BACKGROUND: High-intensity interval training (HIIT) has been promoted as a time-efficient exercise strategy to improve health and fitness in children and adolescents. However, there remains little consensus in the literature regarding its efficacy in children and adolescents with special educational needs (SEN). This study aimed to examine HIIT as a means of improving key health and fitness parameters in children and adolescents with SEN. METHODS: A systematic search was conducted on eight databases (MEDLINE, Embase, SPORTDiscus, Web of Science, Scopus, PsycINFO, CINAHL, and Cochrane Library). Studies were eligible if they 1) included an HIIT protocol, 2) examined parameters related to both physical and mental aspects of health and fitness, and 3) examined children and adolescents with SEN aged 5-17 years. RESULTS: Of the 1727 studies yielded by the database search, 13 (453 participants) were included and reviewed. We found that HIIT generally improved body composition, physical fitness, and cardiometabolic risk biomarkers across a spectrum of SEN (e.g., attention deficit hyperactivity disorder, cerebral palsy, developmental coordination disorder, and mental illness). Improvements in mental health and cognitive performance following HIIT have also been observed. CONCLUSION: This review provides up-to-date evidence for HIIT as a viable exercise strategy for children and adolescents with SEN. Further research investigating the benefits of HIIT in a wider range of SEN populations is warranted. TRIAL REGISTRATION: This study was registered in the International Prospective Register of Systematic Review (PROSPERO; registration number CRD42022352696).

Low-volume, time-efficient high-intensity interval training (HIIT), which involves whole-body (WB) callisthenics exercises, has gained worldwide popularity in recent years. However, the physiological and perceptual impact of WB-HIIT in comparison to specialised, equipment-based training is relatively less studied. This study compared the acute physiological and perceptual responses to a single session of WB-HIIT, ergometer-based HIIT (ERG-HIIT) and conventional moderate-intensity continuous training (MICT). Fourteen physically inactive adults (age: 28.4 ± 6.5 years, VO2peak: 31.0 ± 6.2 mL· kg-1· min-1) underwent three main trials (WB-HIIT: 12 x 30-s high-intensity callisthenics workout; ERG: HIIT: 12 x 30-s high-intensity cycling bouts; MICT: 30-min cycling at 50% peak power output) in a randomized cross-over order 3-7 days apart. The mean session heart rate (HR) and perceived exertion were comparable across all three protocols (p > 0.05). WB-HIIT attained a similar peak HR (87.4 ± 9.4 %HRmax) as that of ERG-HIIT (83.0 ± 8.6 %HRmax), and significantly greater than that of MICT (78.7 ± 5.5 %HRmax, p = 0.001). However, WB-HIIT induced significantly higher blood lactate levels (7.2 ± 1.8 mmol/L) compared to both ERG-HIIT (5.1 ± 1.3 mmol/L, p < 0.05) and MICT (3.1 ± 1.5 mmol/L, p < 0.001). The participants reported higher self-efficacy and greater enjoyment with WB-HIIT compared to MICT (p < 0.05). The mean HR and perceived exertion responses to WB-HIIT are comparable to those of equipment-based HIIT and MICT; however, WB-HIIT results in greater metabolic strain than both other modalities. Despite this, the overall perceptual responses to WB-HIIT are positive, suggesting that it could be a viable exercise alternative, especially for individuals with limited exercise time and restricted access to facilities and equipment.

BACKGROUND: High-intensity interval training (HIIT) or moderate-intensity continuous training (MICT) alone has been shown to improve metabolic health, but the effects of alternating the two training approaches as often practiced in real life remained unclear. PURPOSE: To examine the effects of HIIT or MICT alone or alternating HIIT-MICT on cardiometabolic responses in inactive obese middle-aged men. METHODS: Forty-two participants (age: 42 ± 5 y; BMI: 26.3 ± 2.1 kg m-2) were randomly assigned to four groups: HIIT (12 x 1-min running bouts at 80-90% HRmax interspersed with 1-min active recovery at 50% HRmax), MICT (40-min brisk walk at 65-70% HRmax), alternating HIIT-MICT or a non-exercise control group (CON). Exercise sessions were conducted three times per week for 16 weeks. Maximal oxygen uptake (VO2max), body composition (by bioelectrical impedance analysis), blood pressure, fasting blood glucose, insulin resistance (HOMA-IR) and lipid profile were assessed at baseline and after the 16-week intervention. Enjoyment and self-efficacy were also assessed at the end of intervention. RESULTS: All exercise groups showed a similar VO2max increase of ∼15% (HIIT: 34.3 ± 4.4 vs 39.1 ± 5.4; MICT: 34.9 ± 5.0 vs 39.4 ± 7.2; and alternating HIIT-MICT: 34.4 ± 5.0 vs 40.3 ± 4.6 mL kg-1min-1) compared to baseline and CON (all p < 0.05). Weight, BMI, % fat and waist circumference also showed similar reductions in all exercise groups compared to baseline and CON (all p < 0.05). No significant group difference was observed for all blood markers. Compared to baseline, total cholesterol decreased after HIIT-MICT, while HIIT significantly decreased fasting insulin level and improved insulin resistance (p < 0.05). Enjoyment, self-efficacy and adherence were similar among all exercise groups. CONCLUSION: HIIT or MICT alone or alternating HIIT-MICT similarly improve cardiovascular fitness and body composition in obese middle-aged men despite differences in total training volume and time commitment.

