RESUMO

Orbital diseases, in view of the special situation and anatomic structure of the orbit require greater attention and wide cooperation of doctors of many specialities, being an interdisciplinary problem. The analysis of the presented material showed that almost half the treated orbital diseases were bone fractures (48.4%), resulting mainly from traffic trauma (67.4%). The remaining diseases were orbital inflammations and tumours, and their numbers were similar. The fact is worth attention of decreasing number of orbital inflammations in successive years and increasing tendency for orbital tumour occurrence.

RESUMO

Borreliosis which incidence and epidemic range are constantly increasing is an infectious zoonosis, detected in the town of Lyme, Connecticut, USA in 1975. This disease is caused by Borrelia burgdorferi transmitted by ticks. It relatively easily responds to antibiotic therapy. Making of correct diagnosis, however, causes more problems. The disease has often atypical course, its phases overlap. It may also mimic collagen diseases and other spirochetoses. The relatively low specificity of serological tests additionally makes the diagnosis more difficult.

The authors present the scheme of work, aims and duties of otolaryngology department of the county hospital. Basic activities and aims are as follows: diagnostic and therapeutic procedure, specialistic supervision over other otolaryngology departments and out-patients clinics in the county, teaching otolaryngology department assistants and voluntary workers, postgraduate courses supervised by The Postgraduate Medical Centre, public and educational service. All those activities guarantees the proper work of the department, especially in case of the new country area division.

RESUMO

Laryngological and phoniatric examinations were carried out in 130 alumni from theological seminary. The authors estimated their ability to serve the mass in the future. In 71 (54.6%) cases various pathologies of the ear, nose, oral cavity, pharynx and laryngx were found, which allowed for their early treatment and rehabilitation. It also helped to settle the limitations of their future ministration.

RESUMO

The authors have analyzed 562 cases treated by esophagoscopy in ENT Department of District Hospital Nr 1 in Rzeszów, Poland from 1987 to 1996. There have been 580 esophagoscopy performed during this period: In this number 504 therapeutic procedures and 76 diagnostic ones. The most common indication for therapeutic esophagoscopy has been the suspection or presence of foreign body in the esophagus and rarely other pathology of upper part of digestive tract. Complications of esophagoscopy are rather rare and in the most cases they are the result of perforation of esophageal wall during esophagoscopy. In presented material the complication rate was 0.17%. Impacted parts of false teeth in esophagus should be always removed by experienced otolaryngologist under endotracheal anesthesia and with esophageal muscles relaxation.

The authors described a rare case of craniofacial shot wound in 11 years old girl. The cork pistol shot caused the disseminated lead fragment wounds in cheek, maxillary sinus, sphenoidal-palatal, subtemporal, nasal cavity and nasopharynx.

RESUMO

The authors have presented the description of nasopharyngeal chordoma 28 years after the first diagnose and treatment with Co-60 application of this. Very slow, almost thirty years growth of the tumor has been noticed. The difficulties of the radical treatment, either radiotherapy or surgery have been pointed out in the case of clivus chordoma. The authors have presented too the long-term results of this very rare appearance tumor treatment, based on the review of the literature.

RESUMO

The ways of origin and the methods of diagnosing of the ectopic thyroid gland have been described. The authors have shown the case of follicular carcinoma of ectopic lingual thyroid gland in 35 years woman. The effective surgery (labiomandibuloglossotomy and total extirpation of the tumor) and following irradiation therapy with 131I haven't revealed recurrence of the disease during 2.5 years observation. The permanent substitution therapy with thyroxine has secured the euthyreotic state for the patient.

RESUMO

The authors have described genesis, localization and diagnosis of the first branchial cleft anomaly and the case of the branchial cyst in the deep parotid lobe. Surgical treatment (superficial parotidectomy and cyst extirpation from the superior part of the deep parotid lobe) was effective; one-year follow-up revealed no recurrence of the anomaly.

RESUMO

The authors described the laryngeal cancer surgery results in connection to the clinical stage and surgery method. The main procedure was the total surgery with and without the radical neck dissection and radiotherapy. The 3 years survival rate (131 treated patients) was in 96 (73%) cases. The 5 years survival rate (91 treated patients) was in 51 (56%) cases and 10 years survival rate (56 treated patients) was in 23 (41%) cases.

RESUMO

An extremely rare case of primary adenocarcinoma of the tympanic cavity behind the intact tympanic membrane was presented. A radical surgical treatment and postoperative telegammatherapy was used, with no evidence of recurrence for four years. The authors have described the occurrence of this tumor in the temporal bone, clinical picture, diagnosis difficulties, prognosis and ways of therapy. They have paid attention to the guileful course of disease and non-characteristic symptoms in its initial stage.

