RESUMO

Acute coronary syndrome (ACS) is a leading cause of mortality worldwide. Several studies have shown that certain single nucleotide polymorphisms (SNPs) are linked to the development of ACS. In particular, C-reactive protein (CRP) has emerged as an important predictive biomarker for cardiovascular disease. The current study aimed to investigate four polymorphisms of the CRP gene as possible biomarkers for ACS in a sample of 252 individuals (114 patients with ACS and 138 healthy controls) from Southeastern Mexico. Multivariate analysis adjusted for clinical variables showed that the polymorphism 3872CT for the genotype CC/CT [adjusted Odds Ratio (AdOR)=3.78; 95% Confidence Interval (CI): 1.11-12.92; P=0.034] and the genotype GG/GC of the polymorphisms 2667CG (AdOR=4.82; 95% CI: 1.69-13.72; P=0.02) were associated with ACS. However, the polymorphisms 3006AC genotype AA/AC and 5237GA genotype GG/GC were not found to be associated in the multivariate analysis with ACS (P>0.05). These results suggested that 3872CC/CT and 2667CC/CG polymorphism of the CRP gene plays a significant role in the development of ACS.

OBJECTIVE: To evaluate the impact that the moderate and regular consumption of Cinco Jotas acorn-fed 100% Iberian ham has on overall cardiovascular risk, lipid parameters, blood pressure, and weight. METHODS: A longitudinal, analytical, and quasi-experimental clinical study with repeated measures was carried out with 100 randomly selected individuals in primary care. The sample population included men and women (64%) between the ages of 25 and 55 (42.08, SD 9.6) who were not diagnosed with any cardiovascular illness or diabetes, were not undergoing antihypertensive treatment, nor taking lipid-lowering drugs. There were four visits during a 2-week washout period for the first of three phases. Phases 2 and 3 included an 8-week habitual diet phase followed by an 8-week intervention phase when participants consumed 40 g daily of acorn-fed 100% Cinco Jotas Iberian ham. Measurements of cardiovascular risk factors were taken following the SCORE table. These included total cholesterol, high-density lipoproteins cholesterol (HDL-c), low-density lipoproteins cholesterol (LDL-c), triglycerides (TG), and weight. RESULTS: The average vascular risk (SCORE) was 0.20 (SD 0.49) before the consumption phase and 0.18 (SD 0.48) at the end of the study (p > 0.05). An increase in HDL-c of 5 mg/dl was observed while there was a decrease in LDL-c and TG of 10 mg/dl (p < 0.05). There were no differences in total cholesterol levels, blood pressure, or weight; nor were differences observed in average consumption of calories, protein, lipids, carbohydrates, or alcohol (p > 0.05). CONCLUSIONS: The daily consumption of 40 g of Cinco Jotas acorn-fed 100% Iberian ham does not increase the risk of cardiovascular disease and has a favorable impact on lipid levels without affecting blood pressure or weight.

OBJECTIVE: To describe a collaborative practice focused on coping with the occupational stress among nursing staff in a hospital setting. METHOD: These practices focus on the contextualization of the problems and the design and implementation of actions using the psychopolitical model and the participatory action research (PAR) methodology. Participants were the nurses of 4 units of internal medicine at the public hospital "Virgen Macarena" in Seville. We have used the ISTAS questionnaire, interviews and discussion groups through which nurses and researchers have assessed, defined, proposed and implemented different actions in order to improve their work conditions. RESULTS: Problematic situations detected by the questionnaires are associated to psychological demands, role conflicts and esteem. The main cause of stress in healthcare professionals is the lack of staff, according to the opinion laid by supervisors. In the discussion groups, nurses accorded to get involved in three situations: a) the need of the continuous presence of an orderly to move patients which aren't autonomous; b) the need of controlling visit hours and the number of accompanying people with each patient; and c) the need to improve the registration of the activities assigned to nursing staff. CONCLUSIONS: Among the strength of the psychopolitical model and PAR in a hospital context we must emphasize on the mobilization of professionals and the development of a critical consciousness. Among the weakness, those derived from bureaucratic processes. These barriers imply a challenge for change and organizational development.

Depression is a common disease that may cause severe damage to human health. Imipramine (IMI) and desipramine (DES) are medicaments used for treatment, yet studies on their genotoxic potential have given controversial results. Therefore, we designed the present assay to determine their effect as inducers of micronucleated polychromatic erythrocytes (MNPE) and micronucleated normochromatic erythrocytes (MNNE) in mice. The study was carried out in animals administered daily with the compounds for 4 weeks, and the determination of micronuclei was done each week. We also evaluated the bone marrow cytotoxicity induced by the chemicals. Besides, the same determinations were carried out in the following 4 consecutive weeks, but in this period the animals were not treated with the tested compounds. Our results showed a significant increase in both MNPE and MNNE induced by both compounds from the first week of administration. At the fourth week, IMI increased three times the control level, while the effect of DES was about seven times such level. In the second, 4-week phase, we observed a reduction in the rate of micronuclei approaching the control level. We also detected a bone marrow-mitotic division decrease by the evaluated chemicals. Our results point to the need for cautiousness in the clinical use of the compounds as well as for testing the effect in patients under treatment.

Introducción: Las anormalidades cromosómicas son una causa frecuente de morbilidad y mortalidad en la población humana. Objetivo: Describir el número y tipo de las alteraciones cromosómicas numéricas y estructurales detectadas en estudios citogenéticos realizados prenatalmente y en recién nacidos en el Instituto Nacional de Perinatología (INPer) durante el periodo comprendido de enero a diciembre del 2003. Metodología: Realizar un estudio descriptivo de tipo retrolectivo de los pacientes revisados por el Departamento de Genética con defectos congénitos que presentaron anormalidades cromosómicas. Resultados: Durante el año 2003, 3.26% (189/5795) de los pacientes nacidos en el INPer presentaron defectos al nacimiento, de los cuales veintisiete pacientes mostraron un cariotipo anormal de los cuales 21 (77.7%) presentaron alteraciones cromosómicas numéricas; además, en seis (22.2%) se encontró una alteración cromosómica estructural, lo que representa 0.46% de los pacientes nacidos en el INPer. En seis casos el diagnóstico se realizó en etapa prenatal y se corroboró al nacimiento. Conclusiones: La mayoría de alteraciones cromosómicas se presentan con múltiples defectos al nacimiento y con alteración en el crecimiento y desarrollo mental. Es importante que ante la presencia de pacientes con múltiples defectos mayores estructurales se sospechen este tipo de alteraciones y se realicen los estudios necesarios a la familia para poder brindar un adecuado asesoramiento genético.


Introduction: Chromosomal anomalies are a frequent cause of human disease. Objective: Describe the numerical and structural chromosomal anomalies detected by cytogenetic studies done prenatally and in newborns found in the Instituto Nacional de Perinatología during the period between January and December 2003. Methods: Descriptive study of the patients with congenital defects seen by the Genetics Department who presented chromosomal anomalies. Results: During the year 2003, the 3.46% (189/5795) of the babies born at the INPer had structural anomalies. Twenty patients had a chromosomal anomaly of which 21 (77.7%) had a chromosomal numeric alteration and six (22.2%) a chromosomal structural anomaly which represented 0.46% of the newborns. In six cases the diagnosis was done prenatally and confirmed at birth. Conclusions: Most of the chromosomal anomalies present themselves with multiple congenital anomalies and retarded growth and development. It is very important to implement this type of studies in patients with congenital anomalies, complete the familiar study and provide an accurate genetic counseling to the parents.
