RESUMEN

Background: We aimed to compare the prevention of hypoxemia using High-flow nasal oxygen (HFNO) or regular nasal tubing (CNC) in elderly patients undergoing gastroscopy with sedation. Methods: This study was a prospective, randomized, controlled trial conducted at a single center. We included elective patients aged 65 and above who were undergoing gastroscopy with sedation. In the intervention group (HFNO), we set the oxygen flow rate to 60 liters per minute with an oxygen fraction (FiO2) of 0.6, while in the control group (CNC), it was 6 liters per minute. The primary outcome was the occurrence of hypoxemia (defined as Spo2 < 90%). Results: A total of 125 participants were enrolled (HFNO group: n = 63; CNC group: n = 62). The occurrence of hypoxemia was found to be significantly lower in the HFNO group compared to the CNC group (3.2% vs. 22.6%, p = 0.001). Additionally, a significantly shorter duration of low oxygen levels was observed in the HFNO group [0.0 seconds (0.0-13.0)] compared to the CNC group [0.0 seconds (0.0-124.0), p<0.001]. Moreover, a higher minimum Spo2 value was achieved in the HFNO group [99.0% (98.0-100.0) vs. 96.5% (91.0-99.0), p < 0.001], and a shorter recovery time was recorded [0.5 minutes (0.0-0.5) vs. 0.5 minutes (0.0-1.0), p = 0.016] in comparison to the CNC group. There were no differences in terms of comfort level [0 (0-4) vs. 0 (0-5), p = 0.268] between the two groups. Conclusions: The HFNO system was determined to be a safe and highly effective method for oxygen delivery, leading to a reduction in the occurrence of hypoxemia in elderly patients undergoing gastroscopy with sedation. It is recommended that HFNO be considered as the standard approach for management in this population.

In this study, we propose an innovative composite channel model that considers multi-size bubbles, absorption, and fading caused by scattering for investigating the effect of multiple scattering on the optical properties of a channel. The model is based on Mie theory, geometrical optics and the absorption-scattering model in the Monte-Carlo framework, and the performance of the optical communication system of the composite channel was analyzed for different positions, sizes, and number densities of bubbles. A comparison with the corresponding optical properties of conventional particle scattering indicated that a larger number of bubbles corresponded to greater attenuation of the composite channel, which was manifested by a low power at the receiver, an increased channel impulse response, and the observance of a prominent peak in the volume scattering function or critical scattering angles. Additionally, the effects of the position of large bubbles on the scattering property of the channel were investigated. The proposed composite channel model can provide reference data for designing a more reliable and comprehensive underwater optical wireless communication link.

To improve the performance of underwater wireless optical communication (UWOC) systems, we propose a relay-assisted UWOC system model based on adaptive optics (AO). The closed expressions of the scintillation index, composite channel probability density function, and outage probability of the Gaussian beam before and after AO compensation are derived using the extended Rytov theory and Meijer G-function. The performance variation of an UWOC system with different parameters is analyzed by simulation. The results show that AO correction can compensate for the distorted wavefront and significantly reduce the intensity fluctuation at the receiving end. The proposed system can efficiently alleviate channel fading, improving the outage probability performance of the UWOC system.

In this study, we examined the performance of an underwater wireless optical communication (UWOC) system employing a single-input to multiple-output (SIMO) scheme and proposed an equalization equal gain combining (EEGC) algorithm for it under Gaussian beam conditions. Furthermore, based on a Yue spectrum with the instability of oceanic water stratification and a finite outer scale, we derived the closed analytical formulas for the scintillation index and spatial coherence radius in weak oceanic turbulence for a Gaussian beam, from which we could obtain the threshold of the detector spacing and the strength of oceanic turbulence. We then derived the closed-form formula for the upper bound average bit error rate of the EEGC SIMO system with ON-OFF keying modulation by using the hyperbolic tangent distribution function. Our simulations demonstrate two issues if oceanic water stratification is treated as a steady state: the performance of the diversity receiver system will be significantly underestimated in salinity-dominated weak oceanic turbulence channels and will be significantly overestimated in temperature-dominated weak oceanic turbulence channels. Additionally, the SIMO system performance improvement using the proposed EEGC algorithm was more evident with increasing detector spacing, and the EEGC algorithm reduced the impact of the layout of the avalanche photodiode arrays on the UWOC system performance, in contrast to the equal gain combining algorithm.

