RESUMEN

Inferior vena cava (IVC) aneurysms are a rare finding, whose management and outcomes remain uncertain due to their low incidence and long-term follow-up. As IVC aneurysms remain a poorly understood clinical entity, it is important to expand upon our existing knowledge base as new cases arise. We present a patient with a suprarenal IVC saccular aneurysm and an overview of the current literature regarding IVC aneurysm classification, presentation, and management. Based on the expanding literature, we propose that IVC aneurysms may be simplified into a 2-type classification, which can further guide clinicians on management of the aneurysm.

RESUMEN

As urologists we have an opportunity to reduce the carbon footprint of the procedures we perform. We highlight some areas of interest in urology and potential initiatives to reduce the energy and waste footprint of urology care. Urologists can and should make an impact on the growing climate crisis.

BACKGROUND: The aim of this study was to analyze the epidemiology of patients admitted with finger amputations in the U.S., as well as to evaluate and propose prevention strategies. METHODS: The National Electronic Injury Surveillance System was queried to obtain data on patients that presented to, and were admitted from US emergency departments for treatment of traumatic finger amputations during the period of 2002-2016. The Haddon Matrix, a framework that can be used to analyze the host, agent, and environmental factors of an injury relative to its timing, was then used to evaluate possible contributing factors of amputation events, and thereby explore plausible prevention interventions. RESULTS: From 2002 to 2016, approximately 348,719 people were admitted from the ED for traumatic amputations. The majority were Caucasian and were male. The mean age was 42.3 years old. This was significantly older than those who were not admitted. The top five products responsible for amputations in admitted patients were power saws (40.9% of cases), doors (10.3%), lawn mowers (7.4%), snow blowers (4.3%), and bicycles (2.4%). This list included a higher proportion of powered tools than those with finger amputations who were discharged from the ED with a finger amputation. CONCLUSION: Patients admitted with finger amputations from the ED were older, more likely to be male, and more likely to be victims of powered tools than those that were discharged. Table saws are responsible for a high proportion of the finger amputations that result in hospital admissions. The Haddon Matrix helps us identify factors (host, agent, physical environment, and social environment) to be addressed in prevention strategies. Such approaches might include championing education campaigns, policy measures, and equipment safety features. The effectiveness of such strategies warrants further investigation.

While treatments that induce DNA damage are commonly used as anticancer therapies, the mechanisms through which DNA damage produces a therapeutic response are incompletely understood. Here we have tested whether medulloblastomas must be competent for apoptosis to be sensitive to radiotherapy. Whether apoptosis is required for radiation sensitivity has been controversial. Medulloblastoma, the most common malignant brain tumor in children, is a biologically heterogeneous set of tumors typically sensitive to radiation and chemotherapy; 80% of medulloblastoma patients survive long-term after treatment. We used functional genetic studies to determine whether the intrinsic apoptotic pathway is required for radiation to produce a therapeutic response in mice with primary, Shh-driven medulloblastoma. We found that cranial radiation extended the survival of medulloblastoma-bearing mice and induced widespread apoptosis. Expression analysis and conditional deletion studies showed that Trp53 (p53) was the predominant transcriptional regulator activated by radiation and was strictly required for treatment response. Deletion of Bax, which blocked apoptosis downstream of p53, was sufficient to render tumors radiation resistant. In apoptosis-incompetent, Bax-deleted tumors, radiation activated p53-dependent transcription without provoking cell death and caused two discrete populations to emerge. Most radiated tumor cells underwent terminal differentiation. Perivascular cells, however, quickly resumed proliferation despite p53 activation, behaved as stem cells, and rapidly drove recurrence. These data show that radiation must induce apoptosis in tumor stem cells to be effective. Mutations that disable the intrinsic apoptotic pathways are sufficient to impart radiation resistance. We suggest that medulloblastomas are typically sensitive to DNA-damaging therapies, because they retain apoptosis competence. Cancer Res; 76(11); 3211-23. ©2016 AACR.

RATIONAL AND OBJECTIVES: The objective of this study was to compare the stationary digital breast tomosynthesis (s-DBT) system to a conventional mammography system in a study of breast specimens. Radiologist evaluation of image quality was assessed in a reader study. This study represents the first human tissue imaging with the novel carbon nanotube-based s-DBT device. MATERIALS AND METHODS: Thirty-nine patients, with known breast lesions (Breast Imaging Reporting and Data System 4 or 5) by conventional mammography and scheduled for needle localization biopsy, were recruited under an institutional review board-approved protocol. Specimen images were obtained using a two-dimensional (2D) mammography system with a ×1.8 magnification factor and an s-DBT system without a high magnification factor. A reader study was performed with four breast fellowship-trained radiologists over two separate sessions. Malignancy scores were recorded for both masses and microcalcifications (MCs). Reader preference between the two modalities for MCs, masses, and surgical margins was recorded. RESULTS: The s-DBT system was found to be comparable to magnified 2D mammography for malignancy diagnosis. Readers preferred magnified 2D mammography for MC visualization (P < .05). However, readers trended toward a preference for s-DBT with respect to masses and surgical margin assessment. CONCLUSIONS: Here, we report on the first human data acquired using a stationary digital breast tomosynthesis system. The novel s-DBT system was found to be comparable to magnified 2D mammography imaging for malignancy diagnosis. Given the trend of preference for s-DBT over 2D mammography for both mass visibility and margin assessment, s-DBT could be a viable alternative to magnified 2D mammography for imaging breast specimens.
