RESUMO

These studies present evidence that when human epidermal cells are grown in culture they lose both the ability to stimulate the proliferation of allogeneic T lymphocytes in vitro and their expression of HLA-DR antigens. Our results also show that epidermal cells incubated with anti-HLA-DR serum lose their ability to stimulate the proliferation to allogeneic T lymphocytes in a mixed skin cell-lymphocyte reaction.

In a 65 year old man with hemoptysis, chest pain, weight loss, clubbing of the digits and a large mediastinal mass, the superior vena cava syndrome developed. He was treated for malignancy with radiation therapy and corticosteroids, but he died shortly after his admission to the hospital. Autopsy revealed syphilitic aortitis with an aneurysm of the ascending aorta compressing the superior vena cava and right mainstem bronchus. The postmortem serology corroborated the morphologic findings of tertiary syphilis. In this report we emphasize the important, although now uncommon, association between the superior vena cava syndrome and aneurysm of the aorta.

A 65 year old male with the entity, scleromyxedema, experienced exacerbation of the disease in which the main clinical features involved the central nervous system. He presented with clouded sensorium, disorganized thinking, combative behavior, headache, unsteady gait and grand mal seizures. A few days after hospital admission the symptoms abated. After a 6 day hiatus, the symptoms suddenly recurred, continuing for another week. The symptomatology again suddenly ceased with complete clearance of mental status. During the full-blown delirium, the electroencephalogram had demonstrated diffuse slowing while lumbar puncture, brain scan, E.M.I. scan and cerebral arteriogram failed to contribute to the understanding of the clinical presentation. Scleromyxedema rarely involves the central nervous system. This case illustrates a very unusual manifestation of scleromyxedema, prominent central nervous system involvement presenting as an acute organic brain syndrome. It is the only case which includes formal mental status examination, cerebrospinal fluid findings and electroencephalogram results.

BACKGROUND: Many practitioners perform a thyroid stimulating hormone (TSH) assay as a screening test in older patients. The introduction of sensitive TSH assays with lower normal limits has created a laboratory abnormality that is often of uncertain significance. Mechanisms include autonomous overproduction of thyroid hormone, nonthyroidal illness including medication effects, and hypothalamic/pituitary dysfunction. OBJECTIVE: To characterize the clinical status and course of nursing home residents with low TSH and normal total T4 levels in the absence of treatment with thyroid hormone. DESIGN: Retrospective chart review was performed to determine participants status at the time of low TSH level, with additional recording of follow-up thyroid hormone levels, cardiac rhythm, and mortality. Mortality was compared with that of a control group matched for age and sex. SETTING: A nursing home for veterans and their spouses. MAIN RESULTS: Forty subjects with low TSH and initially normal total T4 were identified. Only three subjects were subsequently diagnosed as hyperthyroid. TSH levels of 18 subjects subsequently normalized, and four additional subjects had low total T3 levels suggesting a nonthyroidal mechanism. Seven subjects died during the first 4 months of follow-up compared with three in a control group (P < .001). Nine of the 40 subjects had a history of or current atrial fibrillation. No new atrial fibrillation was documented during 388 months of EKG follow-up. CONCLUSIONS: Low total T3 levels, TSH normalization, and excess mortality suggest that nonthyroidal illness plays a role in the pathogenesis of low TSH determinants in the nursing home. Autonomous production of thyroid hormone also plays a role. We believe that the term "subclinical hyperthyroidism" should be used only if the clinician believes that autonomous overproduction of thyroid hormone is the cause of a low TSH level. If subsequent research shows correctable adverse consequences associated with subclinical hyperthyroidism from autonomous overproduction of thyroid hormone, a more aggressive diagnostic approach will be needed to define the mechanism of a low TSH level at the time of its discovery.

Nuclear DNA content of paraffin-embedded tissue from 19 clinically benign adrenal pheochromocytomas and 18 control adrenal glands was analyzed using flow cytometry. All control adrenal glands and 6 pheochromocytomas were diploid. Four tumors were tetraploid. Nine were aneuploid with relative DNA indices in the near diploid range in 2, in the peritriploid range in 5, and in the near tetraploid range in 2. These results indicate that aneuploid DNA content is a frequent occurrence in benign adrenal pheochromocytomas. Aneuploidy per se is not a specific marker of malignancy in these tumors as has been suggested by previous reports.

RESUMO

Three to 4 cm-diameter, circular-shaped full thickness sections of the anterior wall of canine bladders were resected. Free grafts of bladder mucosa were lifted from the muscular wall and sewn back as a patch onto the original defect. Fusion of the graft and normal urothelium occurred, and at eight weeks a dense fibrotic external layer and normal urothelium was observed. No diverticuli occurred. A successful closure of recurrent multiple vesicovaginal fistulas in a woman using mucosa alone is reported.

