RESUMEN

Objective: To study the nursing impact of traditional Chinese medicine on children undergoing trichiasis correction surgery with general anesthesia. Methods: A total of 104 children with trichiasis admitted to the hospital for correction under general anesthesia from February 2020 to July 2023 were selected as the research objects. They were randomly divided into a comparison group (52 cases) and an observation group (52 cases). The comparison group was given routine nursing after surgery, and the observation group was given special nursing after surgery. Hamilton Anxiety Scale (HAMA) scores and Wong-Baker Smiley face scale scores of the two groups of children before and after nursing were observed, and the nursing satisfaction of family members of the two groups was evaluated. Results: The anesthesia waking time, operation time, eating time and drinking time of the two groups in the observation group were lower than those in the comparison group, and the differences were statistically significant (P < .05). After nursing, the HAMA and HAMD scores of the two groups decreased, and the HAMA and HAMD scores of the two groups in the observation group were lower than those in the comparison group (P < .05). After nursing, the pain scores of the two groups were decreased, and the pain scores of the observation group were lower than those of the control group (P < .05). The visual acuity of the observation group was significantly better than that of the comparison group, and the difference was statistically significant (P < .05). The total satisfaction rate of the observation group was 94.23%, and that of the comparison group was 80.77%, and the total satisfaction rate of the observation group was higher than that of the comparison group (P < .05). The compliance of the observation group was 96.15%, and that of the comparison group was 84.62%, and the compliance of the observation group was higher than that of the comparison group (P < .05). Within 24h after the operation, the incidence of hunger, thirst, abdominal distension, fever, and irritable crying in the observation group was significantly lower than in the control group, and the difference was statistically significant (P < .05). Conclusion: The application of traditional Chinese medicine-based nursing interventions can effectively alleviate negative emotions, alleviate pain, and enhance the satisfaction of family members towards nursing care for children undergoing trichiasis correction surgery under general anesthesia.

RESUMEN

A nickel-catalyzed cascade addition/cyclization of 2-azido-N-arylacrylamides and cyclobutanone oxime esters for the construction of 3-cyanoalkylated quinoxalin-2(1H)-ones is developed. This reaction proceeds under mild conditions with good functional group tolerance and broad substrate scope. A preliminary mechanistic experiment indicated that the cyanoalkyl radical might be involved in this transformation.

Understanding the electric dipole switching in multiferroic materials requires deep insight of the atomic-scale local structure evolution to reveal the ferroelectric mechanism, which remains unclear and lacks a solid experimental indicator in high-pressure prepared LiNbO3-type polar magnets. Here, we report the discovery of Zn-ion splitting in LiNbO3-type Zn2FeNbO6 established by multiple diffraction techniques. The coexistence of a high-temperature paraelectric-like phase in the polar Zn2FeNbO6 lattice motivated us to revisit other high-pressure prepared LiNbO3-type A2BB'O6 compounds. The A-site atomic splitting (∼1.0-1.2 Šbetween the split-atom pair) in B/B'-mixed Zn2FeTaO6 and O/N-mixed ZnTaO2N is verified by both powder X-ray diffraction structural refinements and high angle annular dark field scanning transmission electron microscopy images, but is absent in single-B-site ZnSnO3. Theoretical calculations are in good agreement with experimental results and suggest that this kind of A-site splitting also exists in the B-site mixed Mn-analogues, Mn2FeMO6 (M = Nb, Ta) and anion-mixed MnTaO2N, where the smaller A-site splitting (∼0.2 Šatomic displacement) is attributed to magnetic interactions and bonding between A and B cations. These findings reveal universal A-site splitting in LiNbO3-type structures with mixed multivalent B/B', or anionic sites, and the splitting-atomic displacement can be strongly suppressed by magnetic interactions and/or hybridization of valence bands between d electrons of the A- and B-site cations.

