BACKGROUND: Some individuals may not retain adequate immunity against measles and rubella years after two doses of measles, mumps, and rubella (MMR) vaccination due to vaccine failure. This study aimed to investigate the rates of vaccine failure and seroconversion by administering an MMR booster to young adults. METHODS: We first assessed measles and rubella antibody levels using the Luminex multiplex assay, VIDAS IgG assay, and plaque reduction neutralization test (PRNT) among individuals aged 18-30 years old who had received two doses of MMR vaccine. Participants with low measles and/or rubella antibody levels as confirmed by VIDAS received an MMR booster. Antibody levels were measured at 1-month post-booster. RESULTS: Among 791 participants, the measles and rubella seroprevalence rates were 94.7% (95% CI: 92.9%-96.0%) and 97.3% (95% CI: 96.0%-98.3%), respectively. Lower seroprevalence rates were observed among older participants. 113 participants who received an MMR booster acquired higher measles and rubella antibody levels at 1-month post-booster compared to baseline. CONCLUSIONS: Although measles and rubella vaccine failures were observed among 5.3% and 2.7% of young adults, respectively, an MMR booster triggered a significant antibody response.

China has experienced substantial coastal reclamation and damming of rivers. These changes have the potential to impact migrations of diadromous fishes between the sea and fresh waters, but the composition of these fishes and the impacts of barriers to their movement in China have received little attention. We inventoried the species composition and distribution of diadromous fishes, and the impacts of barriers on them, in the Hong Kong Special Administrative Region (HKSAR), southern China. Fish assemblages were surveyed using hand-nets, supplemented by cast-netting and single-pass snorkel surveys, in 24 small coastal streams across three regions. Surveys were undertaken on multiple occasions during the wet and dry seasons to account for the monsoonal tropical climate. Twenty-eight diadromous fishes were collected, mostly gobies, amounting to over half (53%) of the total richness of primary freshwater fishes; four additional species are known from literature records. Diadromous richness was 48% greater during the wet season, when all species were encountered. Richness varied substantially among streams, from a maximum of 17 (2 streams that were diversity hot spots) to none (3 streams). The most widespread diadromous fish was Glossogobius giuris (71% frequency of occurrence), followed by Mugil cephalus (58% occurrence) and Eleotris oxycephala (50% occurrence). The remaining 25 diadromous fishes occurred in fewer than half of the streams; 12 species were confined to a single stream and may be locally threatened. There were conspicuous spatial differences in diadromous assemblages across HKSAR, despite its limited extent (1114 km2), the proximity of the surveyed streams, and the broad geographic distribution of most species. Regional species assemblages were influenced by localized habitat characteristics, with a noticeable distinction between areas with and without large, fast-flowing, and highly oxygenated streams. The presence of in-stream barriers (weirs: 0.3-8.7 m high) did not affect spatial patterns in species assemblages, although, on average, diadromous richness was lower in weir-obstructed streams (4.0 vs. 6.9 species in unobstructed streams). In total, 18 species were confined to unobstructed streams or sections below weirs, whereas the remaining 10 species were recorded both above and below weirs. Only the mottled eel (Anguilla marmorata) and a goby (Stiphodon multisquamus) were able to ascend weirs over 2 m. Although at least 400 m of the lower course of each stream was sampled, diadromous fishes were confined to the first 300 m in 12 of the 13 weir-obstructed streams. Remarkably, the tally of 32 diadromous species in HKSAR exceeds the 19 known from mainland China, highlighting the need for further research on composition and conservation status of diadromous fishes.

In order to solve the problem of imbalanced and noisy data samples for the fault diagnosis of rolling bearings, a novel ensemble capsule network (Capsnet) with a convolutional block attention module (CBAM) that is based on a weighted majority voting method is proposed in this study. Firstly, the complete ensemble empirical mode decomposition with adaptive noise (CEEMDAN) method was used to decompose the raw vibration signal into different IMF signals, which are noise reduction signals. Secondly, the IMF signals were input into the Capsnet with CBAM in order to diagnose the fault category preliminarily. Finally, the weighted majority voting method was utilized so as to fuse all of the preliminary diagnosis results in order to obtain the final diagnostic decision. In order to verify the effectiveness of the proposed ensemble of Capsnet with CBAM, this method was applied to the fault diagnosis of rolling bearings with imbalanced and different SNR data samples. The diagnostic results show that the proposed diagnostic method can achieve higher levels of accuracy than other methods, such as single CNN, single Capsnet, ensemble CNN and an ensemble capsule network without CBAM and that it has stronger immunity to noise than an ensemble capsule network without CBAM.