As described in the above paragraphs, efforts in the areas of immunology, cytogenetics, molecular biology, and cytopathology have been applied to the problems urologists face in treating the individual patient with bladder cancer. It is hoped that continued investigations in these disciplines will not only lead to earlier diagnosis of bladder cancer and better prediction of tumor behavior but will further our understanding of the molecular basis of oncogenesis.

We report the pathologic findings in specimens submitted for histologic and cytologic evaluation from 67 patients with the acquired immunodeficiency syndrome. A wide variety of opportunistic pathogens were identified in 41 patients. Mycobacterium avium-intracellulare evoked only a mild host response: granulomas, if present, were poorly formed. Biopsy specimens showing cytomegalovirus gastroenteritis required sections at multiple levels to demonstrate inclusions. Combined histologic and cytologic evaluation can increase the diagnostic yield in pulmonary and esophageal infections. Kaposi's sarcoma was found in biopsy specimens from 29 patients. Early lesions were often extremely subtle, yet distinct from, benign vascular proliferations in involuted lymph nodes. Malignant lymphoma was diagnosed in ten homosexual men who were suspected of having the acquired immunodeficiency syndrome. The lymphomas were characterized by B-cell origin, a diffuse pattern, frequent extranodal presentations, and an aggressive clinical course with prominent central nervous system involvement.

Malignant lymphomas occurring in 29 homosexual men and one thalassemic woman with the acquired immunodeficiency syndrome or the acquired immunodeficiency syndrome-related complex are reported using a working formulation for non-Hodgkin's lymphomas (NHLs). The patients' ages ranged from 25 to 59 years, with an average age of 42 years. Ninety percent of the cases were extranodal; 67% were exclusively extranodal. One case of Hodgkin's disease was encountered. All NHLs were of the diffuse types in both the intermediate- and high-grade categories, with the largest single group (49%) being of the diffuse, large, follicular-center-cell types. The NHLs in this series were classifiable as B-cell neoplasms and were aggressive as evidenced by markedly reduced median survivals. The morphological diagnosis as defined in the working formulation, especially for the intermediate-grade lesions, offered little significant prognostic information.

We investigated the possibility that PGE1, at approximately the minimal seminal plasma concentration reported to be associated with human fertility (3), placed in the vaginas of female dogs might produce local vasodilation and thereby increase vaginal blood flow. Fifteen micrograms of PGE1 in 1 ml, 38 degrees C saline increased vaginal blood flow 2.5 times (0.27 +/- 0.10 to 0.67 +/- 0.10 ml/g X min) as compared to control (1 ml, 38 degrees C saline, intravaginally) without altering arterial pressure, hepatic blood flow, or renal blood flow. We conclude that seminal fluid PGE1 can produce vaginal vasodilation. The role of seminal fluid PG induced increases in vaginal blood flow upon reproductive success, if any, is entirely speculative.

BACKGROUND: The nursing home clinician may encounter euthyroid residents with low total serum thyroxine (T4) values associated with certain medications or nonthyroidal illness (NTI). Hypothyroidism may be incorrectly diagnosed and thyroid hormone prescribed. METHODS: We reviewed all T4 determinations and individuals with low T4 and normal thyroid-stimulating hormone (TSH) levels were identified. Pharmacy and medical records were reviewed. RESULTS: We reviewed 1,153 T4 determinations and identified 22 individuals with low T4 and normal TSH values. Eight of the 22 (36%) were being treated with high-dose salicylates, 4 (18%) with phenytoin, 3 (14%) with carbamazepine, and 2 (9%) with prednisone (15 mg/day). Three residents were on two of these medications. None was on prednisone as a single medication. Eight were on none of the medications, and 5 of these had albumin values of < or = 3.2 g/dL (a marker of NTI). After a low T4 value was determined in these individuals, 6 of the 22 were placed on T4 replacement with no documentation of hypothyroidism. In 5 of these, low T4 level could be attributed to a medication effect. CONCLUSIONS: Low total T4 with normal TSH levels are commonly associated with medications and NTI.

Seventeen women masturbated to orgasm several times in succession while being measured intravaginally by a device that allows continuous oxygen and blood flow readings. Analysis of covariance showed significant differences between fantasy and orgasm and between orgasm and interorgasm relaxation periods. The data do not provide physiological evidence that successive orgasms are either physiologically or subjectively stronger but do provide physiological evidence of a plateau phase of sexual response in women.