BACKGROUND: Although a single bout of postmeal exercise can lower postprandial glucose (PPG), its optimal timing remains unclear. OBJECTIVE: This study aimed to investigate the effect of exercise timing using an individualized approach on PPG in overweight or obese young men. METHODS: Twenty men [age: 23.0 ± 4.3 y; BMI (kg/m2): 27.4 ± 2.8] each completed three 240-min trials in a randomized order separated by 6-14 d: 1) sitting (SIT), 2) walking initiated at each participant's PPG-peak time (PPGP) (iP), and 3) walking initiated 20 min before the PPGP (20iP). For each participant, PPGP was predetermined using continuous glucose monitoring. Walking was performed at 50% maximal oxygen consumption for 30 min. Venous blood was collected at 15- and 30-min intervals for 0-120 min and 120-240 min, respectively. The primary outcome was plasma PPG. Generalized estimating equations were used for comparison between trials. RESULTS: Compared with SIT, the 4-h incremental AUCs (iAUCs) for plasma PPG (-0.6 mmol · L-1 · h; P = 0.047) and insulin (-28.7%, P < 0.001) were reduced in 20iP only, and C-peptide concentrations were lower after iP (-14.9%, P = 0.001) and 20iP (-28.7%, P < 0.001). Plasma insulin (-11.1%, P = 0.006) and C-peptide (-8.3%, P = 0.012) were lower due to the 20iP compared with iP treatment. Finally, PPG reductions due to iP and 20iP occurred only in men with a BMI > 27.5 kg/m2 (iP, -11.2%; 20iP, -14.7%; P = 0.047) and higher glucose iAUC values during SIT (iP, -25.5%; 20iP, -25.7%; P < 0.001). CONCLUSIONS: Walking initiated 20 min before PPGP lowered PPG and plasma insulin and C-peptide concentrations in young men with overweight or obesity, in particular in those with high BMI or glucose iAUC values during SIT; it also lowered plasma insulin and C-peptide concentrations more effectively than did exercise initiated at PPGP. This trial was registered at the Chinese Clinical Trial Registry (http://www.chictr.org.cn/index.aspx) as ChiCTR1900023175.

This study aimed to examine the effects of 3-min light-intensity walking every 30 min (3-min) and 6-min light-intensity walking every 60 min (6-min) compared with prolonged sitting (SIT) on the postprandial metabolic responses in young, centrally obese, Chinese men. Twenty-one Chinese men with central obesity (age, 23 ± 4 years; body mass index, 29.8 ± 3.2 kg·m-2; waist circumference, 98.7 ± 7.1 cm; mean ± SD) underwent three 6-h experiments with a 7-day washout period, SIT, 3-min, and 6-min, in randomized order. Compared with SIT, neither walking condition showed differences in total and net incremental area under the curve (tAUC and iAUC, respectively) for glucose, insulin, nor non-esterified fatty acids. The tAUC and iAUC for triglycerides for the SIT condition (10.8 [9.3, 12.2] and 4.4 [3.7, 5.1] mmol·h·L-1, respectively; mean [95% confidence interval]) was higher than 3-min (10.4 [9.0, 11.8] and 3.8 [3.3, 4.3] mmol·h·L-1, respectively, both Ps <0.05) and 6-min (9.6 [8.1, 11.0] and 3.5 [2.9, 4.2] mmol·h·L-1, respectively, both Ps <0.01) conditions. Interrupting prolonged sitting regardless of frequency-reduced postprandial triglycerides. A higher volume of physical activity may be required to obtain greater glycaemic benefits in young Chinese men with central obesity.

Interval training has been promoted as a superior, time-efficient exercise strategy for enhancing cardiorespiratory fitness in the ageing population. This study assessed the effectiveness of interval training compared with that of moderate-intensity continuous training (MICT) for improving cardiorespiratory fitness in middle-aged and older adults. Six databases were systematically searched for studies with exercise interventions (minimum 2-week duration). Meta-analyses were conducted for within-group and between-group comparisons of maximal oxygen uptake (VO2max). Sub-group analysis was performed using the nature of interval training (high-intensity interval training [HIIT] and sprint interval training [SIT]). Of 3,257 studies, 14 were included (429 participants). Within-group analyses demonstrated significant VO2max improvements (mL/kg/min) following interval training (mean difference: MD = 2.26; 95% confidence interval [CI] = 1.50-3.02) and MICT (MD = 1.34; 95% CI = 0.45-2.23]. When comparing the modes of training, the gain in VO2max was significantly greater following interval training (MD = 1.10; CI = 0.55-1.64). Sub-group analysis showed that HIIT (MD = 1.04; CI = 0.21-1.88) and SIT (MD = 1.18; CI = 0.60-1.76) resulted in superior VO2max gain than MICT. This study provides evidence synthesis for interval training as a viable exercise strategy to improve cardiorespiratory function in healthy ageing.