OBJECTIVE: The utility of dual-time-point (18)F-FDG PET/CT in differentiating benign from malignant processes in pediatric patients was assessed. SUBJECTS AND METHODS: Twenty-one patients (13 girls and eight boys; age range, 1-17 years) with suspected malignancy underwent dual-time-point FDG PET/CT. Scan 1 was performed at approximately 60 minutes after i.v. injection of 5.18 MBq/kg of FDG, and scan 2 was performed at 121 ± 43 minutes after the first scan. Regions of interest were overlaid onto each non-attenuated-corrected image, and semiquantitative analysis was performed using the standardized uptake value (SUV) obtained from early and delayed images. A retention index was calculated according to the following equation: [(delayed SUV - early SUV) / early SUV] × 100. Results were compared prospectively in relation to pathologic examination or other conventional radiologic imaging or clinical follow-up. A retention index of 10% or higher was chosen as a cutoff for differentiating malignant from benign entities. RESULTS: For patients with malignant disease, the average SUV increased from 7.3 ± 1.2 to 10.9 ± 2.7 between the two time points, whereas the SUV changed from 4.5 ± 0.8 to 4.2 ± 1.0 for patients with benign lesions. The average retention index was 37.1% ± 10.8% for patients with malignant lesions versus -9.9% ± 7.1% for benign lesions (p < 0.01). With a cutoff value of 10% or higher for the retention index, the sensitivity and specificity of dual-time-point FDG PET/CT were 77% and 80%, respectively. CONCLUSION: These data show that dual-time-point FDG PET/CT is useful in distinguishing malignant from benign processes in pediatric patients.

Hurling is an Irish national game of stick and ball known for its ferocity, played by 190 000 players. Facial injuries were common but have been significantly reduced by legislation enforcing compulsory helmet wearing. Current standard helmets worn by hurlers do not offer protection to the external ear. Here we describe an emerging pattern of ear injuries and demonstrate the risk of external ear injuries in hurlers complying with current helmet safety standards. A 6-month retrospective analysis was carried out of patients attending Cork University Hospital (CUH) with ear lacerations sustained while hurling. Patient notes were reviewed and helmet manufacturers were interviewed. Seven patients were identified, all of whom sustained complex through ear lacerations while wearing helmets complying with current safety standards. Current helmet design fails to protect the external ear placing it at an increased risk of injury, a potential solution is to include ear protection in the helmet design.

PURPOSE: This work evaluated the prognostic performance of Child-Pugh (CP), albumin-bilirubin (ALBI) and platelet-albumin-bilirubin (PALBI) scores in hepatocellular carcinoma (HCC) patients undergoing radiotherapy (RT). RESULTS: The study included 174 consecutive patients with 63% at CP A5 (n = 110) and 34% at CP A6 (n = 64). The median ALBI score was -2.39 (range: -3.61 to -1.41) with 34.5% at grade A1 (n = 60) and 65.5% at grade A2 (n = 114). The median PALBI score was -2.39 (range -3.39 to -1.24) with 33.3% at grade 1 (n = 58), 41.4% at grade 2 (n = 72) and 25.3% at grade 3 (n = 44). With a median follow-up of 21.7 months, the median OS of the entire cohort was 22.2 months. OS was significantly associated with the PALBI grade (p = 0.002) and for the ALBI grade (p = 0.00495), but not for the CP score (p = 0.46). The PALBI grade has a significantly higher AUC compared than the ALBI grade or CP scores in predicting OS. The PALBI grade was predictive of CP score decline ≥2 (20% grade 3 vs. 5.3% grade 1/2 p = 0.05) but the ALBI and CP scores were not. CONCLUSION: Among CP A HCC patients receiving radiotherapy, the PALBI and ALBI grade maybe a better prognostic tool than the CP score. The role of PALBI in predicting liver toxicity warranted further exploration. METHODS: We retrospectively reviewed HCC patients treated with individualized hypo-fractionated radiotherapy (IHRT) using stereotactic technique from 2006 to 2015. We collected CP, ALBI and PALBI scores prior to treatment and analyzed their correlation with overall survival (OS) and liver toxicity.