We evaluated image cytometric deoxyribonucleic acid ploidy analysis of urine sediments as an adjunct to visual cytology in the detection of urothelial cell carcinoma. Both procedures were performed on 384 specimens obtained from voided urine (78%), bladder washings or by catheterization. Of these specimens 235 came from patients with active urothelial cell carcinoma, 23 from patients with currently inactive urothelial cell carcinoma and 126 from patients with benign disorders of the urinary tract. Visual cytology classified 171 specimens as positive for urothelial cell carcinoma, 143 as negative and 70 as atypical but diagnostically ambiguous. Sensitivity was 72.3% (170 of 235) and specificity was 99.3% (148 of 149, with atypical and negative specimens grouped together). The slides were then restained for ploidy analysis. Each of the deoxyribonucleic acid histograms of the 170 true positive and the 129 true negative specimens was described by a set of order statistics. These sets were entered into a multivariate discriminant analysis. The discriminant function obtained was then applied to the 70 ambiguous specimens. As a result 27 of these specimens, all from patients with active urothelial cell carcinoma, were reclassified as positive. Sensitivity was increased to 83.8% (197 of 235), with no loss of specificity. Image cytometry is a useful adjunct to visual cytology for the detection of bladder cancer in urine sediments.

PGE1 occurs in high concentration in seminal fluid and is apparently necessary for normal fertility, although its specific function(s) is not known. We investigated the possibility that PGE1, at seminal fluid concentration, could increase cervical blood flow and, thus, potentially alter cervical mucus properties, including O2 concentration. Fifteen micrograms PGE1/ml placed directly on the cervix uteri increased cervical blood flow in anesthetized dogs from 0.8 +/- 0.7 to 5.7 +/- 5.1 ml/g.min 5 min following application without affecting arterial pressure or hepatic or renal blood flow. Seminal PGE1 may have effects upon oxygen tension in the cervical mucus, as well as aiding leukocyte accumulation in the cervical canal, by increasing cervical blood flow.

Antral gastrocystoplasty has been previously described as an effective method of bladder augmentation or replacement. Six dogs underwent gastrocystoplasty. Two bladders were augmented utilizing the gastric fundus, one antral augmentation was performed, and three total bladder replacements were created utilizing the antrum. Each dog was followed for at least twelve months or until death from peptic ulcer disease with periodic evaluation for urinary continence, serum gastrin level, serum electrolytes, urine culture, urine pH and the radiographic appearance of the urinary reservoir and upper tracts. All four dogs with antral gastrocystoplasties developed severe hypergastrinemia and two demonstrated gross ulcers. In contrast, the two dogs with fundic augmentation did not show elevation of serum gastrin levels or evidence of peptic ulcer disease. Severe hypergastrinemia can be expected following antral gastrocystoplasty and the resultant ulcer disease can be fatal.

Nuclear DNA content of paraffin-embedded tissue from 48 adrenocortical neoplasms, 18 histologically normal control adrenal glands, and five hyperplastic adrenal glands was analyzed retrospectively using flow cytometry. Aneuploidy was compared with morphologic criteria as a predictor of recurrence. All 18 controls, five hyperplastic glands, and 39 neoplasms were diploid. Nine neoplasms were aneuploid. Compared with their diploid counterparts, aneuploid neoplasms were more likely to weigh more than 50 g (P less than 0.0001) and to have three or more histologic features of carcinoma (P less than 0.0001). Thirty-six neoplasms were followed clinically for at least 2 years (range 24 to 120 months, mean = 64.6 months) or until local recurrence, metastasis, or death. Five were clinically malignant. Neoplasms which recurred or metastasized were more apt to be aneuploid (P less than 0.005) than those showing no evidence of further disease during the follow-up period. They were also more likely to weigh more than 50 g (P less than 0.005) and to have three or more histologic features of carcinoma (P less than 0.0025). However, neither aneuploidy, large size, nor unfavorable histology result was a consistent feature in every malignant neoplasm. Flow cytometric DNA content analysis appears to be as effective a predictor of clinical outcome as size and histology and may be of particular value when the morphologic features are ambiguous.

The threshold for the dark interval between two flashes was used to examine differences in stimulus persistence in 72 subjects aged 20 to 79 years. The data were then combined with results from two previous studies involving children and adolescents; thus, a cross sectional, adult, life-span view of differences in the dark-interval threshold was presented. Results showed that during childhood and adolescence there is a decline in stimulus persistence as a result of decreased receptor sensitivity. In adulthood, however, the dark-interval threshold remains fairly stable until the 70s where it increases significantly. Two possible explanations for this increase in stimulus persistence in old age are discussed.
