PURPOSE: Peptide receptor radionuclide therapy (PRRT) and somatostatin analogues (SSAs) are commonly combined as primary treatment for neuroendocrine neoplasms (NEN), and SSAs given as maintenance. We sought to evaluate whether sequential therapy with PRRT followed by SSAs has progression or survival benefits in patients with NEN after disease control by PRRT. METHODS: This prospective, randomised, single-centre study had as principal eligibility criteria: unresectable, locally advanced, or metastatic, histologically confirmed well-differentiated NEN; no symptoms/biochemical diagnosis of carcinoid syndrome; no SSAs or ≤ 3 months of SSAs before PRRT; and stable disease or partial or complete response after PRRT. Altogether, 115 patients were randomised 2:1 to an SSA group (n = 74) given octreotide acetate LAR every 4 weeks, or a control group (n = 41) receiving only best supportive care. Octreotide treatment was to stop upon intolerable toxicity or patient refusal, or, at physician/patient discretion, upon NEN progression. The primary endpoint was progression-free survival (PFS), the secondary endpoint, and overall survival (OS). RESULTS: Median (25th-75th percentile) follow-up from the first PRRT activity to death or latest observation was 6.6 (3.18-10.22) years. During that time, 71/115 patients (62%) progressed, 52/74 (70%) in the SSA group, and 19/41 (46%) in the control group (p = 0.01). Eighty-eight/115 patients (76%) died, 58/74 (78%) in the SSA group, and 30/41 (73%) in the control group (p = 0.52). Median (95% CI) PFS was 4.7 (2.8-7.7) years in the SSA group, and 6.4 (4.1-not reached) years in controls. Overall, median OS was 6.6 years. Neither PFS nor OS differed between groups (p = 0.129, p = 0.985, respectively). CONCLUSIONS: In patients with disease control after PRRT, subsequent SSA treatment appeared not to be associated with better PFS or OS. Whether to continue SSA administration upon progression after PRRT requires evaluation in a prospective, randomised, controlled multicentre study with a relatively homogeneous sample.

The role of radiotherapy in advanced medullary thyroid carcinoma (MTC) is confined to patients in whom surgical treatment or the administration of tyrosine kinase inhibitors are not possible or contraindicated. High fractionated radiation doses during radiosurgery or fractionated stereotactic radiotherapy are applied to reduce cancer-related symptoms and stabilize irradiated lesions. This study aimed to retrospectively evaluate the therapeutic effect of stereotactic radiotherapy in MTC patients. MATERIAL AND METHODS: The study group involved 11 MTC patients, treated due to 16 cancer lesions, mainly bone metastases (10 lesions), lymph node (2 lesions) metastases, or liver metastases (2 lesions), one primary thyroid tumor, and one MTC recurrence in the thyroid bed. The fractionated and total radiation doses ranged between 5 and 12 Gy and 8-44 Gy, respectively. Six lesions were treated with a single radiation fraction, three lesions with 2 fractions, another 6 lesions with 3 fractions, whereas the remaining one metastatic lesion with 9 fractions of stereotactic radiosurgery. RESULTS: The beneficial effect of stereotactic radiosurgery was obtained in all treated lesions. None of treated lesions progressed in the further disease course. Fourteen lesions were stable (87.5 %), including eight lesions showing progression before radiosurgery (good response). Disease control was obtained in all soft-tissue metastases. Regarding bone metastases, partial regression was achieved in 20 % lesions, whereas in 30 % lesions progressive before radiotherapy, the treatment led to disease stabilization. CONCLUSIONS: Our data pointed to the effectiveness of high-dose fractionated radiotherapy in MTC. However, an observation of a larger group of patients is required to confirm it.

