Liver cancer is very frequent, and hepatocellular carcinoma (HCC) accounts for the majority of liver cancer cases. Its growing incidence has been greatly affected by the increasing prevalence of metabolic-associated fatty liver disease (MAFLD). The latter is a new epidemic in our era. In fact, HCC is often generated from noncirrhotic liver and its treatment benefits from surgical and nonsurgical approaches, potentially bridged by transjugular intrahepatic portosystemic shunt (TIPS) use. TIPS use is an effective treatment for portal hypertension complications, but its application in patients with HCC and clinically significant portal hypertension (CSPH) remains controversial due to concerns about tumor rupture, dissemination, and increased toxicity. The technical feasibility and safety of TIPS use in HCC patients have been evaluated in several studies. Despite concerns about intraprocedural complications, retrospective studies have shown high success rates and low complication rates in TIPS placement for HCC patients. TIPS use in combination with locoregional treatments, such as transarterial chemoembolization (TACE) or transarterial radioembolization (TARE), has been explored as a treatment option for HCC patients with portal hypertension. These studies have shown improved survival rates in patients undergoing TIPS in combination with locoregional treatments. However, the efficacy and toxicity of TACE in combination with TIPS use require careful evaluation, as changes in venous and arterial flow can affect treatment outcomes and complications. The results from studies evaluating the impact of TIPS on systemic therapy and surgical options are also promising. In conclusion, the TIPS is a sufficiently safe, useful item available for physicians treating complications of portal hypertension. Moreover, a TIPS can be used in combination with locoregional therapy in HCC patients. Systemic chemotherapy can also benefit of the use of TIPS placement. A complex interplay affects TIPS use with surgery. The latter needs further data. The TIPS is a useful and safe add-on treatment, changing the natural course of HCC progression. Its use is regulated by a sophisticated physiologic and pathophysiologic flow of evidence.

Nowadays, technology permeates every aspect of human activity, from the professional to the recreational side, and the recent pandemic crisis has only amplified a clearly defined trend. Through portable devices, people play, work, and get information. The immediacy of information and of many aspects of our life is a condition that, once acquired, is difficult to give up. However, immediate information must also be correct, and not all issues can easily be simplified to the point of being understandable in the absence of basic theoretical notions to a large audience. This aspect is particularly relevant when dealing with issues related to human health and the communication problems encountered in recent months concerning the spread of SARS-CoV-2, and the vaccination campaign is there to prove it. With the intent of clarifying the advantages and disadvantages of fast and accessible information, in this article, some of the most widespread applications for mobile devices, dedicated to pollen allergens, air quality, parks, gardens, and green areas, were examined. A widespread lack of clarity on the sources of the data and on the criteria used to formulate synthetic judgments-that often confuse the final users-was noted.

RESUMO

In the last few years, we have witnessed an important development in the medical field of both Mobile Health, such as the use of mobile communication devices, and other telemedicine tools in general, in order to support the surveillance of diseases from the moment of the first diagnosis to the therapeutic follow-up. Long before COVID-19, some authors had analyzed various possible evidence-based scenarios and had indicated how the use of telemedicine could prove to be extremely useful in epidemic situations, especially for the management of chronic patients, such as immune-allergic ones, who are notoriously in greater need of regular follow-up; however, as expected, the advent of the COVID-19 pandemic has amplified the differences between various countries, from the point of view of the propensity to use technological solutions in the health sector. The hope is that one positive outcome of the ongoing pandemic is that it will lead to an acceleration, by all the stakeholders involved, of the process of modernization of health care.

This work reviewed existing literature on airport related activities that could worsen surrounding air quality; its aim is to underline the progress coming from recent-year studies, the knowledge emerging from new approaches, the development of semi-empiric analytical methods as well as the questions still needing to be clarified. To estimate pollution levels, spatial and temporal variability, and the sources relative contributions integrated assessment, using both fixed point measurement and model outputs, are needed. The general picture emerging from the studies was a non-negligible and highly spatially variable (within 2-3 km from the fence line) airport contribution; even if it is often not dominant compared to other concomitant pollution sources. Results were highly airport-specific. Traffic volumes, landscape and meteorology were the key variables that drove the impacts. Results were thus hardly exportable to other contexts. Airport related pollutant sources were found to be characterized by unusual emission patterns (particularly ultrafine particles, black carbon and nitrogen oxides during take-off); high time-resolution measurements allow to depict the rapidly changing take-off effect on air quality that could not be adequately observed otherwise. Few studies used high time resolution data in a successful way as statistical models inputs to estimate the aircraft take-off contribution to the observed average levels. These findings should not be neglected when exposure of people living near airports is to be assessed.

OBJECTIVES: to assess air pollution spatial and temporal variability in the urban area nearby the Ciampino International Airport (Rome) and to investigate the airport-related emissions contribute. DESIGN AND SETTING: the study domain was a 64 km2 area around the airport. Two fifteen-day monitoring campaigns (late spring, winter) were carried out. Results were evaluated using several runs outputs of an airport-related sources Lagrangian particle model and a photochemical model (the Flexible Air quality Regional Model, FARM). MAIN OUTCOME MEASURES: both standard and high time resolution air pollutant concentrations measurements: CO, NO, NO2, C6H6, mass and number concentration of several PM fractions. 46 fixed points (spread over the study area) of NO2 and volatile organic compounds concentrations (fifteen days averages); deterministic models outputs. RESULTS: standard time resolution measurements, as well as model outputs, showed the airport contribution to air pollution levels being little compared to the main source in the area (i.e. vehicular traffic). However, using high time resolution measurements, peaks of particles associated with aircraft takeoff (total number concentration and soot mass concentration), and landing (coarse mass concentration) were observed, when the site measurement was downwind to the runway. CONCLUSIONS: the frequently observed transient spikes associated with aircraft movements could lead to a not negligible contribute to ultrafine, soot and coarse particles exposure of people living around the airport. Such contribute and its spatial and temporal variability should be investigated when assessing the airports air quality impact.

