BACKGROUND: The healthcare practice of dentistry, as well as medicine, is framed within a legal environment. Patients have the right to know all the information related to any action performed on them and dental or medical doctors are obliged to obtain their patient's prior written informed consent (IC) before undertaking any healthcare procedures. MATERIAL AND METHODS: Here we reviewed the legality and jurisprudence in Spain regarding IC. We also used INFLESZ text readability analysis software to analyse a sample of official Spanish informed consent documents (ICDs) from different surgical and interventional procedures related to dentistry and oral cavity interventions. RESULTS: It is a mistake to confound IC with ICDs. This error prevents physicians from considering the former as a care process in which the patient's authorisation signature is the last link in a chain formed, almost in its entirety, by the informative process and deliberation alongside the patient. Multiple factors can influence communication between practitioners and their patients. Importantly, treatment adherence is greater when patients feel involved and autonomous in shared decision-making and when the circumstances of their lives are adequately considered. We concluded that although the ICDs we analysed conformed to the requirements set out in international law, they were somewhat difficult to read according to the reading habits of the general Spanish population. CONCLUSIONS: Knowledge about the legality of IC helps professionals to understand the problems that may arise from their non-compliance. This is because the omission or defective fulfilment of IC obligations is the origin of legal responsibility for medical practitioners. In this sense, to date, there have been more convictions for defective ICs than for malpractice. The information provided in ICs should include the risks, benefits, and treatment alternatives and must be tailored to the needs and capabilities of the patient to enable autonomous decision-making.

RESUMO

Malignant lymphomas represent approximately 5% of all malignant neoplasms of the head and neck area. They are classically divided into two subgroups, Hodgkin's lymphomas (HLs) and non-Hodgkin's lymphomas (NHLs). We describe the clinical characteristics of head and neck lymphomas and the methods to establish the diagnosis. The World Health Organization classification of lymphoid tissues describes more than 50 different histological types, and we analyse the most common staging system for lymphomas, the Ann Arbor staging system. Finally, the different therapeutic approaches are discussed.

RESUMO

The placement of central catheters through the subclavian and jugular venous path can be complicated by the cannulation of an artery or an aberrant venous path. The most frequent anomaly of the embryological development of the caval vein is the persistence of the left superior vena cava (LSVC). The implantation of catheters in the LSVC can be suspected by its anomalous route in thorax radiography. Gasometry and the pressure curve of the vessel make it possible to rule out an arterial catheterisation. Diagnostic confirmation is obtained through angiography, echocardiography, computerised tomography or cardiac resonance. The doctor who regularly implants central venous catheters must be familiar with the anatomy of the venous system and its variants and anomalies, since their presence might influence the handling of the patient.

CONCLUSIONS: The postoperative course was excellent for this type of surgery, and the functional recovery was comparable to that obtained with much more laborious techniques. OBJECTIVES: To compare the advantages and disadvantages of the described technique and oropharyngectomy with labial mandibulotomy. PATIENTS AND METHODS: A total of 46 patients underwent surgery by means of an oropharyngectomy without mandibulotomy. The pharynx was reconstructed using a plasty made of four regional flaps. RESULTS: In addition to obvious esthetic benefits, complications of the osteotomy were absent and surgical time was reduced. Some patients undergoing pull-through oropharyngectomy also underwent a marginal mandibulectomy, markedly reducing the frequency of radionecrosis compared with other statistics of techniques using mandibulotomy.

OBJECTIVE: Hypothyroidism is a common complication when radiotherapy is part of the treatment for head and neck tumours. This study aimed to show the incidence of hypothyroidism and possible risk factors in these patients. METHODS: Factors related to the population, tumour, treatment and occurrence of hypothyroidism were analysed in 241 patients diagnosed with head and neck carcinoma. RESULTS: Approximately 53 per cent of patients were diagnosed with radiation-induced hypothyroidism. Its occurrence was related to: tumour location, laryngeal surgery type, neck dissection type, post-operative complications, cervical radiotherapy and radiotherapy unit type (linear particle accelerator or telecobalt therapy technology). CONCLUSION: Control of thyroid function should be standardised for several years after treatment, particularly in patients with risk factors, such as those treated with telecobalt therapy, those with post-operative complications and for whom the thyroid parenchyma is included in the irradiated area (laryngeal or pharyngeal location and bilateral cervical radiation).