Herein, based on mRNA data from TCGA database, hepatocellular carcinoma (HCC) samples were subjected to a single-sample Gene Set Enrichment Analysis (ssGSEA). Then, HCC samples were finally classified into high-, middle-, and low-immunity groups using K-means consensus clustering (K = 3) according to ssGSEA scores. After the tumor microenvironment of HCC patients was further analyzed using ESTIMATE algorithm, the results indicated high immune score, stromal score, ESTIMATE score and low tumor purity in high-immunity group. HLA family genes and PD-L1(CD274) were remarkably highly expressed in high-immunity group. Immune-related lncRNAs were required by analyzing differentially expressed genes in high- and low-immunity groups. Differential expression analysis was undertaken on HCC samples, with normal samples as the control. After immune-related lncRNAs and differentially expressed lncRNAs were intersected, 321 differentially expressed immune-related lncRNAs were acquired. Later, the prognostic model based on immune-related lncRNAs was obtained following the Cox regression analysis of previous samples. According to the riskScore, the samples in TCGA-LIHC were divided into high- and low-risk groups. Kaplan-Meier survival analysis, ROC curve, and independence analysis confirmed that the immune-related lncRNAs prognostic model was an important factor independent from clinical characteristics. We further analyzed the difference in immune microenvironment and mutational landscapes in both risk groups. Prominent differences were shown in multiple immunity-related gene sets and immune cells in both groups. The mutation rate of TP53 in high-risk group was much higher than the low-risk one. All these conclusions offered references to prognostic evaluations and personalized treatments for patients with HCC.

Candida albicans is a common opportunistic pathogen that can cause serious infection by blood transmission. C. albicans enters the blood circulation and adheres to the endothelial cells of the vascular wall. However, the detailed mechanism of the effect of C. albicans on the endothelial cells remains unclear. In this study, the microarray expression profile of human umbilical vein endothelial cells exposed to C. albicans was analyzed. The 191 up-regulated genes were enriched in TNF, T cell receptor, and NF-kappa B signaling pathways. The 71 down-regulated genes were enriched in pyruvate metabolic, purine nucleotide metabolic, purine nucleotide biosynthetic, and humoral immune response processes. Gene set enrichment analysis showed that apoptosis, oxidative phosphorylation, IL6/JAK/STAT3 signaling pathways were enriched. Moreover, two hub genes with a high degree of connectivity, namely, MYC and IL6, were selected. Molecular screening of traditional Chinese medicine libraries was performed on the basis of the structure of MYC protein. The okanin had the highest docking score. MYC might be used as molecular targets for treatment. In addition, okanin may inhibit the infection of C. albicans. Thus, MYC can be subjected to further research.

Because the underwater channel environment is complicated, it is difficult to do an actual experiment in the ocean to analyze the performance of underwater wireless optical communication (UWOC) systems. In this study, the spot-expansion characteristics and time-domain broadening characteristics of underwater wireless optical signals are simulated and analyzed by a Monte Carlo statistical method. Thus, what we believe is an improved underwater channel transmission-attenuation model and time-domain broadening model based on UWOC are proposed, so the transmission distance characteristics of the UWOC system are obtained by combining the system parameters, and the transmission-rate characteristics can be analyzed by using the Shannon-Hartley theorem. The results show that the transmission distance is linear with the receiver sensitivity, and the transmission rate decays exponentially with the transmission distance and is limited by the receiver sensitivity in the UWOC system.

A division of focal plane (DoFP) polarimeter includes an array of polarized pixels. The response characteristics of polarized pixels are directly affected by inherent defects of a DoFP polarimeter. Correspondingly, the response characteristics are crucial to correction of the inherent defects. However, research on the response characteristics is rarely reported. Therefore, this paper proposes a pixel response model for a DoFP polarimeter. The response model combines the response characteristics of a traditional photoelectric imager and a micro-polarizer array. The proposed model includes six input parameters. They are the major polarization responsivity, minor polarization responsivity, polarization orientation, exposure time, conversion gain, and gamma correction. An experimental setup is constructed to measure the response of a DoFP polarimeter. The proposed model is evaluated by comparing the calculated results and the measured results. The compared results under different artificial parameters show that the each average root-mean-square error value is less than one gray value, which proves the validity of the proposed model.