X-type hexaferrites have been receiving considerable attention due to their promising applications in many magnetic-electronic fields. However, the growth of single-crystal X-type hexaferrite is still a challenge. Herein we reported, for the first time, the preparation of single crystal X-type hexaferrite Sr2Co2Fe28O46 (Sr2Co2X) with high-quality and large size using floating-zone method with laser as the heating source. The crystals show rhombohedral symmetry with space group of R-3m (No. 166, a = 5.8935(1) Å and c = 83.7438(17) Å). Co2+ and Fe3+ oxidation states were confirmed by the X-ray absorption near-edge spectroscopy. The prepared Sr2Co2X exhibits a spin reorientation transition from easy-cone to easy-axis at T2 of 343 K and a ferrimagnetism-paramagnetism transition at Curie temperature (TC) of ∼743 K. The spin reorientation transition was accompanied by magnetocaloric effect (MCE). Both conventional and inverse MCEs were observed near T2 with a magnetic field applied along the c-axis. The maximum value of the magnetic entropy change along the c-axis was evaluated to be 1.1 J/kg·K for a magnetic field change of 5 T.

A novel TBPB-initiated cascade cyclization of 3-arylethynyl-[1,1'-biphenyl]-2-carbonitriles with sulfinic acids via C-S, C-C and C-N bond formation for the synthesis of 3-sulfonated cyclopenta[gh]phenanthridines under metal-free conditions has been developed. This protocol features mild conditions, good functional group tolerance and a broad substrate scope. By using this protocol, a variety of potentially bioactive 3-sulfonated cyclopenta[gh]phenanthridines were facilely synthesized via direct annulation.

RESUMEN

This short overview focuses on the causation and treatment of type 2 diabetes (T2D). Emphasis is given to the historical basis of understanding this disease and the background leading to emergence of the central role of insulin. The strengths of insulin administration in the treatment of diabetes are profound, but these need to be balanced against several serious shortcomings of its extended use. Some alternative approaches to T2D management are considered. Insulin is no longer considered as the first choice for type 2 diabetes, and an expanding range of new therapeutic possibilities is emerging. While these may lack the potency of insulin, at a minimum, they allow a major reduction in the intensity of insulin use. In view of the rising worldwide incidence of this disease, it is imperative to develop safe and inexpensive means of limiting its potential for impairment of normal functioning.

A quadruple perovskite LaMn3Rh4O12 with A' = Mn and B = 4d transition metal was synthesized at high pressure (8 GPa) and temperature (1423 K) for the first time. Room temperature powder X-ray diffraction indicates that LaMn3Rh4O12 forms in cubic symmetry (Im3̅, a = 7.4997(1) Å). X-ray absorption near-edge spectroscopy shows predominantly Mn3+ and Rh3+ oxidation states. An antiferromagnetic transition at TN ∼ 41 K is corroborated by specific heat measurements. The resistivity measurements indicate a three-dimensional Mott variable-range hopping conduction mechanism between 300 and 160 K.

Li2GeTeO6 prepared at ambient pressure adopts the corundum derivative ordered ilmenite structure (rhombohedral R3). When heated at 1073 K and 3-5 GPa, the as-made Li2GeTeO6 can convert into a LiSbO3-derived Li2TiTeO6-type phase (orthorhombic Pnn2), which is the third LiSbO3-derived double A2BB'O6 phase in addition to Li2TiTeO6 and Li2SnTeO6. This Pnn2 Li2GeTeO6 phase spontaneously reverts to the R3 phase if annealed up to 1023 K at ambient pressure. Although the crystal structural analyses and second harmonic generation measurements clearly demonstrate the polar nature of both the R3 and Pnn2 phases, P( E) and dielectric measurements do not show any convincing ferroelectric response. Given the large estimated spontaneous polarization (17 and 80 µC/cm2), the absence of ferroelectric behavior could be attributed to the random domain distribution and leakage due to Li-ion migration.

Double-perovskite Lu2NiIrO6 was synthesized at high pressure (6 GPa) and high temperature (1300 °C). Synchrotron powder X-ray diffraction indicates that its structure is a monoclinic double perovskite (space group P21/ n) with a small, 11% Ni/Ir antisite disorder. X-ray absorption near-edge spectroscopy measurements established Ni2+ and Ir4+ formal oxidation states. Magnetic studies indicate a ferrimagnetic transition at 207 K. The low-temperature magnetization curve of Lu2NiIrO6 features broad hysteresis with a coercive field as high as 48 kOe. These results encourage the search for hard magnets in the class of 3d/5d double-perovskite oxides.

