veterinarians, as well as traditional media and word-of-mouth news. This study provides valuable insights into refining the ASF communication strategies in Ukraine.

The implementation of management activities depends on both the attitude of the people performing the work and their understanding of why the work should be performed. In the context of animal husbandry, the implementation of such practices is crucial for the functionality of biosecurity. Therefore, it is important to know how farmers perceive biosecurity as a whole. An anonymous online survey was conducted among German farmers. In addition to general data about their farm, information about their existing concept of biosecurity, as well as about the assessment of possible introduction routes for animal diseases into the farm with regard to their likelihood, was gathered. Furthermore, information on measures to protect their farm against disease introduction were retrieved. Analysis showed that in general, farmers were aware of the importance of biosecurity and consequently had concepts of biosecurity on their farms. However, awareness about dangerous introduction routes for animal diseases into a farm was associated with a lack of knowledge of how to improve the measures in these areas. The role of the veterinarian in the context of biosecurity was highlighted and further problematic areas were indicated. Overall, the high level of commitment from farmers indicated a good implementation of daily practices.

The implementation of CAD software in the digital production of implant prosthetics stands as a pivotal aspect of clinical dentistry, necessitating high precision in the alignment of implant scanbodies. This study investigates the influence of scanbody geometry and the method of superimposing in CAD software when determining 3D implant position. A standardized titanium model with three bone-level implants was digitized to create reference STL files, and 10 intraoral scans were performed on Medentika and NT-Trading scanbodies. To determine implant position, the generated STL files were imported into the Exocad CAD software and superimposed-automatically and manually-with the scanbody geometries stored within the software's shape library. Position accuracy was determined by a comparison of the 3D-defined scanbody points from the STL matching files with those from the reference STL files. The R statistical software was used for the evaluation of the data. In addition, mixed linear models and a significance level of 0.05 were applied to calculate the p-values. The manual overlay method was significantly more accurate than the automatic overlays for both scanbody types. The Medentika scanbodies showed slightly superior precision compared to the NT-Trading scanbodies. Both scanbody geometry and the type of alignment in the CAD software significantly affect digital workflow accuracy. Manual verification and adjustment of the automatic alignment process are essential for precise implant positioning.

Targeted search for wild boar carcasses is essential for successful control of African swine fever (ASF) in wild boar populations. To examine whether landscape conditions influence the probability of finding ASF-positive carcasses, this study analyzed Global Positioning System (GPS) coordinates of Latvian wild boar carcasses and hunted wild boar, extracted from the CSF/ASF wild boar surveillance database of the European Union, and random coordinates in Latvia. Geographic information system (GIS) software was used to determine the landscape type and landscape composition of carcass detection sites and to measure distances from the carcasses to nearest waterbodies, forest edges, roads and settlements. The results of the automated measurements were validated by manually analyzing a smaller sample. Wild boar carcasses were found predominantly in forested areas and closer to waterbodies and forest edges than random GPS coordinates in Latvia. Carcasses of ASF-infected wild boar were found more frequently in transitional zones between forest and woodland shrub, and at greater distances from roads and settlements compared to ASF-negative carcasses and random points. This leads to the hypothesis, that ASF-infected animals seek shelter in quiet areas further away from human disturbance. A detailed collection of information on the environment surrounding carcass detection sites is needed to characterize predilection sites more accurately.

Since the first occurrence of African swine fever (ASF) in wild boar in Germany in 2020, the disease has primarily affected the wild boar population in the eastern part of the country close to the border with Poland. Local hunters play a crucial role in implementing surveillance and control. To evaluate their perceptions of existing control measures and analyze regional differences between hunters from ASF-affected and non-affected regions, a questionnaire study was conducted among the German hunting community. Hunters from non-affected areas held a more optimistic view regarding the effectiveness of control measures compared to hunters from affected areas. However, control measures that hinder hunting were generally perceived as ineffective. Measures that collided with hunters' understanding of fair hunting practices were regarded as controversial. Financial incentives and reducing bureaucracy were the most favored approaches to increase hunters' participation. Moreover, the possibility of eating or selling the meat of hunted wild boar and the provision of infrastructure for implementing ASF control were considered motivating. Thus, this study highlights the importance of compensating hunters and addressing their concerns to maintain their engagement in ASF control. To enhance compliance with controversial measures, thoughtful communication and raising awareness are essential.