Shortbread biscuits have become one of the most desirable snacks in all population groups but due to its high fat content it is searching for new solutions of fat reduction such as the use of carbohydrate-based fat mimetics. However, the thermal processing of food containing lipids in the presence of certain carbohydrates can contribute to the formation of toxic compounds or alter its levels. Therefore, the aim of the study was to evaluate the use of inulin and pectin gels as partial fat replacers (10-40%) in classical shortbread biscuits in a view of the changes of 3-monochloropropane-1,2-diol ester (3-MCPDE) and glycidyl ester (GE) levels after baking, and also after a two-months storage. The experiment showed that the amount of inulin gel higher than 10% promoted the endogenous formation of 3-MCPDE probably due to the higher amount of Maillard products formed in the biscuits, but after two months the levels decreased and were comparable with those transferred from shortening. Pectin did not demonstrate such phenomenon, owing to the presence of bound water in gel. Glycidyl esters were partially decomposed during baking and also during storage, particularly in the samples with the addition of pectin gel, which presumably caused by low stability of GE under acidic conditions resulting from the occurrence of galacturonic acid in pectin.

This study was designed to determine the effect of feeding female rats with low-calcium diet containing one of three fructan sources (Jerusalem artichoke, yacon, Beneo Orafti Synergy1), on selected bone parameters. Growing Wistar rats were fed modified AIN-93 G diet enriched in fructan sources (8%), added alone or as a strawberry sorbet ingredient. Two of eight groups were a validation model, where the positive control group was fed with recommended calcium dose in the diet (RCD), and negative one - with low calcium diet (LCD). After 12 weeks, femoral Ca content, bone densitometry, architecture and hardness were examined. The positive effects on femoral Ca content and cortical thickness, area and content in distal part of bone was observed after feeding animals diet enriched in Jerusalem artichoke sorbet. Beneficial action on other bone tomographic parameters (particularly trabecular volumetric bone mineral density) in this part of femur were associated mainly with the consumption of the diet with sorbet containing yacon. Our results showed an important role of diet containing frozen strawberry desserts enriched in fructan sources in the maintenance of healthy bones of growing organism. It may suggest possible synergisms between fructans and bioactive substances of strawberry.

Background: Carbohydrate-rich foods, such as breakfast products, snacks and biscuits because of its nutritional or sensory qualities are an inherent part of human diet. However, their production might contribute to the formation of acrylamide, 3-monochloropropane-1,2-diol (3-MCPD) and its esters and glycidyl esters. Objective: The aim of this work was to assess the levels of acrylamide, free and bound 3-MCPD and glycidyl esters in selected carbohydrate-rich, thermal processed products, present on the market in Poland in 2016-2017. Material and Methods: The survey involved 60 samples of snacks, breakfast products and biscuits. Acrylamide and free 3-MCPD was determined using modified QuEChERS approach. Analysis of 3-MCPD and glycidyl esters was based on the acid-catalysed method of sample preparation, derivatisation with PBA and GC-MS analysis. Results: Free 3-MCPD contents were within the values of 9.3-63.3 µg kg-1, with the highest mean content for muesli (33.3 µg kg-1), and the lowest for baby biscuits (11.7 µg kg-1). The levels of bound 3-MCPD were higher (from 9.3 µg kg-1 to 1500 µg kg-1). The highest average content was observed for sugar free biscuits (599 µg kg-1), whereas the lowest for breakfast cereals (50.2 µg kg-1). Glycidyl esters were detected only in four samples with the highest content at the level of 28.8 µg kg-1. The acrylamide levels varied from 195 to 1352 µg kg-1, with the highest content for organic biscuit samples (913 µg kg-1), and the lowest for muesli (348 µg kg-1). Conclusions: Regular consumption of popular snacks such as potato chips, crackers and biscuits may result in risk to human health as the effect of high content of acrylamide or 3-MCPD. Due to a high level of these contaminants detected in some type of breakfast products, and products targeted for children, its consumption should be restricted, especially in younger population groups.