Hepatocellular carcinoma (HCC) is undergoing a transformative shift, with metabolic-associated fatty liver disease (MAFLD) emerging as a dominant etiology. Diagnostic criteria for MAFLD involve hepatic steatosis and metabolic dysregulation. Globally, MAFLD prevalence stands at 38.77%, significantly linked to the escalating rates of obesity. Epidemiological data indicate a dynamic shift in the major etiologies of hepatocellular carcinoma (HCC), transitioning from viral to metabolic liver diseases. Besides the degree of liver fibrosis, several modifiable lifestyle risk factors, such as type 2 diabetes, obesity, alcohol use, smoking, and HBV, HCV infection contribute to the pathogenesis of HCC. Moreover  gut microbiota and genetic variants may contribute to HCC development.The pathophysiological link between MAFLD and HCC involves metabolic dysregulation, impairing glucose and lipid metabolism, inflammation and oxidative stress. Silent presentation poses challenges in early MAFLD-HCC diagnosis. Imaging, biopsy, and AI-assisted techniques aid diagnosis, while HCC surveillance in non-cirrhotic MAFLD patients remains debated.ITA.LI.CA. group proposes a survival-based algorithm for treatment based on Barcelona clinic liver cancer (BCLC) algorithm. Liver resection, transplantation, ablation, and locoregional therapies are applied based on the disease stage. Systemic treatments is promising, with initial immunotherapy results indicating a less favorable response in MAFLD-related HCC.Adopting lifestyle interventions and chemopreventive measures with medications, including aspirin, metformin, and statins, constitute promising approaches for the primary prevention of HCC.Prognosis is influenced by multiple factors, with MAFLD-HCC associated with prolonged survival. Emerging diagnostic biomarkers and epigenomic markers, show promising results for early HCC detection in the MAFLD population.

Background: Increasing rail transportation requires appropriate railway suicide preventive measures. Aims: The investigation of trends in railway suicide during 2008-2018, a period in which preventive measures were taken by Dutch railway infrastructure manager ProRail. Methods: Generalized linear regression models for railway suicide were developed for the period 1970-2007 with general suicide rate, railway traffic intensity, and a combination of these variables as regressors. Subsequently, the best-fitting model was used to investigate trends in railway suicide after 2007 by comparing in retrospect observed values with the expected outcomes of the regression model. Results: An adequate regression model for railway suicide was obtained using both general suicide rate and railway traffic intensity as regressors. Based on this model, while national suicide mortality and railway traffic increased, a distinct relative decline in railway suicides was found from 2012 onward. Conclusions: This decline of railway suicides in the Netherlands may indicate that preventive measures taken by ProRail were effective and prevented around 85 railway suicides annually, a reduction of 30%.

The relevance of dust outbreaks from the Sahara into Italy was evaluated for the period from 2001 to 2007. To this end, days affected by Saharan dust were recognized at four PM(10) background stations belonging to the Regional Agencies for Environmental Protection (ARPA) monitoring networks (one working in the 2005-2007 period only). The analysis included computation of back trajectories, complemented with satellite images and meteorological charts. Results from other models are also collected for comparison. Saharan Dust contribution to PM(10) was calculated referring to a method developed for Iberian monitoring networks. PM(10) exceeding the daily limit value (DLV) of the European Union air quality standard were reviewed also. The provenience of air masses yielding the exceedances was also recognized, allowing to distinguish the Atlantic-European episodes from the (Atlantic)-Saharan ones. The European episodes are due to anthropogenic pollution that affected mainly northern Italy in winter; instead, the Saharan ones are due to dust outbreaks that affected mainly southern Italy in summer. Central Italy is characterised by a transitional situation that is experienced from north to south. A number of dust affected days with DLV exceedance (from 4 to 108 at the considered stations) were identified and grouped into 55 Saharan dust episodes. Analysis showed that DLV exceedances due to Saharan dust advection vanish at all stations when dust contributions are subtracted. A synoptic analysis based on the meteorological charts was carried out allowing a view of Saharan dust transport into Italy for each meteorological season. Notwithstanding that summer is the season most influenced by Saharan episodes at all of the stations, the analysis showed a low-high system of anomalies on the geopotential height distribution causing south-southwesterly flow carrying dust into Italy during the other seasons.

The main objective of this study was to asses the temporal variation (1999 trough 2008) of air quality in Rome, focusing on airborne concentration of selected pollutants (PM10 and PM2.5 mass concentration and particle number concentration, PNC, carbon monoxide, CO, nitrogen oxides, NO and NO2) used for health effects assessment in epidemiological analyses. Time series analysis using Seasonal Kendall test has been applied. A statistically significant decreasing trend was found for primary gaseous pollutants and total particle number concentrations. Moreover a decreasing trend was assessed for PM10, PM2.5 and NO2 measured at traffic oriented sites even if the estimated reduction was lower compared with NO, CO and PNC. The urban background PM10 and NO2 concentrations seem to be practically unchanged since 1999 as no statistically significant trends were found. All the pollutants show higher slope of the estimated trend line at traffic oriented sites compared with those observed at the urban background. Thus a reduction of the intra-city concentration variability throughout the years occurred.
