RESUMO

Under the catalysis of a variety of metabolic enzymes in vivo, such as UDP-glucuronyl transferases, cytochrome P450, carboxylesterase, sulfotransferase, butyrylcholinesterase, catechol-O-methyl transferase and 6-morphine dehydrogenase, the drugs perform glucuronidation, hydrolysis, oxidation, sulfonation and other reactions, then translate into active or inactive metabolites, which are excreted through urination, bile or the other pathways at last. Different drugs own their different metabolic pathways. This paper introduces the studies about the metabolism of drugs in human and animal in recent years, such as morphine-like drugs, amphetamine, ketamine, cannabis and cocaine, and reviews the research progress about the sites of metabolism, metabolic enzymes, metabolites and physiological activity of those drugs metabolic in vivo.

Radical local resection is undoubtedly the method of choice in treating malignant bone and soft tissue tumours in the extremities, provided there is no local recurrence after radical resection. This is even truer today when the chemotherapy of osteosarcoma has achieved encouraging advances in preventing lung metastasis. The diagnostic methods for the evaluation of the tumour infiltration are at a more complete stage, and operative reconstruction techniques have als made rapid progress. Radical local resection is especially suitable for sarcomata of the extremities which are at an early stage, with less infiltrative low-grade malignancy. Amputation is better when the tumour grows rapidly and is large, if the soft tissue is widely infiltrated, if the patient is from 20 to 30 years old, and especially if the tumour is located in the upper tibia where it is difficult to carry out local resection. In this article, we report and discuss the definition, necessity, and possibility of radical local resection as well as the method of surgical reconstruction and our results. The results and prognosis for radical resection of giant-cell sarcoma, chondrosarcoma, and fibrosarcoma are rather good. Three of five cases of osteosarcoma died of lung metastasis one year after surgery. Therefore, improving the results in local resection of osteosarcoma calls for further investigation.

RESUMO

The surgical treatment of two cases of bone tumours and one case of a tumour-like lesion are reported in this article. The tumours were resected radically and the massive bone defect or bone cavity were substituted by vascularized free fibula transfer. All of them were cured by early union. Neither atrophy nor non-union between the graft and the recipient bone occurred. Early restoration of function was observed. Our experiences of isolating the graft and the operative procedure are discussed. We believe that this method has prospects not only in the field of treatment of bone tumours but also in the management of bone defects or non-union due to various other causes.
