RESUMO

To assess stress level induced by multiple stressors in aquatic organism, biomarkers have been adopted as early warning indicator due to their high accuracy, rapidity, and sensitivity. We investigated the effects of ectoparasitic isopod infection on heavy metal bioaccumulation (Fe, Cu, Zn, and Cd) in the fish Nemipterus furcosus and profiled the expression of metallothionein (MT) and heat shock proteins 70 (HSP70) genes of the fish host. Sixty individuals (parasitized and nonparasitized with Cymothoa truncata) were collected from three sites differing in the levels of anthropogenic activities off the South China Sea. Our results revealed no significant difference in heavy metal concentrations between infected and nonparasitized fish. We observed a positive correlation between heavy metal bioaccumulation in the fish host and anthropogenic activities. Accordingly, expression analysis of MT genes in fish liver showed significant differences in expression level between sampling sites, with lowest level in the least exploited site (Batu Rakit). A reverse pattern in HSP70 gene expression was demonstrated in fish muscle, showing the highest expression at Batu Rakit. While cymothoid infection in N. furcosus had no significant impact on fish MT gene expression, it resulted in a reduction of HSP70 level in liver of parasitized fish. These findings highlight the putative roles of MT in heavy metal assessment. Future studies should determine the kinetics of cymothoid infection and other potential stressors in characterizing the HSP70 gene expression profile.

Introduction: Medical aesthetic practice is growing rapidly in Malaysia due to rising market demand, yet public understanding of these practices remains limited. This study evaluated the awareness and attitudes towards medical aesthetics among Malaysians. Methods: A cross-sectional survey was conducted among individuals aged > 18 years old in Malaysia from December 2021 to May 2022. The survey was distributed both online and in public settings. The survey comprised of respondents' sociodemographic, perception of physical attractiveness, knowledge and attitude towards medical aesthetic practices. The factors influencing respondents' attitudes towards medical aesthetic practices in the country were analysed using binary logistic regression, with the significance level set at P<0.05. Results: A total of 382 respondents participated in this study with average age of 30.81 (±9.38) years, ranging from 18 to 68 years. The majority were women (77.5%), Malay (53.7%) and Muslim (57.1%). Although most respondents had no prior experience in medical aesthetic treatment (68.1%), 76.2% respondents demonstrated good knowledge and 70.2% had positive attitude towards these services. Additionally, 53.9% of the respondents highly valued the importance of physical attractiveness. The Buddhists and Hindus exhibited more positive attitude than the Muslims. Conversely, those who placed higher importance on physical attractiveness were 0.5 times less likely to develop a positive attitude towards medical aesthetic services. Conclusion: In conclusion, despite limited experience in medical aesthetic treatments and practice, most Malaysians possess good knowledge and positive attitudes towards medical aesthetics, indicating a growing interest and potential willingness to consider these services for enhancing their appearance.

Safety at the gore areas near diverging ramps is very crucial during planning and implementation of highway safety improvement programs. Limited research has been conducted on safety at the gore areas on arterial roads. This study aims at investigating the impact of improving a sharp gore area in Lincoln, Nebraska by performing a micro simulation before-and-after study with respect to its underlying state of safety and congestion. Data on travel times and traffic volumes for peak hours are incorporated after successful calibration to find out how a geometric intervention can decrease mobility issues as well as the likelihood of crash involvement. This study has utilized VISSIM software package to run the simulation however, to perform safety analysis, Surrogate Safety Assessment Model (SSAM) is used which is developed by the Federal Highway Administration (FHWA). During the model calibration, custom driving behaviors are created to represent driving tendencies of familiar drivers. The simulation results indicated that by adding an auxiliary lane near the gore area, the mobility issues such as bottle necks, lane changing dilemmas and queue lengths are substantially decreased. However, geometric interventions such as provision of a separate lane, increasing ramp spacing, nose spacing, deceleration area and queue storage area considerably reduced the likelihood of rear-end and lane changing crashes. Surrogate safety assessment in diverging ramps, particularly for sharp gores, has not previously been studied, and this study can serve as a primary footmark for future research on ramp-gores safety.

Objective: To study the effectiveness of early percutaneous transthyrohyoid injection laryngoplasty under local anesthesia in improving voice quality in unilateral vocal fold paralysis (UVFP) patients. Methods: Longitudinal data of 29 UVFP patients who underwent injection laryngoplasty within six months from the onset of the symptoms were studied. The injectate (0.5-1 mL) was hyaluronic acid-based material (Juvéderm ULTRA XC; Allergan Industrie, France) that was delivered under local anesthesia with transthyrohyoid approach using a double-bend 21G needle. Multidimensional voice outcomes employing: 1) Malay-Voice-Handicap Index-10 (mVHI-10); 2) maximum phonation time (MPT); and 3) acoustic analysis [jitter%, shimmer% and noise-harmonic ratio (NHR)] were used to assess the treatment progress. The voice parameters were measured at baseline (2 weeks pre-injection), and at the first and third months post-injection. Results: The mean age of the 29 patients was 44.69 years, with a female-to-male ratio of 3.14:1. The voice outcomes measured at different time points were evaluated with repeated measures ANOVA. Significant improvement was observed from baseline to three months post injection laryngoplasty for mVHI-10, jitter, and NHR (p<0.001), shimmer (p=0.005) and MPT (p=0.018). Following the procedure, none of the patients developed any major complications. Conclusion: Office setting early transthyrohyoid injection laryngoplasty using a double-bend needle is a safe and effective procedure in patients with UVFP with evidence of significant improvement in voice and life quality.

Recent studies suggested that accurate species identification in the tropical anguillid eels based on morphological examination requires confirmation by molecular genetic analysis. Previous studies found that two tropical anguillid eels, Anguilla bicolor bicolor and A. bengalensis bengalensis, were found in peninsular Malaysia (West Malaysia) based on morphological and molecular genetic analyses. This study is the first record of A. marmorata in peninsular Malaysia confirmed by both morphological and molecular genetic analyses. The present study also suggests that accurate tropical eel species identification is difficult by morphological identification alone; therefore, molecular genetic analysis is needed for precise species confirmation.