This study examines the association between gender expression, weight status, and the risk of experiencing eating disorders among gender-diverse adults assigned male at birth living in Bangkok, Thailand. Participants completed self-administered questionnaires to provide demographic data and anthropometric measures, and an Eating Attitude Test-26 (EAT-26) to determine the risk of experiencing eating disorders. The associations between gender expression, weight status, and the risk of experiencing eating disorders were analyzed using multivariable logistic regression models. No significant differences were observed in weight-related variables based on gender expression. Participants self-described as feminine/androgynous had lower odds of experiencing a high risk of eating disorders compared to those self-described as masculine (odds ratio (OR) = 0.49; 95% confidence interval (95% CI) = 0.27, 0.88). A higher body mass index (BMI) (OR = 1.07; 95% CI = 1.01, 1.14) and BMI discrepancy (OR = 1.13; 95% CI = 1.03, 1.24) were associated with higher odds of engaging in extreme weight-control behaviors. The risk of experiencing eating disorders among Thai gender-diverse adults assigned male at birth could differ across gender expression and weight status. Further research is needed to expand the understanding of these relationships and develop tailored intervention programs to mitigate the risk.

CONTEXT: Prolonged sitting elevates postprandial metabolic markers, resulting in increased risks of cardiovascular diseases and type 2 diabetes. Interrupting prolonged sitting may reduce these risks. However, more information is needed to understand the patterns of interrupting prolonged sitting to obtain metabolic health benefits. OBJECTIVE: This study examined the effects of interrupting prolonged sitting with different intensities and durations of walking with an equivalent energy expenditure on postprandial metabolic responses in young Chinese men with central obesity. DESIGN: A randomized crossover experimental trial was conducted. SETTING: Participants underwent three 6-hour experiments with a 7-day washout period between each experiment: prolonged sitting, 3 min of light-intensity walking every 30 minutes, and 1.5 minutes of moderate-intensity walking every 30 minutes. PARTICIPANTS AND SAMPLES: Baseline (fasting) and 6-hour postprandial metabolic glucose and lipid levels were analyzed among 18 young Chinese men with central obesity. MAIN OUTCOME MEASURES: Generalized estimating equations (adjusted for the potential confounders explaining residual outcome variance (body mass index) and age), trial order, preprandial values, and lead-in activity) were used, and the incremental areas under the curve (iAUC) of each outcome were compared between prolonged sitting and interrupted prolonged sitting conditions. RESULTS: Compared with prolonged sitting, both interrupting prolonged sitting conditions reduced the iAUCs for glucose (P < .05) but not insulin, C-peptide, triglycerides, or nonesterified fatty acids. CONCLUSIONS: Both conditions of interrupted prolonged sitting reduced postprandial glucose concentrations in young Chinese men with central obesity when the energy expenditure was equivalent.

Aims: To investigate the accuracy of FreeStyle LibreTM flash glucose monitoring (FGM) relevant to plasma glucose (PG) measurements during postprandial rest and different walking conditions in overweight/obese young adults. Methods: Data of 40 overweight/obese participants from two randomized crossover studies were pooled into four trials: (1) sitting (SIT, n = 40); (2) walking continuously for 30 min initiated 20 min before individual postprandial glucose peak (PPGP) (20iP + CONT, n = 40); (3) walking continuously for 30 min initiated at PPGP (iP + CONT, n = 20); and (4) accumulated walking for 30 min initiated 20 min before PPGP (20iP + ACCU, n = 20). Paired FGM and PG were measured 4 h following breakfast. Results: The overall mean absolute relative difference (MARD) between PG and FGM readings was 16.4 ± 8.6% for SIT, 16.2 ± 4.7% for 20iP + CONT, 16.7 ± 12.2% for iP + CONT, and 19.1 ± 6.8% for 20iP + ACCU. The Bland-Altman analysis showed a bias of -1.03 mmol⋅L-1 in SIT, -0.89 mmol⋅L-1 in 20iP + CONT, -0.82 mmol⋅L-1 in iP + CONT, and -1.23 mmol⋅L-1 in 20iP + ACCU. The Clarke error grid analysis showed that 99.6-100% of the values in all trials fell within zones A and B. Conclusion: Although FGM readings underestimated PG, the FGM accuracy was overall clinically acceptable during postprandial rest and walking in overweight/obese young adults.