African swine fever (ASF) emerged in Latvia in 2014. In 2020, the virus has been detected in the German federal state, Saxony. In both regions, the virus was probably introduced by infected wild boar coming from affected neighboring countries. As the current ASF control strategy at EU level had not yet been developed at the time of ASF introduction into Latvia, disease control measures in both study areas differed over time. Assessing the course of ASF in Saxony and the implemented control strategies, the first 18 months of the ASF epidemic were epidemiologically compared between Saxony and Latvia. ASF wild boar surveillance data were analyzed and the prevalence of ASF virus-positive wild boar was estimated. For estimating the wild boar density, the numbers of wild boar per km² were calculated for the respective geographical areas. The number of samples collected from hunted wild boar and wild boar found dead was higher in Saxony. The ASF virus prevalence in Latvia was significantly higher than in Saxony, indicating that Saxony has had more time for getting prepared for dealing with an ASF incursion. Experience from other countries and the rapid implementation of new control strategies may have helped Saxony deal with ASF.

We hypothesise that variations in the importance of wild boar-to-farm and farm-to-farm transmission routes shape these contrasting patterns, and we emphasise the implications for effective ASF control.

Since 2007, African swine fever (ASF) has spread widely within Europe and beyond. Most affected countries recorded outbreaks in domestic pigs and cases in wild boar. Outbreak data from 2014 to 2021 were used to investigate the seasonal pattern of ASF in domestic pigs and wild boar across affected member states of the European Union, since knowledge of seasonal patterns may provide the potential to adapt prevention, surveillance and control during times of increased risk. In domestic pigs, a yearly peak was observed in many European countries in summer (predominantly in July and August). In wild boar, the patterns showed more variability. In many countries, there was a seasonal peak of ASF occurrence in winter (predominantly in January and December), with an additional summer peak in the Baltic States (predominantly in July) and a further spring peak in Poland (predominantly in March). The observed seasonal effects may be related to the abundance and population dynamics of wild boar and to seasonality in pig farming. Moreover, ASF occurrence may also be influenced by human activities in both domestic pigs and wild boar.

African swine fever (ASF) has been present in Estonia since 2014, and pigkeepers play a major role in controlling the spread of viral infection in domestic pigs. Participatory epidemiology may provide insight into livestock keepers' existing attitudes and compliance with implementing control strategies. By conducting focus group discussions and using participatory epidemiology tools, this study aimed to reveal pigkeepers' knowledge and awareness of ASF clinical signs, transmission routes, and preventive measures. Furthermore, opinions including trust in stakeholders in ASF control, acceptability of ASF eradication, and control measures by pigkeepers in Estonia were identified. The most indicative ASF signs in pigs were "increased mortality" and "fever, inappetence, lethargy". The highest risk of virus introduction into the herd was designated to "agricultural machinery, vehicles", "rodents, birds, insects", and "people". "Fencing" was the most liked ASF preventive measure, while "training of staff in biosecurity and informing the visitors" was considered the most effective measure. The least accepted ASF eradication measure was "culling of all animals on the farm". Generally, pigkeepers perceived that healthy animals should not be culled but only ASF-infected pigs. Additionally, healthy pigs could be slaughtered and used for the production of heat-treated canned meat. "Destroying the feed" of ASF-infected farms was not favoured because some pigkeepers proposed using it for other animal species. Regarding ASF territorial restrictions (zoning), "lower meat prices" and "economic losses" were regarded as consequences with the greatest impact. Pigkeepers ranked trust in "Veterinary Authority and Ministry of Rural Affairs" regarding their role in ASF prevention and control as high while perceiving themselves as the most trustworthy stakeholder within the ASF network. This study demonstrates a generally good level of awareness among the pigkeepers in Estonia concerning the clinical manifestation of ASF, the most relevant transmission routes of the virus for their farms, and preventive measures for ASF. Pigkeepers are aware of their own role in preventing ASF, and there is a high level of trust between them and government authorities. Additional efforts are needed from authorities to communicate better the purpose of some ASF control and eradication measures to the pigkeepers. Further incentives are required to support the implementation of costly biosecurity measures, assure compliance, and mitigate the consequences of these measures.