The symmetric splitting of two spin-wave branches in an antiferromagnetic resonance (AFR) experiment has been an essential measurement of antiferromagnets for over half a century. In this work, circularly polarized time-domain THz spectroscopy experiments performed on the low symmetry multiferroic hexagonal HoMnO_{3} reveal an AFR of the Mn sublattice to split asymmetrically in an applied magnetic field, with an ≈50% difference in g factors between the high and low energy branches of this excitation. The temperature dependence of the g factors, including a drastic renormalization at the Ho spin ordering temperature, reveals this asymmetry to unambiguously stem from Ho-Mn interactions. Theoretical calculations demonstrate that the AFR asymmetry is not explained by conventional Ho-Mn exchange mechanisms alone and is only reproduced if quartic spin interactions are also included in the spin Hamiltonian. Our results provide a paradigm for the optical study of such novel interactions in hexagonal manganites and low symmetry antiferromagnets in general.

Objective: The objective of this study was to endoscopically evaluate the graft healing process and graft success rate following cartilage myringoplasty, without trimming of the perforation margin or external ear canal (EAC) packing. Material and Methods: Patients with chronic tympanic membrane (TM) perforation underwent endoscope cartilage underlay myringoplasty, without trimming perforation margin or EAC packing. The healing process of the cartilage graft and the graft success rate were evaluated at 6 months postoperatively. Results: Fifty-eight ears were included in this study. At 1 week postoperatively, clinical inosculation and neovascularization of the graft were observed in small- and medium-sized perforations, but not in large or subtotal perforations. At 2 weeks postoperatively, graft clinical inosculation and neovascularization were completed in the small- and medium-sized perforations; however, neovascularization of the graft had only just begun in the large perforations. At 3 weeks postoperatively, completion of the graft clinical inosculation was achieved in 57 of the 58 ears. At 4 weeks postoperatively, complete neovascularization was achieved in all perforations. Of the 58 ears, postoperative infection resulted in residual perforation in 1 ear, and an insufficient graft resulted in residual perforation in a large perforation without infection. Overall, the graft success rate was 96.6% (56/58). There was no correlation between the graft success rate and graft neovascularization score. Conclusions: The graft healing process experienced the dilation of the blood vessels of the remnant TM, graft clinical inosculation, and neovascularization following cartilage myringoplasty without trimming of the perforation margin and EAC packing; however, the graft success rate was not related to the endoscopic graft neovascularization scores.

Baijiu is an ancient, distilled spirit with a complicated brewing process, unique taste, and rich trace components. These trace components play a decisive role in the aroma, taste, and especially the quality of baijiu. In this paper, the redox reaction between the Fenton reagent and four reducing agents, including o-phenylenediamine (OPD), p-phenylenediamine (PPD), 4-aminophenol (PAP), and 2-aminophenol (OAP), was adopted to construct a four-channel visual sensor array for the rapid detection of nine kinds of common organic acids in baijiu and the identification of baijiu and its adulteration. By exploiting the color-changing fingerprint response brought by organic acids, each organic acid could be analyzed accurately when combined with an optimized variable-weighted least-squares support vector machine based on a particle swarm optimization (PSO-VWLS-SVM) model. What is more, this novel sensor also could achieve accurate semi-quantitative analysis of the mixed organic acid samples via partial least squares discriminant analysis (PLSDA). Most importantly, the sensor array could be further used for the identification of baijiu with different species through the PLSDA model and the adulteration assessment with the one-class partial least squares (OCPLS) model simultaneously.

Baijiu authenticity has been a frequent problem driven by economic interests in recent years, so it is important to discriminate against baijiu with different origins. Herein, we proposed a simple and efficient esters-targeted colorimetric sensor array mediated by hydroxylamine hydrochloride. Esters undergo a nucleophilic addition reaction with hydroxylamine hydrochloride to form hydroxamic acid, which rapidly forms a purplish red ferric hydroxamate under FeCl3·6H2O. Bromophenol blue and rhodamine B enrich the color effects. The array detected 12 esters with a detection limit on the order of 10-5 of most esters and 16 mixed esters with R2 > 0.999 and recoveries close to 100%. Otherwise, for discriminating 34 strong-aroma baijius (SABs), the array has an accuracy of 98% according to the origin, and 95% according to the grades, with a response time of 1 min. This study provides a new strategy for authenticity determination and quality control of baijiu.