INTRODUCTION: Nasoesophageal tube (NT) is very often used by the ear, nose and throat specialists in head and neck oncological surgery. It is well known the irritative effect that it has on the nasosinusal mucosa. The aim of this study is to analyze the relationship between the use of the NT and the pathology of the maxillary sinuses. MATERIAL AND METHOD: The study was carried out in 25 patients treated in our department who had been fed by NT, after surgery. A plain sinus X-ray was performed in the pre-operatory period and a control radiograph in the post-operatory period, and the results have been analyzed. RESULTS AND CONCLUSIONS: No sinusitis had been observed whatever length of time the NT remained in situ.

AIM: High-intensity interval training (HIT) is an exercise model that has been seen to cause similar muscle adaptations and improvements in exercise performance to other traditional exercise models. This study aimed to examine the effects of low-volume HIT exercise on plasma viscosity (PV). METHODS: Ten healthy male subjects (25.80±3.39 years) randomly performed a HIT running protocol (2-min warm up at 8 km/h, 5x2-min bouts at 90% maximal heart rate, separated by 2-min at 8 km/h, finished with another 2-min period at 8 km/h) or an aerobic (AER) running exercise (60'at 55% VO2max). Blood samples were drawn before and after exercise, and after 30-minute recovery. PV, hematocrit (Hct), fibrinogen, total proteins, triglycerides, total-cholesterol and glucose levels were analyzed. Plasma volume loss during exercise was calculated. RESULTS: PV rose after HIT (P<0.05) while Hct rose after both protocols. Plasma volume loss was higher after HIT (-6.35±3.47%) than after AER (-3.11±2.49%) (P=0.045). Total-proteins (P<0.001), triglycerides (P=0.013), total-cholesterol (P<0.001) and glucose (P=0.001) concentrations increased after HIT. After AER no statistically significant differences were found in plasma constituents concentrations. CONCLUSION: A low-volume HIT session causes a sufficient loss in plasma volume that leads to significantly incremented plasma constituents' concentrations and, therefore, a mild transient rise in PV.

The diagnosis of primary ciliary dyskinesia (PCD) depends on electron microscopic examination of cilia obtained from mucosal biopsies of the nasal turbinates or the trachea. This is an invasive, time-consuming, and expensive technique. The low incidence of this abnormality, the large number of infants and children with suggestive symptoms, and the lack of a reliable screening test make the decision to proceed with diagnostic biopsy a common problem in recurrent or chronic respiratory pediatric conditions. To improve this situation early in infancy, nasal mucociliary transport was evaluated in 10 normal newborns using the Tc-99m-labeled seroalbumin technique, and the results obtained were compared with those corresponding to a newborn with Kartagener's syndrome. Transport velocity in normal newborns was similar to that observed in older children and in adults, in contrast to the tracer immobility seen in the affected newborn. This technique is simple, safe, objective, and well tolerated in this age group and might be used as an early screening test to ruling out PCD.

Three methods have been used to study the sympathetic and parasympathetic innervation of the vocal cord: paraformaldehyde-induced fluorescence, electron microscopy, and the peroxidase-antiperoxidase technique. Each method has been successful in determining adrenergic fibers, autonomic endings, and the neuropeptides VIP and NPY, respectively. The close relationship between these neuropeptides and the autonomic endings (adrenergic and cholinergic), and the presence of the neuropeptides in the chorion of the vocal cord, around glands and vessels, gives new evidence that autonomic innervation plays a role in the functioning of this part of the larynx.

The distribution of neurons in the dorsal motor nucleus of the vagus nerve (DMNV) that innervate the supraglottic and glottic areas of the larynx of the guinea pig have been studied using the horseradish peroxidase (HRP) technique. Following soaking of the superior laryngeal nerve in a solution of HRP, labeled neurons were always located ipsilaterally, at levels between the estria acustica and the caudal end of the inferior olivary nucleus. Characteristically, the neurons were small or medium in size.

The action of neurotransmitters is of interest in the normal function of the vocal cord since they regulate mucous secretion and vessel caliber. The intersynaptic space between glandular and myoepithelial cells and the corresponding neuroendocrine endings is the area in which these substances are delivered. This area has not been previously studied in the human vocal cord. Specimens from vocal cords have been studied by electron microscopy. In our study, it seems to be clear that there exists a very close contact between myoepithelial cells and secretory nerve endings. This close relationship is likewise observed between these cells and glandular cells.