While numerous risk factors of African swine fever (ASF) transmission to domestic pigs have been described, ASF virus introduction has often not been traced back to one single defined cause. The large number of ASF outbreaks that occurred in domestic pigs in Lithuania from 2014 through to 2018 raised the question regarding whether outbreak-specific risk factors and transmission routes could be identified. Therefore, a prospective matched case-control study was designed. Data from 18 outbreaks that occurred in Lithuanian in 2019 and 36 control farms were analyzed. Conditional multivariable logistic regression showed that two or more visits by veterinary inspection of a farm had a significant preventive effect on the occurrence of ASF on a farm (Odds ratio (OR) 14.21, confidence interval (CI) 1.09-185.60 for farms not inspected vs. farms inspected twice or more a year), while certain practices (e.g., mushroom picking, sharing equipment, etc.), which might facilitate the indirect introduction of ASF from fields and forests into piggeries, significantly increased the odds of an outbreak (OR 5.18, CI 1.10-24.44). The results of the study highlight the importance of veterinary inspections for increasing the biosecurity level on pig farms and the awareness of ASF. The knowledge on potential protective and risk factors may help to improve the prevention and control of ASF outbreaks in domestic pig farms in Lithuania and other affected countries.

After the first occurrence of African swine fever (ASF) in Germany in September 2020, control measures were implemented that resembled those taken in the Czech Republic and Belgium, the only two countries that succeeded in eliminating ASF from their territory so far in the current epidemic. In the present study, the epidemiological course of ASF in the first 6 months since introduction in these three countries is compared. Within 6 months, Germany experienced more cases than the Czech Republic and Belgium. The affected area in Germany, measured using minimal convex polygons, is much larger than the respective areas in the Czech Republic and in Belgium. All cases in the Czech Republic and in Belgium clustered in one single defined area, suggesting point-source introductions, whereas in Germany four distinct spatial clusters were observed, which indicates that multiple incursions had occurred along the border with Poland. While the overall course of the disease was comparable, when individual clusters were considered, the summarized data showed clear differences between the situation in Germany compared to that in the Czech Republic and Belgium. Germany experienced several independent introductions, caused by continuous infection pressure along the border to Poland, while the infection was only introduced on a single occasion each into the Czech Republic and Belgium. These differences may require appropriate adaptation of control measures, in particular concerning fencing along the border.

African swine fever (ASF) was first detected in Lithuania, Latvia, and Estonia in 2014 and has since been circulating in the Baltic States with a similar epidemiological course characterized by persistence of the disease in the wild boar population and occasional spill-over infections in domestic pigs. The aim of the present study was to evaluate surveillance data on ASF in wild boar from the three countries to improve our understanding of the course of the disease. ASF surveillance and wild boar population data of the countries were analyzed. In all three countries, a decrease in the prevalence of ASF virus-positive wild boar was observed over time. Although somewhat delayed, an increase in the seroprevalence was seen. At the same time, the wild boar population density decreased significantly. Towards the end of the study period, the wild boar population recovered, and the prevalence of ASF virus-positive wild boar increased again, whereas the seroprevalence decreased. The decreasing virus prevalence has obviously led to virus circulation at a very low level. Together with the decreasing wild boar population density, the detection of ASF-infected wild boar and thus ASF control has become increasingly difficult. The course of ASF and its continuous spread clearly demonstrate the necessity to scrutinize current ASF surveillance and control strategies fundamentally and to consider new transdisciplinary approaches.