One phenomenon or social institution often observed in multi-agent interactions is the altruistic punishment, i.e. the punishment of unfair behavior by others at a personal cost. Inspired by the works focusing on punishment and the intricate mechanism behind it, we theoretically study the strategy evolution in the framework of two-strategy game models with the punishment on defectors, moreover, the cost of punishing will be evenly shared among the cooperators. Theoretical computations suggest that larger punishment on defectors or smaller punishment cost incurred by cooperators will enhance the fixation of altruistic cooperation in the population. Through the replicate dynamics, the group size of the randomly selected individuals from the sufficiently large population will notably affect the strategy evolution in populations nested within a dilemma. By theoretical modeling the concept of shared cost for punishment from one point of view, our findings underscore the importance of punishment with shared cost as a factor in real-life decisions in an evolutionary game context.

BACKGROUND AND AIM: Long non-coding RNAs have been confirmed to play a critical role in various cancers. In the present study, the effect of long non-coding RNA (lncRNA) CCAT1 on glioma cell proliferation and its potential mechanism were investigated. METHODS AND RESULTS: Real-time PCR results showed that lncRNA-CCAT1 expression was significantly upregulated in glioma cancer tissues and cell lines compared with controls. After inhibiting CCAT1 expression in glioma cell line U251 with siRNA-CCAT1 (si-CCAT1), the cell viability and cell colony formation were decreased, the cell cycle was arrested in G1 phase, and the cell apoptosis was increased. As reported in bioinformatics software starbase2.0, a total of 22 microRNAs were potentially targeted by CCAT1. It was confirmed that miR-410 was altered most by si-CCAT1. After up-regulating CCAT1 expression in U251 cells, miR-410 level was decreased. Luciferase reporter assay confirmed that CCAT1 targeted miR-410. Correlation analysis showed that CCAT1 expression was negatively related to miR-410 expression in glioma cancer tissues. In addition, down-regulation of miR-410 reversed the inhibitory effect of si-CCAT1 on glioma proliferation. CONCLUSION: These data demonstrated that lncRNA-CCAT1 promoted glioma cell proliferation via inhibiting miR-410, providing a new insight about the pathogenesis of glioma proliferation.

RESUMEN

We propose a novel super-resolution 3D II method using a time-multiplexed coding mask for improving the resolution of 3D imaging based on compressive sensing (CS) theory. Instead of sensing raw pixel data, the recording device measures the compressive samples of the observed 3D scene through a coding mask placed in the aerial pickup plane in a 3D II system. With the aid of CS framework, we design an optimum coding mask pattern and use the time-multiplexed scheme to achieve a sequence of low-resolution elemental images (EIs), which contain the subpixel details of the observed 3D scene. The super-resolution EIs array is further recovered by an optimization algorithm. Both computational reconstruction and optical experimental results show the validity of the proposed method.

Transforming growth factors ß (TGF-ß) pathway has been proven to play important roles in oncogenesis and angiogenesis of gliomas. MiR-132 might be related to TGF-ß signaling pathway and high miR-132 expression was reported to be a biomarker of poor prognosis in patients diagnosed with glioma. However, the expression regulation way involved in TGF-ß pathway and clinical significance of miR-132 have not been investigated in glioma cells. Here we reported that the mRNA level of miR-132 and TGF-ß concentration were both increased in patients with brain glioma. Correlation analysis revealed that TGF-ß concentration was positively correlated with mRNA level of miR-132. In addition, the mRNA level of miR-132 was up-regulated by TGF-ß in a concentration-dependent and time-dependent manner. Furthermore, we found that miR-132 was involved in modulation of the TGF-ß signaling pathway and down-regulation of SMAD7 expression by directly targeting the SMAD7 3'-UTR. MiR-132 was negatively correlated with SMAD7 in patients with brain glioma. Taken together, our results suggest that miR-132 could be stimulated by TGF-ß and might enhance the activation of TGF-ß signaling through inhibiting SMAD7 expression in glioma cells. These findings contribute to a better understanding of the mechanism of the activation of TGF-ß signaling by miR-132.