BACKGROUND: Microbiological and chemical safety concerns regarding frozen fillets from pangasius catfish (Pangasius hypophthalmus) exported to Poland, Germany and Ukraine and Nile tilapia (Oreochromis niloticus) exported to Poland and Germany were investigated by analyzing heavy metal residues, microbiological hazards, biogenic amines, and thiobarbituric acid (TBA) and total volatile basic nitrogen (TVB-N) content. RESULTS: The heavy metal residues from all studied samples were far lower than the limits established by authorities. The most abundant biogenic amine found was histamine, with a maximum content of 9.6 mg 100 g(-1) , found in pangasius exported to Poland. The total viable counts were from 2.8 log cfu g(-1) in pangasius exported to Ukraine to 4.3 log cfu g(-1) in pangasius exported to Germany. Vibrio spp. were present in 70-80% of all studied pangasius groups, whereas there no Vibrio spp. were found in the studied tilapia samples. 30% of Pangasius fillets exported to Poland were contaminated with coagulase-positive staphylococci. No E. coli was found in any of the studied samples. Although the results of TBA analysis differed significantly between studied groups, the malonic aldehyde content in all studied groups was still very low. The TVB-N content in frozen fillets from pangasius was significantly lower than in frozen tilapia fillets. CONCLUSIONS: We reported that pangasius catfish frozen fillets were widely contaminated with Vibrio spp., which could prove hazardous for the final consumer if the fish is eaten raw or undercooked. The rest of the analysis showed no other reason for concern associated with Nile tilapia and Pangasius catfish frozen fillet consumption.

BACKGROUND: According to the WHO data in January 2007, 14 million children were overweight in the countries of the European Union. The problem of childhood obesity should be considered as one of the biggest challenges facing twenty- first century nutrition and pediatrics. OBJECTIVE: The aim of this study was to show the correlation between energy, basic nutrients intake and BMI of school children aged 10-12 years, using Pearson's correlation coefficient. MATERIALS AND METHODS: 169 pupils (69 from Krakow and 100 from Skawina), aged 10-12, participated in these studies. The studies were approved by the Board of Education, school principals and the parents. On the basis of measurements of weight and height, body mass index (BMI) was calculated. 24 h dietary recalls were collected in the spring and autumn of 2006, for 4 selected days per week. RESULTS: Intake of energy, total carbohydrate and fat did not met requirements, but intake of protein was too high. Proper weight, based on BMI values in the population aged 10-12, were found in 74.9% of boys and 81.2% of girls. Deficiencies in body weight occurred in 4.2% and 3.4%, overweight in 15.6% and 10.0% while obesity in 5.3% and 5.4% of respondents. CONCLUSIONS: Inverse correlation was found between the implementation of standards for energy and BMI values of children. It was observed positive correlation between the consumption of total and animal protein and BMI value. The aim of this study was to evaluate the application of quick, easy, cheap, effective, rugged and safe method for simultaneous determination of polycyclic aromatic hydrocarbons and pesticide residues in fresh herbs. In the experiment two extraction solvents and standard types of sorbents were used. The extracts were analyzed using GC-SIM-MS. The results suggest that acetonitrile is more suitable extraction solvent giving more purified samples and better recovery values (71.6 %-116.9 %) with RSD lower than 15 % for most of the compounds. In real samples pesticides were identified in the samples of parsley, tarragon and lovage. In few samples the pesticide levels exceeded the MRL established by EU.

The aim of this study was to analytically evaluate the daily intake of total polyphenols, ß-carotene, and vitamin C by 10- to 12-year-old children. Twenty four-hour dietary recalls were collected in spring and autumn 2006 from students in Krakow and Skawina (Malopolska region, Poland). The content of vitamin C, polyphenols, and ß-carotene was determined in reconstructed daily diets. Statistical analysis did not show any significant differences between the mean intake of measured compounds according to school location, gender, or season. Diets of children contained adequate amounts of polyphenols in relation to the recommended values. Intake of ß -carotene usually exceeded the recommended retinol equivalent; in contrast, vitamin C did not meet requirements. Increased consumption of fruits and vegetables rich in vitamin C is recommended.

RESUMO

Mesenchymal chondrosarcoma (MC) is an infrequent, highly malignant neoplasm of the soft tissues and bone. It is very rare in the pediatric age group, especially in the intraspinal location. Only 24 cases have been reported to date. The authors present a case of a 14-year-old boy with an intraspinal MC who died of the disease 50 months from the initial diagnosis and after the third local recurrence. The patient was treated with a combination of chemotherapy, radiotherapy, and surgery. The authors review the clinical presentation, diagnostics, and the efficacy of treatment of pediatric patients with MC reported in the literature from 1978 to 2010.