RESUMO

The authors have presented the rare case of metastasis the cardiac adenocarcinoma to the middle ear. They have described occurrence of metastatic tumors of the temporal bone, mechanisms of their formation, clinical picture, diagnostic difficulties, prognosis and ways of therapy. A radical surgical treatment and chemotherapy have been used in this case. The patient died 17 months after diagnosis of the cardiac adenocarcinoma and 10 months after diagnosis of metastasis to the temporal bone.

RESUMO

The authors presented 4 cases of patients with a foreign body in the esophagus, which has been tried to remove by means of fiberoscopy and rigid esophagoscopy. The fiberoptic endoscopy has failed either as a diagnostic method or as a therapeutic one. The classical esophagoscopy has turned out to be much better in both cases. The fiberoscopy is a valuable diagnostic and therapeutic procedure in a lot of diseases of digestive tract. Especially it is useful in order to remove some foreign bodies from the esophagus, but at the same time it fails when removing big foreign bodies (e.g. parts of false teeth, large bones). In these cases rigid esophagoscopy is the only effective method when performed under endotracheal anesthesia and with esophageal muscles relaxation.

RESUMO

Authors have described a case of bilateral acinic cell carcinoma of parotid gland. The patient has been treated successfully by left superficial parotidectomy and right total parotidectomy followed by complementary Co 60 therapy. During 3 years after surgery the patient revealed no recurrence of the disease. Clinical course, histology and pathology have also been presented. The authors have pointed out the difficulties in diagnosing ACC because of the variety of histological pictures and unclear terminology.

From 1992 to 1999, 58 thyroid gland operations (41 female and 17 male) were performed in ENT Department of the District Hospital in Rzeszów. In 14 (21.4%) cases (9 female and 5 male) thyroid surgery was done for malignant disorders: papillary carcinoma in 11 (79%) patients, follicular carcinoma in 2 patients and medullary carcinoma in 1 patient. There were neck metastases in 9/14 (64.4%) patients. In 3 cases with papillary carcinoma (all with neck metastases) aerodigestive tract was invaded. One patient had neoplasmatic invasion of the larynx and trachea, one patient had invasion of larynx et pharynx and in one patient tumour invaded the esophageal wall. In those patients radical surgery was done: total thyroidectomy with total laryngectomy and radical neck dissection (2 patients) and subtotal thyroidectomy with conservative neck dissection (1 patient). External beam irradiation and radioactive iodine 131 treatment followed surgery. Two patients are still alive 6 years after the treatment free of disease, and 1 patient died of unrelated causes 3 months after the surgery. Symptoms, diagnostic evaluation and treatment of thyroid papillary carcinoma invading the aerodigestive tract are detailed in paper.

From 1974 to 1994 in Otolaryngology Department of County Hospital No 1 in Rezszów, Poland in 35 patients tumor of neck derived from thyroid gland has been revealed. There were 22 females and 13 males, aged 16 to 78 (mean age 48), among the patients. Pathology examination of surgery removed tumors has revealed malignancy in 19/35 (54%) cases, non-neoplastic tumors in 11/35 (32%) cases and non-malignant tumors in 5/35 (14%) cases. Among malignant tumors metastasis of papillare adenocarcinoma was the most frequent pathology (37%). Authors analyze feature of the thyroid gland derived tumors based on their size, consistency, localization, mobility, towards surroundings and the picture during surgery. Because of the presence of thyroid gland malignant tumor metastasis without clinical symptoms, author emphasised the necessity of its consideration in differential diagnosis.

RESUMO

Bilateral suprahyoid lymphangiectomy have been made in 95 patients because of suspicion of submandibular lymph nodes metastases. The patients with lip carcinoma have been previously treated with surgery, surgery and radiotherapy or with radiotherapy followed by surgery. Histological examinations have revealed lymph node metastases in 62 (65.3%) patients, in 31 (32.6%) patients histology has been negative and in 2 (2.2%) patients lymphonodular tuberculosis has been stated. Authors have analyzed 3, 5 and 10 years survivals in 65 patients after suprahyoid lymphangiectomy during lip cancer treatment.

The authors described 4 cases of neoplasms of middle ear behind the intact tympanic membrane (3 cases of paraganglioma and 1 case of adenocarcinoma). They paid attention to the guileful course of disease and non-characteristic symptoms in its initial stage. Only immediate exploratory operation of the middle ear enables to make early diagnosis and start effective treatment.