We propose a novel three dimensional integral imaging display system with improved viewing angle using a monocentric lens array (MoLA) coupled with fiber bundle. In conventional integral imaging, the off-axis aberrations of the conventional lens array limit the viewing angle in the display stage. The key to our design is a MoLA that eliminates most of the off-axis aberrations and generates a wide-angle image on a spherical surface. The fiber bundle acts as relay optics from the flat-panel display to spherical focal plane of the MoLA. The viewing angle enhancement of the proposed method is analyzed, and the achromatic condition is deduced for the MoLA to correct the chromatic aberration. The experimental result illustrates the capabilities of the proposed method.

The aim of this study is to develop a physiologically based pharmacokinetic (PBPK) model in intra-abdominal infected rats, and extrapolate it to human to predict moxifloxacin pharmacokinetics profiles in various tissues in intra-abdominal infected human. 12 male rats with intra-abdominal infections, induced by Escherichia coli, received a single dose of 40 mg/kg body weight of moxifloxacin. Blood plasma was collected at 5, 10, 20, 30, 60, 120, 240, 480, 1440 min after drug injection. A PBPK model was developed in rats and extrapolated to human using GastroPlus software. The predictions were assessed by comparing predictions and observations. In the plasma concentration versus time profile of moxifloxcinin rats, Cmax was 11.151 µg/mL at 5 min after the intravenous injection and t1/2 was 2.936 h. Plasma concentration and kinetics in human were predicted and compared with observed datas. Moxifloxacin penetrated and accumulated with high concentrations in redmarrow, lung, skin, heart, liver, kidney, spleen, muscle tissues in human with intra-abdominal infection. The predicted tissue to plasma concentration ratios in abdominal viscera were between 1.1 and 2.2. When rat plasma concentrations were known, extrapolation of a PBPK model was a method to predict drug pharmacokinetics and penetration in human. Moxifloxacin has a good penetration into liver, kidney, spleen, as well as other tissues in intra-abdominal infected human. Close monitoring are necessary when using moxifloxacin due to its high concentration distribution. This pathological model extrapolation may provide reference to the PK/PD study of antibacterial agents.

Considering the limited pixel number and large pixel size of common display panel, the captured elemental images (EIs) array of high density pixels cannot be reconstructed sufficiently in the display process of integral imaging, because of matched display requirement. To solve this problem, this paper presents a novel approach to improve integral imaging resolution by designing a coded sub-pixel mask on common display panel. Specifically, multi-pixels in the captured EIs are displayed in a pixel in the common display panel with time multiplexing along with the corresponding aperture switched on/off of the coded sub-pixel mask periodically, in which the resolution of the reconstructed image is determined by the coded aperture size of the sub-pixel mask rather than the pixel size of the display panel. Then, the mapping relationship between the displayed pixel and the position of the switched on aperture of the coded sub-pixel mask is established theoretically. Computational reconstruction and optical experimental results show that this method can match the pixel number of the captured EIs with that of the display panel and the resolution of integral imaging can be improved significantly.

To solve the pseudoscopic problem, we propose a one-step integral imaging system with negative refractive index materials, which can avoid the deterioration in resolution inherent to the optical or digital two-step processes. Specifically, the proposed method is based on the novel feature of negative refractive index materials, bending light to a negative angle relative to the surface normal. The pseudoscopic imaging property of the negative refractive index material slab is theoretically investigated. For formation of orthoscopic reconstructed images, the matching condition of the negative index lens array and the positive index lens array is deduced. Two types of conceptual prototypes of integral imaging system with negative refractive index materials are designed. Experimental results show the validity of the proposed method. To the best of our knowledge, this is the first time to explore the application of negative index materials in eliminating the pseudoscopic effect in integral imaging.

A method for improving the viewing quality of integral imaging (II) is proposed, based on using an aperture-tunable lens array (LA). The proposed method uses a liquid crystal (LC) panel without a backlighting unit to tune the aperture of an LA dynamically. The shape and transmittance of the aperture can be controlled arbitrarily by programming the state of the pixels on the LC panel. Adding the temporal multiplexing technique, the viewing quality can be improved by the after-image effect of the human eye. Moreover, the relationships between the lateral resolution and the aperture tuning pattern and the depth of field and the aperture tuning pattern are derived, respectively. The product of the depth of field, the lateral resolution squared, and the lateral viewing range is proposed as a new figure of merit for an II system. Experimental results show the validity of the proposed method.