BACKGROUND: Adolescence is a time of rapid growth and development of the organism. During intense growth and maturation changes taking place in the body, it cost higher demand for energy and nutrients including minerals. OBJECTIVE: The aim of this study was to assess the intake of sodium, potassium, calcium, phosphorus, magnesium, iron, zinc, copper and manganese with daily diets by children aged 10-12 years. MATERIALS AND METHODS: Assessment of intake of mineral compounds from daily diets was conduced with using 24 hours recall. Data were collected in spring and autumn of 2006 in randomly selected primary schools in Krakow and Skawina. Average intake of minerals (Na, K, Ca, P, Mg, Fe, Zn, Cu, Mn) with daily diets by respondents and the meeting of requirements was estimated with using the computer softwere Diet 2. RESULTS: In almost all cases intake of minerals with daily diets did not meet requirements. Daily dies of children did not met requirements for major minerals i.e. calcium and magnese, and for trace minerals copper, zinc, iron. Insufficient intake of calcium, while fully covering the demand for phosphorus, resulted in improper ratio these minerals in the diet (0.62 at the recommended, of 1). Intake of manganese and potassium fully covered demand, while the sodium and salt were taken in excess. CONCLUSIONS: To low intake of minerals compounds by children may increase the risk of development of diet non-communicable diseases.

Introduction: The aim of this prospective study was to evaluate long-term outcomes in differentiated thyroid cancer (DTC) patients postoperatively treated with distinct RAI activities of 30 mCi, 60 mCi, and 100 mCi. Material and methods: The analysis involved 277 low-risk and 46 intermediate-risk patients, who underwent radioiodine (RAI) ablation with 30 mCi, 60 mCi or 100 mCi under prospective, randomized clinical trials. Seventy-eight patients from the low-risk group received 30 mCi, whereas 125 and 74 patients received 60 mCi and 100 mCi, respectively. Regarding the intermediate-risk group, 20 patients were given 60 mCi, and 26 subjects were given 100 mCi. The mean time of follow-up was 11 years. Results: An excellent treatment response was obtained in 88%, 89% and 90% of low-risk patients treated with 30 mCi, 60 mCi, and 100 mCi, respectively, and in 85% of intermediate-risk patients, who were administered 60 or 100 mCi. An indeterminate response was achieved in 9.4% and 6.5%, whereas an incomplete structural response was obtained in 1.4% and 6.5% of low-risk and intermediate-risk patients, respectively. An incomplete biochemical response was observed only in 2.2% of intermediate-risk patients. The differences in treatment response regarding RAI activity were not significant. Conclusions: RAI activity of 30 mCi demonstrates a comparable efficacy as 60 mCi and 100 mCi in low-risk DTC. RAI activity of 60 mCi seems to be effective in intermediate-risk DTC.

INTRODUCTION: Mycotoxins - secondary mould metabolites with undesirable effects for humans - are common in the environment. These toxins are mainly produced by fungi of the genera Penicilium, Aspergillus and Fusarium. OBJECTIVE: The aim of this study was to evaluate the applicability of various sources of antioxidants (blueberries lyophilisate, cranberries lyophilisate and cinnamon powder), at 5 different concentrations (3%, 5%, 10%, 20%, 30%), to inhibit the formation of mycotoxins during the storage of cereal products. Analysed cereal samples included selected cereal grains, bran and cereal products intended for consumption by children. RESULTS: The results showed that supplementation of oat brans with the highest concentrations of blueberry lyophilisate resulted in a significant decrease in the mycotoxins levels; specifically: 20% concentration reduced the level of HT-2 toxin by 10.7% in one sample, while 30% concentration reduced it by 9.4% and 17.4% in 2 other samples. A similar result was measured for oat bran samples supplemented with the cranberry lyophilisate: specifically, 20% concentration significantly reduced the level of HT-2 toxin by 10.6% in one sample, while 30% concentration reduced it by an average of 18.0% ± 6,0% in 5 other samples. Finally, cinnamon powder supplementation caused a significant reduction in HT-2 levels in all stored samples, even at its lowest concentration. 30% supplementation resulted in HT-2 reduction in cereal samples by 67.1% - 76.1%, in wheat bran samples by 57.5% - 69.2%, in oat bran samples by 83.4% - 87.0% and by 55.0% - 100% in samples of cereal products intended for consumption by children. CONCLUSIONS: Natural products used in the experiment (blueberry, cranberry, cinnamon) inhibited the formation of mycotoxins from the group of trichothecenes.