Given the severe problem of Baijiu authenticity, it is essential to discriminate Baijiu from different origins quickly and effectively. As organic acids (OAs) are the most dominant taste-imparting substances in Baijiu, we proposed a simple, fast, and effective OAs-targeted colorimetric sensor array based on the colorimetric reaction of 4-aminophenol (AP)/4-amino-3-chlorophenol (ACP) under oxidation of Cu(NO3)2 for the rapid discrimination of origins of Baijiu with three main aroma types. Hydrogen ions ionized from OAs induced the protonation of the amino group, which blocked the colorimetric reaction, and the different levels of OAs in Baijiu enabled the array to discriminate different origins of Baijiu. The array was implemented to analyze 10 simple OAs and 16 mixed OAs and further for the discrimination of 42 Baijius with an accuracy of 98%. This method provided an efficient research strategy for a basis for rapid quality analysis of Baijiu.

Soft grippers with good passive compliance can effectively adapt to the shape of a target object and have better safe grasping performance than rigid grippers. However, for soft or fragile objects, passive compliance is insufficient to prevent grippers from crushing the target. Thus, to complete nondestructive grasping tasks, precision force sensing and control are immensely important for soft grippers. In this article, we proposed an online learning self-tuning nonlinearity impedance controller for a tactile self-sensing two-finger soft gripper so that its grasping force can be controlled accurately. For the soft gripper, its grasping force is sensed by a liquid lens-based optical tactile sensing unit that contains a self-sensing fingertip and a liquid lens module and has many advantages of a rapid response time (about 0.04 s), stable output, good sensitivity (>0.4985 V/N), resolution (0.03 N), linearity (R2 > 0.96), and low cost (power consumption: 5 mW, preparation cost

OBJECTIVE: The objective of this study was to evaluate the graft success rate and hearing outcome of modified cartilage inlay graft technique for repairing large perforations under local anesthesia. STUDY DESIGN: Prospective case series. METHODS AND MATERIALS: We performed a prospective study in 31 patients with large perforations who underwent endoscopic local anesthesia-based modified cartilage inlay graft technique with no raising of the squamous epithelium and no rimming of the perforation margins. The graft outcomes and complications were evaluated at 6 and 24 months postoperatively. RESULTS: Thirty-one patients (31 ears) were included in this study. The mean operation time was 42.3 ± 2.6 (ranged 36-51) minutes. The visual analog scale pain scores at intraoperative and 2 and 48 hours postoperatively were, respectively, 2.6 ± 0.6, 1.4 ± 0.2, and 0.6 ± 0.1. The graft success rate was 100.0% (31/31) at 6 months after surgery. However, only 25 (80.6%, 25/31) patients completed a 2-year follow-up and performed a temporal bone CT examination; re-perforation was seen in 2 patients, the graft success rate was 92.0% (23/25). CT revealed the well-developed pneumatization of the mastoids and middle ear in all the patients. Nevertheless, 1 (1/25, 8.0%) patient developed a graft epithelial inclusion cyst, which was removed by endoscope in the outpatient setting. The mean preoperative air bone gap (ABG) was 23.1 ± 4.3 dB, while the mean postoperative ABG after 6 months was 14.9 ± 2.6 dB; the difference between these values was significant (P < .05; Wilcoxon's signed-rank test), and the functional success rate was 93.5% (29/31). CONCLUSION: Endoscopic modified cartilage "inlay" graft technique with no raising the squamous epithelium and no rimming the perforation is a minimally invasive technique with a short operative time and satisfying success rate for closure of large perforations under local anesthesia.