Retrogression and re-aging (RRA) treatment has been proven to effectively overcome the trade-off between strength and corrosion resistance. Current research focuses on the heating rate, temperature, and holding time of retrogression treatment while ignoring the retrogression cooling ways. In this paper, the effects of RRA treatment with different retrogression cooling ways on the microstructure and properties of newly developed T'/η' strengthened Al-Zn-Mg-Cu alloys were investigated by performing tests on mechanical properties, intergranular corrosion (IGC) resistance, and electrochemical corrosion behavior. The results show that the mechanical properties of samples subject to RRA treatment with water-quenching retrogression (ultimate tensile strength, yield strength, and elongation of 419.2 MPa, 370.2 MPa, and 15.9, respectively) are better than those of air-cooled and furnace-cooled samples. The corrosion resistance of water-quenching (IGC depth of 162.2 µm, corrosion current density of 0.833 × 10-5 A/cm2) and furnace-cooled samples (IGC depth of 123.7 µm, corrosion current density of 0.712 × 10-5 A/cm2) is better than that of air-cooled samples. Microstructure characterization reveals that the effect of the retrogression cooling rate on mechanical properties is related to the size of T'/η' precipitates with grains as well as the proportion of T' and η', while the difference in corrosion resistance depends on the continuity of grain boundary precipitates (GBPs). With mechanical properties, corrosion resistance, and time cost taken into consideration, it is appropriate to select water quenching for retrogression. These findings offer valuable insights for further design to achieve superior performance in various applications.

Background: Acute respiratory distress syndrome (ARDS) is a severe complication that can lead to fatalities in multiple trauma patients. Nevertheless, the incidence rate and early prediction of ARDS among multiple trauma patients residing in high-altitude areas remain unknown. Methods: This study included a total of 168 multiple trauma patients who received treatment at Shigatse People's Hospital Intensive Care Unit (ICU) between January 1, 2019 and December 31, 2021. The clinical characteristics of the patients and the incidence rate of ARDS were assessed. Univariable and multivariable logistic regression models were employed to identify potential risk factors for ARDS, and the predictive effects of these risk factors were analyzed. Results: In the high-altitude area, the incidence of ARDS among multiple trauma patients was 37.5% (63/168), with a hospital mortality rate of 16.1% (27/168). Injury Severity Score (ISS) and thoracic injuries were identified as significant predictors for ARDS using the logistic regression model, with an area under the curve (AUC) of 0.75 and 0.75, respectively. Furthermore, a novel predictive risk score combining ISS and thoracic injuries demonstrated improved predictive ability, achieving an AUC of 0.82. Conclusions: This study presents the incidence of ARDS in multiple trauma patients residing in the Tibetan region, and identifies two critical predictive factors along with a risk score for early prediction of ARDS. These findings have the potential to enhance clinicians' ability to accurately assess the risk of ARDS and proactively prevent its onset.

Colorectal cancer (CRC) has a high degree of heterogeneity and identifying the genetic information of individual tumor cells could help enhance our understanding of tumor biology and uncover potential therapeutic targets for CRC. In this study, we identified LPCAT2+ tumor cell populations with less malignancy than LPCAT2- tumor cells in human and mouse CRC tissues using scRNA-seq. Combining in vitro and in vivo experiments, we found that LPCAT2 could inhibit the proliferation of CRC cells by inducing ferroptosis. Mechanistically, LPCAT2 arrested PRMT1 in cytoplasm of CRC cells via regulating acetylation of PRMT1 at the K145 site. In turn, PRMT1 enhanced SLC7A11 promoter activity. Thus, LPCAT2 attenuated the positive regulatory effect of PRMT1 on SLC7A11 promoter. Notably, SLC7A11 acts as a ferroptosis regulator. Furthermore, in LPCAT2 knockout mice (LPCAT2-/-) colon cancer model, we found that LPCAT2-/- mice exhibited more severe lesions, while PRMT1 or SLC7A11 inhibitors delayed the progression. Altogether, we elucidated that LPCAT2 suppresses SLC7A11 expression by inhibiting PRMT1 nuclear translocation, thereby inducing ferroptosis in CRC cells. Moreover, inhibitors of the PRMT1/SLC7A11 axis could delay tumor progression in CRC with low LPCAT2 expression, making it a potentially effective treatment for CRC.