OBJECTIVE: In patients with differentiated thyroid carcinoma (DTC), serum thyroglobulin levels measured at the time of remnant ablation after thyroid hormone withdrawal were shown to have prognostic value for disease-free status. We sought to evaluate serial thyroglobulin measurements at the time of recombinant human thyroid-stimulating hormone (rhTSH)-aided iodine 131 (131I) adjuvant treatment as prognostic markers of DTC. METHODS: Six hundred-fifty patients with DTC given total/near-total thyroidectomy and adjuvant radioiodine post-rhTSH stimulation were evaluated. Thyroglobulin was measured on day 1 (Tg1; at the time of the first rhTSH injection), day 3 (Tg3; 1 day after the second, final rhTSH injection), and day 6 (Tg6; 3 days post-radioiodine administration). Treatment failure was defined as histopathologically confirmed locoregional recurrence, or radiologically-evident distant metastases (signs of disease on computer tomography (CT) or magnetic resonance imaging (MRI), or abnormal foci of radioiodine or [18F] fluorodeoxyglucose ([18F]FDG) uptake. RESULTS: In univariate analysis, Tg1 (p < 0.001) and Tg3 (p < 0.001), but not Tg6, were significantly associated with structural recurrence. In multivariate analysis of the overall cohort, only Tg3 was independently associated with structural recurrence. In multivariate analysis of the subgroup (n = 561) with anti-Tg antibodies titers below the institutional cut-off, 115 IU/mL, Tg1 was an independent prognostic marker. Tg1 and Tg3 cutoffs to best predict structural recurrence were established at 0.7 ng/mL and 1.4 ng/mL, respectively. CONCLUSIONS: Tg1 and Tg3, measurements made after rhTSH stimulation but before radioiodine treatment, independently predict a low risk of treatment failure in patients with DTC. Levels measured post-radioiodine application (e.g., Tg6) are highly variable, lack prognostic value, and hence can be omitted.

Papillary thyroid cancer (PTC) usually has a good prognosis. The treatment, including total thyroidectomy and complementary radioiodine (RAI) therapy, gives complete remission in 90% of patients. However, in 10% of subjects with metastatic disease, the prognosis is poor. In the group of patients with disease progression and no 131I uptake, searching for new therapeutic modalities before all tyrosine kinase inhibitors and other antiangiogenic agents is necessary. The study presents the case of a 55-year-old male with advanced PTC /pT3mNxMo/ diagnosed in 1993. Primary treatment by total thyroidectomy and 131I ablation led to complete remission. In 2000 local as well as lymph node recurrence was diagnosed and successively treated by surgery. In 2006 an increasing serum thyroglobulin level was noted and a single lung metastasis was diagnosed and operated on. In 2007 new foci in CNS and vertebral column with no 131I uptake were stated. Further progression (bones, CNS, and pterygoid muscle) was confirmed by PET-CT. The patient underwent neurosurgical metastasectomy twice and palliative CNS and vertebra's radiotherapy. Liver metastases were diagnosed in 2009. Treatment with increasing doses of thalidomide (up to 800 mg/d) was administered for 3 months with a good tolerance; however, the therapy was withdrawn due to cancer progression. Next, sorafenib (800 mg/d) was given for 16 weeks. Radiological examination performed after 16 weeks confirmed stable disease, whereas 2 months later, after sorafenib withdrawal due to lack of treatment possibility, further progression was observed. Metronomic chemotherapy with Adriamycin was instituted which gave disease stabilization for 6 months. The patient died with advanced disseminated disease due to pulmonary embolism. We present this case to document no adverse effects of therapy with sorafenib in a patient with brain DTC metastases. Sorafenib therapy was only short-term, but no progression occurred in this time.