This meta-analysis aimed to compare the efficacy of montelukast (MKST) combined with budesonide (BUD) and BUD alone in the treatment of pulmonary inflammation and pulmonary function in children with cough variant asthma (CVA). Five electronic databases were searched for studies about MKST+BUD therapy and BUD alone therapy on inflammation and pulmonary function in CVA children from inception to November 23, 2021. Twenty-two articles were included. The results showed that, compared with BUD alone, the combination treatment could achieve better improvement of pulmonary function and lower levels of inflammation (MKST+BUD group: FEV1: SMD = 2.77, 95% CI: 2.07, 3.46; FVC: SMD = 2.54, 95% CI: 1.82, 3.27; PEF: SMD = 2.27, 95% CI: 1.79, 2.75; IgE: SMD = -7.95, 95% CI: -9.66, -6.25; TNF-α: SMD = -4.67, 95% CI: -6.04, -3.31; IL-8: SMD = -8.18, 95% CI: -11.46, -4.90; BUD alone group: FEV1: SMD = 1.83, 95% CI: 1.34, 2.31; FVC: SMD = 1.39, 95% CI: 0.93, 1.84; PEF: SMD = 1.51, 95% CI: 1.13, 1.89; IgE: SMD = -4.93, 95% CI: -6.14, -3.72; TNF-α: SMD = -2.78, 95% CI: -3.76, -1.80; IL-8: SMD = -4.94, 95% CI: -7.10, -2.79). To conclude, compared with BUD alone, MKST+BUD therapy was found to be more effective in improving pulmonary function and reducing inflammation in CVA children. The combined detection of CA125, CEA and AFP is of great significance in the diagnosis of ovarian cancer. Photonic crystal (PhC) barcodes have apparent advantages in multiplex immunoassays of ovarian cancer markers. In this paper, a novel PhC barcode was assembled from dendritic silica nanoparticles (dSiO2) for multiplex detection of ovarian cancer biomarkers. The interconnected macroporous structure of the dSiO2 PhC beads and the open porous topography of dendritic silica particles could increase the surface area to volume ratio for antibody immobilization. We simultaneously detected multiple ovarian cancer markers in one test tube using the sandwich immunization method by utilizing dSiO2 PhC beads as a barcode and CdTe QDs as a detection signal. The detection limits of the three ovarian cancer markers, AFP, CEA and CA125, were 0.52 ng mL-1, 0.64 ng mL-1 and 0.79 U mL-1, respectively (the signal-to-noise ratio was 3). Compared with the classic silica colloidal crystal bead (SCCB) suspension array, the sensitivity of the dSiO2 PhC bead suspension array was increased. In addition, the results showed that this barcode suspension array had acceptable accuracy and good reproducibility.

With the continuous improvement of medical level, the demand for medical devices increases year by year, and it is necessary to track the quality and safety of medical devices. Establishing the cleaning quality control management of medical devices can not only reduce medical accidents and inhibit the spread of counterfeit or substandard medical devices but also help enterprises find the source of problems, determine the flow of the same batch of products, and warn relevant enterprises to take measures after the occurrence of problem products, and ultimately ensure the rights and interests of consumers. This study aims to study the cleaning quality control management of medical equipment in the hospital disinfection supply room based on smart medical care and proposes related concepts of smart medical care, as well as related theories of Internet of Things technology and medical equipment tracking system and related concepts of medical equipment cleaning. In this study, by comparing the cleaning compliance rates of the two groups of medical devices through smart medical care, it can be seen that 100% of the research group applying program-based management is significantly higher than 76.6% of the conventional management path. There was statistical significance in the data comparison between groups (P < 0.05). Therefore, programmatic management is more targeted than conventional management paths.

The timelier and more accurate the diagnosis of the disease, the higher the patient's survival rate. Human epididymal protein 4 (HE4) has great significance as a biomarker of concern for reflecting ovarian cancer. Herein, we prepared a novel optical label that can be used in lateral-flow immuno-dipstick assay (LFIA) for sensitive visual detection of HE4 by implanting hydrophobic gold nanoparticles (Au NPs) at high density in Mg/Fe LDH nanoflowers (MF NFs). MF NFs with large specific surface area, high porosity, abundant active binding sites, and stable structure were employed for the first time as templates to directly anchor Au NPs in the organic phase. After simple modification with an optimized amount of branched polyethyleneimine, not only could MF@Au NFs be dispersed in the aqueous phase, but also amino functional groups were introduced on its surface to facilitate subsequent antibody coupling steps. The limit of detection reaches 50 pM with a detection range of 50 to 1000 pM. This work initially explored how MF NFs can be used to load signal labels with ideal stability and signal amplification capabilities, which greatly improves the practicability of LFIA and highlights its important role in the field of rapid diagnostics.