An experimental model is described for recording laryngeal evoked potentials at the brainstem level through electrical stimulation of the superior laryngeal nerve. Rabbits were divided into two groups according to age: Group 1 consisted of 20 adults (over 6 months old), while group 2 was comprised of 1-month-old rabbits. Under general anesthesia, the superior laryngeal nerve was electrically stimulated and the corresponding evoked potentials were recorded by needle electrodes placed close to the dorsal and ventral aspects of the brain stem. Clear age-related differences were observed for latency, amplitude, and morphology of the potentials. This fact suggests postnatal maturation of the nerve structures, both peripheral and central. Sectioning the superior laryngeal nerve abolished all responses. Adduction of both vocal cords was observed in conjunction with the use of stimulation.

The prognosis for supraglottic squamous cell carcinomas is difficult to establish. A study of 19 histopathological parameters (14 from the primary tumor and 5 from the lymph node metastases) was undertaken in 51 supraglottic epidermoid carcinomas. All patients were subjected to supraglottic horizontal laryngectomy with bilateral functional neck dissection and postoperative radiotherapy. A discriminant analysis was performed to relate the different histopathological variables to 5-year survival. Capsule rupture and the number of metastatic lymph nodes were the only two parameters significantly related to survival (P < .01). Histopathology (in particular, lymph node metastases) proved more effective in predicting patient evolution than TNM staging.

DNA analysis by flow cytometry is considered to be of prognostic value in epidermoid carcinoma of the head and neck. However, few and contradictory studies have been made on laryngeal carcinomas. We studied 48 epidermoid carcinomas in patients subjected to horizontal supraglottic laryngectomy with a 5-year- followup. The technique described by Hedley for fixated and paraffin-embedded tumors was used. Thirteen tumors were excluded on the grounds of presenting variation coefficients in excess of 10. Of the 35 cases analyzed, 28 (80%) were diploid and seven (20%) aneuploid. No correlation was observed between tumor ploidy and patient survival, recurrence, or any of the histopathological variables studied.

Nasal mucociliary transport has been studied using the seroalbumin-Tc99m radioisotope method in 10 adults and 10 normal newborns under 28 days of age. The mean transport velocity was 8 and 6 mm/min for the normal adults and newborns, respectively, with no significant differences between the two (P = 0.07).

A study was made of 106 children between 1 and 15 years of age (mean 6 years) with recurrent upper and lower airways infections since birth. Nasal mucociliary transport (NMT) velocity was determined in all subjects by the Tc99m-labeled seroalbumin technique. In 42 children, NMT was found to be altered. In this group of patients the technique was repeated in a period of between 1 and 2 years later. In 23 cases (55%) transport had normalized, while in 19 (45%) it remained altered. Recurrent pneumonia and constant rhinorrhea were more frequent in this group. Situs inversus was only detected in 2 of these patients. Pathology showed ciliary ultrastructure, the absence of dynein arms and microtubule alterations. The absence of cilia was observed in some patients. Normal cilia were also encountered in children with persistently altered nasal mucociliary transport.

Cystic fibrosis (CF) is a deadly hereditary disease that produces an abnormally thick, viscous and abundant secretion in the respiratory tract. This secretion in turn leads to the development of recurrent respiratory infections and irreversible lung damage. We have studied nasal mucociliary transport by means of an isotopic technique in 12 patients with CF and in 12 healthy volunteers. Nasal mucociliary transport was repeated at 12-18 months in the patients. In five randomly selected patients ciliary ultrastructure was studied. The velocity of nasal mucociliary transport was significantly slower than in healthy persons (P < 0.001) and no significant differences were observed in both studies (P < 0.05). No significant differences were either observed in the CF group between the homo- and heterozygotes (P < 0.5), or in those six patients infected by Pseudomonas aeruginosa (P < 0.05). Ciliary ultrastructure was normal in one patient. In another patient the sample showed no cilia, while the remaining three exhibited changes similar to those observed in chronic respiratory infections: supernumerary peripheral tubules, ciliary disorientation and ciliary complexes.

The topographic distribution of Nucleus Ambiguus (NA) motor neurons innervating the intrinsic laryngeal musculature in the guinea pig has not been studied previously. We used 25 guinea pigs anaesthetized by Penthrane (Methoxyflurane) inhalation. Horseradish peroxidase type VI (40%) was used. The motor neurons traced are distributed in cellular groups elongated rostro-caudally in a tract of the brain stem situated between a rostral plane compatible with the location of the retrofacial nucleus, and a caudal plane some millimetres below the obex. The motor neurons observed do not exhibit differential morphology in relation to the different muscles studied.