INTRODUCTION AND OBJECTIVE: Acrylamide (AA) is a carcinogenic and genotoxic food contaminant occurring in carbohydrate-rich foods produced at high cooking temperatures. The aim of the study was to determine the importance of AA exposure with respect to traditional food and to assess the associated risks. MATERIAL AND METHODS: 165 food samples were collected from local markets in Lesser Poland. The participants enrolled in the study were 500 residents: (males - 179, females - 321) who had purchased food from local markets. Exposure of the participants to AA was assessed by combining the analytical AA results with data on the individual consumption of traditional foods. Risk assessment of AA exposure from traditional foods was estimated and the margin of exposure (MOE) values were calculated. RESULTS: The highest mean AA level was measured in pretzels (92 µg kg -1), followed by bagels (74.81 µg kg-1) and pork paté (59.56 µg kg-1). The average and 95th percentile values of AA exposure were 0.213 and 0.458 [µg kg-1 body weight (BW) day-1]. The calculated values of MOE for the average [798 and 2,019 for both benchmark dose lower confidence limit (BMDL) 0.17 and 0.43 mg kg-1 BW day-1] and 95th percentile AA exposure values (371 and 939 for both BMDL 0.17and 0.43 mg kg -1 BW day-1) suggest that there is a health concern with respect to adult residents. CONCLUSIONS: The results of the study confirm the general recommendation to the consumers, especially certain population groups, to eat a balanced healthy diet and to limit the amount of baked cereal products and fried products.

INTRODUCTION: Osteoporosis, a disease associated with ovarian hormone deficiency following menopause, is the most common cause of age-related bone loss. Although an optimal intake of Ca is vital - both bone accretion during growth and maintenance in adult life - a great percentage of the population consumes far below the recommended amounts of this mineral. On the other hand, there are evidences that fructans enhance not only Ca absorption, but bone calcium as well. OBJECTIVE: In the knowledge that estrogen deficiency and insufficient Ca in the diet during postmenopause cause serious problems with resultant osteoporosis, the aim of this study is to assess the effects of a diet enriched in fructan-containing sources alone, or in the "strawberry matrix", on the structure of bone in OVX rats under calcium hypoalimentation. MATERIAL AND METHODS: Experimental animals were female Wistar rats, sham-operated or ovariectomized. The treatment with Ca-restricted diets also contained one of the sources of fructan (Jerusalem artichoke, yacon, Beneo Orafti Synergy1), in the amount providing 8% of fructans. Femur architecture of rats was assessed by tomography and Ca content by the AAS method. RESULTS: Ovariectomy led to a significant decrease in femoral Ca content, total mineral content and bone density of rats. This study shows that a diet containing inulin-type fructan (especially as a component of strawberry product) improved bone quality (i.e. increase in Ca content in femur, total density in middle part of bone, as well as decrease of endosteal circumference) in OVX rats under calcium hypoalimentation. CONCLUSIONS: These findings suggest that a fructan-enriched diet could be potentially useful for postmenopausal osteoporosis. It is important to determine an optimal dietary level of fructan with the long-term goal of developing a dietary strategy in osteoporosis prevention.

INTRODUCTION: Osteoporosis, the "quiet epidemic", is one of the most serious threats to public health. It is known that estrogen plays a significant role in the regulation of bone turnover, and its loss at menopause causes osteoporosis. Added to this, insufficient calcium intake accelerates bone mass loss, increasing the risk of fractures. OBJECTIVE: The study aimed to answer the question whether a fructan-enriched diet could be helpful in preventing from disturbances in bone turnover caused by calcium restriction combined with ovariectomy-induced estrogen deficiency. The differences related to the kind of fructan and 'matrix effect' of fructan action (form of addition) were also evaluated. MATERIAL AND METHODS: The study was conducted using sham-operated (control groups) or ovariectomized (OVX) rats fed a calcium restricted diet. The treatment diets contained one of three fructan sources - Jerusalem artichoke, yacon and Beneo Orafti Synergy1 - added alone or as an ingredient of strawberry sorbet, all in the amount providing 8% fructans. Analyses of biological material included: serum Ca, Mg and P concentrations, alkaline phosphatase activity (ALP), osteocalcin (OC) and C-telopeptide degradation products from type I collagen (CTX). Densitometric parameters of femora were also assayed. RESULTS: Among markers of bone turnover, the ALP activity depended both on the kind of fructan and the form of addition. The highest value was shown in the OVX group fed a low-calcium diet, whereas administration of diet enriched with Jerusalem artichoke led to an almost 50% decrease in the value of this parameter. Dietary fructans also lowered the OC level. Feeding rats with diet containing sorbet enriched in yacon or Jerusalem artichoke resulted in a decrease of CTX, compared to the diet containing yacon alone or fructan formulation in both forms No significant differences were observed in densitometric parameters between treatment groups. CONCLUSIONS: The obtained findings suggest that fructan administration with a calcium-restricted diet might exert a positive effect on bone turnover parameters. Regarding the form of their addition, it is possible that other constituents of sorbets contributed to the fructan action. It remains open whether this impact would be significant over a longer period of time.

BACKGROUND: Telomerase reverse transcriptase promoter (TERTp) mutations are related to a worse prognosis in various malignancies, including papillary thyroid carcinoma (PTC). Since mechanisms responsible for the poorer outcome of TERTp(+) patients are still unknown, searching for molecular consequences of TERTp mutations in PTC was the aim of our study. METHODS: The studied cohort consisted of 54 PTCs, among them 24 cases with distant metastases. BRAF V600E, RAS, and TERTp mutational status was evaluated in all cases. Differences in gene expression profile between TERTp(+) and TERTp(-) PTCs were examined using microarrays. The evaluation of signaling pathways and gene ontology was based on the Gene Set Enrichment Analysis. RESULTS: Fifty-nine percent (32/54) of analyzed PTCs were positive for at least one mutation: 27 were BRAF(+), among them eight were TERTp(+), and 1 NRAS(+), whereas five other samples harbored RAS mutations. Expression of four genes significantly differed in BRAF(+)TERTp(+) and BRAF(+)TERTp(-) PTCs. Deregulation of pathways involved in key cell processes was observed. CONCLUSIONS: TERTp mutations are related to higher PTC aggressiveness. CRABP2 gene was validated as associated with TERTp mutations. However, its potential use in diagnostics or risk stratification in PTC patients needs further studies.

INTRODUCTION AND OBJECTIVE: Mycotoxins are toxic metabolites produced by certain types of fungi, causing pathological changes in humans and animals. The aim of this study was to assess the degree of contamination of selected cereal grains, bran and cereal products intended for children, with mycotoxins using GCxGC-TOF-MS technique. The study involved mycotoxins belonging to the type A and B trichothecenes group, including T-2 toxin (T-2), HT-2 toxin (HT-2), scirpenol (SCI), 15-monoacetoxyscirpenol (15-MAS), diacetoxyscirpenol (DAS), triacetoxyscirpenol (TAS), fusarenon-X (FUS-X), nivalenol (NIV), deoxynivalenol (DON), 3-acetyl-DON (3-Ac-DON), 15-acetyl-DON (15-Ac-DON). The study also assessed the effect of conditions in which the samples were stored, including temperature (6°C and 28°C) and time (14 and 28 days), on fungal growth and mycotoxin production. Among all studied compounds, only DAS and HT-2 toxins were detected in tested samples, with the exception of products intended for children. Measured HT-2 mycotoxin content in tested samples was in the range 83.9 - 196.4 µg kg -1. RESULTS AND CONCLUSIONS: Experiments with storage conditions showed a statistically significant increase in the HT-2 toxin level after 14 days of storage in all samples, irrespective of temperature. Prolonged storage (additional 14 days) did not cause significant changes in the HT-2 content. Further analyses showed a statistically significant effect of storage temperature on HT-2 toxin levels only in cereal products intended for children after both 14 and 28 days. Interestingly, lower temperature (6°C) was more optimal then higher temperature (28°C) for the HT-2 toxin production. No significant effect of storage temperature on HT-2 level was observed for cereal grains and bran.
